Hirschsprung＇s disease （HD） is a disorder associated with congenital malformation of the enteric nervous system with segmental aganglionosis. Prevailing therapy includes a resection of the affected part of the bowel, However, patients often do not obtain complete functional improvement after surgical treatment. We present the case of a 25-year-old woman who had surgical treatment of lid in early childhood. After that procedure she had clinical features of constipation for years in the end, passing of stool once a week, requiring laxatives and enemas. We diagnosed an incomplete resection of the aganglionic bowel via rectal biopsy and resected the remaining aganglionic segment. Two months after surgery the patient＇s bowel function improved to a frequency of 1-4 stools per day. We conclude that regular follow-up is required to identify lid patients with persistent alterations of bowel function after surgery. In patients presenting with constipation, recognition of a remaining aganglionic segment or other alterations of the enteric nervous system should be aimed at in an early stage. 2005 The WJG Press and Elsevier Inc. HIP cladding is a powder metallurgical coating technique used in the production of wear parts and tools. In many cases the composite components consist of carbide-free hot-work steel as base material and wear resistant carbide-rich PM cold-work steel as coating material. To ensure operativeness a heat treatment matched to the substrate and coating material is required. Dissimilar phase transformation behaviour and different thermal expansion coefficients of layer and substrate entail inner stresses affecting the transformation kinetics in turn. In order to get a deeper insight into these effects Finite Element simulation tools are used. On the one hand, the transient heat conduction problem of the quenching process has to be solved. Non-linear boundary conditions and phase transformation of both, substrate and layer are considered. On the other hand, the mechanical response is calculated. The overall aim of the investigation is an improvement of common heat treatment techniques used for HIP cladded wear parts.

Second-generation direct-acting-antiviral hepatitis C virus treatment: Efficacy, safety, and predictors of SVr12

AIM To gather data on the antiviral efficacy and safety of second generation direct acting antiviral(DAA) treatment with respect to sustained virological response(SVr) 12 wk after conclusion of treatment, and to determine predictors of SVr12 in this *** Two hundred and sixty patients treated with SOF combination partners Pr(n = 51), r(n = 10), SMV(n = 30), DCV(n = 81), LDV(n = 73), or 3D(n = 15).144/260 were pre-treated, 89/260 had liver cirrhosis, 56/260 had portal hypertension with platelets re ≥ 10 and 17/260 were postliver transplantation patients. 194/260 had HCV GT1, 44/260 HCV *** Two hundred and forty/256(93.7%) patients achieved SVr12(m ITT); 4/260 were lost to follow-up. SVr12 rates for subgroups were: 92% for SOF/DCV, 93% for each SOF/SMV, SOF/Pr, 94% for SOF/LDV, 100% for 3D, 94% for pretreated, 87% for liver cirrhosis, 82% for patients with platelets r transplantation, 95% for GT1 a, 93% for GT1 b, 90% for GT3, 100% for GT2, 4, and 6. 12 patients suffered from relapse, 6 prematurely discontinued treatment, of which 4 died. Negative predictors of SVr12 were a platelet count re ≥ 10(P r cirrhosis(P = 0.005) at baseline. In Interferonfree treatment GT3 had significantly lower SVr rates than GT1(P = 0.016). Side effects were mild. CONCLUSION Excellent SVr12 rates and the favorable side-effect profile of DAA-combination therapy can be well translated into "real-world". Patients with advanced liver disease, signs of portal hypertension, especially with platelets re in special need for further research efforts to overcome comparatively higher rates of virological failure.

Improved guided bone regeneration by combined application of unmodified, fresh autologous adipose derived regenerative cells and plasma rich in growth factors:A first-in-human case report and literature review

BACKGrOUND Novel strategies are needed for improving guided bone regeneration(GBr) in oral surgery prior to implant placement, particularly in maxillary sinus augmentation(GBr-MSA) and in lateral alveolar ridge augmentation(LrA). This study tested the hypothesis that the combination of freshly isolated, unmodified autologous adipose-derived regenerative cells(UA-ADrCs), fraction 2 of plasma rich in growth factors(PrGF-2) and an osteoinductive scaffold(OIS)(UAADrC/PrGF-2/OIS) is superior to the combination of PrGF-2 and the same OIS alone(PrGF-2/OIS) in GBr-MSA/*** SUMMArY A 79-year-old patient was treated with a bilateral external sinus lift procedure as well as a bilateral lateral alveolar ridge augmentation. GBr-MSA/LrA was performed with UA-ADrC/PrGF-2/OIS on the right side, and with PrGF-2/OIS on the left side. Biopsies were collected at 6 wk and 34 wk after GBrMSA/LrA. At the latter time point implants were placed. radiographs(32 mo follow-up time) demonstrated excellent bone healing. No radiological or histological signs of inflammation were observed. Detailed histologic,histomorphometric, and immunohistochemical analysis of the biopsies evidenced that UA-ADrC/PrGF-2/OIS resulted in better and faster bone regeneration than PrGF-2/*** GBr-MSA with UA-ADrCs, PrGF-2, and an OIS shows effectiveness without adverse effects.

