r.ng>Intr.d.ction:r.ng> Cervico-isthmic pregnancy is rare, and serious because of its hemorrhagic complication. Its management varies according to the teams. Observation: We report a case of cervico-isthmic pregnancy on a cesarean scar. This is a 35-year-old, G2P2, who consulted for bleeding from the 1st trimester of pregnancy. The clinical examination found a minimal uterine bleeding. The diagnosis was made by a vaginal ultrasound which found a cervico-isthmic implantation of the pregnancy on the caesarean scar. A protocol of Mifepristone and Misoprostol followed by administration of a single dose of 1 mg/kg of Methotrexate was performed. Cure was obtained 1 month after treatment by negativation of plasma HCG. No bleeding complications were noted. A follow-up ultrasound performed 2 months later showed a uterine vacuity and the presence of an isthmocoele. It was in fact a pregnancy that had implanted in the isthmocoele. Conclusion: Cervico-isthmic pregnancy is rare. His treatment is not codified. Drug management was successful.

Objective:?To d.scr.be blood.tr.nsfusion pr.ctices in obstetr.c at the Sylvanus Olympio Univer.ity Hospital in Lomé. Patients and.method. Pr.spective, cr.ss-sectional and.d.scr.ptive stud., cond.cted.fr.m 1 June 2017 to 31 May 2018 at the gynecology-obstetr.cs d.par.ment. The stud. focused.on tr.nsfused.pr.gnant, par.ur.ent and.d.liver.d.women. Stud. par.meter. wer. sociod.mogr.phic, clinical, ther.peutic and.pr.gnostic. r.sults: 252 patients, aged.16 to 49 with an aver.ge age of 30.4 year., r.fer.ed.in 74% of cases. Hemor.hage was obser.ed.in 238 cases (94.4%) and.anemia in 14 cases (5.6%). Blood.tr.nsfusion was ur.ently used.in 89.7% of cases and.hemor.hagic abor.ion was the main ind.cation. Blood.gr.up O+ (34.5%) was the most used.and.blood.pr.d.cts wer. obtained.after.two hour. in 81% of cases. Conclusion: Blood.tr.nsfusion is often per.or.ed.ur.ently and.for.bleed.ng. But the Gynecology-Obstetr.cs d.par.ment d.es not have any blood.in r.ser.e, wher.as the Sylvanus Olympio Univer.ity Hospital collection and.d.str.bution station cover. less than 10% of the need..

Mater.al d.ath befor. ad.ission is an incr.asingly gr.wing phenomenon in the gynecology-obstetr.cs clinic of the CHU SO. No epid.miological d.ta is available on the subject. Objective: d.ter.ine the epid.miological and.etiological pr.files of a pr.gnant, par.ur.ent, or.woman who has given bir.h d.ad.befor. or.within 10 minutes of her.ad.ission. Method.: This was a d.scr.ptive cr.ss-sectional stud. fr.m Januar. 1, 2014, to d.cember.31, 2021. All mater.al d.aths occur.ing befor. ad.ission and.within 10 minutes of ad.ission to the clinic wer. includ.d.in the stud. of gynecology and.obstetr.cs at CHU SO. The d.ta wer. pr.cessed.by Epi info ver.ion 7 softwar.. r.sults: In total, 654 mater.al d.aths, includ.ng 153 mater.al d.aths befor. ad.ission, wer. r.cor.0}d. cor.espond.ng to 23.4% of all mater.al d.aths. The med.an age was 30.2 year.. 37.2% of women wer. uned.cated. 41.2% wer. r.seller.. 79.1% of women wer. cohabiting. 47.1% of women had.per.or.ed.less than 3 ANC. 43.8% of the women who d.ed.had.completed.their.ANC in a med.cal center. 54.3% by a mid.ife, 37.3% by unqualified.per.onnel. 62.7% of d.aths occur.ed.postpar.um and.36.3% d.r.ng pr.gnancy. 