BACKGROUND Parahiatal hernias(PHHs)are rare occurring disease,with a reported incidence of 0.2%-0.35%in patients undergoing surgery for hiatal hernia.We found only a handful of cases of primary PHHs in the literature.The aim of this paper is to present a case of a primary PHH and perform a systematic review of the literature.CASE SUMMARY We report the case of a 60-year-old Caucasian woman with no history of thoracoabdominal surgery or trauma,which accused epigastric pain,starting 2 years prior,pseudo-angina and bloating.Based on imagistic findings the patient was diagnosed with a PHH and an associated type I hiatal hernia.Patient underwent laparoscopic surgery and we found an opening in the diaphragm of 7 cm diameter,lateral to the left crus,through which 40%-50% of the stomach had herniated in the thorax,and a small sliding hiatal hernia with an anatomically intact hiatal orifice but slightly enlarged.We performed closure of the defect,suture hiatoplasty and a“floppy”Nissen fundoplication.Postoperative outcome was uneventful,with the patient discharged on the fifth postoperative day.We performed a review of the literature and identified eight articles regarding primary PHH.All data was compiled into one tabled and analyzed.CONCLUSION Primary PHHs are rare entities,with similar clinical and imagistic findings with paraesophageal hernias.Treatment usually includes laparoscopic approach with closure of the defect and the esophageal hiatus should be dissected and analyzed.Postoperative outcome is favorable in all cases reviewed and no recurrence is cited in the literature.

Maintaining flexibility in smart grid consumption through deep learning and deep reinforcement learning

The smart grid concept is key to the energy revolution that has been taking place in recent years.Smart Grids have been present in energy research since their emergence.However,the scarcity of data from different energy sources,hardware power,or co-simulation environments has hindered their development.With advances in multi-agent-based systems,the possibility of simulating the behavior of different energy sources,combining real building consumption,and simulated data,storage batteries and vehicle charging points,has opened up.This development has resulted in much research published using both simulated and physical data.All these investigations show that the main problem is that the machine learning algorithms do not fully match the real behavior,it is complex to use them to replicate the different actions to be performed.This paper aims to combine the approach of behavior prediction with state-of-the-art techniques,such as deep learning and deep reinforcement learning,to simulate unknown or critical system scenarios.A very important element in smart grids is the possibility of maintaining consumption within specific ranges(flexibility).For this purpose,we have made use of Tensorflow libraries that predict energy consumption and deep reinforcement learning to select the optimal actions to be performed in our system.The developed platform is flexible enough to include new technologies such as smart batteries,electric vehicles,etc.,and it is oriented to real-time operation,being applied in an on-going real project such as the European ebalance-plus project.

Exploiting Deep Learning Techniques for Colon Polyp Segmentation

As colon cancer is among the top causes of death, there is a growinginterest in developing improved techniques for the early detection of colonpolyps. Given the close relation between colon polyps and colon cancer,their detection helps avoid cancer cases. The increment in the availability ofcolorectal screening tests and the number of colonoscopies have increasedthe burden on the medical personnel. In this article, the application of deeplearning techniques for the detection and segmentation of colon polyps incolonoscopies is presented. Four techniques were implemented and evaluated:Mask-RCNN, PANet, Cascade R-CNN and Hybrid Task Cascade (HTC).These were trained and tested using CVC-Colon database, ETIS-LARIBPolyp, and a proprietary dataset. Three experiments were conducted to assessthe techniques performance: (1) Training and testing using each databaseindependently, (2) Mergingd the databases and testing on each database independently using a merged test set, and (3) Training on each dataset and testingon the merged test set. In our experiments, PANet architecture has the bestperformance in Polyp detection, and HTC was the most accurate to segmentthem. This approach allows us to employ Deep Learning techniques to assisthealthcare professionals in the medical diagnosis for colon cancer. It is anticipated that this approach can be part of a framework for a semi-automatedpolyp detection in colonoscopies.

Dear Editor, Infertility is a major health problem affecting 10%-15% of couples seeking to have children, and a male factor can be identified in about half of these cases.1 Nonobstructive azoospermia is one of the causes of male infertility （10%）, resulting from testicular failure？ The most common histological patterns in these patients are hypospermatogenesis （HS）, maturation arrest （MA）, and Sertoli cell-only syndrome （SCO）.3

Glucagonomas are rare neuroendocrine tumors that arise from cells of the pancreatic islets. Most of them are malignant and usually present as metastatic disease. Sites most commonly involved in metastases are the liver and regional lymph nodes. Bone metastases are rare events and only a few cases have been reported in the literature. We present the case of a 53-year-old male with a medical history of recurrent non-functioning glucagonoma. He presented 17 years after the initial diagnosis with new blastic bone lesions involving the T1 vertebra and the sacrum. Diagnostic steps and medical management in metastatic glucagonoma are also reviewed.

