Objective: This study aimed to develop a nomogram to predict the 1-year survival of patients with pancreatic cancer who underwent pancreatectomy following neoadjuvant treatment with preoperatively detectable clinical parameters. Extended pancreatectomy is necessary to achieve complete tumor removal in borderline resectable and locally advanced pancreatic cancer. However, it increases postoperative morbidity and mortality rates, and should be balanced with potential benefit of long-term ***: The medical records of patients who underwent pancreatectomy following neoadjuvant treatment from January 2005 to December 2016 at Severance Hospital were retrospectively reviewed. Medical records were collected from five international institutions from Japan and Singapore for external ***: A total of 113 patients were enrolled. The nomogram for predicting 1-year disease-specific survival was created based on 5 clinically detectable preoperative parameters as follows: age(year), symptom(no/yes), tumor size at initial diagnostic stage(cm), preoperative serum carbohydrate antigen(CA) 19-9 level after neoadjuvant treatment(<34/≥34 U/m L), and planned surgery [pancreaticoduodenectomy(PD)(pylorus-preserving PD)/distal pancreatectomy(DP)/total pancreatectomy]. Model performance was assessed for discrimination and *** calibration plot showed good agreement between actual and predicted survival probabilities;the the Greenwood-Nam-D’Agostino(GND) goodness-of-fit test showed that the model was well calibrated(χ~2=8.24,P=0.5099). A total of 84 patients were used for external validation. AIM:To evaluate the efficacy of simple and extended cholecystectomy for mucosa(T1a) or muscularis(T1b) gallbladder(GB) cancer. METHODS:Original studies on simple and extended cholecystectomy for T1a or T1b GB cancer were searched from MEDLINE(PubMed) ,Cochrane Library,EMBase,and CancerLit using the search terms of GB,cancer/carcinoma/tumor/neoplasm. RESULTS:Twenty-nine out of the 2312 potentially relevant publications met the eligibility *** the 1266 patients with GB cancer included in the publications,706(55.8%) and 560(44.2%) had T1a and T1b GB cancer,*** cholecystectomy for T1a and T1b GB cancer was performed in 590(83.6%) and 375(67.0%) patients,respectively(P<0.01) .In most series,the treatment of choice was simple cholecystectomy for T1a GB cancer patients with a 5-year survival rate of 100%.Lymph node metastasis was detected in 10.9% of the T1b GB cancer patients and in 1.8%of the T1a GB cancer patients,respectively(P<0.01) .Eight patients(1.1%) with T1a GB cancer and 52 patients(9.3%) with T1b GB cancer died of recurrent GB cancer(P<0.01) . CONCLUSION:Simple cholecystectomy represents the adequate treatment of T1a GB *** is no definite evidence that extended cholecystectomy is advantageous over simple cholecystectomy for T1b GB cancer.

AIM: To investigate relationships between colorectal adenoma incidence, metabolic syndrome (MS) components and lifestyle factors. METHODS: We conducted a retrospective cohort study using data from individuals who had multiple sigmoidoscopies for colon cancer at the Health Promotion Center of Ulsan University Hospital in Korea from 1998 to 2007. RESULTS: By multivariate analysis, the incidence of distal colon adenoma was increased by more than 1.76 times in individuals with at least one component of MS compared to those without a component of MS. After adjustment for age, gender, smoking, drinking, and physical exercise, only high body mass index (BMI) was significantly associated with the incidence of distal colon adenoma (Hazard ratio 1.66, 95% confidence interval 1.05-2.62). CONCLUSION: Our results suggest that high BMI may increase the risk of colorectal adenoma in Korean adults.