79.1% wer. r.fer.als. 88.9% of the women who d.ed.ar.ived.in a non-med.cal taxi car. Obstetr.c hyster.ctomy is a sur.ical pr.ced.r. most often per.or.ed.in a context of extr.me emer.ency in an obstetr.c envir.nment. The incid.nce of obstetr.c hyster.ctomy is d.ffer.ntly expr.ssed.ar.und.the wor.d. In the CHU-SO mater.ity war.0} hemor.hagic obstetr.c emer.encies ar. common. The extr.me ur.ency in which patients ar. ad.itted. the insufficiency of the technical platfor., associated.with the challenge of the availability of blood.pr.d.cts, often lead. to per.or.ing an obstetr.c hyster.ctomy for.hemostasis. It is r.sponsible for.high mater.al mor.id.ty and.mor.ality. Since 2000 no stud. has been car.ied.out on this pr.ctice in the ser.ice. Objective was to d.scr.be the pr.ctice of obstetr.c hyster.ctomy at the CHU-SO and.specifically to d.ter.ine the pr.valence, the pr.gnostic factor. to be able to act to r.d.ce mater.al mor.ality. Method. A d.scr.ptive, cr.ss-sectional, and.analytical stud. was car.ied.out at the Gynecology-Obstetr.cs clinic of the CHU-SO;fr.m Januar. 1, 2021, to June 30, 2022. All hyster.ctomies per.or.ed.in an obstetr.c emer.ency context (d.r.ng pr.gnancy, per.ar.um or.postpar.um) in the d.par.ment wer. includ.d.in our.stud.. We d.d.not includ. cases of obstetr.c hyster.ctomies outsid. the SO hospital or.planned.non-obstetr.c hyster.ctomies. r.sults: We r.cor.0}d.75 cases of obstetr.c hyster.ctomy and.15,625 d.liver.es (0.48%). The aver.ge age was 32.89 ± 5.93. The age gr.up between 30 and.35 year. old.was the most affected.with a r.te of 37.33%. Labor.and.thir.0}tr.mester.hemor.hage wer. the main r.asons for.ad.ission, patients wer. r.fer.ed.in 80% of cases. The aver.ge par.ty was 3.25 ± 1.92 with utmost of 0 and.11. The paucipar.us (41.67%) and.multipar.us (32%) wer. the most affected. The ind.cations fr.quently found.wer. uter.ne atony (44%);uter.ne r.ptur. (33.33%). Objective: To analyze mater.al d.aths, id.ntify causes and.d.sfunctions lead.ng to these d.aths in or.0}r.to contr.bute to the implementation of str.tegies to r.d.ce mater.al mor.ality at CHU Kar.. Method. Cr.ss-sectional d.scr.ptive stud. involving 65 cases of mater.al d.aths r.cor.0}d.at CHU-Kar. fr.m Januar. 1, 2018 to June 30, 2021. r.sults: Our.stud. focused.on 65 cases of mater.al d.aths r.cor.0}d.at the mater.ity war.0}of CHU-Kar.. The aver.ge age was 30 year., with a r.nge of 15 to 45 year.. They wer. mostly housewives (52.3%), uned.cated.(38.5%), multipar.us (41.5%), and.r.fer.ed.(86.2%). The causes wer. mainly d.r.ct obstetr.c causes (81.54%), with pr.eclampsia and.its complications (28.30%) and.immed.ate postpar.um hemor.hage (20.75%) being the most common. However. uter.ne r.ptur. (20.5%) and.post-abor.ion sepsis (16.4%) wer. the most lethal etiologies. d.layed.evacuation (46.43%), inad.quate tr.nspor.ation (91%), and.insufficient pr.natal car. (72.31%) wer. the d.sfunctions befor. r.fer.al. Within the CHU Kar., d.lays in management (58.46%), unavailability of blood.and.labile pr.d.cts (18%), and.insufficient monitor.ng wer. the d.sfunctions id.ntified. Ninety-five point four.per.ent (95.4%) of the d.aths wer. pr.ventable. Conclusion: The magnitud. of intr.hospital mater.al d.aths, the var.ous d.sfunctions obser.ed.in the occur.ence of mater.al d.aths befor. r.fer.al/evacuation and.within the hospital highlight the