Malaria is a major health problem facing many tropical countries, including Ghana. The use of modern mechanisms to study environmental factors in malaria occurrence is imperative in malaria control efforts. The aim of this research is to investigate how malaria occurrence is influenced by environmental factors and how Geographical Information Systems (GIS) can be used as a mechanism for improving malaria control in healthcare delivery in Ghana. Environmental factors, elevation, daily temperature and daily rainfall were plotted against prevalence of M and S forms of mosquitoes using ArcGIS 10.1. Prevalence of Anopheles gambiae M was the highest at elevations of 0 m to 200 m and that of S was the highest at elevations between 200 m and 500 m. Prevalence of M was the highest at temperatures between 26.1°C - 27.6°C whilst that of S was the highest at temperatures of between 24.5°C and 26.0°C. Prevalence of M was the highest at rainfall between 10 mm and 35 mm and S was the highest at rainfall between 26 mm and 36 mm and beyond. M form was positively correlated with temperature and negatively with elevation and humidity. The S form was negatively associated with temperature and positively with elevation and rainfall. GIS models can combine with environmental data to estimate vector prevalence under environmental conditions and therefore its use is recommended in Ghana.

Pathology of an internal organ causes significant rectification of electrical currents （diode phenomenon） in related skin areas - organ projection areas （OPAs）, once the resistance ‘breakthrough effect＇ has been induced in the skin. Also, the impedance of the diseased organ＇s projection areas is increased. The aim of this double-blind study was to confirm these phenomena statistically using a broad spectrum of measuring parameters. Skin impedance vs. measurement frequency, skin impedance vs. applied voltage, and skin resistance vs. voltage were evaluated at 335 auricular OPAs related to the healthy internal organs and 203 auricular OPAs related to the diseased organs. These measurements confirmed that the skin electrical characteristics of specific locations are dependent on the health state of the corresponding internal organs. It was found that the impedance of OPAs corresponding to the healthy internal organs equals 185 ＋ 291 k,Q at 10Hz and decreases to 64 ± 49 kΩ at 10kHz; and that the impedance of OPAs related to the diseased organs equals 7.5 ± 2.2 MΩ at 10Hz and 99 ± 50kΩ at 10kHz. The disparity of the impedances between the healthy and diseased organs related OPAs is greater at higher potentials. The skin resistance, when measured with a negatively polarized point electrode, undergoes a rapid resistance decrease of approximately two orders of magnitude, if the applied current is sufficient. After this reversible ‘breakthrough effect＇ is obtained the skin exhibits rectification. The degree of rectification is low for OPAs related to the healthy organs. For OPAs related to the diseased organs, the resistance measured with a positive polarisation of measuring electrode could be five times greater than the resistance measured at the same voltage with the same but negatively polarised electrode. AIM: To define the cost-effectiveness of strategies, including endoscopy and immunosuppression, to prevent endoscopic recurrence of Crohn’s disease following intestinal ***: In the “POCER” study patients undergoing intestinal resection were treated with post-operative drug therapy. Two thirds were randomized to active care (6 mo colonoscopy and drug intensification for endoscopic recurrence) and one third to drug therapy without early endoscopy. Colonoscopy at 18 mo and faecal calprotectin (FC) measurement were used to assess disease recurrence. Administrative data, chart review and patient questionnaires were collected prospectively over 18 ***: Sixty patients (active care n = 43, standard care n = 17) were included from one health service. Median total health care cost was $6440 per patient. Active care cost $4824 more than standard care over 18 mo. Medication accounted for 78% of total cost, of which 90% was for adalimumab. Median health care cost was higher for those with endoscopic recurrence compared to those in remission [$26347 (IQR 25045-27485) vs $2729 (IQR 1182-5215), P < 0.001]. FC to select patients for colonoscopy could reduce cost by $1010 per patient over 18 mo. Active care was associated with 18% decreased endoscopic recurrence, costing $861 for each recurrence ***: Post-operative management strategies are associated with high cost, primarily medication related. Calprotectin use reduces costs. The long term cost-benefit of these strategies remains to be evaluated.

Objective:To evaluate the effect of Salacia lehmbachii (S. lehmbachii) ethanol root bark extract on estrous cycle and sex hormones in female rats. Methods: Forty-eight virgin rats with regular 4-day cycle were grouped into four and each group was further subdivided into 'a' and 'b' (n=6). Each group was orally treated for 28 days with 2 mL of distilled water (control), ethanol root bark extract ofS. lehmbachii in doses of 250, 500 and 750 mg/kg/body weight (groups 2, 3, 4, respectively). Estrous cycle was determined daily using the vaginal smear method. Rats in'a' subgroups were weighed and sacrificed on the 29th day, and blood was collected for serum generation which was used for hormonal assay and sex organs were removed and weighed. Rats in 'b' subgroups were discontinued from treatment for 2 weeks and the parameters above were ***:The mean length of estrous cycle and duration of diestrous of treated rats were prolonged dose dependently compared to control. The increase was significant (P<0.05) at 500 and 750 mg/kg. The other estrous phases were shortened in the same pattern. Relative weights of sex organs were reduced significantly (P<0.05) at the highest dose. Sex hormones levels were significantly (P<0.05) reduced compared to control. The above changes reverted towards the control values two weeks post ***: Ethanol root bark extract ofS. lehmbachii (high doses) has antifertility effect in female rats as it prolongs the estrous and diestrous cycle, and reduces serum sex hormones levels. The observed alterations were reversible.

