Disease progression in chronic hepatitis C patients with normal alanine aminotransferase levels

AIM:To investigate whether the disease progression of chronic hepatitis C patients with normal alanine aminotransferase(ALT) levels differs by ALT ***:A total of 232 chronic hepatitis C patients with normal ALT(leeding gastric or esophageal varices,hepatic encephalopathy,the development of hepatocellular carcinoma,or death related to liver disease,were compared between the two ***:Baseline serum ALT levels were associatedwith disease progression for both *** best predictive cutoff baseline serum ALT level for disease progression was 26 IU/L in males and 23 IU/L in *** mean annual disease progression rate was 1.2% and 3.9% for male patients with baseline ALT levels ≤ 25 IU/L(low-normal) and > 26 IU/L(highnormal),respectively(P = 0.043),and it was 1.4% and 4.8% for female patients with baseline ALT levels ≤ 22 IU/L(low-normal) and > 23 IU/L(high-normal),respectively(P = 0.023).ALT levels fluctuated during the follow-up *** the follow-up,more patients with "high-normal" ALT levels at baseline experienced ALT elevation(> 41 IU/L) than did patients with "lownormal" ALT levels at baseline(47.7% vs 27.9%,P = 0.002).The 5 year cumulative incidence of disease progression was significantly lower in patients with persistently "low-normal" ALT levels than "high-normal" ALT levels or those who exhibited an ALT elevation > 41 U/L during the follow-up period(0%,8.3% and 34.3%,P < 0.001).CONCLUSION:A "high normal" ALT level in chronic hepatitis C patients was associated with disease progression,suggesting that the currently accepted normal threshold of serum ALT should be lowered.

The present work suggested the use of waste oil palm frond as an alternative precursor for nitrogendoped carbon quantum dots(NCQDs)and proposed a straightforward in-situ hydrothermal method for the preparation of NCQDs/TiO_(2)*** elemental composition,morphological,structural and optical characteristics of NCQDs/TiO_(2)nanocomposites have been comprehensively *** successful grafting of NCQDs on TiO_(2)matrix was confirmed by the formation of Ti AOAC bond and the electronic coupling between theπ-states of NCQDs and the conduction band of TiO_(2).For the first time,the oil palm frond-derived NCQDs/TiO_(2)was adopted in the photodegradation of methylene blue(MB)under visible-light *** a result,the photocatalytic efficiency of NCQDs/TiO_(2)nanocomposites(86.16%)was 2.85 times higher than its counterpart TiO_(2)(30.18%).The enhanced performance of nanocomposites was attributed to the pivotal roles of NCQDs serving as electron mediator and visiblelight ***,the optimal NCQDs loading was determined at 4 ml while the removal efficiency of NCQDs/TiO_(2)-4 was the highest at a catalyst dosage of 1 g.L^(-1)under alkaline *** research work is important as it proposed a new insight to the preparation of biomass-based NCQDs/TiO_(2)using a facile synthetic method,which offers a green and sustainable water remediation technology.

AIM:To investigate the current seroprevalence of hepatitis A virus(HAV) antibodies in patients with chronic viral liver disease in *** also tried to identify the factors affecting the prevalence of HAV antibodies. METHODS:We performed an analysis of the clinical records of 986 patients(mean age:49±9 years,714 males/272 females) with chronic hepatitis B virus(HBV) or hepatitis C virus(HCV) infection who had undergone HAV antibody testing between January 2008 and December ***:The overall prevalence of IgG anti-HAV was 86.61%(854/986) in patients with chronic liver disease and was 88.13%(869/986) in age-and gendermatched patients from the Center for Health *** anti-HAV prevalence was 80.04%(405/506) in patients with chronic hepatitis B,86.96%(20/23) in patients with chronic hepatitis C,93.78%(422/450) in patients with HBV related liver cirrhosis,and 100%(7/7) in patients with HCV related liver *** anti-HAV prevalence according to the decade of age was as follows:20s(6.67%) ,30s(50.86%) ,40s(92.29%) ,50s(97.77%) ,and 60s(100%) .The antiHAV prevalence was significantly higher in patients older than 40 years compared with that in patients younger than 40 years of *** analysis showed that age≥40 years,female gender and metropolitan cities as the place of residence were independent risk factors for IgG anti-HAV seropositivity. CONCLUSION:Most Korean patients with chronic liver disease and who are above 40 years of age have already been exposed to hepatitis A virus.

