Hemodynamic Profiling Using a Cardiac Index–Systemic Vascular Resistance Plot in Patients with Fontan Circulation

Background: Elevated Fontan pressure (FP) alone cannot fully predict clinical outcomes. We hypothesized thathemodynamic profiling using a cardiac index (CI)-systemic vascular resistance (SVR) plot could characterize clinicalfeatures and predict the prognosis of post-Fontan patients. Methods: We included post-Fontan patients whounderwent cardiac catheterization at age < 10 years. Patients were classified into four categories: A, CI ≥ 3, SVRindex (SVRI) ≥ 20;B, CI < 3, SVRI ≥ 20;C, CI ≥ 3, SVRI < 20;and D, CI < 3, SVRI < 20. The primary outcome wasfreedom from the combined endpoint: new onset of protein-losing enteropathy or plastic bronchitis, heart transplant,and death. Clinical and hemodynamic variables and freedom from the endpoint were compared betweenthe hemodynamic categories and outcome predictors were evaluated. Results: Eighty-three patients wereincluded. Median follow-up was 5.3 years. Category A/B/C/D consisted of 4/15/53/11 patients, respectively. Allthe patients in category A were New York Heart Association I/II and had a significantly lower pulmonary vascularresistance index (PVRI). Patients in category C had lower pulmonary/systemic blood flow. Patients in category Dhad a higher PVRI and had the poorest freedom from the endpoint (44% at 5 years). Elevated FP and category Dwere outcome predictors. Conclusions: CI-SVR plots was a novel adjunctive method for Fontan hemodynamicprofiling.

BACKGROUND Relapsing polychondritis(RP)is a rare inflammatory disease involving the systemic cartilage,such as the auricle,trachea,and bronchiole,among others.A patient with RP shows variable symptoms based on the involved cartilage.CASE SUMMARY A 72-year-old Japanese woman with a history of redness of the bilateral auricles for 3 d was referred to a clinician.The clinician prescribed antibiotics to the patient;however,the symptoms worsened;thus,she was referred to our hospital.Head and neck magnetic resonance imaging(MRI)showed edematous auricle with remarkable contrast,fluid collection in the bilateral mastoid cells,suggesting otitis media.The eustachian tube(ET)on the right side was also edematous with contrast enhancement.The patient was suspected of RP according to the diagnostic criteria.A biopsy of the auricular cartilage was performed by an otorhinolaryngologist,confirming pathological proof of RP.Treatments with steroids were immediately administered thereafter.CONCLUSION We highlight a rare case of RP with radiologically confirmed involvement of ET in the MRI.

AIM To evaluate the prognostic significance of perioperative carcinoembryonic antigen(CEA) and carbohydrate antigen 19-9(CA19-9) levels in stage Ⅱ/Ⅲ gastric cancer.METHODS From a multi-institutional retrospective database compiled by integrating clinical data from nine institutions, data of 998 patients who underwent curative resection for stage Ⅱ/Ⅲ gastric cancer between 2010 and 2014 were retrieved and analyzed. The prognostic impact of the preoperative and postoperative levels and chronological changes in CEA, CA19-9 and their combination were evaluated. To test whether postoperative adjuvant chemotherapy alters the prognostic impact of perioperative CEA and CA19-9 levels, the hazard ratios for mortality were compared between patients who underwent surgery alone and patients who underwent surgery followed by adjuvant chemotherapy.RESULTS The prognostic impact of postoperative CEA and CA19-9 was superior to that of the preoperative levels. Multivariable analysis identified high postoperative CEA and CA19-9 levels as independent prognostic factors for overall survival.Disease-free survival rates clearly decreased in a stepwise manner in association with postoperative CEA and CA19-9 levels, and patients with high levels of both markers showed significantly poorer prognosis than other patient groups. When we analyzed perioperative changes in serum CEA and CA19-9 levels, patients with high levels before and after surgery had the worst disease-free survival rates among all patient groups. Patients with normalized CEA levels after surgery had a significantly lower disease-free survival rate than those with normal perioperative levels, whereas patients with normalized CA19-9 levels after surgery had equivalent survival to those with normal perioperative levels. The prognostic impact of high CEA levels was observably smaller in patients who underwent adjuvant chemotherapy than in patients who underwent surgery alone, whereas that of high CA19-9 was greater in patients who underwent adjuvant chemotherapy. Clinical utility of the platelet-lymphocyte ratio as a predictor of postoperative complications after radical gastrectomy for clinical T2-4 gastric cancer

