Role of tumor necrosis factor-alpha in the pathogenesis of atrial fibrillation

Background Tumor necrosis factor-alpha （TNF-a） is a pleiotropic proinflammatory cytokine and contributes to many kinds of cardiovascular diseases via its receptors （TNFR1/TNFR2）. We hypothesize that TNF-a plays a role in the pathogenesis of chronic atrial fibrillation （AF）. Methods Sixty-seven consecutive patients who were scheduled to have cardiac surgery were enrolled into the study. Thirty-one patients with rheumatic heart disease （RHD） and AF were enrolled as study group （AF group）. The sinus rhythm （SR） control groups consisted of 20 patients with RHD and 16 patients with coronary artery disease （CAD）. Peripheral blood sample was collected before the operation. About 5 mm3 left atrial tissue was disserted during the operation and was separated into three parts for Western blotting, real time polymerase chain reaction （RT-PCR） and immunohistochemistry （IHC） analysis. Results Compared with the controls （RHD SR and CAD SR）, the levels of TNF-α （（14.40±5.45） pg/ml vs. （4.20±3.19） pg/ml vs. （2.68±2.20） pg/ml, P=0.000） and its soluble receptor 1 （sTNFR1）（（1623.9±558.6） pg/ml vs. （1222.3±175.6） pg/ml vs. （1387.5±362.2） pg/ml, P=0.001） in plasma were higher in patients with AF. TNF-a level had positive correlation with the left atrial diameter （LAD）（r=0.642, P=0.000）. Western blotting analysis showed that the protein levels of TNF-α （0.618±0.236 vs. 0.234±0.178 vs. 0.180±0.103, P=0.000） were higher in patients with AF. The RT-PCR analysis results demonstrated that the mRNA expression of TNF-a （0.103±0.047 vs. 0.031±0.027 vs. 0.023±0.018, P=0.000） increased in patients with AF. IHC analysis displayed that, comparing to the SR, the expression of TNF-α （0.125±0.025 vs. 0.080±0.027 vs. 0.070±0.023, P=0.000） increased in the AF group. The protein level and mRNA expression of TNF-α also had positive correlation with left atrium diameter （LAD）（r=0.415, P=0.000 and r=0.499, P=0.000）. Objective To evaluate the predictive value of red cell distribution width （RDW） on left atrial thrombus （LAT） or left atrial spontane- ous echo contrast （LASEC） in patients with non-valvular atrial fibrillation （AF）. Methods We reviewed 692 patients who were diagnosed as non-valvular AF and underwent transesophageal echocardiography （TEE） in Guangdong Cardiovascular Institute from April 2014 to December 2015. The baseline clinical characteristics, laboratory test of blood routine, electrocardiograph measurements were analyzed. Results Eighty-four patients were examined with LAT/LASEC under TEE. The mean RDW level was significantly higher in LAT/LASEC patients compared with the non-LAT/LASEC patients （13.59% ± 1.07% ws. 14.34% ± 1.34%; P 〈 0.001）. Receiver-operating characteristic curve analysis was performed and indicated the best RDW cut point was 13.16%. Furthermore, multivariate logistic regression analysis indicated that RDW level 〉 13.16% could be an independent risk factor for LAT/LASEC in patients with AF. Conclusion Elevated RDW level is associated with the presence of LAT/LASEC and could be with moderate predictive value for LAT/LASEC in patients with non-valvular AF.

Predictive value of serum uric acid on left atrial spontaneous echo contrast in non-valvular atrial fibrillation patients

Objectives To investigate the relationship between serum uric acid （SUA） and left atrial spontaneous echo contrast （LA-SEC） in non-valvular atrial fibrillation （AF） patients. Methods We retrospectively screened 1,476 consecutive hospitalized patients with AF who underwent transesophageal echocardiography prior to radiofrequency catheter ablation, left atrial appendage closure and electric cardiover- sion at Guangdong General Hospital. Data on the clinical baseline characteristics of all patients were collected from electronic medical re- cords and analyzed. Results After exclusion of patients with left atrial thrombus, 1,354 patients entered into present study and 57 were LA-SEC. The mean female SUA level （380.88 ± 94.35 μmol/L vs. 323.37 ± 72.19μmol/L, P 〈 0.001） and male SUA level （416.97 ± 98.87 μmol/L vs. 367.88 ＋ 68.50 μmol/L, P = 0.008） were both significantly higher in patients with LA-SEC than in the controls. The mean left atrial dimension （41.32 ± 5.12 mm vs. 36.12 ± 5.66 mm, P 〈 0.001） was markedly larger in patients with LA-SEC. In multivariate regression analysis, SUA level was an independent risk factor for LA-SEC （OR： 1.008, P 〈 0.001）. In receiver operating characteristic curve analysis, the corresponding area under the curve for SUA predicting LA-SEC in female and male were 0.670 and 0.657, respectively. SUA level is significantly higher in non-valvular AF patients with LA-SEC. Conclusion SUA level is an independent risk factor and has a moderate predictive value for LA-SEC among non-valvular AF patients in Southern China.