学习任何一种语言,最重要就是掌握一定量的词汇。“零”起点的印尼学生在初级阶段汉语词汇学习中,常常在词义理解和词汇使用方面出现出人意料的错误。本文以笔者在汉语补习机构与一对一私教的教学实践为例,从初级阶段汉语词汇教学方面探讨了教授印尼语为背景的汉语初学者的一些问题及其对策。本文对印尼学生在汉语词汇学习过程中出现的词汇偏误语料进行了分类,并运用对比的方法分析了形成偏误的原因,进而研究如何针对初级阶段印尼学生的词汇教学。本文共分为四章:第一章是引言。主要说明本文的选题缘由、研究目的和研究意义。介绍初级阶段词汇教学中偏误分析与研究的重要性及研究现状等。第二章是在前人研究中介语理论的基础上,对初级阶段印尼学生汉语词汇偏误进行了分析。发现从意义偏误上讲,印尼汉语初学者经常出现动词与名词之间的语义搭配偏误、名词指称偏误、形容词与修饰对象的搭配偏误等;从语法上,偏误主要表现在判断动词“是”的遗漏、“的”字结构误用、否定词误用、方位短语偏误、能愿动词混用和语序错误等。第三章是通过对比分析与研究偏误语料总结了学生偏误原因,并将偏误主要原因归纳为母语负迁移。其表现在语义上和语法上的偏误为最多。当然造成偏误的原因还有其他的可能性,如教师解释不足、关注有盲点等。第四章本文将从初级阶段学习者学习汉语词汇的角度出发提出了相应的教学建议。阐述针对印尼初级阶段学生开展汉语词汇教学的方法及其对策。如采用《基础识字挂图》由单音节扩展双音节词汇;通过语素归类教学,扩充词汇量;通过偏旁教学,促进词语认识;通过情景法教学和游戏法等教学方法掌握词语并扩大词汇量等等。希望就本文的研究成果与笔者提出的教学建议能对提高印尼初级阶段汉语词汇教学有所裨益,同时为汉语国际推广进尽绵薄之力。因为学习者的母语是印尼语,为了论述方便,文中将涉及一些汉-印语的对比。

The supply of highly concentrated oxygen positively affects cognitive processing in normal young adults. However, there have been few reports on changes in cognitive ability in elderly subjects following highly concentrated oxygen administration. This study investigated changes in cognitive ability, blood oxygen saturation （%）, and heart rate （beats/min） in normal elderly subjects at three different levels of oxygen [21% （1 L/min）, 93% （1 L/min）, and 93% （5 L/min）] administered during a 1-back task. Eight elderly male （75.3 ＋ 4.3 years old） and 10 female （71.1 ＋ 3.9 years old） subjects, who were normal in cognitive ability as shown by a score of more than 24 points in the Mini-Mental State Examination-Korea, participated in the experiment. The experiment consisted of an adaptation phase after the start of oxygen administration （3 minutes）, a control phase to obtain stable baseline measurements of heart rate and blood oxygen saturation before the task （2 minutes） and a task phase during which the 1-back task was performed （2 minutes）. Three levels of oxygen were administered throughout the three phases （7 minutes）. Blood oxygen saturation and heart rate were measured during each phase. Our results show that blood oxygen saturation increased, heart rate decreased, and response time in the 1-back task decreased as the concentration and amount of administered oxygen increased. This shows that administration of sufficient oxygen for optimal cognitive functioning increases blood oxygen saturation and decreases heart rate.

