AIM To examine treatment decisions of gastroenterologists regarding the choice of prescribing 5-aminosalycilates(5AsA) with corticosteroids(Cs) versus corticosteroids alone for patients with active ulcerative colitis(UC). METHODs A cross-sectional questionnaire exploring physicians' attitude toward 5AsA + Cs combination therapy vs Cs alone w.s.developed and validated. The questionnaire w.s.distributed to gastroenterology experts in twelve countries in five continents. Respondents' agreement with stated treatment choices were assessed by standardized Likert scale. Background professional characteristics of respondents were analyzed for correlation with responses. REsULTs six hundred and sixty-four questionnaires were distributed and 349 received(52.6% response rate). Of 340 eligible respondents, 221(65%) would continue 5AsA in a patient hospitalized for intravenous Cs treatment due to a moderate-severe UC flare, while 108(32%) would stop the 5AsA(P similarly, 62% would continue 5AsA in an out-patient starting oral Cs. However, only 140/340(41%) would proactively start 5AsA in a hospitalized patient not receiving 5AsA before admission. Most(94%) physicians consider the safety profile of 5AsA as very good. Only 52% consider them inexpensive, 35% perceive them to be expensive and 12% are undecided. On multi-variable analysis, less years of practice and perception of a plausible additive mechanistic effect of 5AsA + Cs were positively associated with the decision to continue 5AsA with Cs. CONCLUsION Despite the absence of data supporting its benefit, most gastroenterologists endorse combination of 5AsA + Cs for patients with active moderate-to-severe UC. Randomized controlled trials are needed to assess if 5AsA confer any benefit for these patients.

环境友好水性聚氨酯(wPU)已广泛应用于涂料和胶粘剂领域。然而相对于溶剂型聚氨酯,水性聚氨酯在耐化学品、耐热和机械性能等方面还较差。在聚合物纳米复合材料中,填料对于改变聚合物的性质扮演着重要的角色。本文通过改变二羟甲基丙酸(DMPA)含量和扩链剂的种类,在硅烷处理氧化锌存在下制备了水性聚氨酯纳米复合材料。水性聚氨酯由以下原料合成:聚酯多元醇、4,4-二环已基甲烷二异氰酸酯(HMDI)、二羟甲基丙酸(DMPA)、三乙胺(TEA)、乙二胺(EDA)、1,4-丁二醇(BD)及经烷基硅烷处理的纳米氧化锌。

we describe an unusual case of extragastrointestinal stromal tumor （EGIsT） presenting as a scrotal mass. A 71- year-old man presented with a gradually enlarging scrotal mass with a 20-year duration. Physical examination re vealed a huge （as large as volleyball）, round, nontender mass occupying the whole scrotum, which w.s.resected completely. Clinical and radiological findings did not comply with any other primary site disease. Under histological examination, the tumor showed a spindle cell pattern with low cellularity, absence of necrotic and mitotic features. Immunohistochemical anlaysis revealed the tumor reactive for CD117 and CD34, while negative for smooth muscle actin, desmin and s-100 protein. To our knowledge, this is the first reported case of an EGIsT involving the scrotum.

目的旨在评估超声成像对儿童造血干细胞移植术(HsCT)后临床可疑肝静脉闭塞性疾病(VOD)的预测价值。方法 216例儿童行造血干细胞移植术,其中70例行彩色多普勒超声检查,检查期间,70例中有59例仅出现单一症状或体征,这些症状不符合临床诊断标准(临床怀疑VOD),最终20例确诊VOD的病人纳入VOD组,另外39例符合其他条件者纳入非VOD组。回顾性对比分析2组间以下指标:门静脉左支(峰值、速度、方向),肝左动脉(收缩末期/舒张末期速度、阻力指数),肝中静脉(峰值、速度、期相性),肝肿大,脾肿大,胆囊癌壁厚和腹水。结果 VOD组与门静脉高反流量(P=0.011)、左肝动脉收缩期峰值速度(P=0.028)、肝中静脉单相(P=0.015)、肝大(P=0.001)、胆囊壁厚度(PsCT术后临床可疑VOD进展至VOD显著相关。