AIM To identify simple and sensitive markers for postoperative complications after gastrectomy, the predictive values were compared among candidate preoperative factors. METHODS Three-hundred and twelve patients with previously untreated clinical T2-4 gastric cancer who underwent a D2 standard gastrectomy(distal gastrectomy or total gastrectomy) were included in the analysis.Correlations between 21 parameters that can be determined by preoperative routine blood tests and clinically relevant postoperative complications(grade Ⅱ or higher according to the Clavien-Dindo classification) were evaluated. The optimal cutoff values and clinical significance of the selected markers were further evaluated by subgroup analyses according to age, body mass index, operative procedure and clinical disease stage.RESULTS Sixty-six patients(21.1%) experienced grade Ⅱ or higher postoperative complications. The plateletlymphocyte ratio(PLR, total lymphocyte count/platelet count × 100) exhibited the highest area under the curve value(0.639) for predicting postoperative complications among the 21 parameters, and the optimal cutoff value was determined to be 0.71(sensitivity = 70%, specificity = 56%). In the univariate analysis, the odds ratio of a low PLR for the occurrence of postoperative complications was 2.94(95%CI: 1.66-5.35, P < 0.001), and a multivariate binomial logistic analysis involving other potential risk factors identified a low PLR as an independent risk factor for postoperative complications(OR = 3.32, 95%CI: 1.82-6.25, P < 0.001). In subgroups classified according to age, body mass index, operative procedure and clinical disease stage, the low PLR group exhibited an increased incidence of postoperative complications. CONCLUSION The preoperative PLR is a simple and useful predictor of complications after curative gastrectomy in patients with clinical T2-4 gastric cancer.

Comparison of Threshold Power between Methylene Blue Degradation and KI Oxidation Reaction Using Ultrasound

Ultrasound is used in various chemical reaction processes, and these reactions are influenced by ultrasonic frequency. A threshold power is required for the ultrasonic degradation reaction and oxidation reaction caused by hydroxyl radicals, and the cavitation threshold power is also influenced by frequency generally. In this study, the effects of frequency on the threshold power of methylene blue degradation and KI oxidation were investigated in the range between 22.8 kHz and 1640 kHz. The threshold power of KI oxidation reaction increased with increasing frequency. This phenomenon well agrees with previous study, and it is revealed that the generation of I-3?ion is caused by oxidation reaction of Iˉ ions with hydroxyl radicals. On the other hand, the threshold power of methylene blue degradation reaction was not affected by frequency. The ultrasonic degradation of methylene blue is considered to be caused by hydroxyl radicals, and there is a linear relationship between degradation rate constant and sonochemical efficiency value. However, it is guessed that the degradation of methylene blue is occurred inside cavitation bubble by pyrolysis at high frequency regions.

Effect of Particle Addition on Ultrasonic Degradation Reaction Rate

The ultrasonic degradation of methylene blue at a frequency of 490 kHz was carried out in the absence and presence of TiO2 or Al2O3 particle, and the effects of amounts of particle on the enhancement of degradation rate constant estimated by assuming first-order-kinetics were investigated. The degradation reaction was enhanced by particle addition, and the apparent degradation rate constant is proportional to the increase in amount of particle. In addition, the constant of proportionality is not influenced by degraded material and ultrasonic frequency. However, particle type influences the constant of proportionality, and the value of TiO2 particle is about 6 times as large as that of Al2O3 particle.

Amino acid profiles in relation to chronic periodontitis and rheumatoid arthritis

