Validation of a Risk-Based Biomarker-Enhanced Scoring System for Lower Respiratory Tract Infections (OPTIMA I Basel)—An Observational Survey

Background: Despiteits recommendation in management guide lines for community acquired pneumonia (CAP), the CURB65 score is frequently not followed for disposition decisions in clinical routine. We therefore proposed an improved CURB65 A score, supplemented by proadre nome dull in (ProADM) levels for patients with CAP and other lower respiratory tract infections (LRTIs). In this study, we vali dated this risk based biomarker enhanced disposition in patients with LRTIs presenting to the emergency department of the University Hospital of Basel. Methods: In this prospective observational cohort study of 85 patients presenting with LRTIs, site of care was decided by the physicians in charge according to their judgement. Retro spectively the CURB65 A score was calculated and a virtual disposition assigned. This was compared with the existing disposition in order to identify efficacy of the novel risk based biomarker enhanced disposition. Results: The novel disposition criteria considered 14 patients suitable for outpatient treatment compared to 11 in the current disposition (p = 0.5). It detected 7 patients to be best treated outside the hospital for nursing reasons, while the current disposition detected only 1 patient requiring geriatric care (p = 0.09). Further, it decreased regular hospitalizations considerably (32 vs. 64, p 0.001). Conclusions: The novel risk based biomarker enhanced disposition is an objective, safe and probably more efficient disposition system to identify outpatient treatment options than the current practice at the University Hospital of Basel.

Catchment soils as a factor of trace metal accumulation in sediments of the reservoir Klingenberg(eastern Ore Mountains, Germany)

The release and accumulation dynamics of trace metals in soils and aquatic sediments were exemplarily investigated in the catchment area of the Reservoir Klingenberg(Germany).Catchment soils were examined for mobilizable and total concentrations of arsenic(As),cadmium(Cd),chrome(Cr),iron(Fe),manganese(Mn),nickel(Ni),lead(Pb),and zinc(Zn)and compared with trace metal quantities accumulated in riverbed and reservoir *** comparison of all samples showed relatively small variations of Cr(7.96–46.0 mg/kg),Fe(7.79–40.4 g/kg),and Ni(6.06–56.5 mg/kg),while stronger differences were found for As(11.2–164 mg/kg),Cd(0.14–30.5 mg/kg),Mn(0.08–1.84 g/kg),Pb(20.7–183 mg/kg),and Zn(69.1–916 mg/kg).The catchment soils were slightly enriched by Cd,Pb,and *** Cd and Zn were characterized by large mobilizable *** mean trace metal concentrations in riverbed sediments were higher than in catchment soils,while reservoir sediments accumulated the highest amounts of the analyzed *** enrichment of trace metals in reservoir sediments was generally determined by the sedimentation of fine particles,while the distribution of As,Fe,and Mn was additionally impacted by redox *** Cd and Zn,which in comparison were most enriched in riverbed and reservoir sediments,a significant release from soils by leaching processes was *** accumulation of As and Pb in reservoir sediments was influenced to a greater extent by soil erosion and by anthropogenic or chalcogen sources in the catchment.

Targeting new ways for large-scale,high-speed surface functionalization using direct laser interference patterning in a roll-to-roll process

Hydrocarbon-Based Contaminants in Drinking Water Sources and Shellfish in the Soku Oil and Gas Fields of South-South Nigeria

Environmentally unfriendly Oil exploration activities have been ongoing in the Soku area of the Niger Delta of Nigeria since 1956. This study evaluated the concentration of hydrocarbons and heavy metals in Shellfish and drinking water sources in the study area. It revealed the absence (nickel and lead were above permissible limits in the fish samples in all standards while PAHs occurred at the cancer risk levels of 10?6. A review of the public health situation in the Soku area with a view to understanding current trends, sources of perturbations and preferable solutions to the potential public health challenges raised in this study is hereby recommended. Also, this study recommends that relevant agencies and developmental partners should launch a national drive to create awareness among people/environmental/public health professionals’/health workers/administrators on this regional concern.

The mining company Sama nickel-CI during its exploration activities in the Bounta area (Biankouma department, western Côte d’Ivoire) identified rocks with magmatic and metamorphic characteristics of felsic to mafic compositions mineralized in sulfide but unknown in the region. Thus, the petrographic characterization of these rocks of Bounta as well as the sulfides they contain, was carried out from macroscopic to microscopic observations. Petrographic analysis of these rocks has shown that these rocks are trondhjemite, tonalite and granodiorite (TTG) suites, composed of the main minerals (quartz, plagioclase, orthopyroxene, amphibole, feldspar) and accessory minerals (biotite, garnet) with magmatic textures supplanted by metamorphic textures. They were set up by fractional crystallization and would be products of Archean crustal anatexis formed from the partial melting of basaltic protocrust during the Liberian orogeny. The Bounta’s series of trondhjemite, tonalite and granodiorite (TTG) contain speckles of sulfide (pyrite and pyrrhotite) which are disseminated in places and are relatively abundant, as well as iron oxides (magnetite) which are of low proportion.

Mafic and ultramafic intrusions observed in the Archean formations of the Sipilou region exhibit occurrences of polymetallic sulphide. Mapping, petrographic and geochemical studies have defined magnetic facies associated with the various geological units. The results of this work reveal that cupronickel sulphides, olivines and pyroxenes as well as spinels are related to ultrabasic formations where strong magnetic facies prevail. Iron sulphides and magnetite are linked to quartzo-feldspathic and jotunite-enderbite formations, which are characterised by moderate magnetic facies. The latter are thought to be derived from anatexite remobilisation within Archean granulites, which have weak magnetic facies.

土壤修复主要通过将受污染土壤进行挖掘和填埋,这种方法在进行大量位点修复时不太切合实际,因此要选择其它棕地(brownfield)修复方法。本研究介绍了评估受铜铬砷(CCA)污染土壤采用含铁(Fe)爆破砂(BS)或嵌氧颗粒(OSG)处理后的化学稳定过程。就工业尺度下混合改良物的可行性和在不同氧化还原条件下的稳定效率对这种稳定技术进行了评估。在野外自然条件下,采用1m^3测渗计调查了这种稳定性并在实验室(10L)对氧化还原条件的效应进行了评估。结果表明改良物的添加率最低时(0.1%和1%)化学计量中还有足够的铁,但是不管在野外还是实验室都是采用高添加率的改良物(8%和17%)效果较好。铁和混合物的颗粒大小会影响稳定效率。厌氧条件的发展(模拟自水饱和度)会提高砷(As^(3+))的份额,因此增强了砷的迁移能力。在有氧条件下,采用高浓度OSG提高了孔隙水中镍(Ni)和铜(Cu)的浓度,但是在厌氧条件下,与未作处理的土壤相比,砷的淋溶率降低了,而镍和铜的淋溶并不是关键的。所处理土壤的最终目的决定了改良方案,也就是说,对于在厌氧条件下填埋的土壤,OSG浓度为10%较合适。也可以在有氧条件下将1%的BS与土壤混合,置于填埋区上层或者棕色地块(brownfield site)原处。另外从长期来看,采用BS处理似乎比用OSG处理效果要好。
