目的探讨瑞马唑仑联合胸椎旁神经阻滞(TPVB)用于肺癌胸腔镜手术的临床效果。方法选取河北省保定市第二医院2021年10月至2023年6月收治的拟行胸腔镜手术的肺癌患者122例,随机分为对照组和观察组,各61例。两组患者均行胸腔镜手术并予TPVB,对照组患者予常规麻醉方案,观察组患者予含苯磺酸瑞马唑仑的其他常规麻醉方案。结果与对照组比较,观察组患者的术毕至拔管时间、引流管留置时间、住院时间均显著缩短,总引流量和补救镇痛患者占比均显著降低(P<0.05)。与对照组比较,观察组患者术后24 h、术后72 h时的平均动脉压(MAP)和心率(HR)及视觉模拟量表(VAS)评分均显著降低,简易智力状态检查(MMSE)量表评分均显著升高(P<0.05),且观察组患者术后72 h时的心肌肌钙蛋白I、脂质过氧化物酶、C反应蛋白水平均显著降低(P<0.05)。观察组术后不良反应发生率为8.20%,显著低于对照组的19.67%(P<0.05)。结论瑞马唑仑联合TPVB用于肺癌胸腔镜手术,可改善患者的镇痛效果及血流动力学稳定性,减轻认知功能障碍及术后心肌损伤、脂代谢紊乱、炎性反应的程度。

Associative memory, one of the major cognitive functions in the hippocampal CA3 region, includes auto-associative memory and hetero-associative memory. Many previous studies have shown that Alzheimer＇s disease （AD） can lead to loss of functional synapses in the central nervous system, and associative memory functions in patients with AD are often impaired, but few studies have addressed the effect of AD on hetero-associative memory in the hippocampal CA3 region. In this study, based on a simplified anatomical structure and synaptic connections in the hippocampal CA3 region, a three-layered Hopfield-like neural network model of hippocampal CA3 was proposed and then used to simulate associative memory functions in three circumstances： normal, synaptic deletion and synaptic compensation, according to Ruppin＇s synaptic deletion and compensation theory. The influences of AD on hetero-associative memory were further analyzed. The simulated results showed that the established three-layered Hopfield-like neural network model of hippocampal CA3 has both auto-associative and hetero-associative memory functions. With increasing synaptic deletion level, both associative memory functions were gradually impaired and the mean firing rates of the neurons within the network model were decreased. With gradual increasing synaptic compensation, the associative memory functions of the network were improved and the mean firing rates were increased. The simulated results suggest that the Hopfield-like neural network model can effectively simulate both associative memory functions of the hippocampal CA3 region. Synaptic deletion affects both auto-associative and hetero-associative memory functions in the hippocampal CA3 region, and can also result in memory dysfunction. To some extent, synaptic compensation measures can offset two kinds of associative memory dysfunction caused by synaptic deletion in the hippocampal CA3 area.

Isothermal hot compression tests of as-cast high-Cr ultra-super-critical（USC） rotor steel with columnar grains perpendicular to the compression direction were carried out in the temperature range from 950 to 1250°C at strain rates ranging from 0.001 to 1 s^（-1）. The softening mechanism was dynamic recovery（DRV） at 950°C and the strain rate of 1 s^（-1）, whereas it was dynamic recrystallization（DRX） under the other conditions. A modified constitutive equation based on the Arrhenius model with strain compensation reasonably predicted the flow stress under various deformation conditions, and the activation energy was calculated to be 643.92 kJ ×mol^（-1）. The critical stresses of dynamic recrystallization under different conditions were determined from the work-hardening rate（θ）–flow stress（σ） and-θ/σ–σ curves. The optimum processing parameters via analysis of the processing map and the softening mechanism were determined to be a deformation temperature range from 1100 to 1200°C and a strain-rate range from 0.001 to 0.08 s^（-1）, with a power dissipation efficiency η greater than 31%.

