Cholangiocarcinoma(CCA)arises from the ductular epithelium of the biliary tree,either within the liver(intrahepatic CCA)or more commonly from the extrahepatic bile ducts(extrahepatic CCA).This disease has a poor prognosis and a growing worldwide *** poor outcomes of CCA are partially explained by the fact that a final diagnosis is challenging,especially the differential diagnosis between hepatocellular carcinoma and intrahepatic CCA,or distal CCA and pancreatic head *** patients present with an advanced disease,unresectable disease,and there is a lack in non-surgical therapeutic *** least,there is an acute lack of prognostic biomarkers which further complicates disease ***,there is a dire need to find alternative diagnostic and follow-up pathways that can lead to an accurate result,either singlehandedly or combined with other *** the"-omics"era,this goal can be attained by various means,as it has been successfully demonstrated in other primary *** variants can reach a biomarker status ranging from circulating nucleic acids to proteins,metabolites,extracellular vesicles,and ultimately circulating tumor ***,given the relatively heterogeneous data,extracting clinical meaning from the inconsequential noise might become a tall *** current review aims to navigate the nascent waters of the non-invasive approach to CCA and provide an evidence-based input to aid clinical decisions and provide grounds for future research.

bACKGrOUND Quality control in colon cancer surgery is an ongoing debate ever since standardization proved to be highly efficient in improving survival in rectal cancer. Complete mesocolic excision(CME) is widely acclaimed as the new goldstandard in colon cancer resections, thus it is imperative to establish quality criteria of CME in order to make it easily understood and verified by surgeons worldwide. One simple and reproducible tool could be the measurement of arterial stumps postoperatively and a straightforward way to test its reliability is to test it in a comparative study between CME and non-CME *** To validate arterial stump measurement as a surgical quality tool by comparing CME with conventional radical *** This was a retrospective study, carried out on a prospective database. We collected data from two groups of patients, divided according to standard CME with D2 central vascular ligation(group A) and non-standardized surgery(group b). The two groups were compared with regard to the arterial stump length after right-and left-sided colectomies for colon cancer. The actual stump lengths of the ileocolic artery(ICA) and inferior mesenteric artery(IMA) were compared with their theoretical best D2 position of predicted ligation levels(D2 PLLs) for calculating the potential for improvement. Measurements on follow-up computed tomography scans were carried out by three observers. Pathological data were recorded(specimen length, lymph node yield) and correlated with stump *** We analysed 58 colectomies. The stump lengths(mean ± SD) in group A were16.97 ± 4.77 mm for ICA and 31.70 ± 15.71 mm for IMA, whereas group b had 49.93 ± 20.29 mm for ICA and 67.24 ± 28.71 mm for IMA. AIM:To investigate the diagnostic accuracy of acoustic radiation force impulse (ArFI) imaging as a noninvasive method for the assessment of liver fibrosis in chronic hepatitis C (CHC) ***:We performed a prospective blind com-parison of ArFI elastography,APrI index and FibroMax in a consecutive series of patients who underwent liver biopsy for CHC in University Hospital bucharest. His-topathological staging of liver fibrosis according to the METAVIr scoring system served as the reference. A to-tal of 74 patients underwent ArFI elastography,APrI index,FibroMax and successful liver biopsy. rESULTS:The noninvasive tests had a good correlation with the liver biopsy results. The most powerful test in predicting fibrosis was ArFI elastography. The diagnostic accuracy of ArFI elastography,expressedas area under receiver operating characteristic curve (AUrOC) had a validity of 90.2% (95% CI AUrOC = 0.831-0.972,P r the diagnosis of significant f ibrosis (F ≥ 2). ArFI sonoelastography predicted even better F3 or F4 fibrosis (AUrOC = 0.993,95% CI = 0.979-1).CONCLUSION:ArFI elastography had very good accuracy for the assessment of liver fibrosis and was superior to other noninvasive methods (APrI Index,FibroMax) for staging liver fibrosis.