In spite of the low temperature during the winter season and the high land environment, the wetland treatment system is gaining popularity in Korea because of its lower construction cost and simplicity in operation and maintenance. Many different types of wetland treatment systems have been built during the last 10 years, among which the free water surface wetland has been predominant. Most of the large-scale systems are government projects for improving the water quality of the streams flowing into the estuary dikes and reservoirs. The covering plants used in this system are different in different areas but cattails and reeds or their combinations are common. Constructed wetlands in Korea can be characterized by their shallow depths and short hydraulic residence times. There is no established flow pattern and configuration rules for constructing wetlands, but many efforts have been made with a view to improving their ecological function. Flow control is the most difficult problem in designing a riverbed or riparian wetland. There have been scores of flow rate control devices developed for wetlands, but none of them guarantee wetlands’ safety against flooding. In earlier wetland construction, the building materials were mainly soil. Recently, strong and durable building materials such as rocks, gravel beds, concrete and steel are used at vulnerable places to protect them from erosion. Our investigation indicated that the wetland system would be an appropriate technology because it is not only cheaper to construct, but also requires less maintenance work. However, we suffer from the reduced effectiveness in performance during the winter. We need to evaluate the partial treatment accomplished during 6 to 7 months per year.

Background:Although several prediction models for the occurrence of postoperative pancreatic fistula(POPF)after pancreatoduodenectomy(PD)exist,all were established using Western ***-scale external validation studies in Eastern cohorts that consider demographic variables including lower body mass index(BMI)are *** purpose of this study was to externally validate POPF prediction models using nationwide large-scale Korean ***:Nine tertiary university hospitals in the Republic of Korea ***'preoperative characteristics,intraoperative factors,and pathologic findings were *** grades were determined according to the 2016 International Study Group on Pancreatic Surgery *** POPF risk models(Callery,Roberts,and Mungroop)were selected for external ***:A total of 1,898 PD patients were enrolled.A non-pancreatic disease diagnosis[hazard ratio(HR),1.856;95%confidence interval(CI),1.223–2.817;P=0.004),higher preoperative BMI(HR,1.069;95%CI,1.019–1.121;P=0.006),and soft pancreatic texture(HR,1.859;95%CI,1.264–2.735;P=0.002)were independent risk factors for clinically relevant POPF(CR-POPF).The area under the receiver operating characteristic curve(AUC)values were 0.61,0.64,and 0.63 on the Callery,Roberts,and Mungroop models,respectively;all were lower than those published in each external validation ***:Western POPF prediction models performed less well when applied to Korean ***,a large-scale Eastern-specific and externally validated POPF prediction model is needed.

Chronic activation of microglial cells endangers neuronal survival through the release of various proinflammatory and neurotoxic factors. The root of Paeonia lactiflora Pall has been considered useful for the treatment of various disorders in traditional oriental medicine. Paeonol, found in the root of Paeonia lactiflora Pall, has a wide range of pharmacological functions, including anti-oxidative, anti-inflammatory and neuroprotective activities. The objective of this study was to examine the efficacy of paeonol in the repression of inflammation-induced neurotoxicity and microglial cell activation. Organotypic hippocampal slice cultures and primary microglial cells from rat brain were stimulated with bacterial lipopolysaccharide. Paeonol pretreatment was performed for 30 minutes prior to lipopolysaccharide addition. Cell viability and nitrite （the production of nitric oxide）, tumor necrosis factor-alpha and interleukin-lbeta products were measured after lipopolysaccharide treatment. In organotypic hippocampal slice cultures, paeonol blocked lipopolysaccharide-related hippocampal cell death and inhibited the release of nitrite and interleukin-lbeta. Paeonol was effective in inhibiting nitric oxide release from primary microglial cells. It also reduced the lipopolysaccharide-stimulated release of tumor necrosis factor-alpha and intefleukin-1β from microglial cells. Paeonol possesses neuroprotective activity in a model of inflammation-induced neurotoxicity and reduces the release of neurotoxic and proinflammatory factors in activated microglial cells.

