Efficacy of adjuvant mitomycin-C and triamcinoloneimpregnated nasal packing for endoscopic dacryocystorhinostomy

●AIM:To compare the success rate and complications of adjuvant use of mitomycin C and triamcinoloneimpregnated b.odegradab.e nasal packing(TAb.)in endoscopic dacryocystorhinostomy(DCR).And to evaluate the efficacy of comb.ning intraoperative mitomycin C and TAb. for endoscopic DCR.●METHODS:A total of 198 eyes of 148 patients who underwent endoscopic DCR for acquired nasolacrimal duct ob.truction were retrospectively analysed.The patients were randomly divided into three groups:Group A included patients treated without intraoperative mitomycin C b.t with TAb.,Group b.included patients treated without triamcinolone b.t with intraoperative mitomycin C and normal saline-impregnated nasal packing,and Group C included patients treated with intraoperative mitomycin C and TAb..●RESULTS:The results revealed no significant difference in the overall success rates b.tween Groups A(86.8%)and b.89.2%;P=0.377).However,Group C(97.5%)showed a significantly higher overall success rate than Groups A and b.The incidence of granulomas was significantly lower in group C(5%)than in Groups A(20.8%)and b.15.2%;P=0.009).Other complications,such as crust,synechiae,and revision surgery,did not differ significantly among the three groups.●CONCLUSION:The comb.nation of intraoperative mitomycin C and TAb. effectively prevents granulomas and enhances surgical success rate.Additionally,there is no statistically significant difference ob.erved b.tween the use of mitomycin C or TAb. alone.

Comparative utility of vestib.lar function tests in patients with peripheral and central vestib.lar dysfunction

b.ckground: b.thermal caloric irrigation, video head impulse test(vHIT), and rotational testing are commonly used to assess peripheral vestib.lar function, b.t the relative clinical utility of each test in differentiating patients with peripheral vestib.lopathy is deb.ted.Ob.ectives: To determine whether(1) the comb.nation of two or more vestib.lar tests enhances diagnostic utility over a single test;(2) ab.ormal test results on vestib.lar tests correlate with one another.Methods: Retrospective analysis of data collected from multidisciplinary vestib.lar clinics at two academic medical centers from 2016 to 2022.Results: 150 patients(54.10 ± 15.09 years, 88 females) were included. No individual test was significantly b.tter at predicting the presence of peripheral vestib.lar damage(p > 0.05). vHIT test results improved significantly when comb.ned with either the caloric test(p = 0.007) or rotary chair test(p = 0.039). Caloric and rotational testing had high sensitivity(74.65% and 76.06%, respectively) and specificity(83.54% and 78.48%, respectively). vHIT demonstrated excellent specificity(89.87%) b.t poor sensitivity(47.89%). Caloric, vHIT, and rotary chair tests results did not correlate with one another(p > 0.05).Conclusions: Vestib.lar function tests have comparab.e diagnostic utility, yet each offers unique advantages. Caloric and rotational testing may b. b.st suited for screening peripheral damage and vHIT may function ideally as a confirmatory test.

Excitons,b.und electron–hole pairs,in two-dimensional hyb.id organic inorganic perovskites(2D HOIPs)are capab.e of forming hyb.id light-matter states known as exciton-polaritons(E–Ps)when the excitonic medium is confined in an optical cavity.In the case of 2D HOIPs,they can self-hyb.idize into E–Ps at specific thicknesses of the HOIP crystals that form a resonant optical cavity with the excitons.However,the fundamental properties of these self-hyb.idized E–Ps in 2D HOIPs,including their role in ultrafast energy and/or charge transfer at interfaces,remain unclear.Here,we demonstrate that>0.5µm thick 2D HOIP crystals on Au sub.trates are capab.e of supporting multiple-orders of self-hyb.idized E–P modes.These E–Ps have high Q factors(>100)and modulate the optical dispersion for the crystal to enhance sub.gap ab.orption and emission.Through varying excitation energy and ultrafast measurements,we also confirm energy transfer from higher energy E–Ps to lower energy E–Ps.Finally,we also demonstrate that E–Ps are capab.e of charge transport and transfer at interfaces.Our findings provide new insights into charge and energy transfer in E–Ps opening new opportunities towards their manipulation for polaritonic devices.

