Sinonasal Polyposis: About 60 Cases at Fann University Hospital Center, Senegal

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. 在发展中国家,大量水果未经加工即以原果直接销售,而加工产品则主要用于出口。芒果(Mangifera indica L),漆树科(Anacardiaceae),是中国热带地区最重要、最普遍、最受欢迎的经济水果之一。但由于粗糙的包装和不恰当的保存,大量芒果腐烂,损失很大。所以,开发新的加工方法以减少损失有重要意义。本论文的目的是开发酶解技术加工芒果浊汁,并研究了芒果浊汁的稳定性,比较了不同杀菌工艺和不同稳定剂对芒果浊汁稳定性的影响。主要研究结果如下: 碳水化合物是中国芒果浆最主要的成分,多酚、胡萝卜素、维生素C、磷的含量也较高。特别是β-胡萝卜素的含量很高,即使经过蒸汽烫4 min,β-胡萝卜素含量依然高达1052μg/100g。原芒果浆和蒸汽热烫过的芒果浆中最重要的风味物质是环己烯。另外,3-蒈烯(3-carene)是芒果中已报道的最重要的风味物质,原果浆中3-蒈烯(3-carene)的含量为3.02%,蒸汽烫过的芒果浆3-蒈烯(3-carene)的含量为0.08%;所以,3-蒈烯(3-carene)对芒果原果浆风味更重要。采用蒸汽热烫钝化芒果内源酶。研究了芒果过氧化物酶(peroxidase, POD)和多酚氧化酶(polyphenoloxidase, PPO)的热稳定性,并且研究了蒸汽烫的时间对芒果颜色的影响。蒸汽烫5 min可以完全钝化POD,蒸汽烫7 min可以完全钝化PPO。蒸汽烫3 min时,POD和PPO分别残留2.85%和8.33%的酶活;与烫5 min的样品相比,残留的酶在保藏期内(20天)对芒果的颜色没有影响。蒸汽烫7 min的芒果的褐变指数稳定但小于蒸汽烫3和5 min的芒果,因为蒸汽热烫的时间太长,非酶褐变较严重。研究发现,黄值(b*)、亮值(L*)与褐变指数(BI)存在正相关关系,而红值(a*)与褐变指数(BI)负相关。使用Pectinex? Ultra SP-L (20-40 ppm)和Cellubrix? L (15-25 ppm)酶制备了芒果浑浊汁,采用响应面实验优化了时间(30–50 min)、温度(35–55 oC)、pH值(4-5.5)等工艺参数。果汁得率、浑浊稳定性、褐变指数结果表明相关系数(R2)高于0.900。最佳工艺参数是:Pectinex 30 ppm、Cellubrix 22 ppm、45℃、pH 5.5、酶解43 min。经过其它一些加工和添加稳定剂,可以延长果汁的货架期、改善颜色稳定性、增加营养成分的保留率。而且,果汁加工中不同的工艺如热处理,对果汁的主要成分如总多酚和β-胡萝卜素的影响并不大。研究了CMC、MCC和XG对果汁的稳定效果。MCC和CMC在低浓度下即能稳定果汁颗粒。这是因为CMC和MCC与XG通过静电相互作用稳定果汁体系。然而,静电相互作用不同(排斥或吸引)造成沉淀。但高浓度的稳定剂使果汁太稠,消费者不能接受。0.1%的(MCC + CMC)能增加芒果汁的浑浊稳定性,从而替代(MCC + GG)和(CMC + GG)。采用HPLC测定了果汁中的β-胡萝卜素,结果表明,普通灭菌方法比HTST法条件温和,对?-胡萝卜素含量的影响更小。而两种杀菌方法对芒果果汁中芒果素(Mangiferin)含量的影响基本一致。但是,UHTMCJ中苯甲酸的含量高于STMCJ。SPME-GC/MS的结果表明,加热灭菌完全破坏了3-蒈烯(3-carene)。另外,95℃灭菌10 min和100℃灭菌7 min的果汁中,环己烯是重要的风味物质,而高温短时灭菌(121℃, 30 s)的果汁中丁酸是最主要的风味物质。这些差异与不同灭菌温度有关。 STMCJ的b*值与UHTMCJ的b*值有轻微差异,可能是因为它们的色素如β-胡萝卜素含量之间有差异。与对照相比,STMCJ和UHTMCJ的L*值、a*都没有大的差别。在较低pH (4.5) 95℃杀菌8 min或高温短时灭菌(121℃, 30 s, pH 5或4.5)能够达到灭菌要求,在贮藏中,果汁腐败菌数目符合卫生质量标准。在贮藏中,样品S1的L*值比样品S0下降更多。S0、S1和S2的a*值的差异可能与加工工艺条件不同有关,而b*值的变化与果汁中一些天然色素如β-胡萝卜素的断裂有关。感官评定结果表明,在相同pH下,STMCJ比UHTMCJ的认可度稍高。在5℃的贮藏时,MCJ有很好的悬浮稳定性,而悬浮稳定性是浑浊果汁最重要的质量特性,所以,长期贮存可以采用5℃,而30℃适于短期(4-5周)贮存。

