The earliest evidence for a microblade adaptation in the remote,high altitude regions of the Tibetan Plateau

Microblade assemblages are among the most common prehistoric archaeological materials found on the Tibetan Plateau(TP)and are thought to indicate large scale migration to and settlement of the *** microblade sites,however,have been systematically excavated,especially in the remotest,highest-elevation regions of the *** timing of the large-scale arrival,spread,and permanent settlement of people on the TP therefore remains *** this paper,we report on a recently excavated site,Locality 3 of the Nwya Devu Site(ND3),located at 4600 meters above sea level(masl),near the shore of Ngoin Lake,on the interior *** analyses reveal a fairly typical microblade technological orientation and two types of microblade cores:wedge-shaped and semi-conical,which are similar to those found throughout North *** Optically Stimulated Luminescence(OSL)dating and AMS^(14)C dating,the age of ND3 ranges from 11 to 10 *** date range indicates ND3 is the oldest microblade site yet recorded in the remote,high-elevation regions of the TP and thus provides important information about when and how hunter-gatherers using microblades began exploiting the higher altitudes of the *** together,studies at ND3 and throughout the TP suggest that a microblade adaptation is associated with the first prolonged human occupation of the plateau and that microblades played a significant role in mediating the risks and facilitating the mobility necessary to permanently inhabit the TP.

细石叶技术产品是青藏高原发现最丰富的史前人类文化遗物,代表了早期人类在高原的大规模迁徙.然而,经过系统发掘、具有可靠年代数据的细石叶技术遗址在青藏高原发现较少,高原腹地更为缺乏,这对理解细石叶技术人群在高原大规模迁徙扩散的时间和过程带来了诸多障碍.本文研究报道了青藏高原腹地海拔4600m处新发现的细石叶技术遗址——尼阿底遗址第3地点.技术-类型学分析结果显示,尼阿底遗址第3地点细石叶的生产主要包括楔形石核和半锥形石核两种技术,与旧石器时代晚期中国华北地区的细石叶技术特征接近.光释光测年(OSL)和碳十四测年(^(14)C)的结果表明,尼阿底遗址第3地点的年代为11000~10000a BP.尼阿底遗址第3地点作为目前青藏高原腹地发现的最早的细石叶技术遗址,为了解拥有细石叶技术的狩猎采集者到达高原腹地的时间及其可能的扩散路线提供了重要信息.另外,通过对整个高原细石叶技术发展的分析,结合分子生物学的成果对比显示,细石叶技术人群迁移进入高原可能与人类对高原的长期占领以及现代藏族人的形成有密切联系.

Intensive care unit complications and outcomes of adult patients with hemophagocytic lymphohistiocytosis: A retrospective study of 16 cases

AIM To study the management, complications and outcomes of adult patients admitted with hemophagocytic lymphohistiocytosis(HLH) in the intensive care unit(ICU).METHODS We performed a retrospective observational study of adult patients with the diagnosis of "HLH" admitted to the two academic medical ICUs of Baylor College of Medicine between 01/01/2013 to 06/30/2017. HLH was diagnosed using the HLH-2004 criteria proposed by the Histiocyte *** Sixteen adult cases of HLH were admitted to the medical ICUs over 4 *** age of presentation was 49 years and 10(63%) were males. Median Sequential Organ Failure Assessment(SOFA) score at the time of ICU admission was 10. Median ICU length of stay(LOS) was 11.5 d and median hospital LOS was 29 d. Septic shock and acute respiratory failure accounted for majority of diagnoses necessitating ICU admission. Septic shock was the most common ICU complication seen in(88%) patients, followed by acute kidney injury(81%) and acute respiratory failure requiring mechanical ventilation(75%). Nine patients(56%) developed disseminated intravascular coagulation and eight(50%) had acute liver failure. 10 episodes of clinically significant bleeding were *** system organ failure was the most common cause of death seen in 12(75%)patients. The 30 d mortality was 37%(6 cases) and 90 d mortality was 81%(13 cases). There was no difference in mortality based on age(above or less than 50 years), SOFA score on ICU admission(more than or less than 10),immunosuppression, time to diagnose HLH or direct ICU admission versus floor *** HLH is a devastating disease associated with poor outcomes in ICU. Intensivists need to have a high degree of clinical suspicion for HLH in patients with septic shock/multi system organ failure and progressive bi/pancytopenia who are not responding to standard management in ICU.