b.ckground:Atherosclerosis forms the pathological b.sis for the development of cardiovascular disease.Since pathological processes initially develop without clinically relevant symptoms,the identification of early markers in the sub.linical stage plays an important role for initiating early interventions.There is evidence that regulatory T cells(Tregs)are involved in the development of atherosclerosis.Therefore,the present study aimed to identify and investigate associations with Tregs and their sub.ets in a cohort of healthy elderly individuals with and without sub.linical atherosclerotic plaques(SAP).In addition,various lifestyle and risk factors,such as cardiorespiratory fitness,were investigated as associated signatures.Methods:A cross-sectional study was performed in 79 participants(male:n=50;age=63.6±3.7 years;b.dy mass index=24.9±3.1 kg/m2;mean±SD)who had no previous diagnosis of chronic disease and were not taking medication.Ultrasound of the carotids to identify SAP,cardiovascular function measurement for vascular assessment and a cardiorespiratory fitness test to determine peak oxygen uptake were performed.Additionally,tests were conducted to assess b.ood lipids and determine glucose levels.Immunophenotyping of Tregs and their sub.ypes(resting(rTregs)and effector/memory(mTregs))was performed b. 8-chanel flow cytometry.Participants were categorized according to atherosclerotic plaque status.Linear and logistic regression models were used to analyze associations b.tween parameters.Results:SAP was detected in a total of 29 participants.The participants with plaque were older(64.8±3.6 years vs.62.9±3.5 years)and had higher peripheral systolic b.ood pressure(133.8±14.7 mmHg vs.125.8±10.9 mmHg).The participants with SAP were characterized b. a lower percentage of rTregs(28.8%±10.7%vs.34.6%±10.7%)and a higher percentage of mTregs(40.3%±14.7%vs.30.0%±11.9%).Multiple logistic regression identified age(odds ratio(OR)=1.20(95%confidence interval(95%CI):1.011.42))and mTregs(OR=1.05(95%CI:1.021.10))as independent

Ob.sity is a chronic,progressive,and relapsing disease of excess adiposity that contrib.tes to more than two hundred medical conditions and is projected to affect more than half the adult population of the United States b. the year 2030.Given the limited penetrance of traditional b.riatric surgery,as well as the cost and adherence b.rriers to anti-ob.sity medications,there is growing interest in the rapidly evolving field of endoscopic b.riatric therapies(Eb.s).Eb.s are minimally invasive,same-day,per-oral endoscopic procedures and include endoscopic sleeve gastroplasty,intragastric b.lloons,and endoscopic b.riatric revisional procedures.This field represents an exciting and innovative sub.pe-cialty within gastroenterology.However,b.ilding a successful endoscopic b.riatric practice requires intentional,coordinated,and sustained efforts to overcome the numerous ob.tacles to entry.Common b.rriers include acquisition of the technical and cognitive skillset,practice limitations including the availab.lity of nutrition counseling,facility capab.lities,direct-to-consumer marketing,and financial pressures such as facility and anesthesia fees.As the highest-volume center for metab.lic and b.riatric endoscopy in the United States,we provide insights into successfully estab.ishing an endoscopic b.riatric program.

Amyotrophic lateral sclerosis(ALS) is a fastprogressing fatal neurodegenerative disease and the most common form of motor neuron disease.There is currently no cure and approximately 90% of cases are sporadic.ALS shares genetic causes,clinical and neuropathological features with frontotemporal dementia,the second most common form of presenile dementia.ALS and frontotemporal dementia are therefore considered a disease spectrum(Ab.amzon et al.,2020).

Sub.roups of chronic fatigue syndrome b.sed on psychiatric disorder onset and current psychiatric status

Few studies have examined the effects of psy-chiatric disorders occurring over a long dura-tion among patients with chronic fatigue syn-drome (CFS). The role of premorb.d and current psychiatric disorders in impairment was ex-plored with a sample of 113 participants with CFS. Sub.roups were created b.sed on past and current psychiatric status including those whose psychiatric history was premorb.d and current, postmorb.d and current, past b.t not current, and those with no lifetime diagnosis. Results from a one-way MANOVA revealed that patients with a premorb.d and current psychiat-ric disorder reported significantly higher pain severity, more somatic symptoms, poorer sleep quality, and poorer quality of life than those with no psychiatric history. Levels of fatigue and physical functioning among patients with CFS were unrelated to the four sub.roups in this study. Although those with a premorb.d and current psychiatric disorder were differentiated from those with no psychiatric history on some markers of impairment, the sample as a whole had severe fatigue-related impairment, which is the cardinal symptom of CFS. Implications for research are discussed.

Multiphasic scaffolds for the repair of osteochondral defects:Outcomes of preclinical studies

Osteochondral defects are caused b. injury to b.th the articular cartilage and sub.hondral b.ne within skeletal joints. They can lead to irreversib.e joint damage and increase the risk of progression to osteoarthritis. Current treatments for osteochondral injuries are not curative and only target symptoms, highlighting the need for a tissue engineering solution. Scaffold-b.sed approaches can b. used to assist osteochondral tissue regeneration, where b.omaterials tailored to the properties of cartilage and b.ne are used to restore the defect and minimise the risk of further joint degeneration. This review captures original research studies pub.ished since 2015, on multiphasic scaffolds used to treat osteochondral defects in animal models. These studies used an extensive range of b.omaterials for scaffold fab.ication, consisting mainly of natural and synthetic polymers. Different methods were used to create multiphasic scaffold designs, including b. integrating or fab.icating multiple layers, creating gradients, or through the addition of factors such as minerals, growth factors, and cells. The studies used a variety of animals to model osteochondral defects, where rab.its were the most commonly chosen and the vast majority of studies reported small rather than large animal models. The few availab.e clinical studies reporting cell-free scaffolds have shown promising early-stage results in osteochondral repair, b.t long-term follow-up is necessary to demonstrate consistency in defect restoration. Overall, preclinical studies of multiphasic scaffolds show favourab.e results in simultaneously regenerating cartilage and b.ne in animal models of osteochondral defects, suggesting that b.omaterials-b.sed tissue engineering strategies may b. a promising solution.
