Femoral Access with Ultrasound-Guided Puncture and Z-Stitch Hemostasis for Adults with Congenital Heart Diseases Undergoing Electrophysiological Procedures

Aims:Although the application of ultrasound-guided vascular puncture and Z-stitch hemostasis to manage femoral access has been widely utilized,there is limited data on this combined application in adult congenital heart disease(ACHD)patients undergoing electrophysiological(EP)procedures.We sought to evaluate the safety and efﬁcacy of ultrasound-guided puncture and postprocedural Z-stitch hemostasis for ACHD patients under-going EP procedures.Methods and Results:The population of ACHD patients undergoing transfemoral EP pro-cedures at the University of Zurich Heart Center between January 2019 and December 2022 was observed and analyzed.During the study period,femoral access(left/right,arterial/venous)was performed under real-time ultrasound guidance.At the end of the procedure,a single Z-stitch was performed at the puncture site.We eval-uated the incidence of in-hospital complications associated with femoral access puncture in this population.Among 101 patients who had a total of 147 previous ipsilateral vascular punctures(mean 1.5 per person),100 patients underwent successful femoral vascular access for EP procedures.The median age of the patients was 47±15 years and 34(34%)were male.Z-stitches were performed after the procedure in 100 patients with 303 femoral vascular accesses(mean 3 punctures per person).No patient developed vascular puncture relevant inguinal hematoma,pseudo aneurysm,arteriovenousﬁstula,venous or arterial thrombosis.Conclusion:In ACHD patients undergoing EP procedures,optimal femoral access management can be achieved with ultra-sound-guided puncture and postprocedural Z-stitch hemostasis.

Dual Control Strategy for Grid-tied Battery Energy Storage Systems to Comply with Emerging Grid Codes and Fault Ride Through Requirements

Battery energy storage systems(BESSs)need to comply with grid code and fault ride through(FRT)requirements during disturbances whether they are in charging or discharging mode.Previous literature has shown that constant charging current control of BESSs in charging mode can prevent BESSs from complying with emerging grid codes such as the German grid code under stringent unbalanced fault conditions.To address this challenge,this paper proposes a new FRTactivated dual control strategy that consists of switching from constant battery current control to constant DC-link voltage control through a positive droop structure.The results show that the strategy ensures proper DC-link voltage and current management as well as adequate control of the positive-and negative-sequence active and reactive currents according to the grid code priority.It is also shown that the proposed FRT control strategy is tolerant to initial operating conditions of BESS plant,grid code requirements,and fault severity.

When is parenteral nutrition indicated?

The indications and contraindications of parenteral nutrition(PN)are discussed in view of recent clinical find-ings.For decades,PN has been restricted to patients unable to tolerate enteral nutrition(EN)intake owing to the perceived risk of severe side-effects.The evolution of the PN substrate composition and delivery of nutrition via all-in-one bags has dramatically improved the application prospects of PN.Recent studies show similar compli-cation rates of nutrition therapy administered through enteral and intravenous routes.Therefore,indications of PN have,based on evidence,extended beyond complete gastrointestinal(GI)failure to include conditions such as insufficient EN generating persistent negative energy balance and insufficient protein intakes,malabsorption,or specific needs that are impossible to cover with EN feeds.

The catalysis of the Hajos-Parrish reaction by cis-and trans-4,5-ethano-proline was explored experimentally and computationally with DFT(ωB97X-D and MN15)and DLPNO-CCSD(T).Both of the new catalysts are as active as proline or cis-and trans-4,5-methano-proline,with the trans-ethano-proline being as enantioselective as proline.A thorough theoretical analysis of the electronic factors influencing catalysis is reported.

Psychological Impacts of Coronavirus Outbreaks on Adults:A Rapid Evidence Review

A recent rapid review highlighted the negative psychological impacts of quarantining during coronavirus out-breaks on the public.However,to date,there has been no review of the psychological impacts of coronavirus on adults using research from community samples and not restricted to people quarantined during coronavirus.A rapid review was conducted to provide timely evidence about the mental health implications of coronavirus outbreaks on adults and to inform psychological research concerning the current COVID-19 outbreak.Three databases and Google Scholar were searched and a total of 27 studies were identiﬁed.Symptoms of anxiety and depression were identiﬁed during coronavirus outbreaks alongside concerns about becoming infected and family becoming infected.Those with pre-existing mental health conditions,young adults,women and those reporting physical symptoms appear to be vulnerable to negative psychological outcomes during coronavirus out-breaks.How people think about and understand coronavirus,and the coping strategies employed by people,may play a role in mitigating negative psychological outcomes.Results demonstrate the adverse psychological impacts of coronavirus outbreaks on adults and the need for continued investment in mental health services for the wider community during these times.Further longitudinal research is required to ascertain the long-term psychological consequences of coronavirus outbreaks.This review can be used to inform continued research on the psycholo-gical impacts of COVID-19 on adults.

France is a country with 67 million inhabitants and is part of the European Union.Antibiotic consumption in humans is higher in France as compared to the European average,especially in outpatient settings(1–2).The burden of antibiotic resistance is also high,as compared to the European average,with an estimated 5,500 deaths per year associated with multidrug-resistant bacterial infections(3).Encouraging results have,however,been observed over the last several years:antibiotic consumption in the community,in nursing homes and in hospitals showed a decreasing trend,as well as certain resistant bacteria,such as 3rd-generation cephalosporin-or fluoroquinolone-resistant Escherichia coli(1–2,4–5).The COVID-19 pandemic has had a dramatic impact on antibiotic consumption and its long-term impact on antibiotic resistance is being carefully monitored(1).