背景强迫症是一种较为常见的慢性、致残的疾病，患者出现无法自控的强迫思维及强迫行为。10％以上的强迫症病人对药物和行为治疗无效。对一些常规治疗无效的难治性强迫症患者，神经外科已经开展一些手术治疗，例如：扣带前回毁损、脑白质切断术、尾状核下束切断、内囊前支毁损。扣带前回毁损相对来说比较安全同时没有明显的副作用，但是也有术后存在认知障碍的报道。为判断立体定向双侧扣带前回毁损是否是一种适合难治性强迫症的治疗，我们评价了立体定向双侧扣带前回毁损术的长期疗效及其副作用。方法患者筛选：在1999年3月至2003年4月间，17例难治性强迫症患者（男10例，女7例）接受了双侧扣带前回毁损术，并且接受了超过2年的随访。患者的选择标准如下：患者符合美国精神障碍诊断与统计手册第4版（DSM-Ⅳ）强迫症的诊断标准，病程超过3年，疾病导致社会心理功能降低。每位患者皆由两位精神科医师依照DSM-Ⅳ的AxisⅠ和AxisⅡ的疾病诊断标准行临床诊断。如伴有精神病性症状，就先用一线药物彻底治疗。手术方法：立体定向的靶点通过与MRI配套的Leksell适配器，在GESigma1．5 T的机器上得以确认。局部麻醉下，在两侧的冠状缝中线外20mm颅骨钻孔。将尖端直径．1．8mm，长度10mm的电极插至靶点，用85℃，90S进行射频毁损。在双侧扣带前回，每侧各4个射频毁损灶，2个针道。第一个射频毁损灶在两侧侧脑室前角后缘15mm，侧脑室顶部上2mm，旁开中线7mm处。射频电极接着回撤8mm，制作第二个射频毁损灶。第三个射频毁损处在两侧侧脑室前角后缘22mm，侧脑室顶部上2mm，旁开中线7mm处。再往上8mm，制作第四个射频毁损灶。结果是出现类似椭圆柱形的射频毁损灶，大约高18mm、前后径13mm，横径6mm。术前和术后随访评估：两位精神科医师用Yale-Brown强迫症量表（Y-BOCS）、临床总体印象量表（CGI）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA），分别在手术前1个月、术后12个月、术后24个月对患者进行评估。评估的精神科医师接受了培训，Kappa值超过0．75，以确保评估的准确性。在我们研究中，患者的Y-BOCS得分改善35％或更高、CGI评分为1（极其显著改善）或者2（显著改善）；由专业的临床精神科医生对患者进行一套固定的神经心理测试，测试分别在术前1个月、术后12个月、24个月进行。这些测试包括：K—WAIS测验，视觉空间功能（RCFT），Hopkins语言学习测验（HVLT），威斯康星卡片分类测验（WCST），口语流畅测验（COWAT）。K-WAIS评分由韦氏成人智力量表（WAIS—R）改编而来，以产生总智商（IQ）、言语IQ和行为IQ来测量智力水平。RCFT评分用于测试空间构建能力。记忆力也包括回忆的精确性、瞬时回忆、延迟回忆和识别指数等的得分。HVLT用于测量口头记忆能力和学习能力。使用WCST和COWAT评分来检查认知的灵活性和执行功能。在WCST中，正确、错误反应的总数，完成的项目数，持续的反应，持续的错误数都用于评分。COWAT评分采用字母翻译和目录翻译的单词数目，字母翻译用来检查语音的流利，目录翻译用来评价语义流利性。使用Wileoxon符号秩检验对术前的Y—BOCS，HAMD，HAMA评分和术后6、12个月的评分进行比较，使用Wileoxon符号秩检验对手术前和手术后12个月的K—WAIS，RCFT，HVLT，WCST，COWAT评分进行比较。统计软件采用SPSS10。结果扣带回毁损术后强迫、抑郁、焦虑症状均改善：Y-BOCS，CGI、HAMD，HAMA评分显示了扣带回毁损术的有效性，在12个月的随访中，与术前Y-BOCS评分相比，改善率为36％，在24个月的随访中，改善率升至48％。术前、术后认知功能和记忆的改变：手术前和手术后24个月的K-WAIS，RCFT，HVLT，COWAT评分没有显著性差异。在WCST评分中，与术前基线相比，术后的错误总数、持续错误数和反应数明显减少。术后的错误总数、持续错误数和反应数的改变说明在扣带回毁损术后执行功能的提高。在术后2—3个月，3例患者有记忆障碍，但持续时间少于2个月。结论强迫症的扣带回毁损术术后24个月的随访结果显示症状明显改善，但没有认知功能的改变，包括智商、言语、视觉空间记忆和执行功能等均没有改变。
