Diagnosing upper tract urothelial carcinoma: A review of the role of diagnostic ureteroscopy and novel developments over last two decades

Objective: The role of ureteroscopy in the diagnosis of upper tract urothelial carcinoma is yet to be fully determined. We aimed to provide an up to date evaluation of its role and the emerging technologies in the field.Methods: A literature search of the last two decades (from 24th May, 2001 to 24th May, 2021) was carried out identifying 147 papers for potential inclusion within this narrative review.Results: Diagnostic ureteroscopy is undeniably useful in its ability to visualise and biopsy indeterminate lesions, and to risk stratify malignant lesions that may be suitable for kidney sparing surgery. However, an increased risk of intravesical recurrence following nephroureterectomy when a prior diagnostic ureteroscopy has been performed, inadequate sampling at biopsy, complications from the procedure, and difficult ureteric access are all potential drawbacks. Furthermore, whilst generally an accurate diagnostic procedure, it risks missing carcinoma in-situ lesions. Despite this, evidence shows that routine use of ureteroscopy changes the management of patients in a large proportion of cases, preventing unnecessary surgery or facilitating kidney sparing surgery. The overall rate of complications is low, and improved biopsy techniques and the use of tissue biomarkers for improved staging and grading are encouraging. The risks of delays to definitive management and post-ureteroscopy intravesical recurrence do not seem to affect survival, and trials are in progress to determine whether intravesical therapy can mitigate the latter. Further promising techniques are being investigated to improve shortcomings, particularly in relation to improved diagnosis of carcinoma in situ and preoperative staging.Conclusion: Ureteroscopy has a role in the diagnosis of upper tract malignancy, though whether it should be used routinely is yet to be determined.

Online Demand Response Characterization Based on Variability in Customer Behavior

This paper proposes an online framework to characterize demand response(DR) over time. The proposed framework facilitates obtaining and updating the daily consumption patterns of customers. The essential concept of response profile class(RPC) is introduced for characterization and complemented by the measure of the variability in customer behavior. This paper uses a modified version of the incremental clustering by fast search and find of density peaks(CFSFDP) algorithm for daily profiles, considering the multivariate normal kernel density estimator and incremental forms of the Davies-Bouldin(i DB) and Xie-Beni(i XB) validity indices. Case studies conducted using real-world and simulated daily profiles of residential and commercial Chilean end-users have demonstrated how the proposed framework can continuously characterize DR. The proposed framework is proven to achieve realistic customer models for effective energy management by estimating the customer response to price signals at the distribution system operator(DSO) level.

A test of the species confidence hypothesis in dusky damselfish

Visual cues are important in both interspecific and intraspecific communication.The species confidence hypothesis proposes that animals are more attracted to conspecific colors and repelled by colors,not on their bodies.Studies on terrestrial lizards and birds have tested the species confidence hypothesis and shown that conspecific colors elicit reduced antipredator behavior.To date,the species confidence hypothesis has not been tested in the marine environment,specifically on coral reefs where color communication is of vital importance.We addressed this knowledge gap by measuring flight initiation distance(the distance an individual moves away from an approaching threat)in dusky damselfish(Stegastes nigricans)in response to an approaching disc of 1 of 4 different color treatments:conspecific,blue,yellow,and black.If the species confidence hypothesis explained variation in damselfish flight initiation distance,then we expected individuals to tolerate closer approaches when approached by a conspecific color.In addition,we calculated the color difference between each stimulus and its corresponding back-ground as a potential alternative explanation for flight responses.Damselfish tolerated the closest approach from the conspecific color stimulus;there were no significant differences between other colors and there was no support for the alternative color difference hypothesis.As with similar terrestrial studies,these results are relevant to ecotourists'choice of swimsuit and wetsuit colors because color choice may modify naturalantipredatorbehavior.

The development of 3D spiral microfluidics has opened new avenues for leveraging inertial focusing to analyze small fluid volumes,thereby advancing research across chemical,physical,and biological disciplines.While traditional straight microchannels rely solely on inertial lift forces,the novel spiral geometry generates Dean drag forces,eliminating the necessity for external fields in fluid manipulation.Nevertheless,fabricating 3D spiral microfluidics remains a labor-intensive and costly endeavor,hindering its widespread adoption.Moreover,conventional lithographic methods primarily yield 2D planar devices,thereby limiting the selection of materials and geometrical configurations.To address these challenges,this work introduces a streamlined fabrication method for 3D spiral microfluidic devices,employing rotational force within a miniaturized thermal drawing process,termed as mini-rTDP.This innovation allows for rapid prototyping of twisted fiber-based microfluidics featuring versatility in material selection and heightened geometric intricacy.To validate the performance of these devices,we combined computational modeling with microtomographic particle image velocimetry(uTPiM)to comprehensively characterize the 3D flow dynamics.Our results corroborate the presence of a steady secondary flow,underscoring the effectiveness of our approach.Our 3D spiral microfluidics platform paves the way for exploring intricate microflow dynamics,with promising applications in areas such as drug delivery,diagnostics,and lab-on-a-chip systems.

