There are five vital signs that healthcare providers assess: temperature, pulse, respiration, blood pressure, and pain. Normal levels for the five vital signs are published by the American Heart Association, and other specialty organizations, however, the sixth vital sign (resilience) which adopts the measure of immune resilience is suggested in this paper. Resilience is the ability of the immune system to respond to attacks and defend effectively against infections and inflammatory stressors, and psychological resilience is the capacity to resist, adapt, recover, thrive, and grow from a challenge or a stressor. Individuals with better optimal immune resilience had better health outcomes than those with minimal immune resilience. The purpose of this paper is to conceptualize, contextualize, and operationalize all six vital signs. We suggest measuring resilience subjectively and objectively. Subjectively, use a 5-item guided interview revised from the Connor-Davidson Resilience Scale (CDRC), a scale of 10 items. The revised CDRC scale is a 5-item scale. The scale is rated on a 5-point Likert scale from 0 (not true) to 4 (true all the time). The total score ranges from 0 to 20, with higher total scores indicating greater resilience. The scale demonstrated good construct validity and internal consistency (α = 0.85) during the development of the scale. The CD-RISC had a good Cronbach’s alpha level of 0.85. The Revised CD-RISC can be completed in 2 - 4 minutes. To measure resilience objectively, we suggest using Immune Resilience (IR) levels, the level of resilience to preserve and/or rapidly restore immune resilience functions that promote disease resistance and control inflammation and other inflammatory stress. IR levels are gauged with two peripheral blood metrics that quantify the balance between CD8 and CD4 T-cell levels and gene expression signatures tracking longevity-associated immunocompetence and mortality- or entropy-associated inflammation. Background:To evaluate the effects on near and intermediate visual performance after bilateral Laser Anterior Ciliary Excision(LaserACE)***:LaserACE surgery was performed using the VisioLite 2.94μm erbium:yttrium-aluminum-garnet(Er:YAG)ophthalmic laser system in 4 oblique quadrants on the sclera over the ciliary muscle in 3 critical zones of physiological importance(over the ciliary muscles and posterior zonules)with the aim to improve natural dynamic accommodative *** was performed on 26 patients(52 eyes).Outcomes were analyzed using visual acuity testing,Randot stereopsis,and the CatQuest 9SF patient ***:Binocular uncorrected near visual acuity(UNVA)improved from+0.20±0.16 logMAR preoperatively,to+0.12±0.14 logMAR at 24 months postoperatively(p=0.0014).There was no statistically significant loss in distance corrected near visual acuity(DCNVA).Binocular DCNVA improved from+0.21±0.17 logMAR preoperatively,to+0.11±0.12 logMAR at 24 months postoperatively(p=0.00026).Stereoacuity improved from 74.8±30.3 s of arc preoperatively,to 58.8±22.9 s of arc at 24 months postoperatively(p=0.012).There were no complications such as persistent hypotony,cystoid macular edema,or loss of best-corrected visual acuity(BCVA).Patients surveyed indicated reduced difficulty in areas of near vision,and were overall satisfied with the ***:Preliminary results of the LaserACE procedure show promising results for restoring visual performance for near and intermediate visual tasks without compromising distance vision and without touchin. the visual *** visual function and visual acuity improvements had clinical *** satisfaction was high postoperatively and sustained over 24 months.

