Changes in Blood Volume and Colloid Osmotic Pressure during Fluid Absorption in Patients Undergoing Endoscopic Urosurgery: An Observational Study

Background and Objective: Anesthesiologists need to be familiar with perioperative changes in blood volume (BV);however, there is no standard method for repeated evaluation of BV over a short interval of time. We evaluated BV in the operation room using repeatable estimation methods. Method: Eighty-five ASA physical status I-II patients scheduled to undergo endoscopic urosurgery using irrigation fluid under general anesthesia at Nippon Medical School Hospital were included in this study. Irrigation with 3% sorbitol in water was commenced after establishment of general anesthesia and volumetric fluid balance, which was defined as control water balance (WB). Hematocrit (Hct), colloid osmotic pressure (COP), total protein (TP) and albumin (Alb) were repeatedly determined before and during anesthesia. BV was calculated using Allen’s formula and the changes in Hct, COP, TP and Alb. Main Outcome Measures: The main outcome was the accuracy of measuring changes in BV (△BV) calculated using the four serum markers. WB and the estimated △BV calculated from Hct, COP, TP and Alb (△BV-Hct, △BV-COP, △BV-TP, and △BV-Alb) were analysed using Pearson’s correlation coefficient test and Bland-Altman analysis. Results: Sixty-five patients were excluded. In the remaining 20 patients, there was a significant correlation between WB and △BV-COP (R2 = 0.72;P < 0.01), WB and △BV-TP (R2 = 0.59;P △BV-Alb (R2 = 0.57;P △BV-Hct (R2 = 0.06). Conclusion: △BV-COP, △BV-TP and △BV-Alb had correlation with WB. However, since COP can be measured repeatedly with simplified instruments under selected clinical circumstances, while TP and Alb cannot. COP is the most useful marker to measure △BV during perioperative period. Hct does not allow precise estimation of △BV.

Pharmacological intervention for chronic phase of spinal cord injury

Spinal cord injury is an intractable traumatic injury. The most common hurdles faced during spinal cord injury are failure of axonal regrowth and reconnection to target sites. These also tend to be the most challenging issues in spinal cord injury. As spinal cord injury progresses to the chronic phase, lost motor and sensory functions are not recovered. Several reasons may be attributed to the failure of recovery from chronic spinal cord injury. These include factors that inhibit axonal growth such as activated astrocytes, chondroitin sulfate proteoglycan, myelin-associated proteins, inflammatory microglia, and fibroblasts that accumulate at lesion sites. Skeletal muscle atrophy due to denervation is another chronic and detrimental spinal cord injury–specific condition. Although several intervention strategies based on multiple outlooks have been attempted for treating spinal cord injury, few approaches have been successful. To treat chronic spinal cord injury, neural cells or tissue substitutes may need to be supplied in the cavity area to enable possible axonal growth. Additionally, stimulating axonal growth activity by extrinsic factors is extremely important and essential for maintaining the remaining host neurons and transplanted neurons. This review focuses on pharmacotherapeutic approaches using small compounds and proteins to enable axonal growth in chronic spinal cord injury. This review presents some of these candidates that have shown promising outcomes in basic research(in vivo animal studies) and clinical trials: AA-NgR(310)ecto-Fc(AXER-204), fasudil, phosphatase and tensin homolog protein antagonist peptide 4, chondroitinase ABC, intracellular sigma peptide,(-)-epigallocatechin gallate, matrine, acteoside, pyrvate kinase M2, diosgenin, granulocyte-colony stimulating factor, and fampridine-sustained release. Anti-Obesity Effects of Dietary d-Allulose and Medium-Chain Triglycerides in High-Fat Diet-Fed Rats

d-Allulose, a rare sugar, exerts anti-obesity effects by inhibiting hepatic lipogenesis and promoting energy expenditure. Medium-chain triglycerides (MCTs) consist of three medium-chain fatty acids connected by glycerol. MCTs have been extensively investigated for their ability to reduce body fat accumulation. We previously investigated the anti-obesity effects of a combination of dietary d-allulose and MCT (5% - 13%) in rats;however, we could not confirm the anti-obesity effects of MCT or observed synergetic effects between d-allulose and MCT on body fat loss. We speculated that our previous studies were influenced by the excessive amount of MCT in the diets. Therefore, in this study, we aimed to investigate the anti-obesity effects of the simultaneous intake of d-allulose and MCT in rats fed an obesity-inducing high-fat diet with a low amount of MCTs (2%). Thirty-two male Wistar rats (3-week-old) were randomly divided into four groups: control, d-allulose, MCT, and d-allulose + MCT groups. Rats in each group were fed ad libitum on a control (no d-Allulose or MCT), 5% d-allulose, 2% MCT, or 5% d-allulose + 2% MCT diets for 16 weeks. Abdominal adipose tissue weights were significantly lower in the d-allulose diet group than in the control group, whereas no differences were observed between results of the MCT-supplemented groups. The total body fat mass was significantly lower in the d-allulose and MCT diet groups than in the control group, but no differences were observed between the MCT-supplemented groups. These results suggested that anti-obesity effects of dietary d-allulose were observed, and the effects of dietary MCTs were weaker than those of d-allulose. Moreover, we confirmed the interaction between dietary d-allulose and MCT on indicators of obesity. Interestingly, their effects were not synergistic, as MCT supplementation offset the anti-obesity effects of dietary d-allulose. 为了预测和评估深部高温岩体的稳定性,运用理论推导的方法研究温度作用下(后)岩石的本构关系。首先,基于已有的岩石统计损伤本构模型,考虑温度对岩石损伤变量的影响,引入了损伤变量修正系数、损伤起裂应力和起裂应变等参数;其次,假设微元体强度服从幂函数分布,并符合Hoek-Brown(H-B)强度准则,针对统计损伤本构模型无法反映峰前较强的微裂纹压密效应的弊端,引入了相应的模型修正系数;再次,建立了考虑温度作用的岩石统计损伤本构模型,并确定了模型参数的表达式;最后,对比不同温度作用下(后)的花岗岩在单轴和常规三轴压缩试验条件下所得的结果与文献中模型的计算结果,验证本文模型的准确性。研究结果表明:所提出的模型的计算结果与文献的试验结果在数值、分布规律和趋势上具有良好的一致性;对于全过程σ-ε曲线的各阶段,本文模型的计算效果较文献中本构模型计算效果更好,与试验曲线的贴合度更高,能够更好地反映高温作用下(后)岩石的损伤本构特征。该模型的参数具有常规性,适用于各类温-压组合作用工况下的岩石。