Chronic periodontitis (CP) and rheumatoid arthritis (RA) are chronic inflammatory conditions, and share many pathologic features. Plasma amino acid profiles have been shown to be associated with RA, but their relevance to CP remains unclear. The aim of the present study is to evaluate amino acid profiles in relation to CP and RA. The study participants consisted of 62 patients with RA (RA group), 30 patients with CP (CP group) and 29 healthy controls (H group) in age-, gender-, smoking status-balanced condition. Clinical periodontal and rheumatologic parameter values and plasma levels of 21 amino acids, C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) were determined. Multiple comparison analyses revealed that the RA group exhibited similar periodontal conditions but significantly higher levels of CRP, IL-6, and TNF-α than the CP group (P < 0.01). A total of four amino acids (glycine, histidine, ornithine, and alpha-aminobutyric acid) were significantly different in the concentrations among the three groups (P < 0.01). The RA group displayed significantly lower levels of glycine and histidine and significantly higher ornithine level than the CP and H groups (P < 0.01). The CP group showed significantly higher alpha-aminobutyric acid level than the RA and H groups (P < 0.01). Of these four amino acids, a significantly positive correlation was found between ornithine level and % of sites with bleeding on probing (P = 0.006) in the RA group. These results suggest a possibility that profiles of four amino acids may play a role in the pathogenesis of CP and RA.

BACKGROUND The ascending pharyngeal artery(APhA)comprises the pharyngeal trunk(PT)and neuromeningeal trunk.The PT feeds the nasopharynx and adjacent tissue,which potentially connects with the sphenopalatine artery(SPA),branched from the internal maxillary artery(IMA).Due to its location deep inside the body,the PT is rarely injured by trauma.Here,we present two cases that underwent transcatheter arterial embolization(TAE)of the PT of the APhA due to trauma and iatrogenic procedure.CASE SUMMARY Case 1 is a 49-year-old Japanese woman who underwent transoral endoscopy under sedation for a medical check-up.The nasal airway was inserted as glossoptosis occurred during sedation.Bleeding from the nasopharynx was observed during the endoscopic procedure.As the bleeding continued,the patient was referred to our hospital for further treatment.Contrast-enhanced computed tomography(CT)demonstrated extravasation in the nasopharynx originating from the right Rosenmuller fossa.TAE was performed and the extravasation disappeared after embolization.Case 2 is a 28-year-old Japanese woman who fell from the sixth floor of a building and was transported to our hospital.Contrastenhanced CT demonstrated a complex facial fracture accompanying extravasation in the left pterygopalatine fossa to the nasopharynx.Angiography demonstrated an irregular third portion of the IMA.As angiography after TAE of the IMA demonstrated extravasation from the PT of the APhA,additional TAE to the artery was performed.The bleeding stopped after the procedure.CONCLUSION Radiologists should be aware that the PT of the APhA can be a bleeding source,which has a potential connection with the SPA.

Novel diagnostics for aggravating pancreatic fistulas at the acute phase after pancreatectomy

AIM:To identify sensitive predictors of clinically relevant postoperative pancreatic fistula(POPF)at the acute phase after pancreatectomy.METHODS:This study included 153 patients diagnosed as having POPFs at postoperative day(POD)3after either open pancreatoduodenectomy or distal pancreatectomy between January 2008 and March 2013.The POPFs were categorized into three grades based on the International Study Group on Pancreatic Fistula Definition,and POPFs of grades B or C were considered to be clinically relevant.The predictive performance for the clinically relevant POPF formation of values at PODs 1,3 and 5 as well as time-dependent changes in levels of inflammatory markers,including white blood cell count,neutrophil count,total lymphocyte count,C-reactive protein(CRP),procalcitonin level,plateletto-lymphocyte ratio and neutrophil-to-lymphocyte ratio,and amylase content in the drain fluid were compared using the receiver operating characteristic(ROC)curve and multivariable analyses.A scoring system for the prediction of clinically relevant POPFs was created using five risk factors identified in this study,and its diagnostic performance was also evaluated.RESULTS:Over time,77(50%)of 153 enrolled patients followed a protracted course and were categorized as having clinically relevant POPFs.ROC curve analyses revealed that changes in CRP levels from POD1 to POD 3 had the greatest area under the curve value(0.767)and that an elevated CRP level of 28.4 mg/L yielded the most optimal predictive value for clinically relevant POPFs.Multivariable analyses for the risk factors of clinically relevant POPFs identified invasive carcinomas of the pancreas and elevation of the CRP level(≥28.4 mg/L,from POD 1 to POD 3)as independent diagnostic factors for clinically relevant POPFs(OR 2.94,95%CI:1.08-8.55,P=0.035 and OR 4.82,95%CI:1.25-20.2,P=0.022,respectively).A gradual increase in the prevalence of clinically relevant POPFs in proportion to the risk classification score was confirmed.A highly elevated CRP level and a risk scor
