ABCD^(2) risk score does not predict the presence of cerebral microemboli in patients with hyper-acute symptomatic critical carotid artery stenosis

Introduction:ABCD^(2) risk score and cerebral microemboli detected by transcranial Doppler(TCD)have been separately shown to the predict risk of recurrent acute *** studied whether ABCD2 risk score predicts cerebral microemboli in patients with hyper-acute symptomatic carotid artery *** and methods:We studied 206 patients presenting within 2 weeks of transient ischaemic attack or minor stroke and found to have critical carotid artery stenosis(≥50% ).86 patients(age 70±1(SeM:years),58 men,83 Caucasian)had evidence of microemboli;72(84% )of these underwent carotid endarterectomy(CeA).120 patients(age 72±1 years,91 men,113 Caucasian)did not have microemboli detected;102(85% )of these underwent *** were analysed using X2 and Mann–Whitney U tests and receiver operating characteristic(ROC)***:140/206(68% :95% CI 61.63 to 74.37)patients with hyper-acute symptomatic critical carotid stenosis had an ABCD2 risk score≥*** was no significant difference in the NICe red flag criterion for early assessment(ABCD^(2) risk score≥4)for patients with cerebral microemboli versus those without microemboli(59/86 vs 81/120 patients:OR 1.05 ABCD2 risk score≥4(95% CI 0.58 to 1.90,p=0.867)).The ABCD2 risk score wasembolising patients and in 39 of 120(31% :95% CI 23 to 39)without cerebral *** adjusting for pre-neurological event antiplatelet treatment(APT),area under the curve(AUC)of ROC for ABCD2 risk score showed no prediction of cerebral microemboli(no pre-event APT,n=57:AUC 0.45(95% CI 0.29 to 0.60,p=0.531);preevent APT,n=147:AUC 0.51(95% CI 0.42 to 0.60,p=0.804)).Conclusions:The ABCD2 score did not predict the presence of cerebral microemboli or carotid disease in over one-quarter of patients with symptomatic critical carotid artery *** the basis of NICe guidelines(refer early if ABCD2≥4),assessment of high stroke risk based on ABCD2 scoring may lead to inappropriate delay in urgent treat

background Cerebral microemboli may lead to ischaemic neurological complications after carotid endarterectomy(CeA).The association between classical cardiovascular risk factors and acute cerebral microemboli following carotid surgery has not been *** aim of this study was to explore whether an established cardiovascular risk score(Pocock score)predicts the presence of cerebral microemboli acutely after *** and methods Pocock scores were assessed for the 670 patients from the Carotid Surgery Registry(age 71±1(SeM)years,474(71%)male,652(97%)Caucasian)managed from January 2002 to December 2012 in the Regional Vascular Centre at University hospitals Coventry and Warwickshire NhS Trust,which serves a population of *** was undertaken in 474(71%)patients for symptomatic carotid stenosis and in 196(25%)asymptomatic patients during the same period.74% of patients were hypertensive,71%were smokers and 49% had *** A high Pocock score(≥2.3%)was significantly associated with evidence of cerebral microemboli acutely following CeA(P=0.039,Mann-Whitney(MW)test).A Pocock score(≥2.3%)did not predict patients who required additional antiplatelet therapy(microemboli signal(MeS)rate>50 hour-1:P=0.164,MW test).Receiver operating characteristic analysis also showed that the Pocock score predicts acute postoperative microemboli(area under the curve(AUC)0.546,95%CI 0.502 to 0.590,P=0.039)but not a high rate of postoperative microemboli(MeS>50 hour−1:AUC 0.546,95% CI 0.482 to 0.610,P=0.164).A Pocock score≥2.3% showed a sensitivity of 74% for the presence of acute postoperative cerebral microemboli.A Pocock score≥2.3% also showed a sensitivity of 77% and a negative predictive value of 90% for patients who developed a high microembolic rate>50 hour−1 after carotid *** These findings demonstrate that the Pocock score could be used as a clinical tool to identify patients at high risk of developing acute postoperative microemboli.