Abdominal aortic aneurysm(AAA)is a progressive aortic dilatation,causing~80%mortality upon ***,there is no approved drug therapy for *** repairs are invasive and risky and thus not recommended to patients with small AAAs which,however,account for~90%of the newly diagnosed *** is therefore a compelling unmet clinical need to discover effective non-invasive strategies to prevent or slow down AAA *** contend that the first AAA drug therapy will only arise through discoveries of both effective drug targets and innovative delivery *** is substantial evidence that degenerative smooth muscle cells(SMCs)orchestrate AAA pathogenesis and *** this study,we made an exciting finding that PERK,the endoplasmic reticulum(ER)stress Protein Kinase R-like ER Kinase,is a potent driver of SMC degeneration and hence a potential therapeutic ***,local knockdown of PERK in elastase-challenged aorta significantly attenuated AAA lesions in *** parallel,we also conceived a biomimetic nanocluster(NC)design uniquely tailored to AAA-targeting drug *** NC demonstrated excellent AAA homing via a platelet-derived biomembrane coating;and when loaded with a selective PERK inhibitor(PERKi,GSK2656157),the NC therapy conferred remarkable benefits in both preventing aneurysm development and halting the progression of pre-existing aneurysmal lesions in two distinct rodent models of *** summary,our current study not only establishes a new intervention target for mitigating SMC degeneration and aneurysmal pathogenesis,but also provides a powerful tool to facilitate the development of effective drug therapy of AAA.

Transepithelial leak in Barrett's esophagus patients: The role of proton pump inhibitors

AIM: To determine if the observed paracellular sucrose leak in Barrett's esophagus patients is due to their proton pump inhibitor (PPI) use. METHODS: The in vivo sucrose permeability test was administered to healthy controls, to Barrett's patients and to non-Barrett's patients on continuous PPI therapy. Degree of leak was tested for correlation with presence of Barrett's, use of PPIs, and length of Barrett's segment and duration of PPI use. RESULTS: Barrett's patients manifested a near 3-fold greater, upper gastrointestinal sucrose leak than healthy controls. A decrease of sucrose leak was observed in Barrett's patients who ceased PPI use for 7 *** initial introduction of PPI use (in a PPI-na ve population) results in dramatic increase in sucrose leak, long-term, continuous PPI use manifested a slow spontaneous decline in leak. The sucrose leak observed in Barrett's patients showed no correlation to the amount of Barrett's tissue present in the esophagus. CONCLUSION: Although future research is needed to determine the degree of paracellular leak in actual Barrett's mucosa, the relatively high degree of leak observed with in vivo sucrose permeability measurement of Barrett's patients reflects their PPI use and not their Barrett's tissue per se .

Purpose:The COVID-19 pandemic has caused 1.4 million deaths globally and is associated with a 3-4 times increase in 30-day mortality after a fragility hip fracture with concurrent COVID-19 ***,death from COVID-19 infection occurs between 15 and 22 days after the onset of symptoms,but this period can extend up to 8 *** study aimed to assess the impact of concurrent COVID-19 infection on 120-day mortality after a fragility hip ***:A multi-centre prospective study across 10 hospitals treating 8%of the annual burden of hip fractures in England between 1st March and 30th April,2020 was *** whose surgical treatment was payable through the National Health Service Best Practice Tariff mechanism for"fragility hip fractures"were included in the ***'120-day mortality was assessed relative to their perioperative COVID-19 *** analysis was performed using SPSS version ***:A total of 746 patients were included in this study,of which 87(11.7%)were COVID-19 *** rates at 30-and 120-day were significantly higher for COVID-19 positive patients relative to COVID-19 negative patients(p<0.001).However,mortality rates between 31 and 120-day were not significantly different(p=0.107),16.1%and 9.4%respectively for COVID-19 positive and negative patients,odds ratio 1.855(95%CI 0.865-3.978).Conclusion:Hip fracture patients with concurrent COVID-19 infection,provided that they are alive at day-31 after injury,have no significant difference in 120-day *** the growing awareness and concern of "long-COVID"and its widespread prevalence,this does not appear to increase mediumterm mortality rates after a hip fracture.

