The large language model called ChatGPT has drawn extensively attention because of its human-like expression and reasoning *** this study,we investigate the feasibility of using ChatGPT in experiments on translating radiology reports into plain language for patients and healthcare providers so that they are educated for improved *** reports from 62 low-dose chest computed tomography lung cancer screening scans and 76 brain magnetic resonance imaging metastases screening scans were collected in the first half of February for this *** to the evaluation by radiologists,ChatGPT can successfully translate radiology reports into plain language with an average score of 4.27 in the five-point system with 0.08 places of information missing and 0.07 places of *** terms of the suggestions provided by ChatGPT,they are generally relevant such as keeping following-up with doctors and closely monitoring any symptoms,and for about 37%of 138 cases in total ChatGPT offers specific suggestions based on findings in the *** also presents some randomness in its responses with occasionally over-simplified or neglected information,which can be mitigated using a more detailed ***,ChatGPT results are compared with a newly released large model GPT-4,showing that GPT-4 can significantly improve the quality of translated *** results show that it is feasible to utilize large language models in clinical education,and further efforts are needed to address limitations and maximize their potential.

Shiga toxin producing Escherichia coli(STEC)outbreak is a public health concern as it can potentially cause a variety of clinical manifestations including diarrhea,hemorrhagic colitis and hemolytic uremic syndrome(HUS).However *** are generally innocuous commensal organisms,and there is a need to discriminate pathogenic from non-pathogenic isolates rapidly and *** this study,we have used standard culture based methods and advanced molecular approaches to characterize *** in food in a local outbreak *** show that the application of DNA based detection methods including real-time PCr and DNA microarray along with a traditional culture method can identify the organism implicated in an outbreak at the strain level for pathogenic potential.

Chromatoprobe as a sample-sparing technique for residual solvent analysis of drug discovery candidates by gas chromatography

In drug discovery research, residual solvent measurement is an integral part of purity analysis for synthesis of a drug candidate before it is used for toxicity testing. This is usually carried out using gas chromatography(GC)with direct injection sample introduction. This method requires testing compounds to be soluble at high concentrations( > 50 mg/mL, usually in DMSO) to achieve acceptable sensitivity, a hurdle which is not always achievable for some samples such as cyclic peptides and oligonucleotides. To overcome the limitation associated with the direct injection approach, a new method using the Chromatoprobe thermal extraction device was developed for quantifying residual solvents of drug discovery compounds. This method not only consumes significantly less material(less than 1 mg), but also shows higher sensitivity than the direct injection *** addition, because no diluent is required with the Chromatoprobe thermal extraction, all residual solvents can be detected and measured without further method optimization. In our study, we compared data from GC residual solvent analysis using the Chromatoprobe solid sample introduction to those of the direct injection method for seven in-house samples. Our results showed a good agreement between the data from these two sample introduction methods. Thus, the Chromatoprobe sample introduction method provided a samplesparing alternative to the direct injection method for the measurement of residual solvents in drug *** method can be particularly useful for residual solvent analysis in samples that are available only in limited amounts, poorly soluble, and/or unstable in the diluents used for the direct injection method.

目的一直以来,围手术期血红蛋白监测依赖于测量血浆红细胞比容(Hct)的血气分析设备计算饱和度。测量总血红蛋白量(t Hb)的碳氧-血氧定量法,可获得对血液稀释更全面的评估。本研究旨在评估施行大心脏手术的住院患者中用碳氧-血氧定量法检测的t Hb和用电导率检测的Hct,与红细胞(red blood cell,rBC)输注、住院时间(length of stay,LOS)和住院费用的相关性。方法回顾性分析2014年1月至2016年6月行冠状动脉旁路移植术(CABG)和/或瓣膜置换(Vr)术患者的临床资料,使用Med Assets出院数据。患者按照检测方式(t Hb和Hct)进行分组,采用详细的账单记录和当代操作术语编码。根据医院特定的成本费用比来计算住院费用。采用多变量logistic回归法来确定rBC输注和资源利用的重要驱动因素。结果本研究纳入18 169例心血管手术患者。其中Hct监测患者占66%,并更有可能进行CABG+Vr双重手术(10.4%vs.8.9%,P=0.006 9)。在控制了患者和医院特征及患者合并症后,Hct组较t Hb组有明显更高的rBC输注风险(Or=1.26,95%CI 1.15~1.38,Prr=1.08,Prr=1.15,PrBC输注和住院费用、缩短住院时间。

Hereditary hemorrhagic telangiectasia(HHT) is arelatively common inherited vascular disorder that was first described in 1864, and is notable for epistaxis, telangiectasia, and arterial venous malformations. While genetic tests are available, the diagnosis remains clinical, and is based on the Curacao criteria. Patients with HHT are at increased risk for both bleeding and clotting events. Because of these competing complications, hematologists are often faced with difficult clinical decisions. While the majority of management decisions revolve around bleeding complications, it is not infrequent for these patients to require anticoagulation for thrombosis. Any anticoagulation recommendations must take into account the bleeding risks associated with HHT. recent reviews have found that HHT patients can be safely anticoagulated, with the most frequent complication being worsened epistaxis. Large clinical trials have shown that factor Ⅱa and Ⅹa inhibitors have less intracranial bleeding than warfarin, and basic coagulation research has provided a possible mechanism. This article describes the anticoagulation dilemma posed when a 62-year-old female patient with a history of bleeding events associated with HHT was diagnosed with a pulmonary embolism. The subsequent discussion focuses on the approach to anticoagulation in the HHT patient, and addresses the role of the new oral anticoagulants.

Our purpose in this study was to assess the dosimetric impact of the Acuros XB algorithm (AXB), in comparison with Anisotropic Analytical Algorithm (AAA) calculations, for esophageal cancer treatment plans created with rapidArc technique. First, we performed a phantom study by comparing the percent depth dose (PDD) calculated by AXB and AAA against the measured PDD in a slab phantom containing a 2 cmair gap thickness. Second, we performed a clinical study using a computed tomography (CT) data set from 10 esophageal cancer patients. The treatment plans calculated by AXB and AAA were evaluated for planning target volume (PTV) coverage, doses to the PTV and organs at risk (OArs). Dose calculations by the AXB and AAA were done for identical beam parameters. The AXB showed better agreement (within ±0.5%) with measurements than did the AAA (?4.9% to ?6.2%). In comparison to the AAA, the AXB predicted a higher maximum PTV dose (2.0%), but lower mean (1.1%) and minimum (2.5%) PTV doses as well as reduced PTV coverage (9.1%). The averaged mean doses to all OArs predicted by the AXB were lower (up to 3.6%), and the percentage of lungs volume receiving at least 20 and 5 Gy were lower by about 3.6% in the AXB plans compared to the AAA plans. The AXB is more accurate than the AAA for dose predictions when air medium is involved. The use of AXB is more likely to avoid dose overestimation or underestimation for the esophageal cancer treatment plans compared to AAA.
