Management of psoriasis patients with hepatitis B or hepatitis C virus infection

The systemic therapies available for the management of Psoriasis (PsO) patients who cannot be treated with more conservative options, such as topical agents and/or phototherapy, with the exception of acitretin, can worsen or reactivate a chronic infection. Therefore, before administering immunosuppressive therapies with either conventional disease-modifying drugs (cDMARDs) or biological ones (bDMARDs) it is mandatory to screen patients for some infections, including hepatitis B virus (HBV) and hepatitis C virus (HCV). In particular, the patients eligible to receive an immunosuppressive drug must be screened for the following markers: antibody to hepatitis B core, antibody to hepatitis B surface antigen (anti-HBsAg), HBsAg, and antibody to HCV (anti-HCV). In case HBV or HCV infection is diagnosed, a close collaboration with a consultant hepatologist is needed before and during an immunosuppressive therapy. Concerning therapy with immunosuppressive drugs in PsO patients with HBV or HCV infection, data exist mainly for cyclosporine a (CyA) or bDMARDs (etanercept, adalimumab, infliximab, ustekinumab). The natural history of HBV and HCV infection differs significantly as well as the effect of immunosuppression on the aforementioned infectious diseases. As a rule, in the case of active HBV infection, systemic immunosuppressive antipsoriatic therapies must be deferred until the infection is controlled with an adequate antiviral treatment. Inactive carriers need to receive antiviral prophylaxis 2-4 wk before starting immunosuppressive therapy, to be continued after 6-12 mo from its suspension. Due to the risk of HBV reactivation, these patients should be monitored monthly for the first 3 mo and then every 3 mo for HBV DNA load together with transaminases levels. Concerning the patients who are occult HBV carriers, the risk of HBV reactivation is very low. Anti-TNF alpha in the treatment of ulcerative colitis:A valid approach for organ-sparing or an expensive option to delay surgery?

Ulcerative colitis (UC) is an inflammatory bowel disease affecting large bowel with variable clinical course. The history of disease has been modified by the introduction of biologic therapy, in particular Infliximab (IFX), that has demonstrated efficacy in inducing fast symptoms remission, promoting mucosal healing and maintaining long-term remission. However, surgery is still needed for UC patients: in case of failure of medical therapy and if acute complications or a malignancy occurred. Surgical treatment is associated with a short-term post-operative mortality and morbidity respectively of 0%-4% and 30%. In this study we systematically analyzed: the role of IFX in reducing the colectomy rate, the risk of post-operative morbidity in pre-operatively IFX-treated patients and the cost-effectiveness of IFX therapy. Four of 5 analyzed randomized controlled trials demonstrated that therapy with IFX significantly reduces the colectomy rate. Moreover, pre-operative treatment with IFX doesn’t seem to increase post-operative infectious complications. By an economic point of view, the cost-effectiveness of IFX-therapy was demonstrated for UC patients suffering from moderate to severe UC in a study based on a cost estimation of the National Health Service of England and Wales. However, the argument is debated.

The efficiency of photo-electrocatalytic（PECa） devices for the production of solar fuels depends on several limiting factors such as light harvesting, charge recombination and mass transport diffusion. We analyse here how they influence the performances in PECa cells having a photo-anode based on Au-modified TiOnanotube（TNT） arrays, with the aim of developing design criteria to optimize the photo-anode and the PECa cell configuration for water photo-electrolysis（splitting） and ethanol photo-reforming *** TNT samples were prepared by controlled anodic oxidation of Ti foils and then decorated with gold nanoparticles using different techniques to enhance the visible light response through heterojunction and plasmonic effects. The activity tests were made in a gas-phase reactor, as well as in a PECa cell without applied bias. Results were analysed in terms of photo-generated current, Hproduction rate and photoconversion efficiency. Particularly, a solar-to-hydrogen efficiency of 0.83% and a Faradaic efficiency of 91%were obtained without adding sacrificial reagents.

