Dendritic cell(DC)-based vaccines continue to be considered an attractive tool for cancer *** require an additional signal from the environment or other immune cells to polarize the development of immune responses toward T helper 1(Th1)or Th2 *** play a role in natural killer(NK)cell activation,and NK cells are also able to activate and induce the maturation of *** investigated the types of NK cells that can induce the maturation and enhanced function of DCs and the conditions under which these interactions *** that were activated by resting NK cells in the presence of inflammatory cytokines exhibited increased expression of several costimulatory molecules and an enhanced ability to produce *** cell-stimulated DCs potently induced Th1 polarization and exhibited the ability to generate tumor antigen-specific cytotoxic T lymphocyte *** data demonstrate that functional DCs can be generated by coculturing immature DCs with freshly isolated resting NK cells in the presence of Toll-like receptor agonists and proinflammatory cytokines and that the resulting DCs effectively present antigens to induce tumor-specific T-cell responses,which suggests that these cells may be useful for cancer immunotherapy.

Low survival rate occurs in patients who initially experience a spontaneous return of circulation after cardiac arrest（CA）. In this study, we induced asphyxial CA in adult male Sprague-Daley rats, maintained their body temperature at 37 ± 0.5°C, and then observed the survival rate during the post-resuscitation phase. We examined neuronal damage in the hippocampus using cresyl violet（CV） and Fluore-Jade B（F-J B） staining, and pro-inflammatory response using ionized calcium-binding adapter molecule 1（Iba-1）, glial fibrillary acidic protein（GFAP）, and tumor necrosis factor-alpha（TNF-α） immunohistochemistry in the hippocampus after asphyxial CA in rats under normothermia. Our results show that the survival rate decreased gradually post-CA（about 63% at 6 hours, 37% at 1 day, and 8% at 2 days post-CA）. Rats were sacrificed at these points in time post-CA, and no neuronal damage was found in the hippocampus until 1 day post-CA. However, some neurons in the stratum pyramidale of the CA region in the hippocampus were dead 2 days post-CA. Iba-1 immunoreactive microglia in the CA1 region did not change until 1 day postCA, and they were activated（enlarged cell bodies with short and thicken processes） in all layers 2 days postCA. Meanwhile, GFAP-immunoreactive astrocytes did not change significantly until 2 days post-CA. TNF-α immunoreactivity decreased significantly in neurons of the stratum pyramidale in the CA1 region 6 hours post-CA, decreased gradually until 1 day post-CA, and increased significantly again 2 days post-CA. These findings suggest that low survival rate of normothermic rats in the early period of asphyxia-induced CA is related to increased TNF-α immunoreactivity, but not to neuronal damage in the hippocampal CA1 region.

The major limitation for the maturation of dendritic cells(DCs)using Toll-like receptor(TLR)agonists is their decreased ability to migrate into lymph nodes compared with conventional ***38 can be used as a multifunctional marker to modulate migration,survival and Th1 responses of ***74 has been shown to negatively regulate DC *** goal of this study was to investigate the combinations of TLR agonists and interferons(IFNs)that most effectively regulate CD38 and CD74 expression on *** TLR agonist stimulation in combination with IFN-a and IFN-c was the best method for regulating CD38 and CD74 expression and inducing the highest secretion of *** in vitro migration assay showed that DCs treated with this combination had significantly enhanced migratory ability,similar to that observed in cells expressing CD38,CD74 and *** results of this study suggest that an alternative maturation protocol in which two TLR ligands are combined with type I and II IFNs generates potent DCs that have both a high migratory capacity and high IL-12p70 production.