Animals have evolved foraging strategies to acquire blends of nutrients that maximize fitness traits. In social insects, nu trie nt regulation is complicated by the fact that few in dividuals, the foragers, must address the diverge nt nutritional n eeds of all colony members simulta neously, includi ng other workers, the reproductives, and the brood. Here we used 3D nutritional geometry design to examine how bumblebee workers regulate their collection of 3 major macronutrients in the presence and absenee of brood. We provided small colonies artificial nectars (liquid diets) and pollens (solid diets) varying in their compositions of proteins, lipids, and carbohydrates during 2 weeks. Colo nies give n a choice betwee n nutrition ally complementary diets self-selected foods to reach a target ratio of 71% proteins, 6% carbohydrates, and 23% lipids, irrespective of the presenee of brood. Whe n con fined to a single nutrition ally imbalanced solid diet, colonies without brood regulated lipid collection and over-collected protein relative to this target ratio, whereas colonies with brood regulated both lipid and protein collection. This brood effect on the regulation of nutrient collection by workers suggests that protein levels are critical for larval development. Our results highlight the importa nee of con sidering bee nu trition as a multidimensi onal phe no men on to better assess the effects of environmental impoverishment and malnutrition on population declines.

Compelling evidence supports the crucial role of the receptor for advanced glycation end-products(rAGE)axis activation in many clinical *** the beginning of the coronavirus disease 2019 pandemic,there is an increasing concern about the risk and handling of severe acute respiratory syndrome coronavirus 2(SArS-CoV-2)infection in inflammatory gastrointestinal disorders,such as inflammatory bowel diseases(IbD).However,clinical data raised during pandemic suggests that IbD patients do not have an increased risk of contracting SArS-CoV-2 infection or develop a more severe course of *** the present review,we intend to highlight how two potentially important contributors to the inflammatory response to SArS-CoV-2 infection in IbD patients,the rAGE axis activation as well as the cross-talk with the renin-angiotensin system,are dampened by the high expression of soluble forms of both rAGE and the angiotensin-converting enzyme(ACE)*** soluble form of rAGE functions as a decoy for its ligands,and soluble ACE2 seems to be an additionally attenuating contributor to rAGE axis activation,particularly by avoiding the transactivation of the rAGE axis that can be produced by the virus-mediated imbalance of the ACE/angiotensin II/angiotensin II receptor type 1 pathway.

Objective:To evaluate the accuracy of our new three-dimensional(3D)automatic augmented reality(AAr)system guided by artificial intelligence in the identification of tumour’s location at the level of the preserved neurovascular bundle(NVb)at the end of the extirpative phase of nerve-sparing robot-assisted radical ***:In this prospective study,we enrolled patients with prostate cancer(clinical stages cT1ce3,cN0,and cM0)with a positive index lesion at target biopsy,suspicious for capsular contact or extracapsular extension at preoperative multiparametric magnetic resonance *** underwent robot-assisted radical prostatectomy at San Luigi Gonzaga Hospital(Orbassano,Turin,Italy),from December 2020 to December *** the end of extirpative phase,thanks to our new AAr artificial intelligence driven system,the virtual prostate 3D model allowed to identify the tumour’s location at the level of the preserved NVb and to perform a selective excisional biopsy,sparing the remaining portion of the *** and postoperative data were evaluated,especially focusing on the positive surgical margin(PSM)rates,potency,continence recovery,and biochemical ***:Thirty-four patients were *** 15(44.1%)cases,the target lesion was in contact with the prostatic capsule at multiparametric magnetic resonance imaging(Wheeler grade L2)while in 19(55.9%)cases extracapsular extension was detected(Wheeler grade L3).3D AAr guided biopsies were negative in all pathological tumour stage 2(pT2)patients while they revealed the presence of cancer in 14 cases in the pT3 cohort(14/16;87.5%).PSM rates were 0%and 7.1%in the pathological stages pT2 and pT3(re 3),***:With the proposed 3D AAr system,it is possible to correctly identify the lesion’s location on the NVb in 87.5%of pT3 patients and perform a 3D-guided tailored nerve-sparing even in locally advanced diseases,without compromising the oncological safety

In this paper, simulated and experimental results on the conical tank level control are presented. PI/PID controllers of integer order （IO） as well as of fractional order （FO） are studied and compared. The tuning parameters are obtained first by using root locus （rL） and Ziegler and Nichols methods, for comparison purposes. Next, particle swarm optimization （PSO） is employed to determine the optimal controllers＇ parameters using as fitness function the integral of the absolute value of tracking error （IAE）. From the experimental results it is concluded that PI/FOPI are the controllers presenting the lowest IAE indexes, whereas PID/FOPID controllers present the lowest energy consumption by the control signal.
