Outcomes following minimally invasive dorsal cheilectomy for hallux rigidus:A systematic review

BACKGROUND Cheilectomy of the 1^(st)metatarsophalangeal joint(MTPj)is one of the most common procedures for the management of hallux rigidus.However,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDC)for the management of hallux rigidus.AIM To evaluate outcomes following MIDC for the management of hallux rigidus.METHODS During November 2023,the PubMed,EMBASE and Cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDC for the management of hallux rigidus.RESULTS Six studies were included.In total,348 patients(370 feet)underwent MIDC for hallux rigidus at a weighted mean follow-up of 37.9±16.5 months.The distribution of patients by Coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 patients,27.1%),Ⅱ(112 patients,52.3%),Ⅲ(44 patients,20.6%).Three studies performed an additional 1^(st)MTPj arthroscopy and debridement following MIDC.Retained intra-articular bone debris was observed in 100%of patients in 1 study.The weighted mean American orthopedic foot and ankle society score improved from a preoperative score of 68.9±3.2 to a postoperative score of 87.1.The complication rate was 8.4%,the most common of which was persistent joint pain and stiffness.Thirty-two failures(8.7%)were observed.Thirty-three secondary procedures(8.9%)were performed at a weighted mean time of 8.6±3.2 months following the index procedure.CONCLUSION This systematic review demonstrated improvements in subjective clinical outcomes together with a moderate complication rate following MIDC for the management of hallux rigidus at short-term follow-up.A moderate reoperation rate at short-term follow-up was recorded.The marked heterogeneity between included studies and paucity of high quality comparative studies limits the generation of any robust conclusions.

Rationale of a Cross Sectional Analytic Study on Determinants of Recurrent Preeclampsia at University Clinics of Kinshasa (Democratic Republic of Congo) and at Victor Dupouy Hospital Center (France)

Research Background: Pre-eclampsia is one of main causes of materno-foetal mortality and morbidity worldwide, with a prevalence of 3% - 7%. Although considered a primiparous condition, it can nevertheless recur. Several factors appear to be associated with risk of recurrence of pre-eclampsia, such as the term of delivery of previous pregnancy, severity of disease, the existence of co-morbidities and the inter-genital space. Purpose: The aim of our study will be to analyse and identify in a population of pregnant women with a history of preeclampsia risk factors associated with occurrence of recurrent preeclampsia at University clinics of Kinshasa (Democratic Republic of Congo) and at Victor Dupouy Hospital Center (France). Methods: In this study, pregnant women with an history of preeclampsia who will give birth between November 2018 and October 2024 at University Clinics of Kinshasa (UCK) and Victor Dupouy Hospital Center (VDHC) will be included. This will be a cross-sectional analytical study, data from previous and subsequent pregnancies will be studied. Expected Result: The prevalence of recurrent preeclampsia in the study population will be determined. And we will highlight the factors that will determine the recurrence of preeclampsia by analysing the risk factors. Conclusion: Knowledge of the factors associated with recurrent preeclampsia could be an excellent tool for predicting and preventing the disease.

Hybridization despite elaborate courtship behavior and female choice in Neotropical tree frogs

The mechanisms of hybridization can be elucidated by analyzing genotypes as well as phenotypes that could act as premating barriers,as the reproductive interactions among heterospeciﬁcs can alter the evolutionary history of species.In frogs,hybrids typically occur among species that reproduce explosively(in dense aggregations)with few opportunities for mate selection but are rare in species with elaborate courtship behaviors that may prevent erroneous mating.Using 21 microsatellite markers,we examined hybridization in the prolonged-breeding tree frogs Bokermannohyla ibitiguara and B.sazimai sampled within a contact zone in the Brazilian savanna(72 tadpoles;74 adults).We also compared acoustic and morphological data.We conﬁrmed both parental species genetically;STRUCTURE results conﬁrmed 14 hybrids,11 of which were second-generation according to NEWHYBRIDS,all with intermediate values of genetic dissimilarities compared to the parentals.Morphological and acoustic analyses revealed that hybrids showed variable but not necessarily intermediate phenotypes.Moreover,2 hybrids exhibited call types different from parentals.The reproduction of B.ibitiguara involves territorial and aggressive males,elaborate courtships with acoustic and tactile stimuli,choosy females,and opportunistic strategies.Our study uncovers a rare case of viable hybridization among closely related frogs with such a combination of complex courtship behaviors and mate choice.We discuss the likely directionality and mechanisms behind this phenomenon,and highlight the importance of investigating hybridization even in species that show elaborate reproduction and female choice to advance our understanding of animal diversiﬁcation.