Effects of Plyometric and Explosive Speed Training on Recreational Marathoners

BACKGROUND: Plyometric training has been shown to improve performance in distance running events up to 5-km, but little research has been done on this type of training for marathon (42.195-km) runners. The purpose of this study was to examine the effects of plyometric and explosive speed training (PLYO) on recreational marathoners. METHODS: Twenty-two subjects (ages 18-23), were randomized to either PLYO or core training (CORE) for a weekly session performed for 12 weeks, in addition to marathon training. Sprint, jump, and distance running performance variables were measured pre- and post-intervention. RESULTS: In the sprint tests, the PLYO group improved in both the 200-m run (P ≤ 0.001) and 60-m run (P = 0.004), and trended toward improvement in the 30-m fly (P = 0.051). The difference from CORE was significant only in the 200-m (P = 0.002). The CORE group did not change in any of the sprint or jump variables. The PLYO group was significantly different from the CORE group in the standing long jump (P = 0.024). There were no differences between groups in distance running performance. Both improved in 2-mile (3.219-km) time trial (p ≤ 0.001), VO2MAX (P = 0.026 for CORE; P = 0.002 for PLYO), and running economy (P = 0.01). CONCLUSION: Weekly PLYO training improves sprint speed and maintains jumping ability in recreational marathoners, but does not augment improvements in distance running performance.

Correlation of Urinary Engrailed-2 Levels to Tumour Volume and Pathological Stage in Men Undergoing Radical Prostatectomy

The aim of this study was to assess the relationship between pre-prostatectomy urinary Engrailed-2 (EN2), a transcription factor secreted by prostate cancer cells, with tumour volume and pathological characteristics in resected prostate specimens. First pass urine samples (10 ml) without prior prostatic massage were collected and stored at –80°C. EN2 levels were measured using an enzyme-linked immunoabsorbent assay. Tumour volume in the prostatectomy specimens was determined histologically. 57 men undergoing RP in one urological cancer network were evaluated. EN2 was detected in 85% of RP patients. EN2 correlated with tumour volume (but not total prostatic volume) in a linear regression analysis, with increasing pathological T stage and margin positivity. Using three “cutoff levels” of tumour volume (0.5 ml, 1.3 ml and 2.5 ml) to define “significant disease”, men with “significant disease” had markedly higher levels of urinary EN2 (p Levels of urinary EN2 may be useful in predicting tumour volume in men with prostate cancer by potentially identifying men with small volume “insignificant” disease. This study justifies a larger multicentre evaluation of urinary EN2 levels as a biomarker of PC significance using cancer volume, pathological and PSA criteria.

Economics of eBay’s buyer protection plan

Background:This paper explores the financial viability of eBay’s Buyer Protection ***:We explore their warranty model assuming exponential auction listing and claims times,under competing assumptions of Normal(classical portfolio theory)versus Paretian claims(industry practice)distributions using a normative ***’s model viability was analyzed under three risk metrics–mean-variance risk of classic portfolio theory;value at risk used in regulation such as the Basel Accords;and tail value at risk which is preferred by ***:Five main findings of the research are:(1)long-term financial viability is set by boundary conditions that can be managed in eBay’s contract terms,(2)commonly used but simplistic assumptions of Gaussian distributions of contract failures can be misleading,and keeping the Buyer Protection Plan viable demands more realistic distributions,(3)value at risk measures provide more information about viability than expectations,(4)tail value at risk measures an lead to sudden and extreme variations in fund value over the parameter range and flawed contracting decisions,and(5)tail value at risk is the preferable risk measure for assessing Buyer Protection Plan viability,but it needs to be used with an assumption of Lomax contract failures to support profitable ***:Our analysis showed substantial improvements in claims management when tail value at risk measures were used instead of conventional VaR *** implies that current strategies,and prices for coverage may result in losses to eBay,in current practice.

US Resident Perceptions of Dairy Cattle Management Practices

As public perception continues to shape the dairy industry, an understanding of consumer views and purchasing behaviors is critically important. The objectives of this paper are to: 1) summarize consumer perceptions or views of 12 common dairy cattle management practices (humane methods of slaughter, humane transportation, increased pen stall or size, access to pasture, feeding of a GMO-free diet, feeding of an organic diet, feeding of a diet the animal would naturally consume, ability to interact with other animals of the same species, access to fresh water at all times, tail docking, use of antibiotics for sick animals, and dehorning), and 2) analyze relationships between respondent’s household demographic characteristics and perceptions of three specific contentious dairy practices, including antibiotic use, tail docking, and dehorning. An online, national survey was used to collect data on household demographics, dairy consumption and purchasing behavior, and perceptions of dairy production practices from 1201 US residents. The findings of this study indicate that 87% of participants consumed dairy products. Of these respondents, 12% had altered their consumption of dairy products over the past three years because of animal welfare concerns and 10% had done so due to food safety concerns. Respondents perceived tail docking and dehorning to have the least beneficial and most negative implications for dairy cattle welfare of the dairy production practices considered.