Diffuse Cervico-Facial Cellulitis: Epidemiological and Clinical Aspects of 297 Cases

Introduction: Cervicofacial cellulitis is a medical and surgical emergency, it is an extensive infection of the aponeurotic spaces of the face and neck. Although frequent in black Africa rather than in developed countries, they remain associated with a poor prognosis and few studies have been done on this subject. Their management must be early, probabilistic and then adapted to the bacteriological results of the pus taken. As Senegal is one of the many sub-Saharan African countries without recent data on cervicofacial cellulitis, we felt it was important to review the situation. The aim of our study was to describe the epidemiological and clinical aspects of cervicofacial cellulitis in the ENT department of Lamine Sine DIOP of the Fann National University Hospital of Dakar. Materials and Method: This was a retrospective study conducted over 5 years from January 2017 to December 2021 in the ENT-CCF LAMINE SINE DIOP Department of the CHNU FANN in Dakar. Our data sources were hospitalization registers and medical records of patients hospitalized for cervicofacial cellulitis. Our data were analyzed using the spss 12 for windows software. Results: During this 5-year period, we collected 297 cases of cervicofacial cellulitis, i.e. 19% of the hospitalizations. The average age was 35 years with a sex ratio of 2.23. The notion of taking NSAIDs was found in 90.57% of the cases, the immunocompromised background was present in 18 patients, i.e. 6.06% of the cases. The site of the swelling was sub-mento-maxillary in 49% of the cases and anterior cervical in 5% of cases. The portal of entry was dental in 189 patients, i.e. 64% of cases, and unknown in 3%. 4 deaths were recorded, i.e. 1.35% of patients hospitalized for cervicofacial cellulitis, and 1 case of mediastinitis with polyseritis was transferred to the Department of Thoracic and Cardiovascular Surgery, which had a favourable outcome. Conclusion: Cervicofacial cellulitis is a fairly common infection with a poor prognosis

Nutritional Contribution of Some Senegalese Forest Fruits Running across Soudano-Sahelian Zone

As a good food supply for the local people in various Senegalese regions, the forest fruits are becoming very important. They are using between foods or main condiment for dishes in many part of Africa. These fruits represent non negligible sources of vitamins, carbohydrates and minerals. Instead of more commercialized species such as Adansonia digitata L., Detarium senegalensis ***., Saba senegalensis (***.) Pichon, Tamarindus indica L., this article focused on nutritional values of other fruits with little interest or neglected. These species were Ficus gnaphalocarpa, L., Cordyla pinnata (Lepr. ex ***.) Milne-Redh. and Icacina senegalensis, harvested in soudano-sahelian zone;while Sarcocephalus latifolius (Sm.) E.A. Bruce belongs to Sudanese zone. The fruit pulps were isolated and freeze dried before analysis. The results showed that acidity, vitamin C and total minerals were significantly different from one fruit to another. S. latifolius with the highest acidity content (402.43 mg/100 g) and best vitamin C content if 1488 mg/100 g were found. Also as results, S. latifolius provided high protein content nearby the 20%. The caloric contributing (kcal/100 g) were 102.5, 177 and 132 respectively for F. gnaphalocarpa, C. pinnata, and S. latifolius. The ash contents were the same for all species (4.5%), indicating the richness in mineral elements. Furthermore, all species in this study were good sources of iron, copper and zinc. However, small amounts of sodium were noticed in all samples. Analysis of total sugars and their profile showed that I. senegalensis and C. pinnata were more appreciated. These results predicted the useful incomes for forest fruits in human being. Lot of medicinal virtues has been recognized from these fruits. The increase in value and the preservation of the biodiversity are necessary particularly for C. pinnata which presents a serious threat because of the strong income for wood production.

This study focused on two woody leafy vegetables Leptadenia hastata Decne and Senna obtusifolia Link, commonly consumed in Senegal. Leaves were col-lected from three regions. Then, proximate analyses and micronutrients were carried out to evaluate their nutritional values. Results revealed that protein level of S. obtusifolia (SO) is richer (21.75%) than Leptadenia hastata (LH) (18.16%). The cellulose and carbohydrate contents of the two vegetable’s leaves are in the same order except those of LH from Widou which are less rich in cellulose (8.31%) and richest in carbohydrate (6.35%). These leaves are also good sources of various mineral elements and especially iron. Leaves of LH appear to be richer in iron and magnesium, while SO appears to be richer in calcium. Vitamin C intakes of SO leaves are better than those of LH and respectively range from 142 to 196.5 and 22.5 to 159.5 mg/100 g. According to the use of this leafy vegetable by the populations, a domestication opportunity is thus justified to ensure availability and accessibility of these significant sources of micronutrients.