Assessing the Effectiveness of Strategies in US Birth Cohort Screening for Hepatitis C Infection

Chronic hepatitis C infection in the USA is a highly morbid condition and current guidelines recommend one-time screening among the birth cohort(1945-1965).Understand-ing strategies to optimize screening can help inform future hepatitis C virus(HCV)screening guidelines.A focused literature search was performed using PubMed and manual abstract review from major hepatology conferences over the past 2 years.The search strategy involved using Medical Subject Headings terms for hepatitis C,screening,birth cohort,baby boomers,and 1945-1965.The review was limited to data from the USA.A total of 327 articles were identified and 36 abstracts were included,with studies published between 2012-2019.Strategies including clinician education,electronic medical record alerts,reflex HCV RNA testing,point-of-care testing,multisite(outpatient,inpatient,emergency department,endoscopy suite)initiatives,direct patient solicitation,and utilization of non-physician providers have increased HCV screening rates.However,broad imple-mentation remains less than optimal.Barriers include lack of patient acceptance to screening and engagement in the HCV care cascade.The Veterans Affairs Healthcare System has achieved higher birth cohort screening rates through an integrated approach requiring high-level engagement by leadership and institutional commitment.Multiple strategies for increasing birth cohort screening have been successful,but overall rates of HCV screening remain low.These strat-egies can inform public health efforts to implement emerging national recommendations for expansion of HCV screening to all U.S.adults age 18 or older.

Visualization of pleural fissures in infants on computed tomography

Purpose For infants with prenatally detected lung lesions,a chest CT is performed prior to surgery.The chest CT is performed as close to the surgery date as possible,because it is presumed that the visualization of lung fissures would be poor in the immediate neonatal setting.However,this presumption has never been formally studied.The purpose of this study is to assess differences in lung fissure visualization on chest CT in different infant age groups.Methods This was a retrospective study of clinically indicated chest CT approved by the institutional review board performed in infants of different ages.The visibility of pleural fissures was subjectively assessed by three pediatric radiologists who were blinded to age group.results In the 0-2 months age group,80%of all fissure segments were visible versus 92%in the 5-6 months group(p=0.04)and 95%in the 7-9 months group(p=0.01).Conclusions The ability to visualize pleural fissures on CT increases with infant age.This observation should be taken into consideration when choosing the optimal timing of preoperative CT for asymptomatic congenital lung lesions.

Effects of temperature and hydric environment on survival of the Panamanian Golden Frog infected with a pathogenic chytrid fungus

Considerable controversy exists concerning whether or not climate changes(particularly global warming)are causing outbreaks of a lethal amphibian pathogen,the chytrid fungus Batrachochytrium dendrobatidis(Longcore,Pessier&D.K.Nichols 1999).In the present study,groups of Panamanian golden frogs(Atelopus zeteki Dunn,1993),a critically endangered amphibian thought to be nearly extinct in Panama,were exposed to varying dosages of zoospores of Batrachochytrium dendrobatidis,temperatures and hydric environments in order to learn whether this species is susceptible to this pathogen and,if so,how environmental factors affect survival.This pathogen proved to be highly lethal for A.zeteki.Frogs exposed to a dosage of 100 Bd zoospores survived significantly(P<0.0001)longer than those that had been exposed to 104 or 106 zoospores.Exposed frogs housed at 23℃survived significantly(P<0.0001)longer than those that were housed at 17℃.Exposed frogs held in dry conditions survived significantly longer than those in wet conditions(P<0.0001).As a laboratory study,these results do not directly test hypotheses about the relation between climate change and the decline of these frogs in the field,but they inform the discussion about how environmental conditions can have an impact on the interaction between a susceptible amphibian and this pathogen.These data do not support the contention that rising global temperatures are necessary to cause the death of amphibians infected with this pathogen because the pathogen was equally lethal at 17 as at 23℃,and frogs at the warmer temperature lived significantly longer than those at the cooler one.

