Predictors of the outcomes of acute-on-chronic hepatitis B liver failure

AIM： To identify the risk factors in predicting the out- come of acute-on-chronic hepatitis B liver failure pa- tients. METHODS： We retrospectively divided 113 patients with acute-on-chronic liver failure-hepatitis B virus （ACLF-HBV） and without concurrent hepatitis C or D virus infection and hepatocellular carcinoma into two groups according to their outcomes after anti-HBV therapy. Their demographic, clinical, and biochemical data on the day of diagnosis and after the first week of treatment were analyzed using the Mann-Whitney U test, Fisher＇s exact test, and a multiple logistic regres- sion analysis. RESULTS： The study included 113 patients （87 men and 26 women） with a mean age of 49.84 years. Fifty- two patients survived, and 61 patients died. Liver failure （85.2%）, sepsis （34.4%）, and multiple organ failure （39.3%） were the main causes of death. Mul- tivariate analyses showed that Acute Physiology and Chronic Health Evaluation （APACHE） Ⅱ scores ≥ 12 [odds ratio （OR） = 7.160, 95% CI： 2.834-18.092, P 〈 0.001] and positive blood culture （OR = 13.520, 95% CI： 2.740-66.721, P = 0.001） on the day of diagnosis and model for end-stage liver disease （MELD） scores 28 （OR = 8.182, 95% CI： 1.884-35.527, P = 0.005） after the first week of treatment were independent predictors of mortality. CONCLUSION： APACHE II scores on the day of diag- nosis and MELD scores after the first week of anti-HBV therapy are feasible predictors of outcome in ACLF- HBV patients.

Image Reconstructions from Limit Views and Angle Coverage Data for a Stationary Multi-Pinhole SPECT System

The goal of this paper is to investigate different reconstruction methods for solving the limited angle problem in reconstructing the projection data from a stationary multi-pinhole system based on a three-head clinical single photon emission computed tomography （SPECT） camera. Three iterative recon- struction approaches were studied including maximum likelihood expectation maximization （MLEM）, maximum a posteriori expectation maximization （MAPEM） with a smoothing prior, and an alternating optimization schemes from MLEM and total variation （TV） optimization. A three-headed multipinhole SPECT was simulated with apertures of nine 1-mm pinholes each, and covered scanning volume of 6-mm diameter. The reconstructions were optimized for various iterations based on visual inspections, and finally 20 iterations were applied for each method. For both MLEM-TV and MAPEM-TV, various initial reconstructions before TV optimization were studied. The smoothing parameter for MAPEM and the gradient descent constant for TV were also investigated through visual comparison. The preliminary results showed the 3 reconstruction methods generated compatible images, and can restore the images from projection data suffering limited angular sampling. However, MLEM was noisy for low-count and highly limited angle data, and thus suitable smoothing in MAPEM alleviated this problem, initial reconstructions were necessary for better edge enhancement in TV. The conclusion is that TV might be potential in producing more edge-enhanced images if all parameters were optimized.

The Clustered Regularly Interspaced Short Palindromic Repeats/CRISPR-associated protein system(CRISPR/Cas)has recently become the most powerful tool available for genome engineering in various *** efficient and proper expression of multiple guide RNAs(gRNAs),the CRISPR/Cas system is particularly suitable for multiplex genome *** the past several years,different CRISPR/Cas expression strategies,such as two-component transcriptional unit,single transcriptional unit,and bidirectional promoter systems,have been developed to efficiently express gRNAs as well as Cas *** progress has been made to optimize gRNA production using different types of promoters and RNA processing strategies such as ribozymes,endogenous RNases,and exogenous endoribonuclease(Csy4).Besides being constitutively and ubiquitously expressed,inducible and spatiotemporal regulations of gRNA expression have been demonstrated using inducible,tissue-specific,and/or synthetic promoters for specific research *** recently,the emergence of CRISPR/Cas ribonucleoprotein delivery methods,such as engineered nanoparticles,further revolutionized transgene-free and multiplex genome *** this review,we discuss current strategies and future perspectives for efficient expression and engineering of gRNAs with a goal to facilitate CRISPR/Cas-based multiplex genome editing.

Aqueous self-assembly and surface-functionalized nanodots for live cell imaging and labeling

Nanoparticles have enormous potential for bioimaging and biolabeling applications, in which conventional organically based fluorescent labels degrade and fail to provide long-term tracking. Thus, the development of approaches to make fluorescent probes water soluble and label cells efficient is desirable for most biological applications. Here, we report on the fabrication and charac- terization of self-assembled nanodots （SANDs） from 3-aminopropyltriethoxysilane （APTES） as a probe for protein labeling. We show that fluorescent SAND probes exhibit both bright photoluminescence and biocompatibility in an aqueous environment. Selective in vitro imaging using protein and carbohydrate labeling of hepatoma cell lines are demonstrated using biocompatible SANDs conjugated with avidin and galactose, respectively. Cytotoxicity tests show that conjugated SAND particles have negligible effects on cell proliferation. Unlike other synthetic systems that require multistep treatments to achieve robust surface functionalization and to develop flexible bioconjugation strategies, our results demonstrate the versatility of this one-step SAND fabrication method for creating multicolor fluorescent probes with the tailored functionalities, effident emission, as well as excellent biocompatibility, required for broad biological use.