Antimicrobial resistance(AMR)poses a critical threat to global health and development,with environmental factors—particularly in urban areas—contributing significantly to the spread of antibiotic resistance genes(ARGs).however,most research to date has been conducted at a local level,leaving significant gaps in our understanding of the global status of antibiotic resistance in urban *** address this issue,we thoroughly analyzed a total of 86,213 ARGs detected within 4,728 metagenome samples,which were collected by the Meta SUB International Consortium involving diverse urban environments in 60 cities of 27 countries,utilizing a deep-learning based *** findings demonstrated the strong geographical specificity of urban environmental resistome,and their correlation with various local socioeconomic and medical *** also identified distinctive evolutionary patterns of ARG-related biosynthetic gene clusters(BGCs)across different countries,and discovered that the urban environment represents a rich source of novel *** study provides a comprehensive overview of the global urban environmental resistome,and fills a significant gap in our knowledge of large-scale urban antibiotic resistome analysis.

Compared to reforming reactions using hydrocarbons,ethanol steam reforming(eSR)is a sustainable alternative for hydrogen(h_(2))production since ethanol can be produced sustainably using *** work explores the catalyst design strategies for preparing the Ni supported on ZSM-5 zeolite catalysts to promote ***,two-dimensional ZSM-5 nanosheet and conventional ZSM-5 crystal were used as the catalyst carriers and two synthesis strategies,i.e.,in situ encapsulation and wet impregnation method,were employed to prepare the *** on the comparative characterization of the catalysts and comparative catalytic assessments,it was found that the combination of the in situ encapsulation synthesis and the ZSM-5 nanosheet carrier was the effective strategy to develop catalysts for promoting h_(2) production via eSR due to the improved mass transfer(through the 2-D structure of ZSM-5 nanosheet)and formation of confined small Ni nanoparticles(resulted via the in situ encapsulation synthesis).In addition,the resulting ZSM-5 nanosheet supported Ni catalyst also showed high Ni dispersion and high accessibility to Ni sites by the reactants,being able to improve the activity and stability of catalysts and suppress metal sintering and coking during eSR at high reaction ***,the Ni supported on ZSM-5 nanosheet catalyst prepared by encapsulation showed the stable performance with~88% ethanol conversion and~65% h_(2) yield achieved during a 48-h longevity test at 550-C.

The combination of urbanization and global warming leads to urban overheating and compounds the frequency and intensity of extreme heat events due to climate ***,the risk of urban overheating can be mitigated by urban green-blue-grey infrastructure(GBGI),such as parks,wetlands,and engineered greening,which have the potential to effectively reduce summer air *** many reviews,the evidence bases on quantified GBGI cooling benefits remains partial and the practical recommendations for implementation are *** systematic literature review synthesizes the evidence base for heat mitigation and related co-benefits,identifies knowledge gaps,and proposes recommendations for their implementation to maximize their *** screening 27,486 papers,202 were reviewed,based on 51 GBGI types categorized under 10 main *** GBGI(green walls,parks,street trees)have been well researched for their urban cooling ***,several other GBGI have received negligible(zoological garden,golf course,estuary)or minimal(private garden,allotment)*** most efficient air cooling was observed in botanical gardens(5.0±3.5℃),wetlands(4.9±3.2℃),green walls(4.1±4.2℃),street trees(3.8±3.1℃),and vegetated balconies(3.8±2.7℃).Under changing climate conditions(2070–2100)with consideration of RCP8.5,there is a shift in climate subtypes,either within the same climate zone(e.g.,Dfa to Dfb and Cfb to Cfa)or across other climate zones(e.g.,Dfb[continental warm-summer humid]to BSk[dry,cold semi-arid]and Cwa[temperate]to Am[tropical]).These shifts may result in lower efficiency for the current GBGI in the *** the importance of multiple services,it is crucial to balance their functionality,cooling performance,and other related co-benefits when planning for the future *** global GBGI heat mitigation inventory can assist policymakers and urban planners in prioritizing effective interventions to reduce the risk of urban overheating,fi

The shuttle effect induced by soluble lithium polysulfides(LiPSs)is known as one of the crucial issues that limit the practical applications of lithium-sulfur(Li-S)***,a titanium dioxide nanoparticle embedded in nitrogen-doped porous carbon nanofiber(TiO_(2)@NCNF)composite is constructed via an interface-induced polymerization strategy to serve as an ideal sulfur *** the protection of the nanofiber walls,the uniformly dispersed TiO_(2) nanocrystalline can act as capturing centers to constantly immobilize LiPSs towards durable sulfur ***,the mesoporous microstructure in the fibrous framework endows the TiO_(2)@NCNF host with strong physical reservation for sulfur and LiPSs,sufficient pathways for electron/ion transfer,and excellent endurance for volume *** expected,the sulfur-loaded TiO_(2)@NCNF composite electrode presents a fabulous rate performance and long cycle lifespan(capacity fading rate of 0.062%per cycle over 500 cycles)at 2.0 ***,the assembled Li-S batteries harvest superb areal capacity and cycling stability even under high sulfur loading and lean electrolyte conditions.

