Management of Pheochromocytoma in Dakar: Diagnostic and Therapeutic Advances throughout 16 Cases

Introduction: Management of pheochromocytoma faces different challenges leading to misdiagnosis and therapeutic delay in developing countries. The aim of our study was to evaluate the diagnostic and therapeutic features as well as the evolution of our management strategies in hospital setting in recent years in Dakar. Patients and Method: We carried out a retrospective study over 12 years (January 2005 to June 2017), in 4 hospitals in Dakar. We only included patients with established diagnosis of pheochromocytoma upon clinical, biological, radiological and histological arguments. Results: During this period, 16 observations have been collected and were all about female patients. The mean age at the time of diagnosis was 36.81 years. The clinical signs were dominated by hypertension found for 14 patients. At biological examination, hypersecretion catecholamine was found in 13 patients, high urinary metanephrines in 7 patients, high plasmatic metanephrines in 2 patients and raised urinary Vanyl Mandelic Acid (VMA) in 4 patients. Dosage of chromogranin A showed high level for one patient. Abdominal CT scan allowed making the adrenal origin of tumors precise. One patient presented bilateral pheochromocytoma. Before being treated surgically, all patients had had medical preparation. The surgical treatment by median laparotomy consisted of an enucleation of the tumor in 2 patients, and adrenal resection in *** evolution after surgery was favorable in 12 patients out of 13. One patient died 2 days after surgery by hemodynamic instability. In 2 patients who presented metastases, multidrug therapy with cyclophosphamide, vincristine and dacarbazine was decided after a multidisciplinary meeting, but they died before treatment. Conclusion: Management of pheochromocytomas in developing countries is obviously improving, but the mortality rate remains high. Renal Disease among HIV Positive Patients in Senegal

Introduction: Renal disease (RD) in human immunodeficiency virus (HIV) infection is a decisive turning point in the development and prognosis of this disease. In Africa, the prevalence varies between 2.5% and 48.6%. In Senegal, little data are available in the literature. The objective of our study was to describe the epidemiological, clinical, paraclinical, therapeutic and progressional aspects in patients living with HIV (PLWHIV). Patients and methods: This was a retrospective, descriptive and analytical study carried out over a 10-year period in the Department of Internal Medicine and Nephrology at the Aristide Le Dantec Hospital in Dakar, Senegal. We included all 15-year old and above PLHIV with available CD4 count and viral load. Results: Out of 248 PLHIV, 32 had kidney disease (KD), which means a hospital prevalence of 12.9%. The mean age was 51.22 ± 10 years (extremes of 36 and 77 years) with a sex ratio (male/female) of 1.28. Renal signs were dominated by glomerular nephropathy syndrome. It was present at 80%. Tubulo-interstitial nephropathy syndrome and chronic uremic syndrome accounted for 6.25% and 3.1% of cases, respectively. Renal function Impairment was present in 21 patients with 18 cases of acute kidney injury (85.7%) and 3 cases of chronic renal failure (14.3%), including 2 in stage 5 of chronic kidney disease. Renal biopsy (RB) was indicated and performed in 20 (62.5%) patients with glomerular signs in 12 patients (60%). Glomerular lesions were dominated by focal and segmental glomerulosclerosis (FSGS) in 6 cases, membraneous nephropathy (MN) in 4 cases and minimal change disease (MCD) in 2 cases. Tubulo-interstitial and vascular lesions were present in 45% and 12.5% of cases, respectively. In highly active antiretroviral therapy (HAART), 12 (37.5%) patients had total remission, 9 (28.12%) had partial remission. Nutritional Profile of Children with Major Sickle Cell Syndrome at the Centre of Medical and Health Advice of Kipé, Conakry, 2018

Introduction: Growth is a reflection of a child’s health and nutritional status. Children with sickle cell disease often have slower statural and weight development. The aim of this study was to evaluate the nutritional profile of children with sickle cell disease (SCD) registered in the CEMECO centre database. Methodology: This was a cross-sectional study with simple random sampling of children aged 1 to 16 years registered in the clinic database. Results: We collected information on 208 children, 121 of whom had sickle cell disease and 87 of whom were normal, with a sex ratio of 1.02. The mean age of the sickle cell patients was 8.7 ± 4.4 years, while that of the non-sickle cell patients was 9.5 ± 4 years. Haemoglobin electrophoresis revealed 103 homozygous (SS), 18 double heterozygous (SC, SBetaThal, SE) and 87 normal (AA) and/or sickle cell trait (AS) sickle cell cases. We observed a significant difference in the height/age ratio (P ¥). Conclusion: The results of our study revealed stunted growth in children with sickle cell disease.