Background:Hemodialysis(HD) per se is a risk factor for *** the growing body of evidence on blood-flow restriction(BFR) exercise in HD patients,identification of possible risk factors related to the prothrombotic agent D-dimer is required for the safety and feasibility of this training *** aim of the present study was to identify risk factors associated with higher D-dimer levels and to determine the acute effect of resistance exercise(RE) with BFR on this ***:Two hundred and six HD patients volunteered for this study(all with a glomerular filtration rate of 70 years old),and HD ***,RE+BFR significantly increases D-dimer after 4 *** with borderline baseline D-dimer levels(400-490 ng/mL) displayed increased risk of elevating D-dimer over the normal range(≥500 ng/mL).Conclusion:These results identified factors associated with a heightened prothrombotic state and may assist in the screening process for HD patients who wish to undergo RE+BFR.D-dimer and/or other fibrinolysis factors should be assessed at baseline and throughout the protocol as a precautionary measure to maximize safety during RE+BFR.

Search for heavy sterile neutrinos in trileptons at the LHC

We present a search strategy for both Dirac and Majorana sterile neutrinos from the purely leptonic decays of W^±→e^±e^±μ ν and μ^±^±e ν at the 14 TeV LHC. The discovery and exclusion limits for sterile neutrinos are shown using both the Cut-and-Count（CC） and Multi-Variate Analysis（MVA） methods. We also discriminate between Dirac and Majorana sterile neutrinos by exploiting a set of kinematic observables which differ between the Dirac and Majorana cases. We find that the MVA method, compared to the more common CC method, can greatly enhance the discovery and discrimination limits. Two benchmark points with sterile neutrino mass m N =20 GeV and 50 GeV are tested. For an integrated luminosity of 3000 fb^-1, sterile neutrinos can be found with 5σ significance if heavy-to-light neutrino mixings ｜U_（Ne）｜^2~｜U_（Nμ）｜^2~10^-6, while Majorana vs. Dirac discrimination can be reached if at least one of the mixings is of order 10^-5.

Nonalcoholic fatty liver disease(NAFLD) encompasses a range of liver histology severity and outcomes in the absence of chronic alcohol *** mildest form is simple steatosis in which triglycerides accumulate within hepatocytes.A more advanced form of NAFLD,nonalcoholic steatohepatitis,includes inflammation and liver cell injury,progressive to cryptogenic *** has become the most common cause of chronic liver disease in children and *** recent rise in the prevalence rates of overweight and obesity likely explains the NAFLD epidemic *** is strongly associated with abdominal obesity,type 2 diabetes,and dyslipidemia,and most patients have evidence of insulin ***,NAFLD shares many features of the metabolic syndrome(MetS),a highly atherogenic condition,and this has stimulated interest in the possible role of NAFLD in the development of *** evidence suggests thatNAFLD is associated with a significantly greater overall mortality than in the general population,as well as with increased prevalence of cardiovascular disease(CVD),independently of classical atherosclerotic risk ***,several studies including the pediatric population have reported independent associations between NAFLD and impaired flow-mediated vasodilatation and increased carotid artery intimal medial thickness-two reliable markers of subclinical atherosclerosis-after adjusting for cardiovascular risk factors and ***,the rising prevalence of obesity-related MetS and NAFLD in childhood may lead to a parallel increase in adverse cardiovascular *** children,the cardiovascular system remains plastic and damage-reversible if early and appropriate interventions are established *** goals for NAFLD should address nutrition,physical activity,and avoidance of smoking to prevent not only end-stage liver disease but also CVD.

Applicability of the Arrhenius model for Ammonia Oxidizing Bacteria subjected to temperature time gradients

The aim of this work is to identify the range of applicability of Arrhenius type temperature dependence for Ammonia Oxidizing Bacteria （AOB） subjected to tem- perature time gradients through continuous titrimetric tests. An innovative online differential titrimetric technique was used to continuously monitor the maximum biologic ammonia oxidation rate of the biomass selected in a pilot scale membrane bioreactor, as a function of temperature time gradients. The monitoring technique is based on the measurement of alkalinity and hydrogen peroxide con- sumption rates in two parallel reactors operated in non- limiting substrate conditions for AOB; both reactors were continuously fed with mixed liquor and in one of them AOB were inhibited with allylthiourea. The effects of temperature decrease rates in the range 1 to 4℃h^-1 were evaluated by controlling the titrimetric reactor in the temperature range 10℃-20℃. The dependence of growth kinetics on temperature time gradients and the range of applicability of Arrhenius model for temperature depen- dency of AOB growth kinetics were assessed. The Arrhenius model was found to be accurate only with temperature gradients lower than 2℃·h^-1. The estimated Arrhenius coefficients （θ） were shown to increase from 1.07 to 1.6 when the temperature decrease rate reached 4℃.h^-1.
