目的为更新1999年法国共识"麻醉中肌肉松弛药与逆转剂的应用"特召集16名专家成立共识委员会。编写开始时宣布庄重的利益声明与分歧监督原则,并在编写过程中予以执行,本指南编写全过程不依赖任何商业资助。所有作者均需遵循推荐分级的评价、制定与评估系统的原则(Grading of Recommendations,Assessment,Development and Evaluation,GRADE?),以评估这些推荐意见的证据质量,规避基于低质量证据做出强推荐的可能性。故有少数推荐意见仍未分等级。方法专家组的讨论主要集中在以下8个问题:(1)在无面罩通气困难标准的情况下,是否有必要在注射肌肉松弛药前检查面罩通气的可行性?面罩通气是否有必要使用肌肉松弛药?(2)为便于气管插管是否有必要使用肌肉松弛药?(3)是否有必要使用肌肉松弛药以便于声门上气道装置的置入及管理相关并发症?(4)是否有必要应用神经肌肉阻滞监测来进行气道管理?(5)是否有必要应用肌肉松弛药以利于手术的进行?如有必要,是哪种手术?(6)术中是否有必要监测神经肌肉阻滞程度?(7)防治肌肉松弛药残留阻滞作用的策略是什么?(8)在特殊人群中(如电休克治疗、肥胖、儿童、神经肌肉疾病、肝/肾功能衰竭、老年患者)使用肌肉松弛药和逆转剂的适应证及注意事项是什么?所有问题都采用研究对象、干预、对照和预后(Population,Intervention,Comparison and Outcome,PICO)模型中的临床问题予以阐明,并生成证据档案,最后采用GRADE?系统评估文献分析的结果并进行推荐。结果SFAR指南专家组提出了31项关于麻醉中应用肌肉松弛药与逆转剂的推荐意见。在这些推荐意见中,有11项是属于强推荐(GRADE 1±),有20项是属于弱推荐(GRADE 2±),有5项应用GRADE?系统无法给出推荐意见,其中2个问题生成了专家意见。经过2轮讨论和1次修订后,所有指南和方案都获得了专家组的一致同意。结论关于麻醉中应用肌肉松弛药与逆转剂的多项强推荐意见,专家间意见基本一致。SFAR推荐在整个麻醉过程中使用神经肌肉功能监测仪来监测神经肌肉阻滞程度。

高温环境中运动会引起体温调节和其它生理压力,继而可导致耐力运动能力的损害。本共识性声明的目的是提供最新的建议以使热环境中体育活动时的运动能力最优化。可用于降低热应激压力和优化运动能力的最重要干预方式是热习服,其应包括1~2周以上反复的运动—高温环境暴露。此外,运动员应在正常水合状态下开始比赛和训练,并将运动中的脱水最小化。随着商用降温系统（如降温背心）的发展,在高温环境中训练或比赛前,运动员可以采取降温策略来促进热的散发或提高蓄热能力。而且,赛事组织者应该设计大面积的遮阳区域,并提供降温和补水设施,按照最小化运动员的健康风险来安排赛事,尤其是在大众参与的赛事中及一年之中炎热天气开始之初。以最近的2008年奥运会和2014年国际足联世界杯为例,当比赛在高温环境中举行时,赛事主管机构应考虑在比赛中或比赛之间允许额外的（或更长的）恢复期以提供补水和降温的时机。

Macrozoospermia,characterized by the presence of largeheaded spermatozoa usually carrying several flagella,is one of the most severe phenotypes of male *** in most cases,the gametes are chromosomally abnormal and cannot be used for assisted reproductive technologies(ART)(Ray et al.,2017).In 2007,subjects from consanguineous families were shown to carry the same homozygous deleterious AURKC variant(NM_001015878.2:c.144delC,later renamed c.145delC,***49TrpfsTer23),establishing AURKC as the first and main gene associated with macrozoospermia(Dieterich et al.,2007).Using flow cytometry,all spermatozoa from patients homozygous for the c.145delC variant were shown to be tetraploid,highlighting a cytokinesis blockage of the first meiotic divisions thus confirming that ART cannot be successful for these patients(Dieterich et al.,2009).

BACKGROUND Since its complete roll-out in 2009,the French colorectal cancer screening program(CRCSP)experienced 3 major constraints[use of a less efficient Guaiac-test(gFOBT),stopping the supply of Fecal-Immunochemical-Test kits(FIT),and suspension of the program due to the coronavirus disease 2019(COVID-19)]affecting its *** To describe the impact of the constraints in terms of changes in the quality of screeningcolonoscopy(Quali-Colo).METHODS This retrospective cohort study included screening-colonoscopies performed by gastroenterologists between jan-2010 and Dec-2020 in people aged 50-74 living in Ile-de-France(France).The changes in Quali-colo(Proportion of colonoscopies performed beyond 7 mo(Colo_7 mo),Frequency of serious adverse events(SAE)and Colonoscopy detection rate)were described in a cohort of Gastroenterologists who performed at least one colonoscopy over each of the four periods defined according to the chronology of the constraints[gFOBT:Normal progress of the CRCSP using gFOBT(2010-2014);FIT:Normal progress of the CRCSP using FIT(2015-2018);STOP-FIT:Year(2019)during which the CRCSP experienced the cessation of the supply of test kits;COVID:Program suspension due to the COVID-19 health crisis(2020)].The link between each dependent variable(Colo_7 mo;SAE occurrence,neoplasm detection rate)and the predictive factors was analyzed in a two-level multivariate hierarchical *** The 533 gastroenterologists(cohort)achieved 21509 screening colonoscopies over gFOBT period,38352 over FIT,7342 over STOP-FIT and 7995 over COVID *** frequency of SAE did not change between periods(gFOBT:0.3%;FIT:0.3%;STOP-FIT:0.3%;and COVID:0.2%;P=0.10).The risk of Colo_7 mo doubled between FIT[adjusted odds ratio(aOR):1.2(1.1;1.2)]and STOPFIT[aOR:2.4(2.1;2.6)];then,decreased by 40%between STOP-FIT and COVID[aOR:2.0(1.8;2.2)].Regardless of the period,this Colo_7 mo’s risk was twice as high for screening colonoscopy performed in a public hospital[aOR:2.

AIM: To assess and compare outcomes of laparoscopic total colectomy performed for a variety of indications. METHODS: Sixty six patients underwent laparoscopic total colectomy for inflammatory bowel disease (IBD) (13) and other diseases (53). Data on demographics, pre and post-operative outcomes were collected ***: Mean operative time was 4.5 h. Conversion rate was 13.6%. Total colectomy performed for IBD was associated with a signifi cantly higher anastomotic leak rate (23.1% vs 1.9%, P < 0.05). On univariate analysis, hand sewn anastomosis and treatment with more than 20 mg of prednisolone for at least 3 mo was associated with a higher anastomotic leak rate (P < 0.05). No signifi cant difference was found in return of gut function and overall morbidity between disease groups. CONCLUSION: Laparoscopic total colectomy is feasible and outcomes are equivalent whatever the indica- tion, except for anastomotic leak rate which is higher for patients with IBD.
