Background: People of Hispanic origin are the fastest growing ethnic minority in the United States and often have hypertension and other comorbidities which increase the risk associated with coronary artery disease(CAD). methods and results: An analysis of the 8045 Hispanic patients enrolled in INVEST was conducted, and comparisons were made to the 14 531 non-Hispanic patients. INVEST was a prospective, randomized, open, blinded end point study in CAD patients with hypertension. After 61 835 patient-years of follow-up, treatment with either a verapamil sustained release(Sr) or atenolol antihypertensive strategy resulted in greater blood pressure control in Hispanic patients, and Hispanic patients were at significantly lower risk of experiencing a nonfatal myocardial infarction, nonfatal stroke, or death(hazard ratio[Hr] 0.87, 95%CI 0.78-0.97). Hispanic ethnicity was associated with an increase(Hr 1.19, 95%CI 1.04-1.36), and randomization to the verapamil Sr strategy was associated with a decrease(Hr 0.85, 95%CI 0.76-0.95), in the risk of new-onset diabetes. Use of trandolapril in the verapamil Sr strategywas associated with reduced risk of new-onset diabetes, whereas increasing doses of atenolol and hydrochlorothiazide in the atenolol strategy were associated with increased risk of new-onset diabetes. Conclusions: The Hispanic cohort of INVEST had better blood pressure control and lower risk of adverse cardiovascular outcomes compared with the non-Hispanic cohort. A verapamil Sr strategy is an alternative to an atenolol strategy for the treatment of Hispanic patients with hypertension and CAD and can reduce the risk of new-onset diabetes.

Due to the importance of vibration effects on the functional accuracy of mechanical systems,this research aims to develop a precise model of a nonlinearly vibrating single-link mobile flexible *** manipulator consists of an elastic arm,a rotary motor,and a rigid carrier,and undergoes general in-plane rigid body motion along with elastic transverse *** accurately model the elastic behavior,Timoshenko’s beam theory is used to describe the flexible arm,which accounts for rotary inertia and shear deformation *** applying Newton’s second law,the nonlinear governing equations of motion for the manipulator are derived as a coupled system of ordinary differential equations(ODEs)and partial differential equations(PDEs).Then,the assumed mode method(Amm)is used to solve this nonlinear system of governing equations with appropriate shape *** assumed modes can be obtained after solving the characteristic equation of a Timoshenko beam with clamped boundary conditions at one end and an attached mass/inertia at the *** addition,the effect of the transverse vibration of the inextensible arm on its axial behavior is *** the axial rigidity,the effect makes the rigid body dynamics invalid for the axial behavior of the ***,numerical simulations are conducted to evaluate the performance of the developed model,and the results are compared with those obtained by the finite element *** comparison confirms the validity of the proposed dynamic model for the *** to the mentioned features,this model can be reliable for investigating the system’s vibrational behavior and implementing vibration control algorithms.

Additive manufacturing(Am)has revolutionized the design and manufacturing of patient-specific,three-dimensional(3D),complex porous structures known as scaffolds for tissue engineering *** use of advanced image acquisition techniques,image processing,and computer-aided design methods has enabled the precise design and additive manufacturing of anatomically correct and patient-specific implants and ***,these sophisticated techniques can be timeconsuming,labor-intensive,and ***,the necessary imaging and manufacturing equipment may not be readily available when urgent treatment is needed for trauma *** this study,a novel design and Am methods are proposed for the development of modular and customizable scaffold blocks that can be adapted to fit the bone defect area of a *** modular scaffold blocks can be combined to quickly form any patient-specific scaffold directly fr.m.two-dimensional(2D)medical images when the surgeon lacks access to a 3D printer or cannot wait for lengthy 3D imaging,modeling,and 3D printing during *** proposed method begins with developing a bone surface-modeling algorithm that reconstructs a model of the patient’s bone fr.m.2D medical image measur.m.nts without the need for expensive 3D medical imaging or *** algorithm can generate both patient-specific and average bone ***,a biomimetic continuous path planning method is developed for the additive manufacturing of scaffolds,allowing porous scaffold blocks with the desired biomechanical properties to be manufactured directly fr.m.2D data or *** algorithms are implemented,and the designed scaffold blocks are 3D printed using an extrusion-based Am *** and instructions are also provided to assist surgeons in assembling scaffold blocks for the self-repair of patient-specific large bone defects.

The hazardous environmental effects of greenhouse gas emissions and climate change demand alternative sources for cementitious materials in the construction *** development of geopolymer structures provides a way of producing 100%cement-free *** this research work,a novel and simple way of deriving nano particles fr.m.waste fly ash particles is *** effect of adding the synthesized nano fly ash particles as a filler.m.dium in geopolymer.m.rtars was investigated by considering strength and durability *** optimization was done by using regression analysis on the geopolymer.m.rtar and the impact of adding nano fly ash particles was studied by varying different percentages of addition ranging fr.m.0 to 7.5%by weight of binder *** the results,it was observed that 1%nano fly ash acted not only as a filler but also as nano-sized precursors of the polymerization process,resulting in denser geopolymer *** can explain the extraordinary gain in strength of 72.11 mPa as well as the denser core with negligible level of chloride ion penetration,making the material suitable for the development of structures susceptible to marine environment.
