A three-factor preoperative scoring model predicts risk of recurrence after liver resection or transplantation in hepatocellular carcinoma patients with preserved liver function

BACKGROUND： No staging systems of hepatocellular carcinoma（HCC） are tailored for assessing recurrence risk. We sought to establish a recurrence risk scoring system to predict recurrence of HCC patients receiving surgical curative treatment（liver resection or transplantation）.METHODS： We retrospectively studied 286 HCC patients with preserved liver function receiving liver resection（n=184） or transplantation（n=102）. Independent risk factors were identified to construct the recurrence risk scoring model. The recurrence free survival and discriminatory ability of the model were analyzed. RESULTS： Total tumor volume, HBs Ag status, plasma fibrinogen level were included as independent prognostic factors for recurrence-free survival and used for constructing a 3-factor recurrence risk scoring model. The scoring model was as follows： 0.758×HBs Ag status（negative： 0; positive： 1）＋0.387×plasma fibrinogen level（≤3.24 g/L： 0; 〉3.24 g/L： 1）＋0.633×total tumor volume（≤107.5 cm3： 0; 〉107.5 cm3： 1）. The cutoff value was set to 1.02, and we defined the patients with the score ≤1.02 as a low risk group and those with the score 〉1.02 as a high risk group. The 3-year recurrence-free survival rate was significantly higher in the low risk group compared with that in the high risk group（67.9% vs 41.3%, P〈0.001）. In the subgroup analysis, liver transplantation patients had a better3-year recurrence-free survival rate than the liver resection patients in the low risk group（80.0% vs 64.0%, P〈0.01）. Additionally for patients underwent liver transplantation, we compared the recurrence risk model with the Milan criteria in the prediction of recurrence, and the 3-year recurrence survival rates were similar（80.0% vs 79.3%, P=0.906）.CONCLUSION： Our recurrence risk scoring model is effective in categorizing recurrence risks and in predicting recurrencefree survival of HCC before potential surgical curative treatment.

BACKGROUND: With the expansion of surgical criteria, the comparative efficacy between surgical resection (SR) and liver transplantation (LT) for hepatocellular carcinoma is inconclusive. This study aimed to develop a prognostic nomogram for predicting recurrence-free survival of hepatocellular carcinoma patients after resection and explored the possibility of using nomogram as treatment algorithm reference. METHODS: From 2003 to 2012, 310 hepatocellular carcinoma patients within Hangzhou criteria undergoing resection or liver transplantation were included. Total tumor volume, albumin level, HBV DNA copies and portal hypertension were included for constructing the nomogram. The resection patients were stratified into low- and high-risk groups by the median nomogram score of 116. Independent risk factors were identified and a visually orientated nomogram was constructed using a Cox proportional hazards model to predict the recurrence risk for SR patients. RESULTS: The low-risk SR group had better outcomes compared with the high-risk SR group (3-year recurrence-free survival rate, 71.1% vs 35.9%; 3-year overall survival rate, 89.8% vs 78.9%, both P0.05). CONCLUSIONS: This nomogram offers individualized recurrence risk evaluation for hepatocellular carcinoma patients within Hangzhou criteria receiving resection. Transplantation should be considered the first-line treatment for high risk patients.

Fibroblast activation protein（FAP） is a key molecule in the field of oncology, with significant impacts on tumor diagnosis and treatment. Importantly, it has paved the way for the development of radiotracers for quinoline-based FAP inhibitors（FAPIs）, which are currently among the most promising radiotracers for PET imaging in cancer. We performed a bibliometric analysis of scientific publications related to FAP and FAPI-based radiotracers, which included the quantification and visualization of current research trends and prospects based on various bibliometric indicators. In our survey of FAP-related studies in the Web of Science Core Collection databases, R and VOSviewer were used for visualization and bibliometric analyses based on country, institute, author, journal, and keywords. We also examined the methodology, radionuclide type, imaging instruments, and major diseases associated with studies on FAPI-based radiotracers. The results revealed 2,664 FAP-related publications from 1992 to the present. Germany, the USA, and China dominated paper publications, multinational collaborations,and societal impacts on FAP research. Southwest Medical University was the most productive institute,while Haberkorn Uwe authored the most cited papers and the highest H-index. The European Journal of Nuclear Medicine and Molecular Imaging and the Journal of Nuclear Medicine were the most influential periodicals. Keywords "FAP", "68Ga-FAPI", and "PET/CT" emerged as the most significant in this *** study may help elucidate current research trends, hotspots, and directions for future research.

