Undifferentiated Pleomorphic Sarcoma of the Cheek with Surface Ulceration: Mimicking Spindle Cell Squamous Cell Carcinoma

Undifferentiated pleomorphic sarcoma (UPS), also called malignant fibrous histiocytoma (MFH), belongs to the soft tissue tumors and is more likely in elder patients. UPS currently accounts for approximately 18% of cutaneous soft tissue sarcoma except Kaposi sarcoma. The most common sites are the extremities and head and neck. UPS is rapidly enlarging, and easily invades subcutaneous tissue and muscle without overlying skin changes. UPS tumors, which are confined to subcutaneous tissue, have been reported to be less than 10%. On the other hand, the involvement of the epidermis, occasionally with ulceration, is rare. Here, we describe a case of UPS of the cheek with surface ulceration. An 83-year-old man presented with a 1-year history of a red nodule on his left cheek. The size of the nodule gradually increased. Clinical examination revealed a 55 × 45 mm red, hard mass in the center of his left cheek with a large cratershaped ulcer involving the subcutaneous tissue. The bottom of the ulcer contained necrotic tissue. No cervical lymph nodes were palpable. A biopsy revealed a high-grade, pleomorphic spindle cell neoplasm and frequent abnormal mitotic figures within the dermis. We temporarily diagnosed and suspected a soft tissue sarcoma, but we could not rule out spindle cell squamous cell carcinoma (SCSCC). The patient underwent tumor resection and skin grafting. The tumor had reached the muscle layer of his left cheek with invasion and ulceration of the overlying skin. Histopathological examination of the totally resected tumor revealed a neoplasm of mesenchymal origin, rich in cells with morphology and immunohistochemical characteristics compatible with UPS. Malignant cells were not found in the epidermis. Immunohistochemical analyses were positive for vimentin, α-anti-trypsin, α-smooth muscle actin (α-SMA), and CD10, and negative for cytokeratins (CAM5.2 and AE1/AE3), lysozyme, epithelial membrane antigen (EMA), CD34, S-100, desmin, and caldesmon. The spread of the new coronavirus(COVID-19)infection in 2020 has had a significant impact on the treatment of cancer *** the COVID-19 pandemic,the biggest challenge for pancreatic surgeons is the difficulty in providing oncological *** this review article,from the standpoint of surgeons,we explain the concept of triaging of patients with pancreatic tumors under the COVID-19 pandemic,and the actual impact of COVID-19 on the treatment of patients with pancreatic *** most vital points in selecting the best therapeutic approach for patients with pancreatic tumors during this pandemic are(1)Oncologists need to tailor the treatment plan based on the COVID-19 phase,tumor malignant potential,and patients’comorbidities;and(2)Optimal treatment for pancreatic cancer should be planned according to the condition of each patient and tumor resectability based on national comprehensive cancer network resectability *** choose the best therapeutic approach for patients with pancreatic tumors during this pandemic,we need to tailor the treatment plan based on elective surgery acuity scale(ESAS).Newly established ESAS for pancreatic tumor and flowchart indicating the treatment strategy of pancreatic cancer,are feasible to overcome this situation.

Management of adult Langerhans cell histiocytosis based on the characteristic clinical features

