BACKGROUND Peritoneal dialysis(PD)catheter migration impedes the efficacy of ***,several techniques involving additional sutures or incisions have been proposed to maintain catheter position in the *** To evaluate the efficacy of creating a short musculofascial tunnel beneath the anterior sheath of the rectus abdominis during PD catheter *** Patients who underwent PD catheter implantation between 2015 and 2019 were included in this retrospective *** patients were divided into two groups based on the procedure performed:Patients who underwent catheter implantation without a musculofascial tunnel before 2017 and those who underwent the procedure with a tunnel after *** recorded patient character istics and catheter complications over a two-year follow-up *** addition,postoperative plain abdominal radiographs were reviewed to determine the catheter angle in the event of *** The no-tunnel and tunnel groups included 115 and 107 patients,*** to the no-tunnel group,the tunnel group showed lesser catheter angle deviation toward the pelvis(15.51±11.30 vs 25.00±23.08,P=0.0002)immediately after the operation,and a smaller range of migration within 2 years postoperatively(13.48±10.71 vs 44.34±41.29,P<0.0001).Four events of catheter dysfunction due to migration were observed in the no-tunnel group,and none occurred in the tunnel *** was no difference in the two-year catheter function survival rate between the two groups(88.90%vs 84.79%,P=0.3799).CONCLUSION The musculofascial tunnel helps maintain catheter position in the pelvis and reduces migration,thus preventing catheter dysfunction.

AIM:To investigate the use of lipid emulsion substitutingfor glucose in postoperative hypocaloric peripheral parenteral nutrition (HPPN).METHODS:This prospective, randomized study was conducted on 20 postoperative gastrointestinal cancer patients. They were randomized and equally divided into interventional group and control group, and both were administered isocaloric and isonitrogenous diets with for lipid emulsion substituting for partial glucose loads in the interventional ***: Nutritional parameters and biochemical data were compared between the two groups before and after 6-d of HPPN. Most investigated variables showed no significant changes after administration of HPPN with lipid emulsion. However, the postoperative triglyceride level was significantly lower in the interventional group than in the control group (P < 0.05). In comparison with lipid emulsion, glucose administration resulted in less decrease in postoperative prealbumin level (P < 0.05).CONCLUSION: In addition to supplementing with essential fatty acid, it seems that HPPN with lipid emulsion is well-tolerated and beneficial to postoperative gastrointestinal cancer patients.

BACKGROUND Radiologic adjacent segment degeneration(ASDeg)can occur after spinal *** segment disease(ASDis)is defined as the development of new clinical symptoms corresponding to radiographic changes adjacent to the level of previous spinal *** pre-existing ASDeg is generally considered to result in more severe ASDis;nonetheless,whether the ASDeg status before index surgery influences the postoperative risk of revision surgery due to ASDis warrants *** To identify possible risk factors for ASDis and verify the concept that greater preexisting ASDeg leads to more severe *** Data from 212 patients who underwent posterior decompression with Dynesys stabilization from January 2006 to June 2016 were retrospectively *** who underwent surgery for ASDis were categorized as group A(n=13),whereas those who did not were classified as group B(n=199).Survival analysis and Cox proportional hazards models were used to compare the modified Pfirrmann grade,University of California-Los Angeles grade,body mass index,number of Dynesys-instrumented levels,and *** The mean time of reoperation was 7.22(1.65–11.84)years in group A,and the mean follow-up period was 6.09(0.10–12.76)years in group *** significant difference in reoperation risk was observed:Modified Pfirrmann grade 3 vs 4(P=0.53)or 4 vs 5(P=0.46)for the upper adjacent disc,University of California-Los Angeles grade 2 vs 3 for the upper adjacent segment(P=0.66),age of60 years(P=0.9),body mass index25 kg/m2(P=0.3),and sex(P=0.8).CONCLUSION Greater preexisting upper ASDeg was not associated with a higher rate of reoperation for ASDis after Dynesys *** overweight tended to increase reoperation risk after Dynesys surgery for ASDis.