AIM: To investigate the prognostic value of preoperative lymphocyte-to-monocyte ratio(LMR) in patients with hepatocellular carcinoma(HCC) undergoing curative ***: Clinicopathological data of 210 hepatitis B virus(HBV)-associated HCC patients who were treated by radical hepatic resection between 2003 and 2010 were retrospectively analyzed. None of the patients received any preoperative anticancer therapyor intraoperative radiofrequency ablation. The diagnosis was confirmed by pathological examination after surgery. Absolute peripheral blood lymphocyte and monocyte counts were derived from serum complete blood cell count before surgery,and LMR was calculated by dividing lymphocyte count by monocyte count. The best cutoff was determined by receiver operating characteristics(ROC) curve analysis. Correlations between LMR levels and clinicopathological features were assessed using the χ2 test. Survival outcomes were estimated using the Kaplan-Meier method and compared by the log-rank test. Univariate and multivariate analyses were performed to evaluate the prognostic impact of LMR and other clinicopathological factors on overall survival(OS) and recurrence-free survival(RFS),using the Cox proportional hazards ***: The optimal cutoff value of LMR for survival analysis was 3.23,which resulted in the most appropriate sensitivity of 55.3% and specificity of 74.7%,with the area under the curve(AUC) of 0.66(95%CI: 0.593-0.725). All patients were dichotomized into either a low(≤ 3.23) LMR group(n = 66) or a high(> 3.23) LMR group(n = 144). A low preoperative LMR level was significantly correlated with the presence of cirrhosis,elevated levels of total bilirubin and larger tumor size. Patients with a low LMR level had significantly reduced 5-year OS(61.9% vs 83.2%,P < 0.001) and RFS(27.8% vs 47.6%,P = 0.009) compared to those with a high LMR level. 目的探究右美托咪定是否通过抑制核因子-κB(NF-κB)通路缓解大鼠高氧诱导急性肺损伤(HALI)。方法将HALI模型大鼠分为模型组、对照组和低、中、高剂量实验组,每组15只;另取15只相同条件下暴露在空气中饲养的大鼠作为假手术组。假手术组和模型组均予以等量0.9%Na Cl,腹腔注射;对照组予以5 mg·kg-1SC75741,腹腔注射;低、中、高剂量实验组分别予以25,50和100μg·kg-1右美托咪定,腹腔注射。用免疫印迹法检测p-JAK激酶2(JAK2)、p-信号传导和转录活化因子3(STAT3)和NF-κB蛋白表达。结果低、中、高剂量实验组和模型组、对照组、假手术组的p-JAK2蛋白相对表达水平分别为(0.63±0.08),(0.58±0.06),(0.32±0.07),(0.88±0.05),(0.23±0.06)和(0.16±0.03),p-STAT3蛋白相对表达水平分别为(0.75±0.03),(0.51±0.11),(0.37±0.05),(0.98±0.12),(0.21±0.03)和(0.15±0.03),NF-κB蛋白相对表达水平分别为(0.78±0.09),(0.64±0.13),(0.48±0.08),(0.95±0.09),(0.21±0.03)和(0.22±0.03),模型组的上述指标与低、中、高剂量实验组、假手术组和对照组相比,差异均有统计学意义(均P<0.05)。结论右美托咪定能够通过抑制NF-κB通路激活,从而发挥对HALI大鼠的保护作用。