Clostridium difficile infection aggravates colitis in interleukin 10-deficient mice

AIM:To investigate the effect of Clostridium difficile(***)infection in an interleukin 10-deficient(IL-10-/-)mouse model of inflammatory bowel ***:Bone marrow-derived dendritic cells isolated from wild type(WT)and IL-10-/-mice were stimulated for 4 h with *** toxin A(200μg/m L),and gene expression of interferon(IFN)-γ,IL-12 and IL-23was determined by real-time reverse transcription polymerase chain *** and IL-10-/-mice(n=20each)were exposed to an antibiotic cocktail for three days and then were injected with clindamycin(i.p.).Mice(n=10 WT,10 IL-10-/-)were then challenged with oral administration of ***(1×105 colony forming units of strain VPI 10463).Animals were monitored daily for 7 d for signs of *** tissue samples were evaluated for cytokine gene expression and histopathologic ***:*** toxin A treatment induced IFN-γgene expression to a level that was significantly higher in BDMCs from IL-10-/-compared to those from WT mice(P<0.05).However,expression of IL-12 and IL-23 was not different among the *** *** administration,mice developed diarrhea and lost weight within 2-3 *** loss was significantly greater in IL-10-/-compared to WT mice(P<0.05).*** infection induced histopathologic features typical of colitis in both IL-10-/-and WT *** histopathologic severity score was significantly higher in the IL-10-/-than in WT mice(mean±standard error;5.50±0.53 vs 2.44±0.46;P<0.05).This was accompanied by a significantly greater increase in IFN-γgene expression in colonic tissues from IL-10-/-than from WT mice challenged with ***(P<0.05).CONCLUSION:These results indicate that colitis is more severe after *** infection in IL-10-/-mice,and that IFN-γexpression is involved in this process.

Mirizzi syndrome is a rare complication of gallstone disease,and results in partial obstruction of the common bile duct or a cholecystobiliary fistula. Moreover,congenital anatomical variants of the cystic duct are common,occurring in 18%-23% of cases,but Mirizzi syndrome underlying an anomalous cystic duct is an important clinical consideration. Here,we present an unusual case of typeⅠMirizzi syndrome with an uncommon anomalous cystic duct,namely,a low lateral insertion of the cystic duct with a common sheath of cystic duct and common bile duct.

AIM: To investigate the effect of Gardenia jasminoides (GJ) on cerulein-induced acute pancreatitis (AP) in ***: C57BL/6 mice weighing 18-20 g were divided into three groups.(1) Normal saline-treated group,(2) treatment with GJ at a dose of 0.1 g/kg,(3) treatment with GJ at a dose of 1 g/*** was administered orally (n = 6 per group) for 1 *** hours later,the mice were given an intraperitoneal injection of cerulein (50 μg/kg),a stable cholecystokinin (CCK) analogue,every hour for a total of 6 h as described *** mice were sacrificed at 6 h after completion of cerulein *** samples were obtained to determine serum amylase,lipase and cytokine *** pancreas was rapidly removed for morphologic examination and scoring.A portion of pancreas was stored at -70℃ and prepared for the measurement of tissue myeloperoxidase (MPO) activity,an indicator of neutrophil sequestration,and for reverse-transcriptase PCR (RT-PCR) and real-time PCR ***: Treatment with GJ decreased significantly the severity of pancreatitis and pancreatitis-associated lung *** with GJ attenuated the severity of AP compared with saline-treated mice,as shown by reduction in pancreatic edema,neutrophil infiltration,serum amylase and lipase levels,serum cytokine levels,and mRNA expression of multiple inflammatory ***: These results suggest that GJ attenuated the severity of AP as well as pancreatitis-associated lung injury.

Is obesity associated with gastropharyngeal reflux disease?

AIM: To examine the association between obesity and gastropharyngeal reflux disease (GPRD) as well as gastroesophageal reflux disease (GERD) METHODS: We conducted a cross-sectional study of consecutive patients undergoing ambulatory 24-h dual-probe pH monitoring from July 2003 to December 2006. The association between body mass index (BMI) and parameters about gastroesophageal or gastropharyngeal reflux was examined in univariate and multivariate ***: A total of 769 patients (307 men and 462 women; mean age 50.7 years) were finally enrolled. Most variables showing gastroesophageal reflux was higher in the obese patients than the patients with normal BMI. There was no difference in all the variables showing gastropharyngeal reflux according to the BMI. After adjustment for age, sex, alcohol intake and smoking, obese patients demonstrated an about 2-fold increase in risk of GERD compared with patients with normal BMI (OR, 1.9; 95 CI, 1.3-2.9), but overweight patients did not demonstrate increased risk of GERD (OR, 1.2; 95 CI, 0.8-1.7). Both obese patients and overweight patients did not demonstrated increased risk of GPRD compared with patients with normal BMI (OR, 1.1; 95 CI, 0.8-1.7; and OR, 0.9; 95 CI, 0.6-1.3, respectively).CONCLUSION: Obesity is not associated with GPRD reflux while it is associated with GERD.