BACKGrOUND:Early detection of graft malfunction or postoperative complications is essential to save patients and organs after orthotopic liver transplantation (OLT).Predictive tests for graft dysfunction are needed to enable earlier implementation of organ-saving interventions following *** study was undertaken to assess the value of indocyanine green plasma disappearance rates (ICG-PDrs) for predicting postoperative complications,graft dysfunction and patient survival following ***:Eighty-six patients undergoing OLT were included in this single-centre ***-PDr was assessed daily for the first 7 days following *** were graft loss or death within 30 days and postoperative complications,graft loss,or death within 30 ***:Postoperative complications of 31 patients included deaths (12 patients) or graft ***-PDr was significantly different in patients whose endpoints were graft loss or death beginning from day 3 and in those whose endpoints were graft loss,death,or postoperative complications beginning from day 4 after *** day 7 measurements,receiver operating characteristic curve analysis revealed an ICG-PDr cut-off for predicting death or graft loss of 9.6% per min (a sensitivity of 75.0%,a specificity of 72.6%,positive predictive value 0.35 negative predictive value 0.94).For prediction of graft loss,death or postoperative complications,the ICG-PDr cut-off was 12.3%per min (a sensitivity of 68.9%,a specificity of 66.7%,positive predictive value 0.57,negative predictive value 0.77).CONCLUSIONS:ICG-PDr measurements on postoperative day 7 are predictive of early patient outcomes following *** added value over that of routinely determined laboratory parameters is low.

Spatially resolved Photoelectric Performance of Axial GaAs Nanowire pn-Diodes

The spatially resolved photoelectric response of a single axial GaAs nanowire pn-diode has been investigated with scanning photocurrent and Kelvin probe force microscopy. Optical generation of carriers at the pn-junction has been shown to dominate the photoresponse. A photocurrent of 88 pA, an open circuit voltage of 0.56 V and a fill factor of 69% were obtained under AM 1.5 G conditions. The photocurrent followed the increasing photoexcitation with 0.24 A/W up to an illumination density of at least 90 W/cm2, which is important for potential applications in concentrator solar cells.

Efficiency of Combined Solar Thermal Heat Pump Systems

In this project, different combinations of solar energy and heat pump systems for preparation of DHW （domestic hot water） and space heating of buildings are analyzed through dynamic system simulations in TrNSYS （Transient System Simulation Program）. In such systems, solar thermal energy can be used, on one hand, directly to charge the buffer storage and, on the other hand, as heat source for the evaporator oftbe HP （heat pump）. In this work systems, in which solar heat is only used directly （parallel operation of solar and HP）, systems using the collectors also as a heat source for the HP are analyzed and compared to conventional air HP systems. With a combined parallel solar thermal HP system, the system performance compared to a conventional HP system can be significantly increased. Unglazed selectively coated collectors as source for the HP have the advantage that the collector can be used as an air heat exchanger. If solar radiation is available and the collector is used as source for the HP, higher temperatures at the evaporator of the HP can be achieved than with a conventional air HP system.

BACKGrOUND Pancreatic panniculitis is an extremely rare condition associated with different underlying pancreatic disorders and characterized by subcutaneous fat necrosis induced by elevated serum lipase *** lesions usually affect the lower extremities and may precede abdominal symptoms of pancreatic *** cell carcinoma(ACC)of the pancreas is a rare pancreatic neoplasm,accounting for only 1%-2%of pancreatic tumors in *** present the case of a 72-year-old man with ACC of the pancreatic head and synchronous liver *** the primary tumor and liver metastases were *** lipase was elevated before surgery and decreased to normal *** serum lipase levels at follow-up led to the diagnosis of hepatic *** disease progression was then accompanied by pancreatic panniculitis,with subcutaneous fat necrosis and acute *** the best of our knowledge,only 4 cases have been reported in the literature and each showed a similar association of serum lipase levels with pancreatic panniculitis and progression of *** Clinical symptoms and progression of ACC may correlate with serum lipase levels,suggesting potential usefulness as a follow-up biomarker.
