Racial, ethnic, and socioeconomic disparities present daunting hurdles that prevent equitable health outcomes for patients with end-stage kidney disease (EsKD) on hemodialysis. Additional resources, such as the Novel Intervention in Children’s Health (NICH) at Lucille Packard Children’s Hospital stanford, provide individualized support to best assist families by assessing barriers to care with the goal of improving health outcomes. In this retrospective cohort study, we reviewed patients with EsKD on hemodialysis involved in NICH to explore if NICH serves as a liaison between the patients and multidisciplinary medical team and to explore if NICH helps patients better manage the challenges of end-stage kidney disease. Through the electronic medical record system, EPIC, we reviewed the patients’ surveys to identify barriers to care, which included school and life engagement difficulty, lack of mental health resources, food and transportation insecurity, and cultural/language barriers. We also tracked the number of hospitalizations and ED visits before and during the patients’ enrollment in NICH. We discovered that through NICH, the aforementioned barriers to care were eliminated, the number of hospitalizations and emergency department visits was reduced, and all patients transitioned from inactive to active on the transplant list. NICH successfully improved the health outcomes of these patients and empowered patients to be more engaged in their care.

Competency-based assessments for healthcare professionals are critical for safe and effective client outcomes. Rehabilitation clinical skill competency assessments must be validated and revised to produce safe and skilled practitioners. The revised American Occupational Therapy Association (AOTA) Fieldwork Performance Evaluation (FWPE) instrument measures occupational therapy student performance to determine readiness for practice. The assessment includes thirty-seven competencies that address both profession specific clinical skills and general professional behavior skills. The objective of this study was to use Rasch methods to explore the use of the revised FWPE in actual fieldwork practice and to determine the instrument’s psychometric properties when separating the item components into two distinct subdomains: General Health Professions Competences and Occupational Therapy-specific Competencies. Internal construct validity and test reliability were analyzed using data from 149 occupational therapy students after completing their initial Level II A fieldwork clinical internship. This study examined the item difficulty hierarchy, item fit, person-fit to model, person separation index, person separation reliability coefficient, strata, ceiling and floor effect, and unidimensionality of the FWPE instrument as a whole and as two separate domains. With the exception of not meeting the criteria for unidimensionality, the full FWPE instrument and the Occupational Therapy-specific Competencies subdomain showed acceptable item-level psychometrics for reliability and precision. While the General Health Professions Competencies subdomain showed good item-level psychometrics, it was below the criterion for reliability and only separated the sample into two strata. We study the acoustomagnetoelectric (AME) effect in two-dimensional graphene with an energy bandgap using the semiclassical Boltzmann transport equation within the hypersound regime, (where represents the acoustic wavenumber and is the mean free path of the electron). The Boltzmann transport equation and other relevant equations were solved analytically to obtain an expression for the AME current density, consisting of longitudinal and Hall components. Our numerical results indicate that both components of the AME current densities display oscillatory behaviour. Furthermore, geometric resonances and Weiss oscillations were each defined using the relationship between the current density and surface Acoustic Wave (sAW) frequency and the inverse of the applied magnetic field, respectively. Our results show that the AME current density of bandgap graphene, which can be controlled to suit a particular electronic device application, is smaller than that of (gapless) graphene and is therefore, more suited for nanophotonic device applications.

Introduction: OsD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee, 20 to 30% of young growing athletes suffer from this pathology, described by Osgood and schlatter in 1903. Objective: To report 2 observations and review the literature. Observation 1: This was a 14-year-old Guinean teenager, Halpoular, living in Conakry, a right-handed footballer who presented with a painful swelling of the right knee that looked mechanical, whose father had HLA B27 positive ankylosing spondylitis. X-ray of the right knee showed fragmentation of the anterior tibial tuberosities, and ossicle with edema of the patellar tendons at stage 4 according to Ehrenbord and Lagergren (1961). Thus, the disease of OsD is retained, He received 120 mg of diclofenac, 20 mg omeprazole for 1 month and the cessation of sports activity, the evolution was favorably marked by the regression of pain. Observation 2: A 16-year-old girl, initials M.L.T, Guinean, residing in Coyah (Republic of Guinea), a handball player, consulted for intense pain (VAs: 8/10) in her knees after a violent shock received when she hit the post. The physical examination found a painful, soft swelling at the anterior tibial tuberosity of the right knee, the rest of the examination was normal. The X-ray of the right knee shows fragmentation of the anterior tibial tuberosity, and ossicle with edema of the patellar tendon at stage 4 according to Ehrenbord and Lagergren 1961, thus the OsD disease is retained, the treatment instituted was 120 mg of diclofenac, the cessation of sports activity which gives a good evolution (EVA at 2/10). Conclusion: OsD is a growth osteochondrosis affecting the apophyseal cartilage of the anterior tibial tuberosity of the knee in young growing athletes, resolving at rest.