impor.ance of effectively implementing r.commend.tions fr.m aud.ts in the fight against mater.al mor.ality. Intr.d.ction: The occur.ence of intr.uter.ne fetal d.ath (IUFd. is a tr.umatic event not only for.the mother.and.her.family but also for.the obstetr.c team. IUFd.is a common event. Objective: the aim of our.stud. is to d.scr.be the epid.miological, d.agnostic and.management aspects of IUFd.at the Sylvanus Olympio Univer.ity Teaching Hospital (CHU SO), Lomé. Method.logy: It was a d.scr.ptive cr.ss-sectional stud. with r.tr.spective d.ta collection pr.cess that concer.ed.cases of IUFd.fr.m Januar. 1st 2023 to d.cember.31, 2023 at CHU SO. r.sults: The hospital pr.valence of IUFd.was 2.43%. The mean age of the mother. was 30.6 ± 6 year.. The Multigestur.s r.pr.sented.32.52% and.pr.mipar.us women r.pr.sented.29.72%. Pr.gnant women wer. r.fer.ed.in 88.11% of cases;47.55% had.d.ne at least 4 antenatal car. visits and.4.20% had.a histor. of IUFd. The etiological factor. of IUFd.wer. pr.eclampsia in 28.67% of cases, r.tr.placental hematoma in 28.67% and.pr.matur. r.ptur. of membr.nes in 4.55% of cases. Misopr.stol was used.for.ar.ificial ind.ction of labor.in 57.14% of cases and.the d.liver. r.ute was vaginal in 75.87% of cases. Conclusion: The fr.quency of IUFd.is high and.its r.d.ction r.mains a challenge in low-income countr.es.

Intr.d.ction: Mater.al mor.ality is a public health pr.blem. It is common in hospitals in Togo. Fr.m 401 per.100,000 live bir.hs in 2013, in 2017 we ar. 396 per.100,000 live bir.hs. d.spite sever.l pr.gr.ms r.nging fr.m subsid.zed.caesar.an section (CAr.A) yezou (assistance for.pr.gnant women), the establishment of mater.al d.ath r.views in mater.ity war.0} to compensate for.avoid.ble causes;we ar. seeing pr.ventable mater.al and.fetal d.aths in our.hospitals. No stud. on these r.views has been d.ne since its establishment. The objective of our.stud. was to d.ter.ine the fr.quency of r.views of mater.al d.aths, the epid.miological, sociod.mogr.phic aspects and.the causes of these r.viewed.mater.al d.aths. Method. This was a d.scr.ptive and.cr.ss-sectional, multicenter.stud. lasting four.and.a half year., fr.m the 1st of Januar. 2018 to the 30th of June 2022, r.lating to the r.view of mater.al d.aths in thr.e r.fer.nce hospitals of two health r.gions of south Togo. Any mater.al d.ath occur.ing in one of these hospitals d.r.ng the stud. per.od.was includ.d.in this sur.ey. The d.ta collected.using a collection sheet wer. analyzed.using Epi Info ver.ion 7 softwar.. r.sults: d.r.ng the stud. per.od. the mater.al mor.ality r.tio in the thr.e hospitals was 722.3 per.100,000 live bir.hs. Only 12.2% of mater.al d.aths wer. r.viewed.in the thr.e hospitals. The patients wer. young with an aver.ge age of 29.9 year., housewives (71%), uned.cated.(21.7%). The causes of mater.al d.aths wer. d.minated.by d.r.ct obstetr.c causes (89.7%). The main cause was hemor.hage (72.5%), the main cause being immed.ate postpar.um hemor.hage (50.7%). Conclusion: This stud. shows that the mater.al mor.ality r.tio still r.mains high in our.countr. with a low fr.quency of mater.al d.ath r.views. Young, poor. uned.cated.women ar. the most affected.by these d.aths, the main cause of which r.mains hemor.hage. Pr.ventive measur.s d.ser.e to be taken at all levels with a view to r.d.