AIM:To evaluate the effect of lens surgery on health-related quality of life(HRQoL)of preschool children with congenital ectopia lentis(CEL).METHODS:A prospective self-controlled study was conducted in Zhongshan Ophthalmic *** aged from 5 to 7y whom were diagnosed with CEL and underwent phacoemulsification with scleral-fixated posterior chamber intraocular lens implantation and their parents were enrolled in this *** of them completed the child and proxy(parental)PedsQL™4.0 before and after the *** preoperative scores were compared to their postoperative *** analyses were performed based on gender and preoperative bilateral presenting visual acuity of the ***:Thirty-two children with CEL successfully underwent surgery without any complications,among whom 8 had monocular surgery and 24 had binocular *** and postoperative questionnaires were completed by 32 child-parent *** intervention could significantly improve the vision of affected children(P<0.001).The medians of physical,psychosocial and total health scores self-reported by the children were 68.75(62.50,81.25),65.00(60.00,80.00)and 67.39(60.87,78.26)preoperatively and were 93.75(87.50,100.00),90.00(83.33,96.67)and 89.13(85.32,95.65)*** preoperative scores of the affected children were significantly lower in all scales than age-matched healthy children(P<0.001).All the postoperative scores were significantly higher than the preoperative scores in affected children and their parents(P<0.001).In the physical functioning evaluation,the preoperative score reported by parents of girls was higher than parents of boys(P=0.041),and the postoperative score of girls was higher than that of boys(P=0.036).CONCLUSION:CEL is associated with significantly worse quality of life in preschool *** intervention can significantly improve the HRQoL in affected children from both personal and family perspective.

Owing to its high theoretical capacity and low cost,Sn has attracted significant attention in sodium-ion ***,the slow kinetics of electrochemical reactions and the rapid decay of capacity resulting from drastic changes in the volume of Sn,as well as persistent side reactions between Sn and the organic electrolyte during the(de)sodium process,have limited its *** improve the electrochemical performance of Sn-based materials,the bottom-up method is normally used to prepare carbon-coated ***,its complex preparation processes and harsh conditions make it unsuitable for practical ***,a carbon-coated hybrid crystal composite(Sn/SnO_(x)@C)was prepared using an up-bottom method with commercial Sn/SnO *** effects accelerate the electrochemical kinetics and inhibit the coarsening of Sn *** Sn/SnO_(x)@C composite electrode exhibited capacity retention of 80.7%even after approximately 1000 cycles(360.4 mAh·g^(−1)) at a current density of 1 A·g^(−1).The high-load Na_(3)V_(2)(PO4)3||Sn/SnO_(x)@C full cell presents a capacity retention rate of 91.7%after 150 cycles at the current density of 0.5 A·g^(−1).This work may significantly accelerate the commercialization of the Sn/SnO_(x)@C composite in sodium-ion batteries with high energy density.

目的建立一种高效的大果山楂果渣果胶提取工艺,并对果胶的初步特性及降糖活性进行研究。方法通过响应面分析法对大果山楂果渣果胶提取工艺进行优化,考察因素包括酶用量、液料比和提取时间等。进一步对其半乳糖醛酸含量、酯化度指标进行分析,并借助α-葡萄糖苷酶抑制实验及胰岛素抵抗细胞实验,探讨大果山楂果渣果胶潜在的降糖活性。结果优化果胶提取工艺参数为:纤维素酶浓度3.4%,液料比7:1(mL:g),提取时间3.5 h,此条件下果胶得率最高,达到13.22%。除蛋白果胶的半乳糖醛酸含量为67.83%,酯化度为45.24%。体外活性实验显示果胶对α-葡萄糖苷酶抑制率达到89.41%,同时可以显著促进地塞米松(3.75μmol/L)和胰岛素(0.01μmol/L)联合诱导的IR-HepG2模型葡萄糖消耗。结论在所述提取工艺条件下,大果山楂的果胶得率最优,所提取的果胶为低酯果胶,表现出良好的降糖功效。
