Degenerative disease of the lumbar spine is a common and increasingly prevalent condition that is often implicated as the primary reason for chronic low back pain and the leading cause of disability in the western world. Surgical management of lumbar degenerative disease has historically been approached by way of open surgical procedures aimed at decompressing and/or stabilizing the lumbar spine. Advances in technology andsurgical instrumentation have led to minimally invasive surgical techniques being developed and increasingly used in the treatment of lumbar degenerative disease. Compared to the traditional open spine surgery, minimally invasive techniques require smaller incisions and decrease approach-related morbidity by avoiding muscle crush injury by self-retaining retractors, preventing the disruption of tendon attachment sites of important muscles at the spinous processes, using known anatomic neurovascular and muscle planes, and minimizing collateral soft-tissue injury by limiting the width of the surgical corridor. the theoretical benefits of minimally invasive surgery over traditional open surgery include reduced blood loss, decreased postoperative pain and narcotics use, shorter hospital length of stay, faster recover and quicker return to work and normal activity. this paper describes the different minimally invasive techniques that are currently available for the treatment of degenerative disease of the lumbar spine.

A key topic for aneurysmal subarachnoid hemorrhage is neuroinflammation.Neuroinflammation can predispose to aneurysm formation and rupture.Neuroinflammation can also result from the blood breakdown products after aneurysm rupture.Recent evidence has shown that perpetual neuroinflammation can contribute to vasospasm and hydrocephalus.targeting neuroinflammation is a novel mechanism for preventing subsequent neurologic sequalae.In this review,we highlight the pathophysiology of aneurysm formation,the neuroinflammatory surge after rupture including the involved cytokines,and ultimately tie in the contributory clinical relevance.In the last sections,we look at the pre-clinical data and novel avenues for further discovery.this paper will be a useful resource to both the clinician and scientific investigator.

Communication after cancellations in orthopaedics: the patient perspective

AIM: to examine patients' perceptions on communication surrounding the cancellation of orthopaedic operations and to identify areas for improvement in communication.MEtHODS: A prospective survey was undertaken at a university teaching hospital within the department of trauma and Orthopaedics. Patients admitted to an acute orthopaedic unit, whose operations were cancelled, were surveyed to assess patient satisfaction and preferences for notification of cancellation of their operations. Patients with an abbreviated mental test score of tients unable to complete the survey independently, those under 16 years of age, and any patient notified of the cancellation by any of the authors were excluded from this study. Patients were surveyed themorning after their operation had been cancelled thus ensuring that every opportunity was given for the medical staff to discuss the cancellation with the patient. the survey included questions on whether or not patients were notified of the cancellation of their surgery, the qualifications of the person discussing the cancellation, and patient preferences on the process. Satisfaction was assessed via 5-point Likert scale questions.RESULtS: Sixty-five consecutive patients had their operations cancelled on 75 occasions. Fifty-four point seven percent of the patients who had cancellations were notified by a nurse and 32% by a doctor. No formal communication occurred for 13.3% cancellations and no explanation was provided for a further 16%. Patients reported that they were dissatisfied with the explanation provided for 36 of the 75(48%) cancellations. Of those patients who were dissatisfied, 25(69.4%) were notified by a nurse. twenty-three of the 24(96%) patients notified by a doctor were satisfied with the explanation and that communication. Of those patients who were notified by a nurse 83% patients reported that they would have preferred it if a doctor had discussed the cancellation with them. there was a significant difference in satisfaction between those counselled by a nu

Hermansky-Pudlak syndrome（HPS） is a recessive disorder with bleeding diathesis, which has been linked to platelet granule defects. Both platelet granules and endothelial Weibel-Palade bodies（WPBs）are members of lysosome-related organelles（LROs） whose formation is regulated by HPS protein associated complexes such as BLOC（biogenesis of lysosome-related organelles complex）-1,-2,-3, AP-3（adaptor protein complex-3） and HOPS（homotypic fusion and protein sorting complex）. Von Willebrand factor（VWF） is critical to hemostasis, which is stored in a highly-multimerized form as tubules in the WPBs. In this study, we found the defective, but varying, release of VWF into plasma after desmopressin（DDAVP） stimulation in HPS1（BLOC-3 subunit）, HPS6（BLOC-2 subunit）, and HPS9（BLOC-1 subunit）deficient mice. In particular, VWF tubulation, a critical step in VWF maturation, was impaired in HPS6 deficient WPBs. this likely reflects a defective endothelium, contributing to the bleeding tendency in HPS mice or patients. the differentially defective regulated release of VWF in these HPS mouse models suggests the need for precise HPS genotyping before DDAVP administration to HPS patients.

two problems confront the evaluation of the survival rate: the natural chronicity and difficulty of diagnosis of lymphosarcoma. two to eight percent of the patients survive naturally over periods of 10 years or more without specific therapy. With radiotherapy 10 to 15% survive in good health.Although controversy still exists concerning the identity of lymphosarcoma, the prevalent tendency is to adhere to lymphosarcoma as a clinical and pathologic entity. Further segregation of lymphosarcoma on a morphological basis has been fully justified by clinical facts.A perusal of statistics on radiotherapeutic results does not disclose a convincing evidence that cure of lymphosarcoma might be permanent, nor is there any conclusive proof to the contrary. However, an analysis according to types and stage of the disease reveals more significant results. In the early stage the rate of lasting survival in lymphosarcoma of the upper respiratory tract, the stomach and cecum has been high, far exceeding that of the overall statistics covering ail localities and stages. A considerable number of patients with giant follicular lymphosarcoma and primary reticulum cell lymphosarcoma of the bone remain well for prolonged periods. It is reasonable to expect that patients who have survived in good health 15 or 20 years or more after treatment are permanently cured.
