Wire Rope Isolators (WRI) are well known and used for theprotection of sensitive equipment against non-contact underwaterexplosions (UNDEX) on board Naval Ships, amongst others, whichare extremely destructive and can comple`tely impair the ship’scombat *** WRI exhibit a number of definite advantages, such aslarge deflection capability, modularity and insensitivity to aggressiveenvironment when proper materials are used. However, theirinherent nonlinearity does not always provide the best solution interms of shock attenuation. (Stiffening tension characteristics)Fortunately, there are ways to overcome this problem, namelyincreasing their damping and/ or changing their aspect ratio. It is thepurpose of this presentation to show how to overcome this problemand through analysis, by use of validated models of the shock testingmachines. The existing technology will be briefly presented andHDWRI (high damping wire rope isolators) vs. conventional WRIresponses will be compared on the US Navy shock testing machinesof MIL-S-901D.

高浓磨浆过程是制浆造纸、中密度纤维板等行业的重要操作单元,高浓磨浆机的运行效果直接影响纤维质量及过程能耗。本文以高浓磨浆过程的重要表征参数磨浆强度为出发点,系统综述了高浓磨区蒸汽及浆料的运动、浆料在磨区的停留时间、磨齿冲击频次及磨浆强度等方面的理论及实验研究现状,旨在全局把握高浓磨浆强度的研究脉络并解决现有研究存在的问题,本文还展望了高浓磨浆磨区对纤维动态冲击的作用机理及强度模型的未来研究及应用前景,对高得率制浆高浓磨浆过程的节能降耗与纤维性能调控具有积极意义。

Over the past few years, the research team of ProfessorDing-feng SU has reported an impressive quantity of experimental data about the relationships between blood pressurevariability (BPV) and end-organ damage, a topic of obviousclinical interest. This research work has been summarized ina paper that appeared in the August issue of the renownedjournal Trends in Pharmacological Sciences[1]. The studiesby Su et al provide convincing evidence that BPV is an independent cardiovascular risk factor that should be considered as such and, therefore, might become an important target for therapeutic interventions. Geometrical optics and the Monte Carlo method are very flexible in dealing with the interaction of light with non-spherical particles, but usually diffraction is not considered. To cover this gap, the Heisenberg Uncertainty Monte Carlo （HUMC） model is applied to calculate separately the diffraction of a ray or a photon. In this paper, we report an improvement of the HUMC model by specifying the phase of the photon subject to the Fraunhofer diffraction condition. After validating the model by comparing its results with analytical results for apertures of simple shapes, the HUMC model is then applied in simulations of Fraunhofer diffraction by apertures of complex shapes, such as those composed of one or two elliptical openings. We have shown that the diffracted intensity distributions of simple apertures obtained by the HUMC model are in good agreement with the results calculated from analytical expressions. The simulations of diffraction by apertures composed of two square or elliptical openings prove that the HUMC model is a powerful and flexible too] for predicting the Fraunhofer diffraction by a complex optical system.

Introduction: Osteosarcoma is the most common primary malignant bone tumor in children. It is highly aggressive and has a poor prognosis. A late presentation modifies and makes difficult the management affecting the survival of children. We report the case of a large conventional osteosarcoma in a 13-year-old girl. Case Presentation: Adolescent girl admitted for painful swelling of the left shoulder with absolute functional impotence of the thoracic limb and severe anemia. The painful swelling was thought to have been caused by a minor trauma that had occurred six months previously. The patient’s general condition was poor, and she presented with a large, shiny, painful mass over the shoulder and upper 2/3 of the left arm, measuring 28 cm long by 28 cm wide and 57 cm in circumference, and a large fistulous axillary adenopathy. CT scan showed a tumour lesion of the left humerus with liver and lung metastases, raising suspicion of osteogenic osteosarcoma. The tumor was classified according to TNM staging: T2N1M1(a + b). Management was modified when uncontrolled bleeding developed. It consisted of an extended amputation of the left thoracic limb. Pathological analysis showed a high-grade conventional osteosarcoma. Quality improvement was obtained for thirty days, followed by the onset of dyspnea. The evolution was towards death at forty days post-operatively. Conclusion: Osteosarcoma is a highly aggressive cancer. Delayed treatment leads to a fatal outcome. Early diagnosis is one of the challenges to be met in order to improve survival.

Introduction: Pre-eclampsia is a complication of pregnancy that generally occurs in the third trimester. It is associated with a maternal and foetal mortality rate of around 27%. In view of the grim picture painted by this condition for both mother and newborn, studies have been carried out into the early detection of patients at risk of developing pre-eclampsia. These make it possible to introduce pregnancy-specific monitoring and preventive strategies to reduce the incidence of the condition. Objective: To establish the link between placental ultrasound characteristics and the onset of pre-eclampsia. Methodology: A multicentre prospective cohort study was conducted in two hospitals in Yaoundé, namely the Yaoundé Gynaecological Obstetrics and Paediatrics Hospital and the Nkolndongo Health and Social Animation Centre, in the gynaecology and radiology departments over a period of 11 months, from October 2022 to August 2023. It included pregnant women who had undergone obstetric ultrasound between 12 and 18 weeks’ gestation. In addition to routine obstetric ultrasound, we performed obstetric Doppler measurements in these patients. The resistance index of the left and right uterine arteries, the umbilical artery and the placental volume were the characteristics sought. At the end of this examination, two groups were formed: cases (pathological Doppler group) and controls (normal Doppler group). Blood pressure and urine dipstick were taken at each antenatal visit until delivery, then during the immediate postpartum period and finally at 7 days, 21 days and 42 days after delivery. Results: Sixty-seven (67) patients were included. Of these, 35 (47.8%) had pathological Dopplers. Sixteen patients had arterial hypertension associated with proteinuria and were therefore labelled as pre-eclampsia (PE). This gives a prevalence of 23.9%. Background and objective: Classically, diabetic subjects are at high risk of anaesthesia compared with general population. However, some recent publications have shown contrasting and sometimes contrary results. The aim of our study was to evaluate morbidity and mortality during and after anaesthesia in patients with versus without diabetes operated on at Monkole Hospital over the last ten years. Methods: Retrospective cohort study including all patients who underwent all-comers surgery excluding cardiac surgery between 2011 and 2021. Each diabetic patient was matched to 2 non-diabetic controls on age and sex. The evaluation criterion was the frequency of occurrence of at least one perioperative complication and/or death up to day 30. A multivariate analysis using a Cox model was used to determine the factors associated with the occurrence of this morbidity and mortality. The model was adjusted for comorbidities, preoperative hyperglycaemia, ASA score, type of anaesthesia and severity of surgery. Results: A total of 351 diabetic patients (mean age 53.3 ± 14.18 years) and 701 non-diabetic patients (mean age 53.52 ± 14.7 years) were included and analysed. Preoperatively, hyperglycaemia (blood glucose > 180 mg/dl) was observed in 24.3% of diabetic patients compared with 1.6% of non-diabetic patients. The incidence of overall perioperative complications was 25.6% in diabetic patients compared with 28.6% in non-diabetic patients (p = 0.27). The risk factors associated with this morbidity were general anaesthesia with oro-tracheal intubation vs loco-regional anaesthesia (OR = 3.06 [95%CI: 1.91 - 4.94];p Conclusion: This study shows that there is not significant increase in perioperative morbidity and mortality in diabetic patients compared with non-diabetic ones of similar severity. These results suggest that diabetes itself (excluding associated comorbidities) has only a minor impact on perioperative morbidity and mortality.