RGB displays effectively simulate millions of colors in the eyes of humans by modulating the rela-tive amount of light emitted by 3 differently colored juxtaposed lights （red, green, and blue）. Therelationship between the ratio of red, green, and blue light and the perceptual experience of thatlight has been well defined by psychophysical experiments in humans, but is unknown in *** perceptual experience of an animal looking at an RGB display of imagery designed for humansis likely to poorly represent an animal＇s experience of the same stimulus in the real world. This isdue, in part, to the fact that many animals have different numbers of photoreceptor classes thanhumans do and that their photoreceptor classes have peak sensitivities centered over differentparts of the ultraviolet and visible spectrum. However, it is sometimes possible to generate videosthat accurately mimic natural stimuli in the eyes of another animal, even if that animal＇s sensitivityextends into the ultraviolet portion of the spectrum. How independently each RGB phosphor stimu-lates each of an animal＇s photoreceptor classes determines the range of colors that can be simu-lated for that animal. What is required to determine optimal color rendering for another animal is adevice capable of measuring absolute or relative quanta of light across the portion of the spectrumvisible to the animal （i.e., a spectrometer）, and data on the spectral sensitivities of the animal＇sphotoreceptor classes. In this article, we outline how to use such equipment and information togenerate video stimuli that mimic, as closely as possible, an animal＇s color perceptual experienceof real-world objects.

Diabetes is a widespread, rapidly increasing metabolic disease that is driven by hyperglycemia. Early glycemic control is of primary importance to avoid vascular complications including development of retinal disorders leading to blindness, end-stage renal disease, and accelerated atherosclerosis with a higher risk of myocardial infarction, stroke and limb amputations. Even after hyperglycemia has been brought under control, 'metabolic memory,' a cluster of irreversible metabolic changes that allow diabetes to progress, may persist depending on the duration of hyperglycemia. Manipulation of bile acid (BA) receptors and the BA pool have been shown to be useful in establishing glycemic control in diabetes due to their ability to regulate energy metabolism by binding and activating nuclear transcription factors such as farnesoid X receptor (FXR) in liver and intestine as well as the G-protein coupled receptor, TGR5, in enteroendocrine cells and pancreatic β-cells. The downstream targets of BA activated FXR, FGF15/21, are also important for glucose/insulin homeostasis. In this review we will discuss the effect of BAs on glucose and lipid metabolism and explore recent research on establishing glycemic control in diabetes through the manipulation of BAs and their receptors in the liver, intestine and pancreas, alteration of the enterohepatic circulation, bariatric surgery and alignment of circadian rhythms.

New risk assessment methodology for coal mine excavated slopes

This paper presents a new risk assessment methodology for coal mine excavated slopes. This new empirical-statistical *** assessment m. ethodology （ssAM! is intended for use by geotechnical engineers at both the design review and operational stages of a mine＇s life to categonse the risk of an excavated coal mine slope. A likelihood of failure is determined using a new slope stability classification system for excavated coal mine slopes developed using a database of 119 intact and failed case studies sourced from open cut coal mines in Australia. Consequence of failure is based on slope height and stand-off distance at the toe of the excavated slope. Results are presented in a new risk matrix, with slope risk being divided into low, medium and high categories. The ssAM is put forward as a new risk assess- ment methodology to assess the potential for, and consequence of, excavated coal mine slope failure. Unlike existing classification systems, assumptions about the likely failure mode or mechanism are not required. Instead, the ssAM applies an approach which compares the conditions present within the exca- vated slope face, with the known past performance of slopes with similar geotechnical and geometrical conditions, to estimate the slope＇s propensity for failure. The ssAM is novel in that it considers the depo- sitional history of strata in an excavated slope and how this sequence affects slope stability. It is further novel in that it does not require explicit measurements of intact rock, rock mass and/or defect strength to rapidly calculate a slope＇s likelihood of failure and overall risk. Ratings can be determined entirely from visual observations of the excavated slope face. The new ssAM is designed to be used in conjunction with existing slope stability assessment tools.
