Prospective randomized comparison of oral sodium phosphate and polyethylene glycol lavage for colonoscopy preparation

AIM: To compare the effectiveness, patient acceptability, and physical tolerability of two oral lavage solutions prior to colonoscopy in a Taiwan Residents population. METHODS: Eighty consecutive patients were randomized to receive either standard 4 L of polyethylene glycol (PEG) or 90 mL of sodium phosphate (NaP) in a split regimen of two 45 mL doses separated by 12 h, prior to colonoscopic evaluation. The primary endpoint was the percent of subjects who had completed the preparation. Secondary endpoints included colonic cleansing evaluated with an overall assessment and segmental evaluation, the tolerance and acceptability assessed by a selfadministered structured questionnaire, and a safety profile such as any unexpected adverse events, electrolyte tests, physical exams, vital signs, and body weights. RESULTS: A significantly higher completion rate was found in the NaP group compared to the PEG group(84.2% vs 27.5%, P＜0.001). The amount of fluid suctioned was significantly less in patients taking NaP vs PEG (50.13±54.8 cc vs 121.13±115.4 cc, P＜0.001),even after controlling for completion of the oral solution(P = 0.031). The two groups showed a comparable overall assessment of bowel preparation with a rate of 'good' or 'excellent' in 78.9% of patients in the NaPgroup and 82.5% in PEG group (P = 0.778). Patients taking NaP tended to have significantly better colonic segmental cleansing relative to stool amount observedin the descending (94.7% vs 70%, P = 0.007) andtransverse (94.6% vs 74.4%, P = 0.025) colon. Slightly more patients graded the taste of NaP as 'good' or 'very good' compared to the PEG patients (32.5% vs 12.5%;P = 0.059). Patients' willingness to take the same preparation in the future was 68.4% in the NaP compared to 75% in the PEG group (P = 0.617). There was a significant increase in serum sodium and a significant decrease in phosphate and chloride levels in NaP group on the day following the colonoscopy without any clinical sequelae. HELIOCENTRIC THEORY IN CHINA——IN COMMEMORATION OF THE QUINCENTENARY OF THE BIRTH OF NICOLAUS COPERNICUS

This paper deals mainly with the arduous struggles of the Copernican theory in China. In the first place, it gives an account of the Chinese conception of the motion of earth before Copernicus. From the Warring States Period down to the sixteenth century, ideas advocating an earth in motion or an earth at rest constantly antagonize each other in China. In Ch’ung-Chen-Li-Shu which was compiled in 1634, a good deal of materials were translated from De revolutionibus orbium coelestium. However, because of the concealment and distortion made by the Jesuit missionaries, the Copernican theory was not introduced into China until mid-eighteenth century. It finally won the triumph at the end of the nineteenth century after protracted struggles.

Three New Cucurbitacins from Hemsleya lijiangensis

Three new cucurbitacins were isolated from the BuOH extract of the rhizomes of Hemsleya lijiangensis. Their structures were elucidated as 23, 24 -dihydro cucurbitacin F-16, 25-diacetate(1), 23, 24-dihydro cucurbitacin F-16, 25-diacetate-2-O-β-D glucopyranoside (2), 23, 24-dihydro-cucurbitacin F-16-acetate(3), respectively; by spectral analyses.

Long-term outcome and efficacy of endoscopic hemorrhoid ligation for symptomatic internal hemorrhoids

AIM:To assess the long-term outcome of endoscopic hemorrhoid ligation (EHL) for the treatment of symptomatic internal ***:A total of 759 consecutive patients (415 males and 344 females) were *** presentations were rectal bleeding (593 patients) and mucosal prolapse (166 patients).All patients received EHL at outpatient *** severity was classified by Goligher's *** mean follow-up period was 55.4 mo (range,45-92 mo).RESULTS:The number of band ligations averaged 2.35 in the first session for bleeding and 2.69 for prolapsed *** was controlled in 587 (98.0%) patients,while prolapse was reduced in 137 (82.5%) *** treatment,93 patients experienced anal pain and 48 patients had mild *** subjective satisfaction was 93.6%.Repeat treatment or surgery was performed if symptoms were not relieved in the first *** the bleeding group,the recurrence rate was 3.7% (22 patients) at 1 year,and 6.6%and 13.0% at 2 and 5 *** the prolapsed group,the recurrence rate was 3.0%,9.6% and 16.9% at 1,2 and 5 years,***:EHL is an easy and well-tolerated procedure for the treatment of symptomatic internal hemorrhoids,with good long-term results.

Large-vessel thrombosis in intestinal Behet's disease complicated with myelodysplastic syndrome and trisomy 8

Behet's disease is characterized by recurrent oral ulcers, genital ulcers, uveitis and skin lesions. Myelodysplastic syndrome (MDS) is characterized by problems due to ineffective hematopoiesis. Several studies have identified a relationship between MDS and Behet's disease, especially intestinal Behet's disease. Trisomy 8 seems to play an important role in these disorders as well. The present case was a 24-year-old woman who had a huge tonsil ulcer with initial symptoms of odynophagia and intermittent fever. We also noted folliculitis on her upper back. Five days later, she began to experience diarrhea and abdominal pain. Abdominal computed tomography and subsequent surgery revealed ileum perforation and enterocolitis with multiple ulcers. Later, she was admitted again for a vulvar suppurative ulcer and suspicious Bartholin's cyst infection. The patient's clinical presentations met the criteria for Behet's disease. Six months after the bowel perforation event, we noted the development of pancytopenia in a routine laboratory examination. All the examinations led to the diagnosis of MDS with trisomy 8. The most unusual finding was that multiple large vessel thrombi developed during follow-up. Previous studies have suggested that trisomy 8 in MDS leads to concurrent intestinal Behet's disease. Moreover, the inflammatory and immune genes related to thrombus formation are overexpressed in cases of MDS with trisomy 8. Trisomy 8 must play a role in thrombosis. Further studies are needed to help clarify the pathophysiology and pathogenesis of these disorders.

Their structures were determined on the bases of the spectral and chemical evidences.