BACKGROUND： An increasing number of studies have shown the effects of aging in basic cognitive processing and higher cognitive functions using functional magnetic resonance imaging （fMRI）. However, little is known about the aging effects in diverse cognitive abilities, such as spatial learning and reasoning. OBJECTIVE： To investigate the effect of aging on spatial cognitive performance and regional brain activation based on fMRI. DESIGN, TIME, AND SETTING： A block design for fMRI observation. This study was performed at the fMRI Laboratory, Brain Science Research Center, Korea Advanced Institute of Science and Technology from March 2006 to May 2009. PARTICIPANTS： Eight right-handed, male, college students in their 20s （mean age 21.5 years） and six right-handed, male, adults in their 40s （mean age 45.7 years）, who graduated from college, participated in the study. All subjects were healthy and had no prior history of psychiatric or neurological disorders. METHODS： A spatial task was presented while brain images were acquired using a 3T fMRI system （ISOL Technology, Korea）. The spatial tasks involved selecting a shape that corresponded to a given figure using four examples, as well as selecting a development figure of a diagram. MAIN OUTCOME MEASURES： The accuracy rate （number of correct answers/total number of items x 100%） of spatial tasks was calculated. Using the subtraction procedure, the activated areas in the brain during spatial tasks were color-coded by T-score. The double subtraction method was used to analyze the effect of aging between the two age groups （20s versus 40s）. RESULTS： The cerebellum, occipital lobe, parietal lobe, and frontal lobe were similarly activated in the two age groups. Increased brain activations, however, were observed in bilateral parietal and superior frontal lobes of the younger group. Lipidomics coverage improvement is essential for functional lipid and pathway construction.A powerful approach to discovering organism lipidome is to combine various data acquisitions,such as full scan mass spectrometry(full MS),data-dependent acquisition(Dda),and data-independent acquisition(DIA).Caenorhabditis elegans(***)is a useful model for discovering toxic-induced metabolism,highthroughput drug screening,and a variety of human disease *** determine the lipidome of *** and investigate lipid disruption from the molecular level to the system biology level,we used integrative data *** methyl-tert-butyl ether method was used to extract L4 stage *** after exposure to triclosan(TCS),perfluorooctanoic acid,and nanopolystyrene(nPS).Full MS,Dda,and DIA integrations were performed to comprehensively profile the *** lipidome by Q-Exactive Plus *** annotated lipids were then analyzed using lipid ontology and pathway *** annotated up to 940 lipids from 20 lipid classes involved in various functions and *** biological investigations revealed that when *** were exposed to nPS,lipid droplets were disrupted,whereas plasma membrane-functionalized lipids were likely to be changed in the TCS treatment *** nPS treatment caused a significant disruption in lipid ***,glycerophospholipid,and ether class lipids were those primarily hindered by ***,toxicant exposure frequently involved numerous lipid-related pathways,including the phosphoinositide 3-kinase/protein kinase B *** conclusion,an integrative data acquisition strategy was used to characterize the *** lipidome,providing valuable biological insights into hypothesis generation and validation.

有效而准确的预测商品混凝土价格变动趋势,对各类建筑的施工规划具有重要意义。相比其他预测模型,随机森林模型具有更高的预测精度。然而不同的数据结构都有其独特之处,针对特定数据结构进行模型优化,有助于提高算法在特定数据上的处理性能。我们针对时间序列分类(TSC:Time Series Classification)的特征提出一种改进随机森林算法。首先将随机森林创建训练子集时的随机抽样调整为倾斜抽样,然后将决策树分裂时的随机特征向量抽样调整为分层抽样,最后以加权投票取代平均投票。实证结果表明相比原始随机森林算法,改进模型具有明显优势,对商品混凝土价格变动的预测准确率达98.4%,预测精度、召回率和F1评分分别为:98.7%,98.2%,98.4%,可以实现了商品混凝土价格变动趋势的精准预测。

AIM: To evaluate whether the Milan criteria are useful in patients with hepatocellular carcinoma (HCC) who received transarterial chemoembolization (TACE) before liver transplantation (LT). METHODS: Thirty-six HCC patients who fulfilled the Milan criteria after having received TACE and subsequently underwent LT were included (TACE + LT group) in the study. As controls, 21 patients who also met the Milan criteria and underwent LT without prior treatment were selected (LT group). Post-LT clinical outcomes, such as HCC recurrence, survival rate, and histologic features of explanted livers, were compared between the two groups. RESULTS: Baseline characteristics were not different between the two groups. Pre-LT maximal tumor diameter in TACE + LT group was similar to that of LT group (2.0 ± 0.6 cm vs 2.3 ± 0.9 cm; P = 0.10). Post-LT histologic findings also revealed similar maximal tumor diameter in the two groups (2.4 ± 1.4 cm vs 2.3 ± 0.9 cm; P = 0.70). Explanted livers showed similar incidence of unfavorable pathologic features. The morality within 60 d after transplantation was not different between the two groups (8.3% vs 9.5%; P = 0.99). Post-LT 5-year survival rate (57% vs 74%; P = 0.70) and cumulative recurrence rate (8.3% vs 4.8%; P = 0.90) were not significantly different between the two groups. CONCLUSION: The Milan criteria are still a useful selec- tion criteria showing favorable outcomes in HCC patientsreceiving TACE before LT.