Hepatic steatosis affects 20% to 30% of the general adult population in the western world. Currently, the technique of choice for determining hepatic fat deposition and the stage of fibrosis is liver biopsy. However, it is an invasive procedure and its use is limited, particularly in children. It may also be subject to sampling error. Non-invasive techniques such as ultrasound, computerised tomography (CT), magnetic resonance imaging (MRI) and proton magnetic resonance spectroscopy (1H MRS) can detect hepatic steatosis, but currently cannot distinguish between simple steatosis and steatohepatitis, or stage the degree of fibrosis accurately. Ultrasound is widely used to detect hepatic steatosis, but its sensitivity is reduced in the morbidly obese and also in those with small amounts of fatty infiltration. It has been used to grade hepatic fat content, but this is subjective. CT can detect hepatic steatosis, but exposes subjects to ionising radiation, thus limiting its use in longitudinal studies and in children. Recently, magnetic resonance (MR) techniques using chemical shift imaging have provided a quantitative assessment of the degree of hepatic fatty infiltration, which correlates well with liver biopsy results in the same patients. Similarly, in vivo 1H MRS is a fast, safe, non-invasive method forthe quantification of intrahepatocellular lipid (IHCL) levels. Both techniques will be useful tools in future longitudinal clinical studies, either in examining the natural history of conditions causing hepatic steatosis (e.g. non-alcoholic fatty liver disease), or in testing new treatments for these conditions.

河漫滩上沉积物的沉积伴随着与沉积物相结合的营养和污染物质的沉积。已有研究表明,河漫滩是与沉积物相结合的磷的沉积地。采用137Cs技术,并结合河漫滩沉积物中全磷(TP)含量的剖面信息,可以调查近年河漫滩沉积物中的TP含量,计算TP储量。本文根据英国Devon郡Culm河8个河漫滩采样环沉积物中的数据,重建了在过去40～50年内TP储量和含量的变化。结果表明,在Culm河河漫滩沉积物中,在1963～2000年间TP平均含量为0.60～1.96gkg-1,该值自河流上游向下游和自过去到现在有逐渐增加的趋势;在相同的河漫滩沉积物中,在相同的时期内TP储量为18.62～435.48gm-2,即0.49～11.46gm-2a-1,最大值出现在河流的中游。

Interrupted Time Series Analysis of Military and Civilian Regimes in Nigeria: A Statistical Evidence from Gross Domestic Product (GDP)

Governments influence the economy by changing the level and types of taxes, the extent and composition of spending, and the degree and form of borrowing. Governments directly and indirectly influence the way resources are used in the economy. Higher taxes, fees, and greater regulations can stymie businesses or entire industries and the resulting impact is reflected on the country’s economy status (strong or weak). The growth rate of GDP is often used as an indicator of the general health of the economy. In broad terms, an increase in real GDP is interpreted as a sign that the economy is doing well. So it is important to study and pay more attention to country’s GDP growth rate. In this paper, an intervention analysis approach was applied to Nigeria GDP data in order to evaluate the performances of military and civilian rules in the country. Data on Nigeria GDP were collected and subjected to interrupted (intervention) time series model. Based on the Alkaike Information Criterion (AIC), Bayesian Information Criterion (BIC) and sigma2 values, the interrupted time series model ARIMA (1, 1, 0) with exogenous variables (per capita per capita GDP, intervention, year and yearAfter) was identified as the best model amongst other competing models. It was observed that the intervention (civilian rule) was significant at the 10% level of significance in increasing the Nigeria GDP by 10B US$ on the average since 2005 till 2021 while controlling for the effects of other determinants. Also, the ARIMA (1, 1, 0) forecasts indicate that the Nigeria GDP will continue increasing during the civilian rule. As a result, changing from military rule to civilian rule in Nigeria significantly increased the GDP of the country.

The treatment of advanced pancreatic cancer has not moved much beyond single agent gemcitabine until recently when protocols such as FOLFIRINOX(fluorouracil,leucovorin,irinotecan and oxaliplatin)and nab-paclitaxelgemcitabine have demonstrated some improved *** in technology especially in massively parallel genome sequencing has progressed our understanding of the biology of pancreatic cancer especially the candidate signalling pathways that are involved in tumourogenesis and disease *** has allowed identification of potentially actionable mutations that may be targeted by new biological *** heterogeneity of pancreatic cancer makes tumour tissue collection important with the aim of being able to personalise therapies for the individual as opposed to a one size fits all approach to treatment of the *** paper reviews the developments in this area of translational research and the ongoing clinical studies that will attempt to move this into the everyday oncology practice.