Thermal energy management in metal-organic frameworks(MOFs)is an important,yet often neglected,challenge for many adsorption-based applications such as gas storage and *** its importance,there is insufficient understanding of the structure-property relationships governing thermal transport in *** provide a data-driven perspective into these relationships,here we perform large-scale computational screening of thermal conductivity k in MOFs,leveraging classical molecular dynamics simulations and 10,194 hypothetical MOFs created using the ToBaCCo 3.0 *** found that high thermal conductivity in MOFs is favored by high densities(>1.0 g cm^(−3)),small pores(ected metal *** also found that 36 MOFs exhibit ultra-low thermal conductivity(hich is primarily due to having extremely large pores(~65Å).Furthermore,we discovered six hypothetical MOFs with very high thermal conductivity(>10 W m^(−1) K^(−1)),the structures of which we describe in additional detail.

Objectives:Determine variability in intra-and post-operative management of tracheostomies(trachs)at our institution as existing literature suggests that trachs are a frequent trigger for airway-related *** the development of an institution-wide protocols for trach ***:A 39-question online survey was sent to 55 providers who perform open and percutaneous trachs at three of the hospitals within our large,urban,academic medical *** providers were identified by surveillance of the operating room schedules for 1 ***:The survey was completed by 40 of the 53 eligible providers(75.5%).Response rate by question *** included members of all departments that perform trachs at our institution(Otorhinolaryngology,Trauma Surgery,Thoracic Surgery,General Surgery,Cardiovascular Surgery and Interventional Pulmonology).While most responses demonstrated uniformity in practice,notable variations included the following:80%of percutaneous trach providers stated that morbid obesity was not a contraindication to performing a trach outside of the operating room(n=20)while 58%of open trach providers stated that morbid obesity was a contraindication;only 35%of open trach providers perform a Bjork flap(n=350).The survey also identified significant variability in practice with regards to timing of trach suture ***:Lack of uniformity was identified in several practices related to intra-and post-operative tracheostomy *** did,however,trend toward consensus in many *** results are being used to establish a more consistent approach to tracheostomy management across our institution to ensure standardization of practice amidst the rapidly evolving practices of trach *** for practice:With ongoing evolution in the methods of trach placement and its management,the concepts put forth here will be a resource for health care providers at other institutions to consider intra-institutional analysis and

Repeated hydrogen peroxide dosing briefly reduces cyanobacterial blooms and microcystin while increasing fecal bacteria indicators in a eutrophic pond

This study explored the effects of h_(2)O_(2)on Cyanobacteria and non-target microbes using fluorometry,microscopy,flow cytometry,and high throughput DNA sequencing of the 16S rRNA gene during a series of mesocosm and whole-ecosystem experiments in a eutrophic pond in NY,*** addition of h_(2)O_(2)(8 mg/L)significantly reduced Cyanobacteria concentrations during a majority of experiments(66%;6 of 9)and significantly increased eukaryotic green and unicellular brown algae in 78%and 45%of experiments,*** heterotrophic bacteria declined significantly following h_(2)O_(2)addition in all experiments,bacteria indicative of potential fecal contamination(escherichia coli,enterococcus,fecal coliform bacteria)consistently and significantly increased in response to h_(2)O_(2),evidencing a form of‘pollution swapping’.h_(2)O_(2)more effectively reduced Cyanobacteria in enclosed mesocosms compared to whole-ecosystem *** whole-pond h_(2)O_(2)applications over a twoyear period temporarily reduced cyanobacterial levels but never reduced concentrations below bloom thresholds and populations always rebounded in two weeks or *** bacterial phyla of Cyanobacteria,Actinobacteria,and Planctomycetes were the most negatively impacted by h_(2)O_(2).Microcystis was always reduced by h_(2)O_(2),as was the toxin microcystin,but Microcystis remained dominant even after repeated h_(2)O_(2)*** h_(2)O_(2)favored the growth of eukaryotic algae over potentially harmful Cyanobacteria,the inability of h_(2)O_(2)to end cyanobacterial blooms in this eutrophic waterbody suggests it is a non-ideal mitigation approach in high biomass ecosystems and should be used judiciously due to potential negative impacts on non-target organisms and promotion of bacteria indicative of fecal contamination.