To find out the most appropriate management,clinical features of 18 cases of adult multisystem langerhans cell histiocytosis(LCH) have been analyzed. The patients comprising of 9 males and 9 females were median age of 36 years,ranging from 18-53 years at diagnosis. Regarding the initial symptoms,7 patients(2 males and 5 females) showed central diabetes insipidus(CDI) and other endocrine symptoms with thickened pituitary stalk or a mass at the hypothalamic region. Additional 2 patients initiated the disease with CDI with no immediate diagnosis. In the remaining patients,the diseasebegun with single(n = 3) or multiple(n = 1) spinal bone lesion(s) in 4 patients(all males),with multiple bone lesions in 3 patients(1 male and 2 females),with single skull lesion in one female patient and with ambiguous symptoms including hypothyroidism in the remaining one male patient. We also recognized the correlation between pregnancy/childbirth and LCH in 4 patients. In terms of treatment,9 patients received systemic immuno-chemotherapy alone,of which the majority received vinblastine-based chemotherapy while 4 received 2-chlorodeoxyadenosine. Five had a combination of immuno-chemotherapy with surgical resection or radiotherapy,2 had immunotherapy alone,2 had surgical resection followed by observation alone to date. Three patients received hematopoietic stem cell transplantation after extensive chemotherapy. In terms of outcome,15 patients are alive(9 with active disease,6 without active disease),with a median of 66 mo(range 17-166 mo),two died of disease while the remaining 1 lost to follow-up. Based on these results,we think that early diagnosis and rapid introduction of appropriate treatment are essential,in order to overcome the problems relevant to adult LCH.

Effects of the Percolation Patterns and the Thickness of Soil Dressing on Reducing Cadmium Uptake and and Yields of Rice Plants （Oryza Sativa L.）

The objective of this study is to clarify the effects of the thickness of soil dressing, the percolation patterns of plowsole and subsoil on reducing cadmium （Cd） uptake, and growth and yields of rice plants. Six stratified paddy field models, three patterns of soil dressing layer and two percolation patterns were used for the experiments. These models had 12.5 cm, 15.0 cm and 25.0 cm thickness of soil dressing layer and 15 cm thickness of underlying polluted soil layer, whose Cd concentration was about 1.81 mg·kg^-1. As a result Cd concentration of brown rice was lower than 0.04 mg·kg^-1 for the models with the close system percolation, while that in the open system percolation models were lower than 0.17 *** the thickness of soil dressing became lower, Cd concentrations with the open system percolation models showed significantly higher values than those of the other models （5% of significant level）. But any significant difference was not found in the growth and yield among these models. As above mentioned, it was found that Cd concentration in rice grains was affected by the thickness of soil dressing and percolation patterns.

Streptococcal SspB Peptide Analog Inhibits Saliva-Promoted Adhesion and Biofilm Formation of Streptococcus mutans

Background: Streptococcus gordonii, a pioneer colonizer of dental plaque biofilm, expresses surface protein adhesin SspB by which the bacteria bind to salivary agglutinin (gp340). SspB has extensive homology with PAc, a surface adhesin of Streptococcus mutans. Hence, SspB of S. gordonii competes with PAc of S. mutans for the same niche environment in the salivary pellicles. The aim of this study was to develop anti-adherence agents that enabled us to control cariogenic biofilms by using the streptococcal SspB peptide analog SspB (A4K-A11K). Methods: First, we performed ELISA to determine the S. mutans-saliva interaction and saliva-binding activities of SspB (A4K- A11K). The inhibitory effects of SspB (A4K-A11K) were then evaluated by examining S. mutans adhesion to saliva-coated hydroxyapatite disks (s-HA). To determine peptide interference with biofilm formation, S. mutans biofilms were quantified by counting CFUs on MS agar plates and by measuring the absorbance at 492 nm of safranin-stained biofilms on s-HA. Results: Saliva, particularly salivary gp340 peptide, promoted adherence of S. mutans to polystyrene surfaces. SspB (A4K-A11K) significantly bound to saliva and inhibited the adhesion of S. mutans to s-HA without bactericidal activity. Furthermore, biofilms of S. mutans on s-HA were successfully reduced by pretreatment with SspB (A4K-A11K). Conclusion: SspB (A4K-A11K) peptide competitively blocked S. mutans adhesion to experimental pellicles through SspB-gp340 interaction, thereby inhibiting biofilm formation. These findings will contribute to the control cariogenic biofilms.

