What is already known on this topic?The global efforts to address the hepatitis C virus(HCV)are progressing,but there are still significant gaps in the diagnosis and treatment of HCV,leading to an increasing number of deaths related to HCV.What is added by this report?An extensive investigation was conducted to assess HCV RNA diagnosis,treatment uptake,and associated factors among individuals infected with HCV within Jiangsu Province.The study encompassed a large geographical area and utilized a substantial sample size.What are the implications for public health practice?Implementing focused interventions to improve the timely diagnosis of HCV RNA and increase the uptake of HCV treatment could effectively reduce the future burden of HCV-related health problems,deaths,and healthcare expenses.This is essential for achieving the global target of eliminating hepatitis C.

Background With the variability in emerging data,guidance on the isolation duration for patients with coronavirus disease 2019(COVID-19)due to the Omicron variant is controversial.This study aimed to determine the predictors of prolonged viral RNA shedding in patients with non-severe COVID-19 and construct a nomogram to predict patients at risk of 14-day PCR conversion failure.Methods Adult patients with non-severe COVID-19 were enrolled from three hospitals of eastern China in Spring 2022.Viral shedding time(VST)was defined as either the day of the first positive test or the day of symptom onset,whichever was earlier,to the date of the first of two consecutively negative PCR tests.Patients from one hospital(Cohort I,n=2033)were randomly grouped into training and internal validation sets.Predictors of 14-day PCR conversion failure were identified and a nomogram was developed by multivariable logistic regression using the training dataset.Two hospitals(Cohort II,n=1596)were used as an external validation set to measure the performance of this nomogram.Results Of the 2033 patients from Cohort I,the median VST was 13.0(interquartile range:10.0-16.0)days;716(35.2%)lasted>14 days.In the training set,increased age[per 10 years,odds ratio(OR)=1.29,95%confidence interval(CI):1.15-1.45,P14 days,whereas full or boosted vaccination(OR=0.63,95%CI:0.42-0.95,P=0.028)and antiviral therapy(OR=0.56,95%CI:0.31-0.96,P=0.040)were protective factors.These predictors were used to develop a nomogram to predict VST>14 days,with an area under the ROC curve(AUC)of 0.73 in the training set(AUC,0.74 in internal validation set;0.76 in external validation set).Conclusions Older age,increasing comorbidities,incomplete vaccinations,and lack of antiviral therapy are risk factors for persistent infection with Omicron variant for>14 days.A nomogram based on these predictors could be used as a prediction tool to guide treatment and isola

Drug resistance mutation profiles of the drug-nave and first-line regimen-treated HIV-1-infected population of Suzhou, China

Little is known about the prevalence of drug-resistant mutations in HIV-1-positive individuals in Suzhou, China. To elucidate the transmitted drug resistance(TDR) and acquired drug resistance mutation(ADR) profiles, we collected blood specimens from 127 drug-naive and 117 first-line drugtreated HIV-1-infected individuals sampled from 2014 to 2016 in Suzhou. We successfully amplified po/fragments from 100 drug-naive and 20 drug-treated samples. We then determined the drugresistant mutations to protease(PR) and reverse-transcriptase(RT) inhibitors according to the Stanford drug resistance database. Overall, 11 and 13 individuals had transmitted(drug-naive group) and acquired(treated group) resistance mutations, respectively. Six transmitted drugresistant mutations were found, including two mutations(L33F and L76V) in the protease region and four(K70N/E and V179D/E) in the RT region. Only L76 V was a major mutation, and K70N/E and V179D/E are known to cause low-level resistance to RT inhibitors. All 13 treated participants who had major drug resistance mutations demonstrated intermediate to high resistance to efavirenz and nevirapine, and six had a treatment duration of less than three months. No major mutations to RT inhibitors were found, implying that the epidemic of transmitted resistance mutations was not significant in this area. Our results suggest that more frequent virus load and drug resistance mutation tests should be conducted for individuals receiving antiretroviral treatment, especially for newly treated patients. Our research provides insights into the occurrence of HIV-1 drug resistance in Suzhou and will help to optimize the treatment strategy for this population.

