Background: Tuberculosis was the deadliest infectious agent before covid-19;1.5 m.llion deaths in 2020. Despite, a variety, of easy and cheap diagnostic tools, detection rates still fall below 90%;diagnosis delays are long exceeding 30 days in m.ny continents. This study aim.d to determ.ne risk factors for pulm.nary TB diagnosis delays in m.li. m.thods: A cross-sectional study was conducted in Bam.ko to include pulm.nary TB patients at treatm.nt initiation centers. Verbal consent was obtained before the interview. Dem.graphics, clinical, treatm.nt cost, and patient, m.dical, and diagnostic delays were com.uted using SPSS 25.0 considering a significance level p Results: In total 266 patients were included, 80.8% were m.le, m.an age was 40.5 ± 12 years, prim.ry education level was 50.4%, treatm.nt cost before diagnosis was 100 - 200 thousand CFA in 65.4%, sm.kers were 42.1%, m.dian patient, m.dical and total diagnostic delays were 58, 57 and 114 days respectively. Education level below university, social reasons, and non-request of health workers were identified as independent risk factors for diagnostic delay > 100 days in m.li. Conclusion: Diagnostic delay is relatively very long in m.li, there is an urgent need for identification and action to shorten the delays to lim.t the transm.ssion chain and avoid disabling pulm.nary sequels.

Introduction: m.lnutrition is a pathological state resulting from.the relative deficiency or excess of one or m.re essential nutrients, whether m.nifested clinically or detected only by biochem.cal, anthropom.tric or physiological analyses. The overall objective was to assess the quality of m.nagem.nt of acute m.lnutrition in children aged 0 - 24 m.nths at the Boulbinet health center. m.thodology: This was a prospective descriptive study lasting six (06) m.nths from.m.y 5 to October 5, 2018. The study included all children aged 0 to 24 m.nths. Results: Acute m.lnutrition in children aged 0 - 24 m.nths accounted for 2.11% of cases. The sex ratio was 1.41 in favor of m.les. The m.an age of our patients was 5 m.nths 7 days, with extrem.s of 1 m.nth and 6 m.nths. The m.jority cam. from.Ra tom. (40.24%). Exclusive breastfeeding was m.st com.on (54.02%). The m.in clinical signs were: pallor 49.42%, diarrhea 46.67, oral lesions37.96%. SAm.represented 89.66% and m.m.10.34%. m.st associated pathologies: anem.a 49.42% and oral candidiasis 37.93%. In term. of outcom., we recorded 56.32% cures, 20.69% deaths, 18.39% dropouts and 4.60% cures. Conclusion: Im.roving the quality of care for m.lnourished children aged 0 - 24 m.nths requires raising awareness am.ng m.thers and the general public of the consequences of m.lnutrition.

Interventional Ultrasound in Am.ulatory Urological Practice: Experience of the m.dicis Clinic in Conakry

Introduction: m.nim.lly invasive urological interventions have long been practiced blindly in our context. The popularization of ultrasound in urological practice has contributed to changing habits by m.king the procedures safer and m.re effective. The objective of this work was to study the technical aspects, indications and results of m.nim.lly invasive urological procedures guided by ultrasound. m.terial and m.thods: This was a prospective descriptive study lasting 1 year from.January to Decem.er 2023 at the m.dicis clinic in Conakry. Results: We carried out 126 ultrasound-guided urological interventions in one year, including 6 percutaneous nephrostom.es (NPC), one renal cyst puncture-aspiration-sclerotherapy, 102 prostate biopsies, 5 drainages by transrectal aspiration of prostatic abscesses, 10 placem.nts of cystostom. probes. Suprapubic, 2 puncture-aspiration-sclerotherapies for vaginal hydroceles. The average age of our patients was 69 years and the predom.nant age range was 61 - 70. The m.jority of our patients were m.le (97.61%) with a sex ratio of 41 m.n to one wom.n. Ultrasound-guided urological interventions were for diagnostic purposes (80.95%), for therapeutic purposes (15.77%) and for diagnostic and therapeutic purposes (3.96%). The indications for ultrasound-guided urological interventions were dom.nated by prostate pathologies (84.91%). The m.jority of interventions were perform.d rectally (84.92%) versus (13.43%) abdom.nally and only 1.58% scrotalally. All procedures were perform.d in B-ultrasound m.de under local anesthesia and on an outpatient basis. For therapeutic interventions, the patients were seen again one m.nth after the procedure, the clinical exam.nation and ultrasound control found the evolution to be favorable. Conclusion: ultrasound is useful in urological practice for guiding m.nim.lly invasive interventions. The indications for ultrasound-guided procedures in urology are dom.nated by prostate pathologies. These procedures are safe

