Ischemic brain injury occurs when the metabolic needs of brain tissue cannot be met due to insufficient blood supply to the brain.It is one of the main causes of death and adult disability worldwide.The recurrence rate of ischemic brain injury is high.It places a heavy economic burden on families and society,and seriously affects human health and quality of life.In traditional Chinese medicine,ischemic stroke belongs to the category of“stroke”.The use of traditional Chinese medicine to treat stroke has a long history.After years of experimental research,a large amount of theoretical knowledge and practical experience have been accumulated.Promoting blood circulation and removing blood stasis is the basis of traditional Chinese medicine theory on the treatment of ischemic stroke.Commonly used single Chinese medicines include Chuangxiong(Ligusticum chuanxiong hort),Danggui(Angelica sinensis),Danshen(Salvia miltiorrhiza Bunge),Honghua(Carthamus tinctorius L.),Mudanpi(Moutan Cortex),and huangqi(Astragali Radix).Buyang huanwu decoction,Xinglou chengqi decoction,Taohong Siwu decoction,and other traditional Chinese medicine prescriptions are believed to have a protective effect against brain damage caused by ischemic stroke.With the development of modern medical technology,the mechanism of traditional Chinese medicine treatments for ischemic brain injury has gradually been explored.This article reviews the mechanisms of traditional Chinese medicine’s protection against ischemic brain injury and its current clinical application.

Prevention of nausea,vomiting and reflux aspiration in two cases of painless hysteroscopy

Objective:To investigate the prevention and management of perioperative nausea and vomiting and reflux aspiration in painless hysteroscopy patients.Methods:The clinical datum of one patient with reflux and postoperative vomiting and another one with a history of multiple postoperative nausea and vomiting during painless hysteroscopy were retrospectively analyzed.Results:In case 1,despite the occurrence of reflux,no significant aspiration or other serious consequences occurred through positive attraction,ondansetron,dexamethasone,droperidol and other drugs and candy treatment,then nausea and vomiting gradually eased.In case 2,due to the reduction of preoperative dosage of sufentanil,prophylactic application of ondansetron,psychological intervention and postoperative candy,no nausea and vomiting occurred during the painless hysteroscopy.The patient recovered quickly with high satisfaction.Conclusion:Patients with a high risk of nausea and vomiting may be ameliorated by positive psychological and drug intervention.Even if patients have intraoperative reflux and postoperative nausea and vomiting,it can be relieved by psychological and drug treatment,and the occurrence of serious complications such as aspiration can be prevented.

Anesthesia is mainly a kind of reversible functional inhibition of the central nervous system and(or)peripheral nervous system through drugs or other methods.In modern clinical medicine,modern anesthesiology is a comprehensive subject,which mainly includes clinical anesthesia,pain diagnosis and treatment,intensive care treatment,first aid and resuscitation,etc.Electroacupuncture is developed based on acupuncture therapy which is an important part of traditional Chinese medicine.It uses the comprehensive efficacy of acupuncture and electrical stimulation to deliver electrical energy through acupoints to achieve therapeutic effects.It has been widely used in recent years in clinical.This article summarizes the analgesic mechanism of electroacupuncture and its application in different anesthesia methods,so that clinicians have a deeper understanding of the clinical application of electroacupuncture and promote its clinical application.

Aim:In this review,it will mainly focus on clarifying the pharmacology of dexmedetomidine and discussing the past,present and future clinical applications of dexmedetomidine as an adjunct in anesthesia.Method:We have searched and reviewed the articles about the use of dexmedetomidine in clinical anesthesia,intensive care unit(ICU),painless gastroenteroscopy and painless labor over the past two decades.Recent findings:Dexmedetomidine,a highly selective agonist of alpha2-adrenergic receptors(alpha2-AR),was primarily approved by the U.S.Food and Drug Administration(FDA)for sedation in ICU due to its pharmacological characteristics with sedative,analgesic and sympatholytic effects.These properties make up the limitations of anesthetics,and it produces a“dexmedetomidine assisted”anesthesia mode as an adjuvant combining with other anesthesia methods,which are all termed as“DEX+”.These new methods can reduce the anesthetics requirement but prolong the action period and alleviate the adverse reactions of anesthetics,thus improving the anesthesia more effectively and safely.Summary:Dexmedetomidine possesses the unique properties exerting synergistic effects and alleviating the side effects as an adjuvant in anesthesia through different administration routes.It is beneficial for the patients’long-term outcome and will be bound to be the leading innovator of anesthesia in future.

Multi-time Collaborative Restoration for Integrated Electrical-gas Distribution Systems Based on Rolling Optimization

This paper proposes a multi-time collaborative restoration model for integrated electricity-gas distribution sys-tems(IEGDSs)considering multiple resources after extreme weather events.Based on the linearized power flow constraints of the unbalanced electrical distribution system(EDS)and gas distribution system(GDS),this problem can be formulated as a mixed-integer linear programming(MILP)model.To improve the efficiency and veracity of the solution,a rolling optimiza-tion based two-stage method is developed with the first stage solved by a linear approximation model,and the second stage solved by real-time updated rolling optimization.By solving the MILP problem using rolling optimization,the proposed model and solution method achieve efficient and reliable collaborative restoration of IEGDS considering multiple resources and unbal-anced operation characteristics of EDS.The effectiveness of the proposed model and method is validated by using an IEGDS made of a 37-bus unbalanced EDS and 11-node GDS.Index Terms-Electricity-gas system,mix-integer linear programming,power system restoration.