Gastrointestinal stromal tumor is the most common mesenchymal tumor in the gastrointestinal tract. It may coexist with other type of cancers,and if so,the tumors usually involve the stomach. The most common associated cancers are gastrointestinal carcinomas. We report a 65-year-old woman with a history of gastric gastrointestinal stromal tumor who had undergone subtotal segmental gastrectomy. New polypoid lesions were detected on a follow-up gastroscopy one year later. The lesions were biopsied and found to be carcinoid tumors. There was serum hypergastrinemia,and type 1 gastric carcinoid tumor was diagnosed. A total gastrectomy was performed. Pathologic examination revealed both carcinoid tumors and a recurrent gastrointestinal stromal tumor.

Hepatitis B virus(HBV)infection is a leading cause of chronic hepatitis,liver cirrhosis,and hepatocellular carcinoma *** to the shared modes of transmission,coinfection with HBV and human immunodeficiency virus(HIV)is not *** is estimatedthat 10%of HIV-infected patients worldwide are coinfected with *** areas where an HBV vaccination program is implemented,the HBV seroprevalence has declined *** HIV/HBV-coinfected patients,HBV coinfection accelerates immunologic and clinical progression of HIV infection and increases the risk of hepatotoxicity when combination antiretroviral therapy(cART)is initiated,while HIV infection increases the risk of hepatitis events,cirrhosis,and end-stage liver disease related to chronic HBV *** the advances in antiviral therapy,concurrent,successful longterm suppression of HIV and HBV replication can be achieved in the cART *** reduce the disease burden of HBV infection among HIV-infected patients,adoption of safe sex practices,avoidance of sharing needles and diluent,HBV vaccination and use of cART containing tenofovir disoproxil fumarate plus emtricitabine or lamivudine are the most effective ***,due to HIV-related immunosuppression,using increased doses of HBV vaccine and novel approaches to HBV vaccination are needed to improve the immunogenicity of HBV vaccine among HIV-infected patients.

Taipei is a city full of oriental traditional *** from other Chinese cities,Taipei has a very specific urban fabric and development context that provides excellent conditions for the development of cultural and creative industries(CCIs).Since 2011,Taipei City Government has set up 11 creative quarters that have demonstrated the unique features and lifestyles of Creative *** this paper,the authors introduce Creative Taipei in 4 parts:examining the CCIs conditions of Taipei,identifying strengths of Taipei’s CCIs,understanding governance strategies and policies,and analyzing creative city governance through case *** into the future of Creative Taipei,this paper concludes that the clear leading policies,the attractive investment environment for private investors,and the cross-boundary integration are three dimensions to further strengthen the power of CCIs in Taipei.

The purpose of this study was to identify the availability of authoring tools that support vertical flow function based upon empirical experiments. There were five EPUB authoring tools selected for this study. Four major editing procedures were designed for the project. The first was the support of Chinese character input. The second was vertical form editing. The third was ruby position editing, and finally, testing whether the editing software was capable of outputting a document as an e-Book in EPUB 3.0 format. Five programs were tested, and only one of the five was able to fulfill the aforementioned requirements. The usability of each program is positively verified through this study.

Identification of carriers of fragile X syndrome(FXS) with the subsequent prenatal diagnosis and knowledge of FXS-associated genetic profiles are essential for intervention in specific populations. We report the results of carrier screening of 39,458 East Asian adult women and prenatal diagnosis from 87 FXS *** prevalence of FXS carriers and full mutation fetuses was estimated to be 1/581 and 1/3124 in East Asian populations, respectively. We confirmed the validity of the current threshold of CGG trinucleotide repeats for FMR1 categorization;the integral risks of full mutation expansion were approximately 6.0%,43.8%, and 100% for premutation alleles with 55—74, 75—89, and ≥ 90 CGG repeats, respectively. The protective effect of AGG(adenine-guanine-guanine nucleotides) interruption in East Asian populations was validated, which is important in protecting premutation alleles with 75—89 CGG repeats from full mutation expansion. Finally, family history was shown not an effective indicator for FXS carrier screening in East Asian populations, and population-based screening was more cost-effective. This study provides an insight into the largest carrier screening and prenatal diagnosis for FXS in East Asian populations to date. The FXSassociated genetic profiles of East Asian populations are delineated, and population-based carrier screening is shown to be promising for FXS intervention.