Dorsal Psammomatous Meningioma Revealed by Sciatica Case Report

Introduction: Meningiomas are benign tumors most localized in brain. Intraspinal meningiomas are rare and may have an intra or extradural topography. We report the case of a dorsal psammomatous meningioma revealed by sciatica. Case description: We present a 58-year-old female hospitalized for bilateral painful and disabling L5 sciatica rebellious to medical treatment and physiotherapy. At physical examination, she presented a lumbar spinal syndrome without root deficit. MRI (Magnetic Resonance Imaging) showed a borderline lumbar canal with a paramedian L4-L5 herniated disc. She undergone a first surgery a L4 decompressive laminectomy and there was no postoperative complications. The follow-up was marked by partial pain regression. Three months later, after a fall she presented low back pain and progressive worsening gait troubles. A MRI was realized and shown L3-L4, L4-L5 duct narrowing. A second surgery a L3 and L5 decompressive laminectomy was performed. The postoperative follow up was characterized

Vertebroplasty and Kyphoplasty: Indications and Results, a Preliminary Senegalese Experience from the Neurosurgery Department of the CHNU of Fann

Introduction: Vertebroplasty and kyphoplasty are percutaneous techniques that consist in injecting an acrylic cement into the body of a pathological vertebra. This work aims to report the experience the of Fann’s neurosurgery department in treating certain spinal pathologies by vertebroplasty and kyphoplasty. Methods: During a 3-year period from July 1, 2019 to July 31, 2022, we conducted a retrospective, descriptive and analytical study, including patients who underwent vertebroplasty or kyphoplasty for dorsolumbar spinal pathology. Results: The mean age of the 13 patients in our study was 51.61 years. Female gender was predominant in 62% (n = 8). The context of spontaneous onset was found in six patients. Nine patients had a VAS (visual analogue scale) ≥ 8 (69.23%). On clinical examination, all patients had a syndrome without neurological deficits. 84.61% of patients had a CT scan (n = 11). The dorsolumbar hinge was most affected with 53.85% of cases. Seven patients had a vertebral compression of between 25% and. The average degree of kyphosis was 8˚ and seven patients had a degree of kyphosis ≥ 10˚. Tumour aetiology accounted for 46.15% of cases. Kyphoplasty was performed in 61.53% (n = 9) of the cases and vertebroplasty was performed in 38.47% (n = 4) of the patients. Kyphoplasty was associated with biopsy in two cases and with osteosynthesis in one patient. Vertebroplasty was always associated with a biopsy. The evolution was favourable with a significant reduction in pain and vertebral kyphosis. The mean VAS decreased from 8.15 to 0.69 three months after treatment and the mean kyphosis decreased from 8˚ to 2˚. Conclusion: Kyphoplasty and vertebroplasty as percutaneous techniques allow consolidation of the vertebral body and pain relief. Kyphoplasty alone not only reduces pain but also restores the height of the compacted vertebral body.

A New Method for the Determination of Cyanide Ions and Their Quantification in Some Senegalese Cassava Varieties

Cassava (Manihot esculenta Crantz) is a starchy staple food that previous researches have showed to contain cyanogenic compounds, precursors of hydrocyanic acid, undoubtedly toxic for humans. With the aim to determine food security in cassava, this study developed a simple, fast and less expensive step for quantifying cyanide ions by using micro-diffusion with modified Conway cells. After an enzymatic degradation, the cyanide ions were quantified by electrochemical procedures. The validation of this method is estimated. The concentration of cyanide ions at different part of the samples was determined. The results showed high toxicity in some fresh Senegalese consumed cassava varieties (>100 mg HCN·kg﹣1). However, in the processed cassava products, less than 10 mg HCN·kg﹣1 was found in the different varieties studied except for the chips where the levels of CN﹣ contents were important (>49 mg HCN·kg﹣1).

Comparative Study between Dielectric Properties and Compaction Parameters: Case Study on Three Laterite Quarries

This work studies the variability of the relationship between relative permittivity and compaction parameters as a function of certain geotechnical properties for different laterite types. This study allows to purpose the dielectric permittivity as a third compaction parameter allowing to obtain a non destructive control méthod. Preliminary studies on Diack laterite had shown a good correlation. Additional investigations are carried out to verify the possibility of generalizing this correlation to three new laterite careers: Ngoudiane, Yéba and Fandene. To proceed, particle size analysis, Atterberg limits, specific weight tests and compaction according to the modified Proctor test were performed on laterite samples. Using the radar method, experimental permittivities are determined for laterite samples by the point method of propagation times and confirmed by the diffraction hyperbole method. The geotechnical and radar data obtained allowed correlations between permittivity and water content on the one hand and between permittivity and dry density on the other. The results show that the maximum dry density as a function of permittivity corresponds with the optimum Proctor, which confirms the results previously obtained on Diack laterite.
