Procedural Sedation and Analgesia in Children in Emergency Department—Role of Adjunct Therapies

Objective: To compare sedation efficacy and parent/consultant satisfaction between standard sedation, sedation with music listening, and sedation with Certified Child life Specialists (CCLS) in children undergoing procedural sedation in the Pediatric Emergency Department (PED). Methods: Eligible children, aged 3 - 18 years, were randomly allocated to one of 3 groups: 1) standard sedation;2) sedation with music listening;3) sedation with CCLS intervention. All 3 groups received intravenous ketamine. The child life group received age appropriate comforting measures, while the music group listened to music of their choice during the procedure. The primary outcome was sedation efficacy, measured by Ramsay Sedation scale, FACES-P scale and need for re-dosing. The secondary outcome was parent/consultant satisfaction. Results: Fifty nine patients were analyzed (standard sedation: 20;sedation with music listening: 20;and sedation with CCLS: 19). There was no significant difference in mean initial ketamine dosing (1.58 ± 0.44 vs. 1.68 ± 0.36 vs. 1.42 ± 0.47, p = 0.26). There was no significant difference in median Ramsay Sedation scores [6(IQR:4,6) vs. 6 (IQR:4,6) vs. 6 (IQR:5,6)], FACES-R pain score [0 (IQR:0.0) vs. 0 (IQR:0.0) vs. 0 (IQR:0.0)] and need for re-dosing [9/20 (45%) vs. 4/20 (20%) vs. 8/19 (42.1%)] amongst the 3 groups. Parent and consultant satisfaction was high in all 3 groups. Conclusion: Our pilot study did not demonstrate a difference in sedation efficacy or parent/consultant satisfaction when adjunct therapies were used during PSA. Further studies with a large sample size are needed to define the role for such adjunct therapies during procedural sedation in PED.

Cancer remains one of the diseases with the highest incidence and mortality globally.Conventional treatment modalities have demonstrated threatening drawbacks including invasiveness,noncontrollability,and development of resistance for some,including chemotherapy,radiation,and surgery.Sono-photodynamic combinatorial therapy(SPDT)has been developed as an alternative treatment modality which offers a non-invasive and controllable therapeutic approach.SPDT combines the mechanism of action of sonodynamic therapy(SDT),which uses ultrasound,and photodynamic therapy(PDT),which uses light,to activate a sensitizer and initiate cancer eradication.The use of phthalocyanines(Pcs)as sensitizers for SPDT is gaining interest owing to their ability to induce intracellular oxidative stress and initiate toxicity under SDT and PDT.This review discusses some of the structural prerequisites of Pcs which may influence their overall SPDT activities in cancer therapy.

Spinal cord injury can be traumatic or non-traumatic in origin,with the latter rising in incidence and prevalence with the aging demographics of our society.Moreove r,as the global population ages,individuals with co-existent degenerative spinal pathology comprise a growing number of traumatic spinal cord injury cases,especially involving the cervical spinal cord.This makes recovery and treatment approaches particula rly challenging as age and comorbidities may limit regenerative capacity.For these reasons,it is critical to better understand the complex milieu of spinal cord injury lesion pathobiology and the ensuing inflammatory response.This review discusses microglia-specific purinergic and cytokine signaling pathways,as well as microglial modulation of synaptic stability and plasticity after injury.Further,we evaluate the role of astrocytes in neurotransmission and calcium signaling,as well as their border-forming response to neural lesions.Both the inflammatory and reparative roles of these cells have eluded our complete understanding and remain key therapeutic targets due to their extensive structural and functional roles in the nervous system.Recent advances have shed light on the roles of glia in neurotransmission and reparative injury responses that will change how interventions are directed.Understanding key processes and existing knowledge gaps will allow future research to effectively target these cells and harness their regenerative potential.

The recent increase in placenta accreta spectrum has been correlated with a rise in the rate of cesarean sections.A recent study provides evidence that hampered wound healing results in cesarean scar defects that lead to a failure in the normal process of decidualization and deeper adherence of trophoblasts.Matrix metalloproteinase(MMP)is crucial in every step of wound healing as it alters the wound matrix,facilitating cell migration,as well as tissue remodeling.MMP-9 expression is higher in placental and decidual tissue in cases of placenta accreta.Based on these findings,assessment of MMP-9 expression can shed new light on the etiopathology of placenta accreta spectrum disorder and can be a potential diagnostic marker.

