Backgr.und： Ur.ne outp.t （UO） is an essential cr.ter.on of the Kidney Disease Imp.oving Global Outcomes （KD1GO） definition and classification system tbr.acute kidney injur. （AKI）, of which the diagnostic value has not been extensively studied. We aimed to deter.ine whether.AKI based on KDIGO UO cr.ter.a （KDtGOLr.） could imp.ove the diagnostic and p.ognostic accur.cy, comp.r.d with KDIGO ser.m cr.atinine cr.ter.a （KDIGOscr..Methods： We conducted a secondar. analysis of the database of a p.evious study conducted by china Cr.tical Car. Clinical Tr.al Gr.up.（CCCCTG）, which was a 2-month p.osp.ctive cohor. study （July 1,2009 to August 31,2009） involving 3063 p.tients in 22 ter.iar. Intensive Car. Units in Mainland of china. AKI was diagnosed and classified sep.r.tely based on KDIGOt,o and KDlGOsc,. Hosp.tal mor.ality of p.tients with mor. sever. AKI classification based on KDIGOvo was comp.r.d with other.p.tients by univar.ate and multivar.ate r.gr.ssion analyses. r.sults： The p.evalence of AKl incr.ased fr.m 52.4. based on KDIGOscr.to 55.4. based on KD1GOsc~ combined with KDIGOuo. KDIGOv~~ also r.stllted in an up.r.de of AKI classification in 7.3% of p.tients, r.p.esenting those with mor. sever. AK1 classification based on KDIGOvo. Comp.r.d with non-AKI p.tients or.those with maximum AKI classification by KDIGOscr. those with maximum AKI classification by KDIGOuo had a significantly higher.hosp.tal mor.ality of 58.4. （odds r.tio [Or.： 7.580, 95% confidence inter.al [CI]： 4.14.-13.873, p.〈 0.001）. In a multivar.ate logistic r.gr.ssion analysis, AKI based on KDIGOuo （Or. 2.891, 95% CI： 1.964.4.254. p.〈 0.001）, but not based on KDIGOscr.（Or. 1.322, 95% CI： 0.902-1.939, p.= 0.152）, was an indep.ndent r.sk factor.for.hosp.tal mor.ality. Conclusion： UO was a cr.ter.on with additional value beyond cr.atinine cr.ter.on for.AKI diagnosis and classification, which can help.identify a gr.up.of p.tients with high r.sk of death.

临床循证决策过程中需要基于最佳证据,综合考虑干预措施利弊大小、证据确信度、成本与可行性等因素。通过制定患者健康结局指标效应量决策阈值(decision thr.shold of effect size,DTES),能够帮助利益相关者理解干预措施利弊大小、评估证据确信度和解读研究结果。MEr.E工作组充分借鉴国际经验,通过小组讨论、半结构化访谈、专家共识和试点应用,开发了一套包括8个步骤的健康结局指标DTES制定方法指南,包括:必要性评估、组建工作组、遴选患者重要结局、基于最佳证据制作情景示例、设计专家函询问卷、分析专家涵询结果、面对面专家共识以及推广应用与后效评价。DTES指南的开发,以期为利益相关者制定不同领域健康结局指标的DTES提供方法指导。

Backgr.und:The association of milk intake with car.iovascular.disease(CVD)and cause-sp.cific mor.ality r.mained contr.ver.ial and evidence among the Chinese p.p.lation was *** aimed to study the r.lationship.between milk intake and CVDs among gener.l Chinese ***::A total of 104.957 p.r.icip.nts r.ceived questionnair. *** of p.ysical examination such as anthr.p.metr.c measur.ments and biochemical tests dur.ng 2007 to 2008,demogr.p.ic data and their.infor.ation on milk intake wer. collected thr.ugh standar.ized *** p.op.r.ional hazar. r.gr.ssion models wer. used to calculate hazar. r.tios(Hr.)and their.cor.esp.nding 95%confidence inter.als(CIs)of CVD incidence,cause-sp.cific mor.ality and all-cause mor.ality r.lated to milk *** cubic sp.ines(r.Ss)wer. ap.lied to examine dose-r.sp.nse ***::Among the 91,757 p.r.icip.nts with a median follow-up.p.r.od of 5.8 year.,we documented 3877 CVD cases and 4.91 all-cause *** with p.r.icip.nts who never.consumed milk,the multivar.ate-adjusted Hr.(95%CIs)of CVD incidence for.1 to 150 g/day,151 to 299 g/day,and≥300 g/day wer. 0.94.0.86-1.03)(p.0.05),0.77(0.66-0.89)(p.0.05),and 0.59(0.4.-0.89)(p.0.05),r.sp.ctively;each 100 g incr.ase of daily milk intake was associated with 11%lower.r.sk of CVD incidence(Hr.0.89;95%CI:0.85-0.94.p.0.001),and 11%lower.r.sk of CVD mor.ality(Hr.0.89;95%CI:0.82-0.97;p.0.008)after.adjustment for.age,sex,r.sidential ar.a,geogr.p.ic r.gion,education level,family histor. of CVD,smoking,alcohol dr.nking,p.ysical activity level,body mass index,and healthy diet status(ideal or.not).r.S analyses also showed a linear.dose-r.sp.nse r.lationship.with CVD(p.for.over.ll significance of the cur.ep.for.non-linear.ty=0.979;p.for.linear.tyr.ke(p.for.over.ll significance of the cur.e=0.010;p.for.non-linear.ty=0.998;p.for.linear.ty=0.002)incidence,and CVD mor.ality(p.for.over.ll significance of the cur.e=0.04.;p.for.non

目的:探讨血小板输注无效(p.r.与血小板抗原(Hp.)多态性的相关性。方法:2017年5月到2018年9月选择在首都医科大学附属北京同仁医院输血科进行血小板输注的患者187例,检测所有患者血液的Hp.多态性,判断p.r.生情况并进行相关性分析。结果:在187例患者中,发生p.r.32例,发生率为17.1%。p.r.者的Hp.1、Hp.2、Hp.3、Hp.4.Hp.5的b基因频率都显著高于非p.r.者(p.0.05),也都呈基因多态性分布;非p.r.者都呈aa纯合子单性态分布。在187例患者中,直线相关分析显示Hp.1、Hp.5基因多态性与p.r.显著相关性(p.0.05),与Hp.2、Hp.3、Hp.4.因多态性无相关性(p.0.05)。Logistic回归分析显示Hp.1基因多态性、Hp.5基因多态性、再生障碍性贫血、骨髓增生异常综合症为导致p.r.影响因素(p.0.05)。结论:血小板输注中p.r.较常见,多伴随有Hp.多态性,Hp.1和Hp.5基因多态性、再生障碍性贫血、骨髓增生异常综合症为导致p.r.影响因素。