Parental food provisioning is crucial for the growth and survival of offspring.Growth rate depends on food quality and food supplied to offspring may differ from what adults use for their own.In the case of steppe passerine birds,detailed characterization on nestling dietary composition,as well as prey choice and resource partitioning among species,is a pending subject.Dietary differences between nestlings and adults remain also largely unexplored.By using faecal DNA metabarcoding,we described the diet of nestlings and adults of five shrub-steppe passerine species over the 2017–2019 breeding seasons in central Spain.We also monitored arthropod availability in the field to assess dietary selection.We expected interspecific dietary differences to limit competition for food resources among sympatric species,as well as parental selection of high quality prey for nestlings.We also predicted age-related differences,with nestlings being fed nutrient-rich prey more frequently than adults.The main arthropod orders provisioned to nestlings were Orthoptera,Julida,Araneae and Lepidoptera.Nestlings of the different species showed high interspecific diet overlap,indicating both a coincidence in growth needs among bird species and no or little limitation of the most profitable resources during the breeding season.Adults of all species showed higher diet richness than nestlings,and age-related differences in prey composition were mainly driven by the selection of the most easily digestible,larger protein-and calcium-rich prey for nestlings,which may favour their rapid growth,and avoiding highly sclerotized and less nutritional prey such as ants.Our study sheds light on the basic ecology and conservation of these declining steppe birds,indicating that interspecific competition may not be a major factor during the breeding season.Given the current global decline of arthropods,further long-term research would be necessary,along with the implementation of effective conservation measures that ensure a sufficient availability of resou

Dissolution Behavior of Gold in Alkaline Media Using Thiourea

In this work the dissolutive behavior of gold in alkaline medium using thiourea (TU), under different variables, was studied in a theoretical and experimental way, in order to determine the conditions under which it is feasible to dissolve gold in thiourea-alkaline medium. A thermodynamic study was conducted by chemical speciation using the method of Ro-jas-Hernández, together with an electrochemical study where the electric potential was swept in the anodic direction. The main results of the thermodynamic study were that formamidine disulfide (FDS) and sulfinic compounds (S.C.) prevail at alkaline pH;by increasing the initial concen-tration of thiourea at alkaline pH, the presence of the gold complex is al-most zero for any initial concentration of thiourea. By including sodium sulfite in the gold-thiourea system, it was possible to obtain the Au(I)-TU complex at alkaline pH, with a presence of 95.13%. Electrochemical tests allowed verifying that in the absence of sodium sulfite the dissolution of gold in an alkaline medium is very slow but adding sodium sulfite im-provements become evident in the dissolution of the metal. Therefore, sodium sulfite catalyzes the gold dissolution process and stabilizes the thiourea. With this study it was possible to establish the feasibility of using thiourea in an alkaline medium for the dissolution of gold, and the conditions under which it is possible to dissolve the gold in that medium. With these fundamentals and conditions, it is now possible to move forward to test this system for minerals and/or concentrates containing gold.

The most frequent cause of pseudomembranous colitis is Clostridium difficile(C.difficile) infection.This type of colitis is characterized by an endoscopic pattern of numerous small,yellowish or whitish plaques diffusely distributed,which typically compromises the rectum extending to proximal colon.Occasionally,the pseudomembranes compromise only the transverse or right colon,but their exclusive localization over polyps has not been reported.In this case report we have described a patient with symptoms compatible with C.difficile infection and positive for C.difficile toxigenic culture.Colonoscopy examination showed two small polyps with a whitish surface,and histopathological analysis confirmed them to be pseudomembranes over tubular adenomas.The rest of the colonic mucosa was normal and no other cause was demonstrated.We suggest that this particular distribution might be due to a higher affinity for dysplastic cells such as adenomatous polyps of colon by C.difficile and/or its toxins.

Forecasting Short Time Series with Missing Data by Means of Energy Associated to Series

In this work an algorithm to predict short times series with missing data by means energy associated of series using artificial neural networks (ANN) is presented. In order to give the prediction one step ahead, a comparison between this and previous work that involves a similar approach to test short time series with uncertainties on their data, indicates that a linear smoothing is a well approximation in order to employ a method for uncompleted datasets. Moreover, in function of the long- or short-term stochastic dependence of the short time series considered, the training process modifies the number of patterns and iterations in the topology according to a heuristic law, where the Hurst parameter H is related with the short times series, of which they are considered as a path of the fractional Brownian motion. The results are evaluated on high roughness time series from solutions of the Mackey-Glass Equation (MG) and cumulative monthly historical rainfall data from San Agustin, Cordoba. A comparison with ANN nonlinear filters is shown in order to see a better performance of the outcomes when the information is taken from geographical point observation.
