目的:探讨中医治疗特发性肺纤维化(IPF)的辨证用药规律,为临床治疗IPF提供参考。方法:在中国知网、万方数据知识服务平台、PubMed、Embase等数据库中,检索建库至2021年8月8日辨证应用中药汤剂治疗IPF的相关文献,提取证型、症状及中药处方信息,构建IPF医案数据库,采用隐结构模型、频繁项集等数据挖掘方法分析中药汤剂内服治疗IPF的辨证用药规律。结果:共纳入文献39篇,包含病例1497例。纳入文献共包含症状163个,如咳嗽、乏力、气短等,将频次3的53个症状作为显变量构建IPF的隐结构模型,该模型提示气虚血瘀是IPF的常见证型。纳入文献包含的处方中共涉及中药163味,其中高频药物包括黄芪、甘草、丹参、当归、党参等;挖掘出“症状-中药”频繁项集5项,如“胸痛乏力+舌有瘀斑+脉沉细+丹参+桃仁+党参”;“证型-症状-中药”频繁项集4项,如“气虚血瘀+胸闷乏力+舌有瘀斑+黄芪+丹参+当归”。结论:气虚血瘀是IPF的临床常见证型,中药汤剂治疗IPF多以黄芪、甘草、丹参为主要药物,药物配伍根据临床辨证选择。

混合痔术后尿潴留发生的影响因素

目的观察混合痔患者术后尿潴留(UR)发生情况,探讨其发生的影响因素。方法 2023年1月—2023年12月郑州大学附属郑州中心医院行外剥内扎联合内痔硬化剂注射术的混合痔患者974例,有肛裂、肛瘘、肛乳头纤维瘤等疾病者同时行相应手术进行处理。术后发生尿潴留者68例为尿潴留组,未发生尿潴留者906例中随机选取68例为非尿潴留组。比较2组性别、年龄、体质量指数(BMI)、痔疮病程、痔疮分度、基础疾病及术前血红蛋白、白蛋白水平;记录联合手术比率、套扎内痔点数、外痔切除点数、手术时间、术中液体入量、术后3 h视觉模拟评分(VAS);采用多因素logistic回归分析混合痔术后发生尿潴留的影响因素。结果 (1)尿潴留组BMI[(22.91±2.94)kg/m^(2)]小于非尿潴留组[(24.58±4.11)kg/m^(2)](t=-2.730,P=0.007),联合手术比率(77.9%)高于非尿潴留组(23.5%)(χ^(2)=38.126,P0.05)。(2)BMI(OR=0.801,95%CI:0.668~0.961,P=0.017)、术后3 h VAS(OR=10.245,95%CI:3.769~19.180,P=0.001)、联合手术(OR=8.502,95%CI:3.460~30.338,P=0.001)是混合痔患者术后发生尿潴留的影响因素。结论 BMI小、行联合手术、术后VAS高的混合痔患者行外剥内扎联合内痔硬化剂注射术后尿潴留的发生风险增大。

Auxetic metamaterials,which exhibit the negative Poisson’s ratio(NPR)effect,have found wide applications in many engineering ***,their high porosity inevitably weakens their bearing capacity and impact *** improve the energy absorption efficiency of auxetic honeycombs,a novel vertex-based hierarchical star-shaped honeycomb(VSH)is designed by replacing each vertex in the classical star-shaped honeycomb(SSH)with a newly added self-similar *** analytical model is built to investigate the Young’s modulus of VSH,which shows good agreement with experimental results and numerical *** in-plane dynamic crushing behaviors of VSH at three different crushing velocities are investigated,and empirical formulas for the densification strain and plateau stress are *** results reveal more stable deformation modes for VSH,attributed to the addition of self-similar star-shaped ***,compared with SSH under the same relative densities,VSH exhibits better specific energy absorption and higher plateau ***,VSH is verified to be a better candidate for energy absorption while maintaining the auxetic *** study is expected to provide a new design strategy for auxetic honeycombs.

This paper considers the problem of approximating the infinite-horizon value function of the discrete-time switched LQR *** particular,the authors propose a new value iteration method to generate a sequence of monotonically decreasing functions that converges exponentially to the value *** method facilitates us to use coarse approximations resulting from faster but less accurate algorithms for further value iteration,and thus,the proposed approach is capable of achieving a better approximation for a given computation time compared with the existing *** numerical examples are presented in this paper to illustrate the effectiveness of the proposed method.

分子动力学模拟通常用于晶体硅热力学性质的研究,因原子间采用复杂的多体作用势,分子模拟通常面临较高的计算负载,导致计算的时间和空间尺度受限。图形处理器(GPU)采用并行多线程技术,用于计算密集型处理任务,在分子动力学模拟领域中显示巨大的应用潜力。因此,充分利用GPU硬件架构特性提升固态共价晶体硅分子动力学模拟的时空尺度对晶体硅导热机制的研究具有重要意义。基于固态共价晶体硅分子动力学模拟算法,提出面向GPU计算平台的固定邻居算法设计与优化。利用数据结构、分支结构优化等方法解决分子动力学模拟的固定邻居算法全局访存和分支结构的耗时问题,降低数据访存消耗和分支冲突,通过改变线程并行调度方式,在GPU计算平台上实现高性能并行计算,有效解决计算负载问题。实验结果表明,LAMMPS双精度固态晶体硅分子动力学模拟与双精度固定邻居算法的加速比为11.62,HOOMD-blue双精度固态晶体硅分子动力学模拟与双精度固定邻居算法和单精度固定邻居算法的加速比分别为9.39和12.18。

In comparison to conventional hexagonal honeycomb structures,auxetic metamaterials with re-entrant configurations have exhibited superior mechanical properties in terms of energy *** further enhance the energy absorption capacity of these materials,a novel re-entrant honeycomb configuration,named novel auxetic re-entrant honeycomb(NARH),is developed by incorporating“”-shaped cell walls into the conventional auxetic re-entrant honeycomb(ARH).Two analytical models for the plateau stress are formulated to consider the plastic deformation of NARH during quasi-static compression and the dynamic impact using the linear momentum ***-static compression tests on 3D printed NARH honeycomb specimens and finite element simulations are performed to verify the effectiveness of the theoretical *** exhibits higher plateau stresses compared with ARH during compression,which can be attributed to the presence of more plastic hinges formed in *** hinges,the embedded parts with inclined cell walls,not only improve stability by forming stable triangles during compression but also enhance the energy absorption capacity.A parametric study is conducted to analyze the effect of impact velocity,thickness,and incline angle of cell walls on *** simulations demonstrate higher sensitivity of the mechanical properties to impact velocity and cell wall *** ribs to the“”-shaped cell walls in NARH further reduces the initial peak force during dynamic crushing while maintaining high energy *** research provides valuable guidelines for the design of energy absorption metamaterials.