Interventional Ultrasound in Ambulatory Urological Practice: Experience of the Medicis Clinic in Conakry

Introduction: Minimally invasive urological interventions have long been practiced blindly in our context. The popularization of ultrasound in urological practice has contributed to changing habits by making the procedures safer and more effective. The objective of this work was to study the technical aspects, indications and results of minimally invasive urological procedures guided by ultrasound. Material and methods: This was a prospective descriptive study lasting 1 year from January to December 2023 at the Médicis clinic in Conakry. Results: We carried out 126 ultrasound-guided urological interventions in one year, including 6 percutaneous nephrostomies (NPC), one renal cyst puncture-aspiration-sclerotherapy, 102 prostate biopsies, 5 drainages by transrectal aspiration of prostatic abscesses, 10 placements of cystostomy probes. Suprapubic, 2 puncture-aspiration-sclerotherapies for vaginal hydroceles. The average age of our patients was 69 years and the predominant age range was 61 - 70. The majority of our patients were male (97.61%) with a sex ratio of 41 men to one woman. Ultrasound-guided urological interventions were for diagnostic purposes (80.95%), for therapeutic purposes (15.77%) and for diagnostic and therapeutic purposes (3.96%). The indications for ultrasound-guided urological interventions were dominated by prostate pathologies (84.91%). The majority of interventions were performed rectally (84.92%) versus (13.43%) abdominally and only 1.58% scrotalally. All procedures were performed in B-ultrasound mode under local anesthesia and on an outpatient basis. For therapeutic interventions, the patients were seen again one month after the procedure, the clinical examination and ultrasound control found the evolution to be favorable. Conclusion: ultrasound is useful in urological practice for guiding minimally invasive interventions. The indications for ultrasound-guided procedures in urology are dominated by prostate pathologies. These procedures are safe

In the tropics, lowland rice cultivation is often confronted with the problem of iron toxicity. The solution proposed by research in general is the use of industrial silicon. However, the high cost of industrial silicon limits its adoption by farmers. A study was carried out in Zakogbeu;Center-West of Côte d’Ivoire, to assess the potential of kaolin to mitigate the effect of this soil constraint on the root of the rice plant. Five kaolin-based treatments were analyzed (T0 = 0 kg kaolin ha−1, T1 = 366 kg kaolin ha−1, T2 = 736 kg kaolin ha−1, T3 = 1097 kg kaolin ha−1 and T4 = 1465 kg kaolin ha−1 are 0, 200, 400, 600 and 800 kg SiO2 ha−1) in a device in complete random blocks, with 5 repetitions. The results obtained show that kaolin supply increases the length of the root tissue as well as the number of branching of the root of the rice plant. Root tissue increased from 10 cm with T0 treatment to more than 15 cm with treatment T4. The microscopic observation of the roots shows that in the treatment T0, the roots present only primary ramifications and the tertiary and quaternary ramifications are observed with the treatments T3 and T4. The contribution of kaolin is an alternative to inhibit the effect of iron toxicity on the rice plant root development in iron toxicity condition.

This study assessed the contribution of climate projections to improving rainfall information for cocoa crops in the central and southern regions of Côte d’Ivoire. Particular attention was paid to fourteen localities in these two climatic zones. Simulation data were obtained from the CORDEX ensemble and observation data from CHIRPS. They cover the period 1991-2005 for the reference period and the future period from 2021 to 2050 for the RCP4.5 and RCP8.5 scenarios. In addition, the study was based on the water requirements necessary during the critical phase of the cocoa tree (the flowering phase) for a good yield from the cocoa production chain on the one hand, and on a selection of three climate indices CDD, CWD and r95PTOT to study their spatio-temporal changes over two future periods 2021-2035 (near future) and 2036-2050 (medium-term) on the other. These climatic indices influence cocoa cultivation and their use in studies of climatic impacts on agriculture is of prime importance. The analysis of their spatio-temporal changes in this work also contributes to providing climate services based on rainfall, to which cocoa crops are highly sensitive. Our results show that the CDD and CWD indices vary from one region to another depending on latitude. For the fourteen localities studied, the number of consecutive dry days (CDD) could increase between now and 2050, while the number of consecutive wet days (CWD) could decrease over the period 2021-2035 and then increase over the period 2036-2050. The localities of Tabou, Aboisso and San-Pedro record high numbers of CDD index and CWD index for both projection scenarios. In comparison with the RCP4.5 and RCP8.5 scenarios, these results show that the RCP8.5 scenarios are having an impact on cocoa growing in Côte d’Ivoire.