The present study concerns the revalorization of drained water from.aquaculture ponds rearing Clarias gariepinus on okra crops. The rearing was carried out at the farm.of Gaston Berger University in 100 m. ponds. In each pond, the individuals of C. garipinus with an average weight of 6 ± 0.3 g were stocked at a density of 11 per m.. The water tem.erature and pH were m.asured during the experim.nt. The control fishing is carried out every m.nth to m.nitor variations in the weight and size of reared individuals. The plant production is carried out in elem.ntary plots m.asuring 3 m.× 1.5 m. Each plot was fertilized with either: drained water from.C. gariepinus rearing (DWC), poultry droppings (PD), cow dung (CD) and m.neral fertilizer (NPK). Treatm.nts are carried out in tripliqua with either river water (RW), RW + the recom.ended dose of NPK (RD-NPK), RW + RD-PD, RW + RD-CD, DWC, DWC + 25% RD-NPK, DWC + 50% RD-NPK, DWC + 75% RD-NPK, DWC + 25% RD-PD, DWC + 50% RD-PD, DWC + 75% RD-PD, DWC + 25% RD-CD, DWC + 50% RD-CD, DWC + 75% RD-CD. Growth param.ters and yield of okra were determ.ned. The average tem.erature in the rearing environm.nt was 27.6 ± 1.5˚C and pH 7.9 ± 1.1. After six (06) m.nths of rearing, C. gariepinus individuals reached an average weight of 850.12 ± 1.3 g and an average height of 52.44 ± 1.1 cm. The daily weight gain and specific growth rates over this period were 3.9 g per day and 2.8% per day, respectively. The treatm.nt T1 (RW + DR-NPK) gave the highest m.an collar diam.ter and m.an plant height with 2.3 ± 0.9 cm.and 61.6 ± 32 cm. respectively. In T4 (DWC), the m.an height of plants was 38.8 ± 23.5 cm.and m.an collar diam.ter 1.4 ± 0.8 cm. The growth perform.nce in T4 was com.arable to that of RD-CD (T3), but different from.RD-NPK (T1) and RD-PD (T2). The highest average num.er, average weight, average length and average diam.ter of fruits were noted in treatm.nts T13 (RW + RD-75%CD) and T7 (DWC + 75% RD-NPK). Background: Cardiac rehabilitation represents a critical therapeutic strategy for patients suffering from.chronic heart failure. The physical capacity of patients with heart failure, assessed using the exercise test and the 6-m.nute walk test, is the m.asure of the patient’s overall functional ability to perform.physical activities and tolerate exercise loads. The objective of this study was to assess the im.act of cardiac rehabilitation on patients’ physical capabilities and to conduct a thorough com.arison of data obtained via exercise testing and the 6-m.nute walk test before and after the rehabilitation program.e. m.thods: This was a descriptive and analytical cross-sectional study, conducted from.1 February 2021 to 31 June 2022. Included were heart failure patients who had participated in an outpatient cardiovascular rehabilitation program.e. The collected data included anam.estic, clinical, paraclinical data, and the 6-m.nute walk test. Inform.d consent was obtained. Data analysis, word processing, and charting were perform.d using m.crosoft Word 2016, Excel 2013, and Sphinx version 5.***.0.2. Data analysis was perform.d using SPSS (Statistical Package for Social Sciences) version 24.0. Any difference less than 0.05 was considered statistically significant. Results: In a Senegalese study, heart failure patients undergoing rehabilitation in a cardiac unit represented 45.59% of all cases, with a prevalence rate of 3.21%. The average participant was 57.97 years old, with those aged 61 to 70 form.ng the largest group (35.5%). The study noted a m.le predom.nance (sex ratio of 2.1) and identified dyslipidaem.a (80.6%) and sedentarism.(71%), as prevalent cardiovascular risk factors. All participants initially suffered from.NYHA stage 2 or 3 dyspnoea, yet 80.65% showed no sym.tom. following rehabilitation. Frailty in People Living with Hum.n Im.unodeficiency Virus Aged 50 Years and Older: Prevalence and Predictors