Forced mixing to a single-phase or supersaturated solid solution(SSS)and its prerequisite microstructure evolution in immiscible systems has been a focus of research for fundamental science and practical applications.Controlling the formation of SSS by shear deformation could enable a material design beyond conventional equilibrium microstructure in immiscible systems.Here,a highly immiscible Cu-50 at.%Cr binary alloy(mixing enthalpy of∼20 kj mol^(−1))was employed to investigate the microstructure evolution and localized tendencies of SSS during severe shear deformation.Our results demonstrate the dislocation mediated microstructural refinement process in each phase of the binary alloy and the mechanisms associated with localized solute supersaturation as a function of shear strain.Pronounced grain refinement in the softer Cu phase occurs owing to the strain localization driving the preferential dynamic recrystallization.The grain refinement of the Cr phase,however,is enabled by the progressive evolution of grain lamination,splitting,and fragmentation as a function of shear strain.The solute supersaturation is found to be strongly dependent on the local environments that affect the dislocation activity,including the level of microstructure refinement,the interfacial orientation relationship,the mechanical incompatibility,and the localized preferential phase oxidation.Ab initio simulations confirm that it is more favorable to oxidize Cr than Cu at incoherent Cu/Cr interfaces,limiting the mass transport on an incoherent boundary.Our results unveil the mechanism underpinning the non-equilibrium mass transport in immiscible systems upon severe deformation that can be applied to produce immiscible alloys with superior mechanical properties.

Towards a precision psychiatry approach to anxiety disorders with ecological momentary assessment:the example of panic disorder

Background Treatments for anxiety disorders are among the most effective in psychiatry.Yet,there is considerable room for improvement.Aim In this paper,we discuss the value of ecological momentary assessment as a research method and clinical tool.Methods We begin by describing ecological momentary assessment and its advantages,including the ability to collect ecologically valid information about mental disorders,in real time,in individual patients.We then illustrate the value of this approach for anxiety disorder treatment using two patients with panic disorder who completed ecological momentary assessments for 2 weeks before and after a cognitive-behavioural therapy intervention.We focus especially on two key pieces of information provided by ecological momentary assessment data:information about symptom dynamics and information about the relationships among symptoms as they unfold overtime within individual patients.Perspective Although considerable work is needed to further develop this methodology in the context of anxiety disorder treatment,we believe that these pieces of information may ultimately inform our understanding of how anxiety disorder treatments have their effect and how those treatments can be tailored to individual patients.

Association of homocysteine and smoking with cerebral microemboli in patients with mechanical heart valves:a transcranial Doppler study

Objectives Microembolic signals(MES)on transcranial Doppler(TCD)predict stroke and cognitive decline.Plasma levels of total homocysteine(tHcy),a prothrombotic factor,are higher in patients with microemboli in carotid stenosis and in patients with paradoxical embolism.In this study we assessed the association between the level of tHcy and the number of MES in patients with mechanical heart valves(MHVs).Methods TCD monitoring was performed to detect MES before and after breathing 100% oxygen and repeated every 2-4 weeks up to six times.results Twenty-five patients with MHVs(mean age:63.60±10.15 years)participated in this study;15 were men(66.47±7.25 years)and 10 were women(59.30±12.60 years).In total,there were 126 study visits.In multiple regression,higher tHcy was associated with more MES in both preoxygenation(OR 1.34(95%CI 1.07 to 1.68,P=0.009))and postoxygenation(OR 1.40(95% CI 1.07 to 1.83,P=0.01))phases.Current smoking and the length of time between the operation and monitoring also correlated with a higher number of MES before and after breathing oxygen,particularly in women.conclusions Higher tHcy and smoking were associated with a higher MES count in both preoxygenation and postoxygenation phases.Because smoking can be stopped and hyperhomocysteinaemia is treatable,these are clinically important findings.