Imaging of target with complicated motion using ISAR system based on IPHAF-TVA

The technique of imaging a target with a complicated motion using an Inverse Synthetic Aperture Radar(ISAR) system is an effective tool in the field of radar signal processing. After the translational compensation, the received signal reflected from the target can take the form of a multi-component Polynomial Phase Signal(m-PPS), and the high quality ISAR image can be provided via the combination between the estimated parameters of the m-PPS and the Range Instantaneous-Doppler technique(RID). For a target with a high maneuvrability, the occurrence of scatterers Migration Through Resolution Cell(MTRC), caused by the rotational movement could be appearing. That is why the variation in the amplitude of the echo during the time of observation cannot be neglected. The purpose of this study is the parameters estimation of the m-PPS signal with order three in the case of the Time Varying Amplitude(TVA). The Improved-version of the Product High-order Ambiguity Function(IPHAF) with TVA is proposed to improve the quality of the ISAR image compared with traditional techniques based on a constant amplitude;the experimental outcomes confirm that the new IPHAF-TVA method presented in this study is an effective technique to make the ISAR image very clear.

Left Hemispherical Subdural Hematoma on the Same Side as the Motor Deficit (Kernohan)

Chronic subdural hematoma represents 25% of traumatic subdural collections;common in elderly subjects with a clear male predominance. It occurs over the age of 50 in more than 90% of cases and 50% of patients have no history of head trauma even if the latter remains the main risk factor with others such as coagulopathy, anticoagulant treatment, chronic alcoholic poisoning. Its discovery is rarely fortuitous and has an important clinical polymorphism including an intracranial hypertension syndrome (complete or incomplete). We report a clinical case of a left hemispheric subacute subdural hematoma with post-traumatic falcorial involvement in a 70-year-old patient admitted with a picture of impaired consciousness and weakness of the left hemibody. The objective of this work is to draw the attention of neurosurgeons and neurologists to the paradoxical existence of the neurological deficit on the same side as the causal brain lesion.

Interactions of zinc and cadmium toxicity in their effects on growth and in antioxidative systems in tomato plants (Solanum lycopersicum)

The interaction between zinc and cadmium was investigated in tomato plants （Solanum lycopersicum）. Ten-day-old seedlings were treated with 10 μmol/L CdC12 associated to different concentrations of ZnC12 （10, 50, 100, and 150 μmol/L）. Zn supply clearly reduced Cd accumulation in leaves and simultaneously increased Zn concentration. Cd induced oxidative stress in leaves as indicated by an increase in thiobarbituric acid-reactive substances （TBARS） level and chlorophyll breakdown. Furthermore, compared with control, Cd- treated plants had significantly higher activities of superoxide dismutase （SOD, EC 1.***.1.1）, whereas, catalase （CAT, EC 1.***.1.6）, ascorbate peroxidase （APX, EC 1.***.1.11）, and glutathione reductase （GR, EC 1.***.4.2） activities were significantly suppressed by Cd addition. Zn supplementation, at low level, restored and enhanced the functional activity of these enzymes （SOD, CAT, APX and GR） as compared to Cd-alone-treated plants. The beneficial effect of adequate Zn level on Cd toxicity was confirmed by a significant decrease in TBARS level and restoration of chlorophyll content. However, when Zn was added at high level in combination with Cd there was an accumulation of oxidative stress, which was higher than that for Cd or excess Zn alone treatments. These results suggested that higher Zn concentrations and Cd are synergistic in their effect on plant growth parameters and oxidative stress.

Thermal performance of lightweight concrete applications in building envelopes in Lebanon

Innovative building materials are being used in building envelopes for reducing their heating and cooling *** paper aims to assess the thermal impact of using lightweight concrete in Lebanese building constructions by pouring an 8 cm thickness of lightweight concrete on the roof and the slab and replacing traditional hollow concrete block by lightweight concrete *** properties of two different samples were experimentally determined:the first one(558 kg/m^(3))used for the roof and the slab and the second one(1074 kg/m^(3))used for the *** numerical simulations were carried out for a Lebanese traditional detached house using the characteristics of these two *** thermally improved Light Weight Concrete building(LWC)was compared to a traditional Lebanese house base case(BC)using a dynamic building energy simulation tool in the four different Lebanese climate zones:coastal,mid-mountain,mountain,and inland *** results highlight the effectiveness of integrating LWC to building envelopes by reducing energy consumption and improving thermal comfort in both winter and summer climate conditions and in the different Lebanese climatic *** paper demonstrates that the use of LWC in the vertical walls replacing the traditional hollow blocks can reduce the heating needs by up to 9%and by up to 13%for cooling *** the other hand,adding a LWC roof screed has a very high impact on cooling and heating energy consumption,which can reach up to 74%in cooling energy savings and up to 24%in heating energy savings.
