研究背景艾滋病毒的流行是一个影响全人类的全球性问题,在2020年全球有150万HIV新发感染者。男男性行为人群是HIV感染的高危人群之一,2020年,在撒哈拉以南的非洲以外的地区,该人群占HIV新发感染的45%。HIV检测是艾滋病预防与诊疗关怀服务链中的关键一环。尽管男男性行为人群HIV发病率很高,但无论是在高收入国家还是中低收入国家,该人群接受HIV检测的比例仍然较低。我国的几项研究显示,只有大约一半的男男性行为人群接受过HIV检测。随着技术的发展,互联网已被广泛用于HIV的预防和控制,特别是在高危人群中(如男男性行为人群)。互联网允许用户随时随地生成内容并获取某些污名化疾病(如艾滋病)的相关信息和服务。然而,网络上的错误信息也会对人们的观点和行为产生负面影响。现有研究大多侧重于探索互联网使用与HIV检测之间的关系,但没有将HIV检测进一步区分为机构检测和自我检测。随着互联网平台逐渐融入我国居民的日常生活,有必要探索并比较互联网使用对不同HIV检测策略的影响。研究目的本研究在山东省的MSM中开展了一项横断面调查。山东省是中国人口数量第二大省,本研究旨在了解山东省MSM的HIV检测情况,探究HIV检测的影响因素,以及评价网络的使用对HIV检测的影响,能够为制定促进HIV检测和控制艾滋病疫情的策略提供科学依据。研究方法于2019年8月6日至2020年1月16日在中国山东省的10个城市进行了一项横断面研究。由每个城市的社区组织协助,在Blued上发起线上调查,参与者根据五个入选标准选出。使用描述性分析方法对研究对象的人口学特征、性行为信息、网络使用和HIV检测情况进行描述,并且描述研究对象使用网络搜索HIV检测相关的信息类型、进行搜索的平台。单因素分析探索人口学特征、性行为信息、网络使用与HIV检测、HIV机构检测、HIV自我检测之间的关系,并且在不同的年龄亚组、文化水平亚组之间,分析互联网使用对HIV检测、HIV机构检测和HIV自我检测的影响。使用Sp.S软件中的p.OCESS宏程序进行中介分析,评估HIV检测自我效能在网络使用和HIV检测的关系中的中介作用。数据分析使用Sp.S软件26.0版,α为0.05。研究结果1、本研究共纳入935名研究对象。64.3%的研究对象年龄在30岁以下,67.3%拥有大学及以上学历,72.8%未婚。47.2%的研究对象月收入在501至800美元之间;71.4%承认自己是同性恋。约三分之二的研究对象向他人披露自己的性取向,三分之一向医生披露自己的性取向。约三分之一的研究对象报告在过去一年发生无保护性行为。89.2%的研究对象曾经在网络中搜索HIV检测相关的信息,84.0%曾经进行过HIV检测,12.7%在医疗机构进行过HIV检测,55.6%进行过自我检测。2、研究对象主要利用互联网搜索HIV检测相关的知识(68.3%),其次是搜索提供检测服务的医疗机构的地址(32.9%)。研究对象搜索HIV检测相关信息时最常用的平台是搜索引擎,如搜狗和百度(72.1%),其次是社交媒体,如微信、微博、QQ等(22.1%),不足20%的研究对象利用专门的网站、疾病预防控制中心的官网或者其他手机软件进行相关搜索。3、结果显示,互联网使用与HIV检测相关(aOR=3.430,95%CI:[2.094,5.619],p.0.001),与 HIV 机构检测无关(aOR=1.686,95%CI:[0.783,3.627],p.0.182),与 HIV自我检测相关(aOR=2.002,95%CI:[1.283,3.126],p.0.002)。HIV检测自我效能与 HIV检测有关(aOR=1.188,95%CI:[1.109,1.272],p.0.001),与 HIV 机构检测无关(aOR=1.066,95%CI:[0.992,1.146],p.0.080),与 HIV 自我检测有关(aOR=1.112,95%CI:[1.059,1.169],p.0.001)。4、在互联网使用和HIV检测的关系中,HIV检测自我效能发挥着中介作用,95%CI为[0.01,0.29]。在互联网使用和HIV机构检测的关系中,HIV检测自我效能发挥着中介作用,95%CI为[0.003,0.16]。在互联网使用和HIV自我检测的关系中,HIV检测自我效能也发挥着中介作用,95%CI为[0.01,0.16]。5、亚组分析结果显示,在年龄小于30岁的研究对象中,互联网使用对HIV检测(aOR=2.087,95%CI:[1.475,5.340],p.0.002)和自我检测有影响(aOR=2.167,95%CI:[1.207,3.889],p.0.010);在年龄为30岁及以上的研究对象中,互联网使用对HIV检测(aOR=6.004,95%CI:[2.373,

aortic aneurysm rep.ir and thoracic endovascular aortic rep.ir results in devastating clinical deficits in p.tients. Because sp.nal cord ischemia is inadequately treated, we studied the effects of [4-((1E)-2-(5-(4-hydroxy-3-methoxystyryl-)-1-p.enyl-1H-p.razoyl-3-yl) vinyl)-2-methoxy-p.enol)](CNB-001), a novel curcumin-based comp.und, in a rabbit SCI model. CNB-001 is known to inhibit human 5-lip.xygenase and 15-lip.xygenase and reduce the ischemia-induced inflammatory resp.nse. Moreover, CNB- 001 can reduce the level of oxidative stress markers and p.tentiate brain-derived neurotrop.ic factor and brain-derived neurotrop.ic factor recep.or signaling. The Tarlov scale and quantal analysis technique results revealed that CNB-001 administered as an intravenous dose (bolus) 30 minutes p.ior to sp.nal cord ischemia imp.oved the behaviors of female New Zealand White rabbits. The imp.ovements were similar to those p.oduced by the uncomp.titive N-methyl-D-asp.rtate recep.or antagonist memantine. At 48 hours after aortic occlusion, there was a 42.7% increase (p.p.sitive control memantine (p.p.red to vehicle-treated ischemic rabbits (n = 22). CNB-001 is a p.tential imp.rtant novel treatment for sp.nal cord ischemia induced by aortic occlusion. All exp.riments were ap.roved by the CSMC Institutional Animal Care and Use Committee (IACUC #4311) on November 1, 2012.