AIM： To analyze 67 cases of splenic abscess in a medica center of Taiwan during a period of 19 years. METHODS： From January 1986 to December 2004, a total of 67 patients with splenic abscess were enrolled for the retrospective study. The clinical characteristics, underlying diseases, organism spectra, therapeutic methods, APACHE Ⅱ scores, and mortality rates were analyzed. RESULTS： There were 41 males and 26 females with the mean age of 54.14-14.1 years. Multiple splenic abscesses （MSA） account for 28.4% and solitary splenic abscess in 71.6% of the patients. Twenty-six of sixtyseven patients （35.8%） had extrasplenic abscesses, with leading site of liver （34.6%）, Microbiological cultures were positive in 58 patients （86.6%）, with 71.8% in blood culture and 93.5% in abscess culture. Gram negative bacillus （GNB） infection predominated （55.2%）, with leading pathogen of Klebslella pneumoniae （22.4%）, followed by gram positive coccus （GPC） infection （31%）. Splenectomy was performed in 26 patients （38.8%）, percutaneous drainage or aspiration in 21 （31.3%）, and antibiotic therapy alone in 20 patients （29.9%）. Eventually, 12 of 67 patients expired （17.9 %）. By statistics, spleen infected with GNB was likely to develop multiple abscesses compared with infection with GPC （P=0.036）. Patients with GNB infection （P=0.009） and multiple abscesses （P=0.011） experienced a higher mortality rate than patients with GPC infection and solitary abscess. The mean APACHE Ⅱ score of 12 ex- pired patients （16.3±3.2） was significantly higher than that of the 55 survivals （7.2 ± 3.8）（P〈 0.001）. CONCLUSION： MSA, GNB infection, and high APACHE Ⅱ scores are poor prognostic factors. Early surgical intervention should be encouraged when these risk factors are present.

Electric vehicle has attracted more and more attention all around the world in recent years because of its many advantages such as low pollution to the environment. However, due to the limitation of current technology, charging remains an important issue. In this paper, we study the problem of finding and making reservation on charging stations via a vehicular ad hoc network (VANET). Our focus is on the privacy concern as drivers would not like to be traced by knowing which charging stations they have visited. Technically, we make use of the property of blind signature to achieve this goal. In brief, an electric vehicle first generates a set of anonymous credentials on its own. A trusted authority then blindly signs on them after verifying the identity of the vehicle. After that, the vehicle can make charging station searchin. queries and reservations by presenting those signed anonymous credentials. We implemented the scheme and show that the credential signing process (expected to be the most time consuming step) can be completed within reasonable time when the parameters are properly set. In particular, the process can be completed in 5 minutes when 1024 bits of RSA signing key is used. Moreover, we show that our scheme is secure in terms of authentication and privacy-preserving.

Increased levels of surfactant protein D (SP-D) and lipid-laden foamy macrophages (FMs) are frequently found under oxidative stress conditions and/or in patients with chronic obstructive pulmonary disease (COPD) who are also chronically exposed to cigarette smoke (CS). However, the roles and molecular mechanisms of SP-D and FMs in COPD have not yet been determined. In this study, increased levels of SP-D were found in the bronchoalveolar lavage fluid (BALF) and sera of ozone- and CS-exposed mice. Furthermore, SP-D-knockout mice showed increased lipid-laden FMs and airway inflammation caused by ozone and CS exposure, similar to that exhibited by our study cohort of chronic smokers and COPD patients. We also showed that an exogenous recombinant fragment of human SP-D (rfhSP-D) prevented the formation of oxidized low-density lipoprotein (oxLDL)-induced FMs in vitro and reversed the airway inflammation and emphysematous changes caused by oxidative stress and CS exposure in vivo. SP-D upregulated bone marrow-derived macrophage (BMDM) expression of genes involved in countering the oxidative stress and lipid metabolism perturbations induced by CS and oxLDL. Our study demonstrates the crucial roles of SP-D in the lipid homeostasis of dysfunctional alveolar macrophages caused by ozone and CS exposure in experimental mouse emphysema, which may provide a novel opportunity for the clinical application of SP-D in patients with COPD.

基于地雷独特机械特性和声-地震耦合原理的声-地震耦合探雷技术,在埋设地雷的安全有效探测方面具有广阔的应用前景,但针对实用工程探雷系统的研究还需要做大量工作。其中,声波耦合的地表振动信号非常微弱复杂,如何对其进行精确快速测量是一个关键难题。本文在声-地震耦合探雷技术原理的基础上,对地表振动的非接触激光测量技术(包括激光多普勒干涉技术、电子散斑干涉技术和激光自混合干涉技术)进行综述分析,并分析了电子剪切散斑干涉技术用于声-地震耦合探雷的可行性。