Complex atrial tachycardia with alternating cycle length:What is the mechanism?

Background Atrial tachycardia(AT)with cycle length(CL)alternans is uncommon and conventional mapping of this AT remains challenging. We used an ultrahigh density mapping system to rapidly map complicated circuits with sufficient spatial resolution and electrogram quality to elucidate the precise mechanism of this special ATs. Methods Of 210 consecutive patients with clinical ATs who underwent catheter ablation with the ultrahigh density mapping system,4 patients(1.9%)with CL alternans were identified. The AT alternating cycles mapped by the Rhythmia mapping system for long CL were 317±51(range 245-355)ms and for short CL were 282±51(range 235-333)ms. Both long and short cycles followed in 1∶1 sequence in all 4 patients(longshort-long-short). Results By comparing the separate maps with long and short CL,we classified ATs with CL alternans into 2 types. In type 1,CL alternans resulted from an intermittent 2∶1 conduction block through the slow conduction channel in the small circuit. In type 2,CL alternans caused by the alternated conduction velocity passing through the conduction gap were manifested. Ablation at the fractionated potentials contributes to the termination of AT in 3 of the 4 patients. Conclusions Ultrahigh density mapping system can accurately identify the mechanism of complex ATs with alternating CL. The CL alternans may be related to the intermittent conduction block within the channel of the small circuitor different conduction velocity through the identical channel. Fractionated electrogram recorded in the common isthmus or some"conduction gaps"may be a reasonable approach to terminate these ATs.

Influenced factors of radiofrequency catheter ablation for ventricular arrhythmias:A single-center experience

Background There is little information on the factors that have influenced the long-term success rate of radiofrequency ablation of ventricular arrhythmias(VAs)at different *** objective of this study was to investigate the characteristics of the ablation of VAs and the factors influencing the long-term success rate in our *** Totally,2751 consecutive patients(mean ages 44±15 years,male/female 58.6%/41.4%)with VAs underwent radiofrequency ablation in Guangdong Provincial People’s Hospital from 2013 to 2018 were *** data were retrieved from the medical record ***-term follow-up data were obtained by telephone *** mainly recorded the patient’s last post-ablation holter,and the symptoms after *** regressions were applied to assess the relationship between the long-term success rate and the influence *** Through the baseline data of 2751 patients,we found that the acute success rate of ablation was 95.1%.The average follow-up time was 16±5 months(ranging from 1 to 80).In all,304 patients(11%)lost follow-up,while 2447 patients(89%)were successfully *** long-term success rate of ablation was 82.56%.By means of multivariable regression analyses,it was discovered that cardiovascular diseases,such as coronary heart disease(OR:0.568,95%CI:0.327-0.987),hypertension(OR:1.554,95%CI:1.100-2.198),were independently associated with the long-term success rate,as well as ablation approaches(OR:0.212,95%CI:0.082-0.550).Lower ablation success rates of the patients whose VAs were originated from some uncommon region,like left ventricular papillary muscle,Summit,*** *** Ablation approaches,concomitant cardiovascular diseases,and ablation targets were related to the long-term clinical outcome of VAs ablation.

We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,NICER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by NICER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.