A prospective cohort of men with localized prostatecancer on active surveillance protocol in Hong Kong,China:what did we learn?

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,China.We investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease progression.Moreover,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 lesions.The study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated biopsy.The combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Determination of critical state line(CSL)for silty-sandy iron ore tailings subjected to low-high confining pressures

The disposal of filtered tailings in high dry stacks can induce particle breakage,changing the material's behaviour during the structure's lifetime.The grading changes influence material properties at the critical state,and this is not mature for intermediate artificial soils(tailings)in a broad range of confining pressures.In this paper,it aims to describe the behaviour of iron ore tailings in a spectrum of confining pressures broader than the reported in previous studies.A series of consolidated drained(CD)triaxial tests was carried out with confining pressures ranging from 0.075 MPa to 120 MPa.These results show that the amount of breakage plays an essential role in the response of iron ore tailings.The existence of curved critical state line(CSL)in both specific volume(ν)-logarithm of mean effective stress(p′)and deviatoric stress(q)-mean effective stress(p′)planes,and different responses in the deviatoric stress-axial strain-volumetric strain curves were verified.An inverse S-shaped equation was proposed to represent the silty-sandy tailings'behaviour up to high pressures onν-lnp′plane.The proposed equation provides a basis for enhancing constitutive models and considers the evolution of the grading up to severe loading conditions.The adjustment considered three regions with different responses associated with particle breakage at different pressure levels.

Use of tree species by three species of Magicicada(Hemiptera:Cicadidae)in an Appalachian forest

Periodical cicadas(Magicicada spp.)are endemic to deciduous forests in the eastern United States.In successional forests,they must partition resources such as host trees to coexist.We measured tree size,emergence holes,oviposition scar bundles,and chorusing center abundances of Magicicada species on 12 common tree species in a deciduous forest to understand host-tree use.We predicted that the abundance of periodical cicadas and use of specific host-tree species would change depending on the Magicicada species and tree life stage.We considered the size of the tree(diameter at breast height)as a covariate to control for tree size and collected eggs for a greenhouse experiment to assess whether nymphs prefer to feed on Quercus rubra or Acer saccharum.More emergence holes were found below Quercus species than any other tree species.The abundance of periodical cicadas on host trees used for chorusing centers varied depending on the Magicicada species,but were most abundant on Quercus species.Oviposition scar bundles were also more frequent on Quercus.More nymphs were found on Quercus than Acer in the nymph preference study.Though periodical cicadas used Quercus hosts more than other tree species,their abundances on different host tree sizes and species differed significantly.Periodical cicada species may use specific host species and life stages as a way to partition resources and minimize competition among the Magicicada species during emergence years.

Ischemic stroke represents a heavy burden on public health.Currently,recanalization is the only effective therapy for ischemic stroke in a small population of eligible patients.However,there is no effective adjunct medication for preventing neuron loss after reperfusion to mitigate longlasting brain injury.Extrasynaptic N-methyl-Daspartate receptors (esNMDARs) are the major cause of ischemic neuronal death.However,there is no inhibitor selectively ta rgeting esNMDARs without compromising the function of other"beneficial"syna ptic NMDARs.

Natural nest substrates influence squamate embryo physiology but have little effect on hatchling phenotypes

Vertebrate embryos require access to water;however,many species nest in terrestrial habitats that vary considerably in moisture content.Oviparous,non-avian reptiles have served as models to understand how environmental factors,like moisture availability,influence development because eggs are often exposed to prevailing environments in the absence of parental care.Though much research demonstrates the importance of water absorption by eggs,many ecological factors that influence moisture availability in natural nests have received little attention.For example,the type of substrate in which nests are constructed is understudied.We experimentally incubated eggs of the brown anole lizard(Anolis sagrei)in 2 naturally occurring nest substrates that were treated with varying amounts of water to determine how natural substrates influence development at different moisture concentrations.One substrate consisted of sand and crushed seashells and the other was mostly organic material(i.***.decayed plant material).Both are common nesting substrates at our field site.When controlling for water uptake by eggs,we found that egg survival and hatchling phenotypes were similar between substrates;however,embryos developed more quickly in the sand/shell substrate than the organic substrate,indicating substrate-specific effects on embryo physiology.These results demonstrate that different natural substrates can result in similar developmental outcomes if the water available to eggs is comparable;however,some aspects of development,like developmental rate,are affected by the type of substrate,independent of water availability.Further study is required to determine how natural substrates influence embryo physiology independent of water content.