Excision of malignant and pre-malignant rectal lesions by transanal endoscopic microsurgery in patients under 50 years of age

BACKGROUND The most common technique for treating benign and early malignant rectal lesions is transanal endoscopic microsurgery(TEM).Local excision is an acceptable technique for high-risk and elderly patients,but there are hardly any data regarding young patients.AIM To describe TEM outcomes in patients under 50 years of age.METHODS We collected demographic,clinical,and pathological data from all patients under the age of 50 years who underwent the TEM procedure at Hasharon Rabin Medical Center from January 2005 to December 2018.RESULTS During the study period,a total of 26 patients under the age of 50 years underwent TEM procedures.Their mean age was 43.3 years.Eleven(42.0%)were male.The mean operative time was 67 min,and the mean tumor size was 2.39 cm,with a mean anal verge distance of 8.50 cm.No major intraoperative or postoperative complications were recorded.The median length of stay was 2 d.Seven(26.9%)lesions were adenomas with low-grade dysplasia,four(15.4%)were high-grade dysplasia adenomas,two were T1 carcinomas(7.8%),and three were T2 carcinomas(11.5%).No residual disease was found following endoscopic polypectomy in two patients(7.8%),but four(15.4%)had other pathologies.Surgical margins were negative in all cases.Local recurrence was detected in one patient 33 mo following surgery.CONCLUSION Among young adult patients,TEM for benign rectal lesions has excellent outcomes.It may also offer a balance between the efficacy of complete oncologic resection and postoperative quality of life in the treatment of rectal cancer.In some cases,it may be considered an alternative to radical surgery.

Association of COVID- 19 with respiratory syncytial virus (RSV) infections in children aged 0-5 years in the USA in 2022: a multicentre retrospective cohort study

Objective To investigate whether COVID-19 infection was associated with increased risk for incident respiratory syncytial virus(RSV)infections and associated diseases among young children that might have contributed to the 2022 surge of severe paediatric RSV cases in the USA.Design This is a retrospective population-based cohort study.Five outcomes were examined,including overall RSV infection,positive lab test-confirmed RSV infection,clinically diagnosed RSV diseases,RSV-associated bronchiolitis and unspecified bronchiolitis.Risk ratio(RR)and 95%CI of the outcomes that occurred during the 2022 and 2021 RSV seasons were calculated by comparing propensity-score matched cohorts.Setting Nationwide multicentre database of electronic health records(EHRs)of 61.4million patients in the USA including 1.7million children 0–5 years of age,which was accessed through TriNetX Analytics that provides web-based and secure access to patient EHR data from hospitals,primary care and specialty treatment providers.Participants The study population consisted of 228940 children of 0–5 years with no prior RSV infection who had medical encounters in October 2022.Findings were replicated in a separate study population of 370919 children of 0–5 years with no prior RSV infection who had medical encounters in July 2021–August 2021 during a non-overlapping time period.Results For the 2022 study population(average age 2.4 years,46.8%girls,61%white,16%black),the risk for incident RSV infection during October 2022–December 2022 was 6.40%for children with prior COVID-19 infection,higher than 4.30%for the matched children without COVID-19(RR 1.40,95%CI 1.27 to 1.55);and among children aged 0–1year,the overall risk was 7.90%for those with prior COVID-19 infection,higher than 5.64%for matched children without(RR 1.40,95%CI 1.21 to 1.62).For the 2021 study population(average age 2.2 years,46%girls,57%white,20%black),the risk for incident RSV infection during July 2021–December 2021 was 4.85%for children with prior COVID-19 infection,higher than 3.68

Objective The long-term time trend and seasonality variations of first-time medically attended respiratory syncytial virus(RSV)infections among young children are unknown.We aim to examine the time trend of medically attended first-time RSV infections among young children in the USA from January 2010 through January 2023.Design This is a population-based cohort study using electronic health records(EHRs).Monthly incidence rate of medically attended first-time RSV infection(cases per 10000000 person-days).A time-series regression model was used to model and predict time trends and seasonality.Setting Multicenter and nationwide TriNetX Network in the USA.Participants The study population comprised children aged 0–5 years who had medical visits during the period of January 2010 to January 2023.Results The data included 29013937 medical visits for children aged 0–5 years(46.5%girls and 53.5%boys)from January 2010 through January 2023.From 2010 through 2019,the monthly incidence rate of first-time medically attended RSV infection in children aged 0–5 years followed a consistent seasonal pattern.Seasonal patterns of medically attended RSV infections were significantly disrupted during the COVID-19 pandemic.In 2020,the seasonal variation disappeared with a peak incidence rate of 20 cases per 1000000 person-days,a decrease of 97.4%from the expected peak rate(rate ratio or RR:0.026,95%CI 0.017 to 0.040).In 2021,the seasonality returned but started 4 months earlier,lasted for 9 months,and peaked in August at a rate of 753 cases per 1000000 person-days,a decrease of 9.6%from the expected peak rate(RR:0.90,95%CI 0.82 to 0.99).In 2022,the seasonal pattern is similar to prepandemic years but reached a historically high rate of 2182 cases per 10000000 person-days in November,an increase of 143%from the expected peak rate(RR:2.43,95%CI 2.25 to 2.63).The time trend and seasonality of the EHR-based medically attended RSV infections are consistent with those of RSV-associated hospitalisations from the Centers for Disease Control an