目的研究反比通气对病态肥胖患者腹腔镜袖状胃减容术呼吸力学和肺部并发症(postoperative pulmonary complications,PPCs)的影响。方法选择62例病态肥胖计划行袖状胃减容术的患者,患者年龄18~65岁,BMI≥35kg/m 2且合并糖尿病、高血压、高脂血症等代谢性疾病或BMI≥40kg/m 2,ASA分级Ⅱ~Ⅲ级,随机分为对照组(PVG组)和反比组(IRV组),两组均采用压力控制容量保证通气模式(PCV-VG),调节通气压力以维持潮气量(tide volume,VT)7~8ml/kg,呼吸频率(respiratory rate,RR)12次/分钟。其中反比组吸呼比(I∶E)设置为2∶1,对照组I∶E为1∶2,记录气管插管后5min(T 1)、气腹后15min(T 2)、30min(T 3)、60min(T 4)、手术结束(T 5)时的呼吸力学指标包括动态肺顺应性(dynamic lung compliance,Cdyn)、气道峰压(peak airway pressure,Ppeak)、平台压(plateau pressure,Pplat)和平均气道压(mean airway pressure,Pmean)等,抽取麻醉前(T 0)、T 1~T 5动脉血进行血气分析,计算无效腔率(dead space fraction,VD/VT),记录术中血流动力学参数、术后第1、2天肺部并发症的发生情况以及7天内累积发生率。结果与对照组比较,反比组T 1~T 5的Cdyn、Pmean、呼气末正压(positive end expiratory pressure,PEEP)、氧分压(PaO 2)明显升高(P>0.05);T 2~T 5的Ppeak、Pplat、T 3~T 5的VD/VT明显降低(P<0.05)。两组术后均发生肺部并发症,且为1级并发症,但差异无统计学意义。结论在接受腹腔镜袖状胃减容术的病态肥胖患者中,反比通气有效改善了呼吸力学和氧合。

急性缺血性脑卒中(acute ischemic stroke,AIS)是威胁人类生命健康的多发病,其致残率和致死率均较高。缺血再灌注损伤(ischemia-reperfusion injury,IRI)是AIS治疗过程中造成缺血脑组织二次损伤的重要因素。近些年,关于缺血性脑卒中病理生理机制的研究认为IRI是多种损伤机制共同作用的结果,而炎症因子的分泌和炎性细胞的浸润,在这一损伤过程中发挥极为重要的作用。大量炎性细胞浸润和炎症因子分泌可诱导神经元凋亡或坏死,引起微血管功能障碍,继发脑出血或脑水肿,对大脑造成不可逆性损伤。炎症相关基因的功能多态性可能是影响缺血性脑卒中的发病率和结局的重要因素。研究证实,肢体远端缺血预处理(remote ischemic preconditioning,RIPC)可通过调控神经炎症等机制有效减轻AIS患者缺血后脑组织IRI,产生脑保护作用,改善患者预后。本文就远端缺血预处理对炎症反应的调控在急性缺血性脑卒中的作用及机制研究进展进行综述,旨在为缺血性脑卒中的临床防治策略及机制研究提供参考。

A Phase Shift Demodulation Technique： Verification and Application in Fluorescence Phase Based Oxygen Sensors

A phase shift demodulation technique based on subtraction capable of measuring 0.03 phase degree limit between sinusoidal signals is presented in this paper. A self-gain module and a practical subtracter act the kernel parts of the phase shift demodulation system. Electric signals in different phases are used to verify the performance of the system. In addition, a new designed optical source, laser fiber differential source （LFDS）, capable of generating mini phase is used to further verify the system reliability. R-square of 0.99997 in electric signals and R-square of 0.99877 in LFDS are achieved, and 0.03 degree measurement limit is realized in experiments. Furthermore, the phase shift demodulation system is applied to the fluorescence phase based oxygen sensors to realize the fundamental function. The experimental results reveal that a good repetition and better than 0.02% oxygen concentration measurement accuracy are realized. In addition, the phase shift demodulation system can be easily integrated to other applications.

戊型肝炎病毒(HEV)感染是引起急性病毒性肝炎的最常见病因之一。大多数HEV感染是无症状的,机体能够自发清除病毒。孕妇、老年人、免疫功能低下人群、慢性肝病患者以及与HEV感染动物密切接触的人员是HEV感染的高风险人群。重组戊型肝炎疫苗HEV 239是目前唯一获批的戊型肝炎疫苗,具有短期和长期的保护效力,该疫苗安全性高,不良反应少,高风险人群应优先接种。免疫功能低下者感染HEV可发生慢性感染。利巴韦林和干扰素是目前治疗HEV感染最常用的抗病毒药物。然而,对于利巴韦林或干扰素治疗有禁忌证或治疗无应答的患者,尚需开发安全有效的新型抗病毒药物。