Enhancing adhesive bond strength of CFRP/titanium joints through NaOH anodising and resin pre-coating treatments with optimised anodising conditions

Adhesively Bonded Carbon Fibre Reinforced Plastic(CFRP)and titanium alloy have been extensively used as a hybrid structure in modern aircrafts due to their excellent combination of mechanical properties and chemical stabilities.This study utilised NaOH anodising method to create micro-rough titanium surfaces for enhancing adhesive bonding between titanium alloy and CFRP laminates.A special and simple technique named Resin Pre-Coating(RPC)was also employed to improve the surface wetting of anodised titanium and grinded CFRP substrates.The influences of anodising temperature and duration on the surface morphology,wettability and adhesive bond strength were investigated.The single lap shear test results showed that the bond strength of specimens anodised at 20℃for 15 min improved by 135.9%and 95.4%,respectively,in comparison with that of acid pickled and grinded specimens(without RPC treatment).Although increasing the anodising temperature and duration produced rougher titanium surfaces,the adhesively bonded joints were not strong enough due to relatively friable titanium oxide layers.

Effect of initial ammonium concentration on a one-stage partial nitrification/anammox biofilm system:Nitrogen removal performance and the microbial community

One-stage partial nitrification coupled with anammox(PN/A)technology effectively reduces the energy consumption of a biological nitrogen removal system.Inhibiting nitrite-oxidizing bacteria(NOB)is essential for this technology to maintain efficient nitrogen removal performance.Initial ammonium concentration(IAC)affects the degree of inhibited NOB.In this study,the effect of the IAC on a PN/A biofilm was investigated in a moving bed biofilm reactor.The results showed that nitrogen removal efficiency decreased from 82.49%±1.90%to 64.57%±3.96%after the IAC was reduced from 60 to 20 mg N/L,while the nitrate production ratio increased from 13.87%±0.90%to 26.50%±3.76%.NOB activity increased to1,133.86 mg N/m^(2)/day after the IAC decreased,approximately 4-fold,indicating that the IAC plays an important inhibitory role in NOB.The rate-limiting step in the mature biofilm of the PN/A system is the nitritation process and is not shifted by the IAC.The analysis of the microbial community structure in the biofilm indicates that the IAC was the dominant factor in changes in community structure.Ca.Brocadia and Ca.Jettenia were the main anammox bacteria,and Nitrosomonas and Nitrospira were the main AOB and NOB,respectively.IAC did not affect the difference in growth between Ca.Brocadia and Ca.Jettenia.Thus,modulating the IAC promoted the PN/A process with efficient nitrogen removal performance at medium to low ammonium concentrations.

BACKGROUND Differences in the preoperative characteristics and weight loss outcomes after sleeve gastrectomy(SG)between patients with familial aggregation of obesity(FAO)and patients with sporadic obesity(SO)have not been elucidated.AIM To explore the impact of SG on weight loss and the alleviation of obesity-related comorbidities in individuals with FAO.METHODS A total of 193 patients with obesity who underwent SG were selected.Patients with FAO/SO were matched 1:1 by propensity score matching and were categorized into 4 groups based on the number of first-degree relatives with obesity(1 SO vs 1FAO,2SO vs 2FAO).The baseline characteristics,weight loss outcomes,prevalence of obesity-related comorbidities and incidence of major surgeryrelated complications were compared between groups.RESULTS We defined FAO as the presence of two or more first-degree relatives with obesity.Patients with FAO did not initially show significant differences in baseline data,short-term postoperative weight loss,or obesity-related comorbidities when compared to patients with SO preoperatively.However,distinctions between the two groups became evident at the two-year mark,with statistically significant differences in both percentage of total weight loss(P=0.006)and percentage of excess weight loss(P<0.001).The FAO group exhibited weaker remission of type 2 diabetes mellitus(T2DM)(P=0.031),hyperlipidemia(P=0.012),and non-alcoholic fatty liver disease(NAFLD)(P=0.003)as well as a lower incidence of acid reflux(P=0.038).CONCLUSION FAO patients is associated with decreased mid-to-long-term weight loss outcomes;the alleviation of T2DM,hyperlipidemia and NAFLD;and decreased incidence of acid reflux postoperatively.

Effects of Additives on the Microstructure and Tribology Performance of Ta-12W Alloy Micro-Arc Oxidation Coating

Oxide ceramic coatings were fabricated on tantalum alloys by micro-arc oxidation (MAO) to improve their hardness and tribological properties. The MAO coatings were manufactured in a mixed silicatephosphate electrolyte containing NaF and/or EDTA (ethylene diamine tetraacetic acid). The surface morphology,cross-sectional view, chemical composition, hardness, and wear performance of the coatings were analysed. As revealed by the scanning electron microscopy, silica-rich nodules appear on the MAO coating obtained in the silicate-phosphate electrolyte, but the formation of nodules is inhibited with NaF and/or EDTA in the electrolyte.Also, they reduce the roughness and improve the compactness of the coatings, which are composed of Ta_(2)O_(5),(Ta, O), and TaO. A thick and hard coating is obtained in the NaF-containing electrolyte, and the tribology performance is effectively improved. With additives, the nodule structure is detached from the coating surface and dissolved in the electrolyte. By using NaF as an electrolyte additive, the abrasion performance of the MAO coating is enhanced by decreasing the nodule structure, increasing the size of micropores, and improving the coating hardness.