GPS Vector Tracking Loop with Fault Detection and Exclusion

In this paper,both the integrity monitoring and fault detection and exclusion(FDE)mechanisms are incorporated into the vector tracking loop(VTL)architecture of the Global Positioning System(GPS)receiver for reliability *** the VTL,the tasks of signal tracking and navigation state estimation no longer process separately and a single extended Kalman filter(EKF)is employed to simultaneously track the received signals and estimate the receiver’s position,velocity,*** contrast to the scalar tracking loop(STL)which utilizes the independent parallel tracking loop approach,the VTL technique is beneficial from the correlation of each satellite signal and user *** VTL approach provides several important *** of the merits is that the tracking loop can be assisted for overcoming the problem of signal *** the VTL architectures provide several important advantages,they suffer some fundamental *** example,the errors in the navigation solutions may degrade the tracking *** most significant drawback is that failure of tracking in one channel may affect the entire tracking loop and possibly lead to loss of *** reliability enhancement,the EKF based integrity monitoring and FDE algorithms are developed to prevent the error from spreading into the entire tracking *** integrity monitoring is utilized to check the possible fault in the pseudorange and the pseudorange rate,followed by the FDE mechanism employed to exclude the abnormal satellite *** assessment and evaluation for the proposed approach will be presented.

On the resonance analysis for compliant bionic jellyfishes

A bionic jellyfish is a robot that can mimic the swimming of a real jellyfish. In practical application, the volume and the capacity of its carrying power are limited, hence its power consumption is a crucial factor for continuous swimming. The purpose of this paper is to propose an approach for the resonance analysis of bionic jellyfishes in order to investigate their energy efficiency in swimming. First, a suitable rigid-body bionic jellyfish was chosen from the previous study, and then its design approach was presented, Then, it was transformed into a compliant design, by the proposed method, in order to mimic the motion of a real jellyfish. Furthermore, its solid model was drawn, and the approach of modal analysis by using ANSYS software was addressed. After that, two terms, specific energy and energy ratio, were defined in order to evaluate its energy efficiency. Finally, a design example was given for illustration, and its motion simulations with and without resonance input were conducted by using ADAMS software. Additionally, their specific energies and energy ratios were found. The simulation results showed that the required input energy would be significantly reduced if a bionic jellyfish swims at its resonance frequency.

PI3K-AKT-mTOR signaling in prostate cancer progression and androgen deprivation therapy resistance

Prostate cancer （PCa） is the second most common malignancy among men in the world. Castration-resistant prostate cancer （CRPC） is the lethal form of the disease, which develops upon resistance to first line androgen deprivation therapy （ADT）. Emerging evidence demonstrates a key role for the PI3K-AKT-mTOR signaling axis in the development and maintenance of CRPC. This pathway, which is deregulated in the majority of advanced PCas, serves as a critical nexus for the integration of growth signals with downstream cellular processes such as protein synthesis, proliferation, survival, metabolism and differentiation, thus providing mechanisms for cancer cells to overcome the stress associated with androgen deprivation. Furthermore, preclinical studies have elucidated a direct connection between the PI3K-AKT-mTOR and androgen receptor （AR） signaling axes, revealing a dynamic interplay between these pathways during the development of ADT resistance. Thus, there is a clear rationale for the continued clinical development of a number of novel inhibitors of the PI3K pathway, which offer the potential of blocking CRPC growth and survival. In this review, we will explore the relevance of the PI3K-AKT-mTOR pathway in PCa progression and castration resistance in order to inform the clinical development of specific pathway inhibitors in advanced PCa. In addition, we will highlight current deficiencies in our clinical knowledge, most notably the need for biomarkers that can accurately predict for response to PI3K pathway inhibitors.

CD14 promoter polymorphism in Chinese alcoholic patients with cirrhosis of liver and acute pancreatitis

AIM： To investigate the relationship between genetic polymorphism of the CD14 promoter and the occurrence of alcoholic cirrhosis and alcoholic pancreatitis, and to challenge the conclusion made earlier that the patients with acute alcoholic pancreatitis and patients with alcoholic cirrhosis of liver are two different subpopulations. METHODS： Using the polymerase chain reaction-restriction fragment length polymorphism （PCR-RFLP） method, we determined the polymorphism of CD14 gene and aldehyde dehydrogenase gene 2 （ALDH 2） in 335 alcoholic patients with different organ complications i.e., cirrhosis of liver （n = 100）, acute pancreatitis （n = 100）, esophageal cancer （n = 82） and avascular necrosis of hip joint （AVN）（n = 53） and 194 non-alcoholic controls in a Chinese group. RESULTS： The results showed that the carriage of T allele was not different among alcoholic patients with cirrhosis of liver, alcoholic patients with other complication and non-alcoholic controls. On the other hand, the carriage of the C allele was significantly more prevalent for alcoholic pancreatitis than for esophageal cancer （0.79 vs 0.60, P〈0.001）, alcoholic AVN （0.79 vsO.65, P〈0.025） and nonalcoholic controls （0.79 vs 0.68, P〈0.025）. Furthermore, when only subjects with ALDH2 1-1 genotype were examined, the C allele frequency was significantly more prevalent for alcoholic pancreatitis than for alcoholic liver cirrhosis （0.82 vs 0.69, P〈0.025）, esophageal cancer （0.82 vs 0.61, P〈0.01）, alcoholic AVN （0.82 vs 0.64, P〈0.01） and non-alcoholic controls （0.82 vs0.69, P〈0.05）. CONCLUSION： The C allele may be associated with some mechanism, which is important in the pathogenesis of alcoholic pancreatitis, and that alcoholic patients with acute pancreatitis and cirrhosis of liver are probably two different subpopulations.
