Effect of liver cirrhosis on long-term outcomes after acute respiratory failure: A population-based study

AIM To assessed the effect of liver cirrhosis(LC) on the poorly understood long-term mortality risk after firstever mechanical ventilation(1-MV) for acute respiratory *** All patients in Taiwan given a 1-MV between 1997 and 2013 were identified in Taiwan's Longitudinal Health Insurance Database 2000. Each patient with LC was individually matched, using a propensity-score method, to two patients without LC. The primary outcome was death after a *** A total of 16653 patients were enrolled: 5551 LC-positive (LC^([Pos])) patients, including 1732 with cryptogenic LCs and 11102 LC-negative(LC^([Pos])) controls. LC^([Pos]) patients had more organ failures and were more likely to be admitted to medical department than were LC^([Pos]) controls. LC^([Pos]) patients had a significantly lower survival rate(AHR = 1.38, 95%CI: 1.32-1.44). Moreover, the mortality risk was significantly higher for patients with non-cryptogenic LC than for patients with cryptogenic LC(AHR = 1.43, 95%CI: 1.32-1.54) and patients without LC(AHR = 1.56, 95%CI: 1.32-1.54). however, there was no significant difference between patients with cryptogenic and without LC(HR = 1.05, 95%CI: 0.98-1.12).CONCLUSION LC, especially non-cryptogenic LC, significantly increases the risk of death after a 1-MV.

Perihepatic nodes detected by point-of-care ultrasound in acute hepatitis and acute-on-chronic liver disease

AIM: To study the manifestations of perihepatic lymph nodes during the episode of acute hepatitis flare by point-of-care ***: One hundred and seventy-six patients with an episode of acute hepatitis flare(ALT value > 5 × upper normal limit) were enrolled retrospectively. Diagnosis of etiology of the acute hepatitis flare was based on chart records and serological and virological assays. The patients were categorized into two groups(viral origin and non-viral origin) and further defined into ten subgroups according to the etiologies. An ultrasonograpy was performed within 2 h to 72 h(median, 8 h). The maximum size of each noticeable lymph node was measured. Correlation between clinical parameters and nodal manifestations was analyzed RESULTS: Enlarged lymph nodes(width ≥ 5mm)were noticeable in 110(62.5%) patients, mostly in acute on chronic hepatitis B(54.5%). The viral group had a higher prevalence rate(89/110 = 80.9%) and larger nodal size(median, 7 mm) than those of the non-viral group(21/66 = 31.8%; median, 0 mm)(P howed strong significance in multivariate analysis(P hange is valuable for clarifying the etiologies in an episode of acute hepatitis flare.

BACKGROUND Although important for determining long-term outcome, pathologic stage of hepatocellular carcinoma (HCC) is difficult to predict before surgery. Current state-of-the-art magnetic resonance imaging (MRI) using gadoxetic acid provides many imaging features that could potentially be used to classify single HCC as pT1 or pT2. AIM To determine which gadoxetic acid-enhanced MRI (EOB-MRI) findings predict pathologic stage T2 in patients with solitary HCC (cT1). METhoDS Pre-operative EOB-MRI findings were reviewed in a retrospective cohort of patients with solitary HCC. The following imaging features were examined: Hyperintensity in unenhanced T2-weighted images, hypointensity in unenhanced T1-weighted images, arterial enhancement, corona enhancement, washout appearance, capsular appearance, hypointensity in the tumor tissue during the hepatobiliary (HB) phase, peritumoral hypointensity in the HB phase, hypointense rim in the HB phase, intratumoral fat, hyperintensity on diffusionweighted imaging, hypointensity on apparent diffusion coefficient map, mosaic appearance, nodule-in-nodule appearance, and the margin (smooth or irregular). Surgical pathology was used as the reference method for tumor staging. Univariate and multivariate analyses were performed to identify predictors of microvascular invasion or satellite nodules. RESULTS There were 39 (34.2%;39 of 114) and 75 (65.8%;75 of 114) pathological stage T2 and T1 HCCs, respectively. Large tumor size (≥ 2.3 cm) and two MRI findings, i.e., corona enhancement [odds ratio = 2.67;95% confidence interval: 1.101-6.480] and peritumoral hypointensity in HB phase images (odds ratio = 2.203;95% confidence interval: 0.961-5.049) were associated with high risk of pT2 HCC. The positive likelihood ratio was 6.25 (95% confidence interval: 1.788-21.845), and sensitivity of EOB-MRI for detecting pT2 HCC was 86.2% when two or three of these MRI features were present. Two-terminal(2-T)perovskite(PVK)/CuIn(Ga)Se_(2)(CIGS)tandem solar cells(TSCs)have been considered as an ideal tandem cell because of their best bandgap matching regarding to Shockley–Queisser(S–Q)***,the nature of the irregular rough morphology of commercial CIGS prevents people from improving tandem device *** this paper,D-homoserine lactone hydrochloride is proven to improve coverage of PVK materials on irregular rough CIGS surfaces and also passivate bulk defects by modulating the growth of PVK *** addition,the minority carriers near the PVK/C60 interface and the incompletely passivated trap states caused interface recombination.A surface reconstruction with 2-thiopheneethylammonium iodide and N,N-dimethylformamide assisted passivates the defect sites located at the surface and grain ***,LiF is used to create this field effect,repelling hole carriers away from the PVK and C60 interface and thus reducing *** a result,a 2-T PVK/CIGS tandem yielded a power conversion efficiency of 24.6%(0.16 cm^(2)),one of the highest results for 2-T PVK/CIGS TSCs to our *** validation underscores the potential of our methodology in achieving superior performance in PVK/CIGS tandem solar cells.

hong Kong has a long historical record of harmful algal blooms(HABs).In the 1980s-1990s,HABs were mainly pollution-related and most of the events happened in estuaries and enclosed embayment such as Tolo Harbour and Port *** major cause of HABs in 1980s-1990s was closely related to nutrients enrichment;included but not limited to changes in the concentration and ratio of soluble N and P in *** major causative organisms of HABs in hong Kong by then were meso-plankton such as selected species of diatoms and dinofl ***,zooplankton was also a cause of red tides(the common name of HABs).There has been gradual change after the *** is attributed to higher and higher influences from the region,namely the Zhujiang(Pearl)River *** are increasing influences from Chinese mainland due to extraordinary social and economic growths during the past 20 *** the past10 years,HABs in hong Kong was mainly subject to regional impacts in the Zhujiang River *** the duration period and covered areas have been enlarged which overwhelmed the localized influences of stream pollution and self-contamination of aquaculture *** flagellates of uncommon happening become dominant species in HAB ***,the seasonal impact of Noctiluca scintillans in late winter to mid spring remained.