目的探讨阵发性心房颤动患者肿瘤坏死因子（TNF）-α、白细胞介素（IL）-6和可溶性肿瘤坏死因子受体1 （sTNFR1）的变化及其可能来源.方法连续采集30例行射频消融术治疗的阵发性心房颤动患者（阵发性心房颤动组）股静脉、高位右房、冠状窦及左房血液样本,酶联免疫吸附法检测血清TNF-α、sTNFR1和IL-6水平,与相同部位采血的20例阵发性室上性心动过速患者（阵发性室上性心动过速组,左侧旁路）进行比较.结果阵发性心房颤动组股静脉、高位右房、冠状窦及左房血清TNF-α和IL-6水平较阵发性室上性心动过速组增高[TNF-α：（4.45±1.76） ng/L比（0.59±0.36） ng/L、（6.67±1.43） ng/L比（0.51±0.30） ng/L、（8.35±2.03） ng/L比（0.85±0.50） ng/L、（9.97±2.70） ng/L比（0.28±0.29） ng/L;IL-6：（2.02±0.87） ng/L比（1.04±0.63） ng/L、（1.51±0.68） ng/L比（0.74±0.26） ng/L、（2.00±0.51）ng/L比（0.88±0.35） ng/L、（1.32±0.47） ng/L比（0.48±0.28） ng/L],差异有统计学意义（P=0.000）.阵发性心房颤动组高敏C反应蛋白（hs-CRP）水平高于阵发性室上性心动过速组[（2.41±1.35） mg/L比（1.10±0.53） mg/L,P=0.002].阵发性心房颤动组左房TNF-α水平较其他三个部位增高,差异有统计学意义（P=0.000）；IL-6水平在股静脉和冠状窦增高,与高位右房和左房比较差异有统计学意义（P＜ 0.05）; sTNFR1水平在股静脉、高位右房、冠状窦比较差异无统计学意义（P＞0.05）,但均高于左房,差异有统计学意义（P＜0.05）.各部位血清TNF-α、IL-6水平及肘静脉hs-CRP水平与左房内径（LAD）存在相关性（P＜ 0.01或＜0.05）；左房sTNFR1水平与LAD呈正相关,而右房sTNFR1水平与LAD呈负相关（P＜0.01）.结论阵发性心房颤动患者血清hs-CRP、TNF-α及IL-6水平增高,心房颤动患者TNF-α、IL-6水平增高可能来源于心肌分泌,并与左房增大有关.

目的评价风湿性心脏病瓣膜置换术后心房颤动(房颤)消融的效果和安全性。方法入选2008年至2013年在广东省人民医院心内科接受房颤导管消融治疗的风湿性心脏病瓣膜置换术患者,分析其临床特征、消融策略及消融成功率。结果共纳入23例患者,男8例,女15例,平均年龄(51.0±9.2)岁。单纯二尖瓣置换术患者13例(56.5%),二尖瓣、主动脉瓣双瓣置换术10例(43.5%),其中5例同时进行三尖瓣置换或整形术。外科术后阵发性房颤患者14例(60.9%),非阵发性房颤9例(39.1%);这些患者在外科术前心律情况为9例窦性心律,4例阵发性房颤,10例非阵发性房颤。导管消融距离外科手术时间为(6.9±5.8)年,外科术后发生房颤病程(3.1±3.2)年,左、右心房内径分别为(44.1±5.9)mm、(48.1±9.0)mm,左心室射血分数64.0%±8.3%。平均消融手术时间(156.8±46.6)min,X线曝光时间(27.3±11.2)min。随访(29.7±21.2)个月,其中4例(17.4%)患者接受再次消融术;14例(60.9%)维持窦性心律(6例服用胺碘酮),1例死亡,2例失访,6例复发(包括2例持续性房颤,1例阵发性房颤,2例偶发性心房扑动,1例阵发性房性心动过速)。结论风湿性心脏病瓣膜置换术后房颤导管消融有效、安全,步进式导管消融策略可能较为合适。

目的评价导管消融治疗迷宫术后复发房性心动过速(房速)的中、远期疗效,探讨房速复发的相关因素。方法纳入2010年4月至2015年6月在广东省人民医院接受导管消融治疗的迷宫术后复发房速患者59例。应用三维标测系统进行高密度电解剖标测,结合拖带标测指导导管消融。术后定期随访,采用多因素Cox比例风险回归模型分析导管消融治疗迷宫术后房速复发的危险因素。结果共记录到88种房速,其中1例患者房速为短阵性发作,与窦性心律交替出现,难以进行电解剖标测及拖带标测。平均每例患者有(1.49±0.75)种房速,以折返性房速多见,占76.1%(67/88),局灶性房速占22.7%(20/88)。59例患者中,56例(94.9%)即时消融成功。术后平均随访(30.8±17.7)个月,首次导管消融治疗后3例患者即时消融失败,12例复发,术后复发时间为3.5(1.3,12.0)个月,消融成功率为74.6%(44/59)。6例患者接受再次导管消融治疗,二次导管消融治疗后总体成功率为79.7%(47/59)。多因素分析提示左心房内径为预测复发的独立危险因素(HR1.108,95%CI 1.002~1.226,P=0.045)。结论迷宫术后复发房速患者行导管消融治疗安全、有效,左心房内径是房速消融术后复发的独立危险因素。

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical Design Report(TDR)follows the Preliminary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.