Helicobacter pylori eradication with moxifloxacin-containing therapy following failed first-line therapies in South korea

AIM:To investigate moxifloxacin-containing triple therapy as second-line treatment for Helicobacter pylori(***)infection following failed first-line ***:The sample included 312 patients for whom first-line treatment failed between January 2008and May 2013;27 patients were excluded,and a total of 285 patients received 7-or 14-d moxifloxacincontaining triple therapy as second-line treatment for *** *** line regimens included 7-d standard triple(n=172),10-d bismuth-containing quadruple(n=28),14-d concomitant(n=37),or14-d sequential(n=48)*** status was evaluated using 13C-urea breath testing 4 wk later,aftercompletion of the *** primary outcome was the *** eradication rate analyzed using intentionto-treat(ITT)and per protocol(PP)*** secondary outcome was the occurrence of serious adverse *** and clinical factors were analyzed using Student’s t-tests and Pearson’sχ2 tests according to first-and second-line regimens.A P value of less than 0.05 was considered statistically ***:The eradication rate of moxifloxacincontaining triple therapy was 68.4%(ITT;95%CI:62.8-73.5)and 73.9%(PP;95%CI:68.3-78.8).The eradication rate was significantly higher with 14 d compared to 7 d of treatment(77.5%vs 62.5%,P=0.017).Peptic ulcer patients had a higher eradication rate than the patients without ulcers(82.9%vs 70.6%,P=0.046).The demographic and clinical characteristics were not significantly different between the groups according to first-line *** and PP analyses of the moxifloxacin-containing triple therapy indicated the following eradication rates:70.9%(95%CI:63.8-77.2)and 77.2%(95%CI:70.1-83.1)for standard triple;67.9%(95%CI:51.5-84.2)and 67.9%(95%CI:51.5-84.2)for bismuth-containing quadruple;60.4%(95%CI:46.3-73.0)and 70.7%(95%CI:54.0-80.9)for sequential;and 67.6%(95%CI:51.5-80.4)and67.6%(95%CI:51.5-80.4)for concomitant *** were no statistically significant differe

Background There were limited data comparing the major clinical outcomes between first-generation(lG)-drug eluting stents(DES)and second-generation(2 G)-DES in patients with acute myocardial infarction(AMI) after percutaneous coronary intervention(PCI) during very long follow-up periods. We thought to investigate the comparative efficacy and safety of 2 G-DES compared with 1 G-DES in AMI patients during 5-year follow-up periods. Method A total of 1016 eligible AMI patients who underwent PCI with 1 G-DES [paclitaxel-,sirolimus-, lG-zotarolimus-eluting stent(endeavor?or endeavor sprint?), n = 554] or 2 G-DES [2 G-zotarolimus(endeavor resolute?)-or everolimus-eluting stent, n = 462] were enrolled. The primary endpoint was the occurrence of major adverse cardiac events(MACE) defined as total death, non-fatal mycx^ardial infarction(MI), target lesion revascularization(TLR), target vessel revascularization(TVR), non-target vessel revascularization(Non-TVR) and the secondary endpoint was stent thrombosis(ST) at 5 years. Results Two propensity score-ma?tched(PSM) groups(232 pairs, n = 464, C-statistic = 0.802) were generated. During the 5-year follow-up period, the cumulative incidence of TLR [hazard ratio(HR): 3.133; 95% confidence interval(Cl): 1.539-6.376; P = 0.002], TVR(HR: 3.144; 95% Cl: 1.596-6.192; P = 0.001)and total revascularization rate(HR: 1.874; 95% Cl: 1.086-3.140; P = 0.023) were significantly higher in 1 G-DES compared with 2 G-DES after PSM. However, the incidence of total death, non-fatal MI and ST were similar between the two groups. Conclusion In this sin?gle-center and all-comers registry, 2 G-DES,s superiorities for TLR, TVR and total revascularization in AMI patients suggested during 5-year clinical follow-up periods.
