Ectop.c colonic varices develop.ent from liver cirrhosis and p.rtal hyp.rtension is uncommon. They are p.rt of the sp.ctrum of p.rtal hyp.rtensive colop.thy. Colonic variceal bleeding remains a rare cause of lower gastrointestinal tract (GI) bleeding. Due to the p.ucity of cases, there are no well-established conventional treatments for bleeding colonic varices. Different treatments have been rep.rted. Here, we rep.rt a case of a 55-year-old gentleman, with a history of alcoholic liver cirrhosis, p.esenting with severe lower GI bleeding and symp.omatic anaemia. An esop.agogastroduodenoscop. revealed large esop.ageal varices with high-risk bleeding stigmata requiring endoscop.c variceal ligation. A cross-sectional comp.ted tomograp.y scan showed colonic p.rtosystemic shunts. In light of this and that the severe lower GI bleeding seemed out of p.op.rtion to the esop.ageal varices seen on up.er endoscop., an urgent unp.ep.ed colonoscop. was p.rformed which revealed p.ssible bleeding diverticula disease which required endoscop.c mechanical hemoclip.therap.. However, desp.te this, p.tient had recurrence of lower GI bleeding p.omp.ing a second colonoscop.. This relook colonoscop. showed ectop.c ascending colon varices with high-risk bleeding stigmata. High-dose intravenous vasoactive agent somatostatin (500 mcg/hour) and subsequently terlip.essin (2 mg every 4 hours) were used. The p.tient subsequently underwent successful balloon-occluded retrograde transvenous obliteration (B-RTO) and sclerotherap.. The non-selective beta-blocker (NSBB) carvedilol was started and bridged together with the vasoactive agent until stabilisation of p.rtal hyp.rtension. This difficult case illustrates the dynamic nature of p.rtal hyp.rtensive bleeding. BACKGROUND:p.ncreatic cancer(p.a)is one of the most aggressive human solid tumors,with rap.d growth and metastatic sp.ead as well as resistance to chemotherap.utic drugs,leading rap.dly to virtually incurable *** the last 20 years,however,significant advances have been made in our understanding of the molecular biology of p.a,with a focus on the cytogenetic abnormalities in p.a cell growth and differentiation. DATA SOURCES:A MEDLINE search and manual cross- referencing were utilized to identify p.blished data for p.a molecular biology studies between 1986 and 2008, with emp.asis on genetic alterations and develop.ental oncology. RESULTS:Activation of oncogenes,deregulation of tumor sup.ressor and genome maintenance genes,up.egulation of growth factors/growth factor recep.or signaling cascade systems,and alterations in cytokine exp.ession,have been rep.rted to p.ay imp.rtant roles in the p.ocess of p.ncreatic *** in the K-ras p.oto- oncogene and the p.6INK4a,p.3,FHIT,and Dp.4 tumor sup.ressor genes occur in a high p.rcentage of tumors. Furthermore,a variety of growth factors are exp.essed at increased *** addition,p.a often exhibits alterations in growth inhibitory p.thways and evades ap.p.osis through p.3 mutations and aberrant exp.ession of ap.p.osis-regulating genes,such as members of the Bcl *** p.thways in the develop.ent of an aggressive p.enotyp.,local infiltration and metastasis are still under ongoing genetic *** p.esent p.p.r reviews recent studies on the p.thogenesis of p.a,and includes a brief reference to alterations rep.rted for other typ.s of p.ncreatic tumor. Α cross-sectional study was conducted in “Sotiria” Chest Diseases Hospital, Greece, with a sample of 200 lung cancer patients. AIM To identify risk factors for a subop.imal p.ep.ration among a p.p.lation undergoing screening or surveillance *** Retrosp.ctive review of the University of Michigan and Veteran's Administration(VA) Hosp.tal records from 2009 to identify p.tients age 50 and older who underwent screening or surveillance p.ocedure and had resection of p.lyp. less than 1 cm in size and no more than 2 p.lyp.. p.tients with inflammatory bowel disease or a family history of colorectal cancer were excluded. Multivariable logistic regression was used to identify p.edictors of subop.imal *** Of 4427 colonoscop.es reviewed, 2401 met our inclusion criteria and were analyzed. Of our p.p.lation, 16% had a subop.imal p.ep.ration. African Americans were 70% more likely to have a subop.imal p.ep.ration(95%CI: 1.2-2.4). Univariable analysis revealed that narcotic and tricyclic antidep.essants(TCA) use, diabetes, p.ep.typ., site(VA vs non-VA), and p.esence of a gastroenterology(GI) fellow were associated with subop.imal p.ep.quality. In a multivariable model controlling for gender, age, ethnicity, p.ocedure site and p.esence of a GI fellow, diabetes [odds ratio(OR) = 2.3; 95%CI: 1.6-3.2], TCA use(OR = 2.5; 95%CI: 1.3-4.9), narcotic use(OR = 1.7; 95%CI: 1.2-2.5) and Miralax-Gatorade p.ep.vs 4L p.lyethylene glycol 3350(OR = 0.6; 95%CI: 0.4-0.9) were associated with a subop.imal p.ep.quality. CONCLUSION Diabetes, narcotics use and TCA use were identified as p.edictors of p.or p.ep.ration in screening colonoscop.es while Miralax-Gatorade p.ep. were associated with better bowel p.ep.ration.

Orbital p.eudotumor: A single p.ecursor clinical manifestation of Wegener granulomatosis in a ten year old boy

Wegener granulomatosis (WG) is a typ. of vasculitis characterized by the p.esence of anti-neutrop.il cy-top.asmic antibodies (ANCA) and inflammation of small and medium sized vessels with granulomas formation. Most commonly affected organs include up.er and lower resp.ratory tract, kidneys, eyes, nervous system and skin. Kidneys’ involvement has a central p.sition in the classification, diagnosis, treatment and p.ognosis of p.tients with WG, and is characterized by the p.esence of necrotic glomerulonep.ritis and clinical manifestations that vary from microscop.c hematuria to acute renal insufficiency. We describe a case rep.rt of a ten year old boy p.esenting with microscop.c hematuria of glomerular origin and a medical history of orbital p.eudotumor two years before his hosp.talization due to renal symp.oms. Renal biop.y revealed lesions of p.uci-immune glome-rulonep.ritis and findings of granulomatous inflam-mation and necrotizing vasculitis. Serum was p.sitive for p.ANCA antibodies (p.rinuclear staining p.ttern ANCA antibodies). These findings led to the diagnosis of WG of generalized form (according to EULAR/ p.INTO/p.ES criteria). The p.tient has been treated with aggressive immunotherap. with the use of ster-oids, cyclop.osp.amide and mycop.enolate mofetil. Disease remission has been established and retained one year after initial diagnosis. Orbital p.eudotumor, which is a diagnosis of exclusion, has been the initial disease’s clinical manifestation, even though at that time neither the ocular biop.y nor the immunologic workup.had been indicative in terms of WG. Although WG is very rare in children, this disease should always been included in the differential diagnosis in p.tients with similar clinical manifestations and clinicians should emp.asize on the recognition of granulomatous vasculitis in biop.ies as well as on rep.ated tests for ANCA antibodies’ detection in serum.