Matrine promotes neural circuit remodeling to regulate motor function in a mouse model of chronic spinal cord injury

In chronic phase of spinal cord injury, functional recovery is more untreatable compared with early intervention in acute phase of spinal cord injury. In the last decade, several combination therapies successfully improved motor dysfunction in chronic spinal cord injury. However, their effectiveness is not sufficient. We previously found a new effective compound for spinal cord injury, matrine, which induced axonal growth and functional recovery in acute spinal cord injury mice via direct activation of extracellular heat shock protein 90. Although our previous study clarified that matrine was an activator of extracellular heat shock protein 90, the potential of matrine for spinal cord injury in chronic phase has not been sufficiently evaluated. Thus, this study aimed to investigate whether matrine ameliorates chronic spinal cord injury in mice. Once daily intragastric administration of matrine(100 μmol/kg per day) to spinal cord injury mice were starte at 28 days after injury, and continued for 154 days. Continuous mat rine treatment improved hindlimb motor function in chronic spinal cord injury mice. In injured spinal cords of the matrine-treated mice, the density of neurofilament-H-positive axons was increased. Moreover, matrine treatment increased the density of bassoon-positive presynapses in contact with choline acetyltransferase-positive motor neurons in the lumbar spinal cord. These findings suggest that matrine promotes remodeling and reconnection of neural circuits to regulate hindlimb movement. All protocols were approved by the Committee for Animal Care and Use of the Sugitani Campus of the University of Toyama(approval No. A2013 INM-1 and A2016 INM-3) on May 7, 2013 and May 17, 2016, respectively.

Impact of solids on biphasic biodegradation of phenanthrene in the presence of hydroxypropyl-β-cyclodextrin(HPCD)

The consequence of polycyclic aromatic hydrocarbons(PAHs)in the environment is of great *** hydrophobic properties of PAHs significantly impact phase distribution causing limited *** biodegradation has been extensively carried out by surfactants and the redeployment effect was ***,the quantitative relationship concerning the impact of solids was rarely reported.A batch of biphasic tests were carried out by introducing Mycobacterium vanbaalenii PYR-1 and hydroxypropyl-β-cyclodextrin(HPCD)into a mixture of phenanthrene solution and various glass beads(GB37-63,GB105-125,and GB350-500).The comparative results demonstrated that HPCD had little effect on microbial growth and was not degradable by bacterium.A model was proposed to describe the biodegradation *** regression results indicated that the partition coefficient kd(1.234,0.726 and 0.448 L·g–1)and the degradation rate k(0 mmol·L^(–1):0.055,0.094,and 0.112;20 mmol·L^(–1):0.126,0.141,and 0.156;40 mmol·L^(–1):0.141,0.156 and 0.184 d^(–1))were positively and negatively correlated with the calculated total surface area(TSA)of solids,*** enhanced in the presence of HPCD,and the enhancing factor f was calculated(20 mmol·L^(–1):15.16,40.01,and 145.5;40 mmol·L^(–1):13.29,37.97,and 138.4),indicating that the impact of solids was significant for the enhancement of biodegradation.

Experimental Losses and Optimum Conditions for Phenanthrene Extraction

Polycyclic aromatic hydrocarbons （PAHs） are a class of pollutants that are widespread in the en- vironment. Their hydrophobic properties make accurate analyses very difficult. In addition, the experimental losses during extraction of PAHs are inevitable and of great concern. In this study, dichloromethane （DCM）, chloroform （CF）, and carbon disulfide （CDS） were used as solvents to extract phenanthrene （PHN）, a typical PAH, at various temperature and pressure reduction conditions. The extraction was improved using a multi- ple microtube extraction methodology. The results indicate that the CDS had the best performance followed by CF and DCM at 25~C and standard pressure. This was ascribed to the intensity of the molecular interac- tions between the solvent, the water, and the PHN due to their different molecular structures leading to dif- ferent stereo and hydrogen bonding effects. The optimum conditions for extraction of PHN by CDS were determined by correlating the experimental results. The regression shows that the experimental loss rate is linearly proportional to the pressure reduction and exponentially related to the temperature. The correlation can be used to improve extraction efficiencies.