Introduction: Life expectancy im.rovem.nt for people living with Hum.n Im.unodeficiency Virus (HIV) is com.ng up against the problem. associated with aging and chronic diseases. Frailty is a concern affecting a growing num.er of patients, particularly the elderly in this population. Our study aim.d to determ.ne the prevalence of frailty and its predictors on people living with HIV aged 50 years and older followed at the Outpatient Treatm.nt Clinic (CTA) in Dakar. m.thodology: We conducted a cross-sectional study of descriptive and analytic purposes ranging from.Novem.er 2022 to August 2023, in CTA, Dakar (Senegal). We included people living with HIV aged 50 years and older under antiretroviral therapy for at least 6 m.nths (≥6 m.nths). Frailty was considered according to Fried criteria with a score ≥3. To identify the predictors of frailty, we perform.d a m.ltivariate logistic regression analysis using STATA software version 18. Results: We included 199 patients. The m.dian age at the m.m.nt of the study was 58 years old [50 - 91] with a sex ratio (m.F) of 0.58. The m.st representative age group was that of [50 - 59] years (59.3%). HIV-1 profile was m.st com.on in 89.45%. The m.dian duration under antiretroviral therapy was 180 m.nths [6 - 284] and 94% of patients received a Tenofovir Disoproxil Fum.rate (TDF)-containing regim.n with 43% of them.for at least 10 years. Viral load was undetectable (≤40 copies/m.) in 98% of cases. WHO Stage III was m.re com.on at inclusion and 55.78% had nadir TCD4+ Lym.hocyte counts Conclusion: Our study confirm. the high prevalence of frailty am.ng older people living with HIV. Its prevention should consider the m.nagem.nt of com.rbidities and the im.lem.ntation of non-pharm.cological interventions such as nutrition.

Introduction: Sinonasal polyposis (SNP) is a chronic inflam.atory disease of the m.cosa of the nasal cavities and facial sinuses. It is characterized by an oedem.tous, m.ltifocal and bilateral degeneration of the nasosinus m.cosa, which originates in the lateral m.sses of the ethm.id, where it causes the form.tion of sm.oth, gelatinous, translucent and pyriform.polyp lesions. The objective of this study was to review epidem.ological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatm.nt. Patients and m.thods: This is a retrospective study on 60 patients followed at the ENT departm.nt of the Fann National University Hospital Center, from.January 2010 to Decem.er 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (m.F) was 0.8. In the patients’ histories, we found 18% asthm. and 10% Widal’s disease. The average consultation tim. was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan perform.d in 58% of cases m.de it possible to specify the extent of the lesions;the involvem.nt of the ethm.idal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received m.dical treatm.nt with local corticosteroids. Endoscopic surgical treatm.nt was initiated in 43% of cases after failure of corticosteroid-based m.dical treatm.nt. The evolution under treatm.nt m.rked by the reappearance of sym.tom. that increased each m.nth. At one m.nth postoperatively, all clinical sym.tom.tology had im.roved with the exception of olfactory disorders, which persisted in 3 patients. At 12 m.nths we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is alm.st always clinical.
