Histopathological changes in the oesophageal mucosa in Egyptian children with corrosive strictures: A single-centre vast experience

BACKGROUND The caustic ingestion continues to be a major problem worldwide especially in developing countries. The long-term complications include stricture and increased life time risk of oesophageal carcinoma. Patients suffered from corrosive induced oesophageal strictures have more than a 1000-fold risk of developing carcinoma of the *** To determine the possibility of oesophageal mucosal dysplasia after prolonged dilatation in post corrosive *** This observational study was conducted at the Paediatric Endoscopy Unit in Cairo University Children's Hospital. It included children of both sexes older than 2 years of age who had an established diagnosis of post-corrosive oesophageal stricture and repeated endoscopic dilatation sessions for more than 6 mo. All patients were biopsied at the stricture site after 6 mo of endoscopic dilatation. A histopathological examination of an oesophageal mucosal biopsy was performed for the detection of chronic oesophagitis, inflammatory cellular infiltration and *** The mean age of the enrolled children was 5.9 ± 2.6 years; 90% of the patients had ingested an alkaline corrosive substance(potash). The total number of endoscopic dilatation sessions were ranging from 16 to 100 with mean number of sessionswas 37.2 ± 14.9. Histopathological examination of the specimens showed that 85%of patients had evidence of chronic oesophagitis(group A) in the form of basal cell hyperplasia, hyperkeratosis and subepithelial fibrosis. Serologic, endoscopic and pathologic findings in pediatric celiac disease: A single center experience in a low/middle income country

A comparative study of liquid and solid inner contact roxatidine acetate ion-selective electrode membranes

A comparative study was conducted using two designs of a roxatidine acetate （ROX）-selective electrode; a conventional liquid inner contact called electrode A and a graphite-coated solid contact called electrode 13. The fabrication of electrodes was based on roxatidine-tetraphenylborate （ROX-TPB） as an ion-association complex in a PVC matrix using different plasticizers. electrode A has a linear dynamic range of 2.2 ×10^-5 mol/L to 1.0 ×10^-2 mol/L, with a Nernstian slope of 54.7 mV/decade and a detection limit of 1.4 ×10^-6 mol/L. electrode B shows linearity over the concentration range of 1.0×10^-6 mol/L to 1.0×10^-2 tool/L, with a Nernstian slope of 51.2 mV/decade and a limit of detection of 1.1×10^7 mol/L which is remarkably improved as a result of diminishing ion fluxes in this solid contact, ion-selective electrode. The proposed sensors display useful analytical characteristics for the determination of ROX in bulk powder and its pharmaceutical formulation. The present electrodes show clear discrimination of ROX from several inorganic, organic ions, sugars, some common drug excipients and the degradation product （3-[3-（1-piperidinyl methyl） phenoxy] propyl amine） of ROX. Furthermore, the proposed electrodes were utilized for the determination of ROX in human plasma, where electrode B covers drug Cmax which indicated its applicability to pharmacokinetic, bioavailability and bioequivalent studies. The results obtained by the proposed electrodes were statistically analyzed and compared with those obtained by a reported HPLC method. No significant difference for either accuracy or precision was observed.

目的:通过节细胞内网状层(GCIPL)评估中度和重度青光眼的损伤程度并比较其与视盘周围视网膜神经纤维层(PRNFL)的诊断效能,包括敏感性与特异性及ROC曲线下面积(AUC)。方法:前瞻性研究。共210眼(包括中度青光眼患者30例54眼,重度青光眼患者34例59眼和正常人50例97眼)纳入本研究。所有参与者均接受全面眼科检查,包括视野检查、3D-OCT视盘检查和3D-OCT黄斑部垂直扫描。记录并比较所有参与者的GCIPL和PRNFL的AUC、敏感性与特异性。结果:在中度和重度青光眼组中,上部、下部及整体GCIPL和PRNFL厚度明显变薄(P<0.001)。在中度青光眼组中,GCIPL的敏感度与特异性高于PRNFL(仅上半部分敏感性相同)。重度青光眼组,上部、下部及整体GCIPL的敏感度均低于PRNFL。整体GCIPL的特异性低于PRNFL。上部GCIPL的特异性高于PRNFL。下部GCIPL的特异性与PRNFL相同。结论:对于区分中度和重度青光眼,黄斑GCIPL参数的功能远高于PRNFL。两者联合在病情分析中效果最优,能够提供更准确的损伤程度评估。

Genetic Polymorphisms of HBS1L-MYB (rs4895441 and rs9376090) in Egyptian Patients with Hemoglobinopathy

Objective: Study the HBS1L-MYB (rs4895441 and rs9376090) genetic polymorphisms in Egyptian patients with β-thalassemia major and sickle cell disease and its relation to Hb F and severity of the disease. Background: Hb F is a predominant modulator for the severity of β-thalassemia major & sickle cell disease. Genetic polymorphism in the intergenic region (HBS1L-MYB) between GTP-binding elongation factor HBS1L and myeloblastosis oncogene MYB on chromosome 6q is associated with high fetal hemoglobin levels. Subjects and Methods: 150 subjects were included in this study. For all studied groups: Complete blood picture and serum ferritin were evaluated. For patients, hemoglobin variants were separated by High-performance liquid chromatography. Genotyping of HBS1L-MYB (rs4895441 & rs9376090) was evaluated by real-time polymerase chain reaction technique using TaqMan probe. Results: AG, CT genotypes, and G, C alleles of HBS1L-MYB (rs4895441 & rs9376090) were significantly high in sickle cell patients [OR (3.400);95% C.I (1.482 - 7.799)], (p = 0.003) & [OR (4.522);95% C.I (1.854 - 11.029)], (p = 0.001) respectively. AIM:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to ely compared to those ely. The prevalence of HBV infection was significantly higher among children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P 0.05. C ON C LU S I ON :T he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.