Flame Synthesis of Zr/ZSM-5 Catalysts with Tunable Acidity for the Oxidative Dehydrogenation of Propane to Propene

Tuning the surface acidity of ZSM-5 catalyst is essential to achieve desired propene selectivity and yield.Here several ratios of Zr were utilized to modify ZSM-5 via flame spray pyrolysis technique coupled with a pulse spray evaporation system.The interaction between Zr and ZSM-5 in the flame influenced the physicochemical and acidity properties of the Zr/ZSM-5.The increasing Zr ratio in ZSM-5 shows coated layers of irregular nano-sized Zr with an increase in crystallite sizes due to the synergetic effect between Zr and ZSM-5.The surface chemical analysis revealed increased lattice oxygen on the Zr modified ZSM-5(1:4) sample compared to other catalysts.The acidity analysis revealed the Lewis and Br?nsted acid distribution in the weak and medium acid sites on the catalyst surface.However,the increase in Zr loading decreased the concentration of Br?nsted acid sites and tuned the catalyst surface to more Lewis acidity,promoting propene selectivity and hindering the over-oxidation of propene.The modified ZSM-5 catalysts were examined in a fixed bed reactor within 300℃-700℃ at a gas hourly space velocity(GHSV) of 6000 mL·g(catalysts)^(-1)·h^(-1) for the oxidative dehydrogenation of propane(ODHP) to propene.Among the catalysts,Zr/ZSM-5(1:4) exhibited the best propene yield, with 57.19% propane conversion and 75.54% selectivity to propene and the highest stability.This work provides a promising strategy for tuning the surface acidity of ZSM-5 with Zr for ODHP applications.

Effects of positive end-expiratory pressure on intracranial pressure,cerebral perfusion pressure,and brain oxygenation in acute brain injury:Friend or foe?A scoping review

Background Patients with acute brain injury(ABI)are a peculiar population because ABI does not only affect the brain but also other organs such as the lungs,as theorized in brain–lung crosstalk models.ABI patients often require mechanical ventilation(MV)to avoid the complications of impaired respiratory function that can follow ABI;MV should be settled with meticulousness owing to its effects on the intracranial compartment,especially regarding positive end-expiratory pressure(PEEP).This scoping review aimed to(1)describe the physiological basis and mechanisms related to the effects of PEEP in ABI;(2)examine how clinical research is conducted on this topic;(3)identify methods for setting PEEP in ABI;and(4)investigate the impact of the application of PEEP in ABI on the outcome.Methods The five-stage paradigm devised by Peters et al.and expanded by Arksey and O'Malley,Levac et al.,and the Joanna Briggs Institute was used for methodology.We also adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)extension criteria.Inclusion criteria:we compiled all scientific data from peer-reviewed journals and studies that discussed the application of PEEP and its impact on intracranial pressure,cerebral perfusion pressure,and brain oxygenation in adult patients with ABI.Exclusion criteria:studies that only examined a pediatric patient group(those under the age of 18),experiments conducted solely on animals;studies without intracranial pressure and/or cerebral perfusion pressure determinations,and studies with incomplete information.Two authors searched and screened for inclusion in papers published up to July 2023 using the PubMed-indexed online database.Data were presented in narrative and tubular form.Results The initial search yielded 330 references on the application of PEEP in ABI,of which 36 met our inclusion criteria.PEEP has recognized beneficial effects on gas exchange,but it produces hemodynamic changes that should be predicted to avoid undesired consequences on cerebral blood flow

Parkland trees on smallholder farms ameliorate soil physical-chemical properties in the semi-arid area of Tigray,Ethiopia

Proposed agroforestry options should begin with the species that farmers are most familiar with,which would be the native multipurpose trees that have evolved under smallholder farms and socioeconomic conditions.The African birch(Anogeissus leiocarpa(DC.)Guill.&Perr.)and pink jacaranda(Stereospermum kunthianum Cham.)trees are the dominant species in the agroforestry parkland system in the drylands of Tigray,Ethiopia.Smallholder farmers highly value these trees for their multifunctional uses including timber,firewood,charcoal,medicine,etc.These trees also could improve soil fertility.However,the amount of soil physical and chemical properties enhanced by the two species must be determined to maintain the sustainable conservation of the species in the parklands and to scale up to similar agroecological systems.Hence,we selected twelve isolated trees,six from each species that had similar dendrometric characteristics and were growing in similar environmental conditions.We divided the canopy cover of each tree into three radial distances:mid-canopy,canopy edge,and canopy gap(control).At each distance,we took soil samples from three different depths.We collected 216 soil samples(half disturbed and the other half undisturbed)from each canopy position and soil depth.Bulk density(BD),soil moisture content(SMC),soil organic carbon(SOC),total nitrogen(TN),available phosphorus(AP),available potassium(AK),p H,electrical conductivity(EC),and cation exchange capacity(CEC)were analysed.Results revealed that soil physical and chemical properties significantly improved except for soil texture and EC under both species,CEC under A.leiocarpus,and soil p H under S.kunthianum,all the studied soils were improved under both species canopy as compared with canopy gap.SMC,TN,AP,and AK under canopy of these trees were respectively 24.1%,11.1%,55.0%,and 9.3% higher than those soils under control.The two parkland agroforestry species significantly enhanced soil fertility near the canopy of topsoil through improving soil physical and chemica