Changes in Blood Volume and Colloid Osmotic Pressure during Fluid Absorption in Patients Undergoing Endoscopic Urosurgery: An Observational Study

Background and Objective: Anesthesiologists need to be familiar with perioperative changes in blood volume (BV);however, there is no standard method for repeated evaluation of BV over a short interval of time. We evaluated BV in the operation room using repeatable estimation methods. Method: Eighty-five ASA physical status I-II patients scheduled to undergo endoscopic urosurgery using irrigation fluid under general anesthesia at Nippon Medical School Hospital were included in this study. Irrigation with 3% sorbitol in water was commenced after establishment of general anesthesia and volumetric fluid balance, which was defined as control water balance (WB). Hematocrit (Hct), colloid osmotic pressure (COP), total protein (TP) and albumin (Alb) were repeatedly determined before and during anesthesia. BV was calculated using Allen’s formula and the changes in Hct, COP, TP and Alb. Main Outcome Measures: The main outcome was the accuracy of measuring changes in BV (△BV) calculated using the four serum markers. WB and the estimated △BV calculated from Hct, COP, TP and Alb (△BV-Hct, △BV-COP, △BV-TP, and △BV-Alb) were analysed using Pearson’s correlation coefficient test and Bland-Altman analysis. Results: Sixty-five patients were excluded. In the remaining 20 patients, there was a significant correlation between WB and △BV-COP (R2 = 0.72;P < 0.01), WB and △BV-TP (R2 = 0.59;P △BV-Alb (R2 = 0.57;P △BV-Hct (R2 = 0.06). Conclusion: △BV-COP, △BV-TP and △BV-Alb had correlation with WB. However, since COP can be measured repeatedly with simplified instruments under selected clinical circumstances, while TP and Alb cannot. COP is the most useful marker to measure △BV during perioperative period. Hct does not allow precise estimation of △BV.

Inhibitory Effects of Pu-erh Tea Leaves on Mouse Type IV Allergy

Pu-erh tea is one of post fermented tea manufactured through oxidative maturing by microorganisms. We investigated the preventive effects of pu-erh tea extract on oxazolone-induced type IV allergy in male (ICR) mice. Oral administration of 50 mg/kg water extract of pu-erh tea leaves resulted in significant preventive effects against mouse type IV allergy. The hydrophilic Theabrownin-like fraction (TBW-ND), with a high molecular weight of approximately 12,000 or higher, was prepared from pu-erh tea leaves by a solvent-extraction method, followed by Diaion HP-20 column chro-matography and dialysis. Oral and percutaneous administrations of TBW-ND at doses of 18.7 mg/kg and 0.037 mg/ear also resulted in significant preventive effects, which included preventing increases in levels of the proinflam-matory cytokine interleukin-12. The anti-allergic ingredients in TBW-ND were predicted to be highly complex compounds containing of polyphenols, polysaccharides and/or proteins. The results suggest pu-erh tea leaves could be a beneficial food for protecting against delayed-type allergy.

BACKGROUND Ischemic gastritis is a clinically rare and highly fatal disease that occurs when the hemodynamics of a patient with vascular risk is *** diagnosis and treatment are possible only with upper endoscopy after symptom *** report seven cases of ischemic gastritis and its clinical features,prognosis,and indicators that may help in early *** SUMMARY Of the seven patients,six had vascular risk and five died within 2 wk of *** symptoms included hematemesis and *** surgery is a choice for radical treatment,not all patients were *** such patients,conservative treatment was selected,but all of them *** contrast,patients who underwent repeat endoscopy showed improved mucosal findings,suggesting that this improvement may not affect *** ischemic changes such as wall thickening,mural emphysema,and fluid retention in the stomach were observed before diagnosis through endoscopy and computed tomography(CT).The CT scan can be effective for early detection,and improvement in circulatory failure and aggressive treatment may save the lives of patients with this *** The characteristic CT findings enable early detection of ischemic *** diagnosis increases the chance of survival if early therapeutic intervention and improvement of circulatory dynamics can be achieved in this highly fatal disease.
