The Editor-in-Chief of Hepatobiliary Surgery and Nutrition invited me to write an Editorial on the article entitled"10th Anniversary of ALPPS-Lessons Learned and quo Vadis"by Lang et *** was published in the 2019 January issue of Annals of Surgery(1).This article was coauthored by 15 world-renowned experts in associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)who met during the 12th Biennial Congress of the European-African Hepato-Pancreato-Biliary Association in Mainz,Germany from 23 to 26 May,*** aim of the meeting and the subsequently published article was to make"the most updated inventory of what has been achieved,and what remains unclear in ALPPS".

BACKGROUND: Following curative treatment for hepatocellular carcinoma (HCC), 50%-90% of postoperative death is due to recurrent disease. Intra-hepatic recurrence is frequently the only site of recurrence. Thus, any neoadjuvant or adjuvant therapy, which can decrease or delay the incidence of intra-hepatic recurrence, or any cancer chemoprevention which can prevent a new HCC from developing in the liver remnant, will improve the results of liver resection. This article systematically reviewed the current evidence of neoadjuvant, adjuvant, and chemoprevention in partial hepatectomy of HCC. DATA SOURCES: Studies were identified by searching MEDLINE and PubMed databases for articles from January 1990 to November 2008 using the keywords 'hepatocellular carcinoma', 'hepatectomy', 'adjuvant therapy', 'neoadjuvant therapy', and 'regional therapy'. Additional papers and book chapters were identified by a manual search of the references from the key articles. RESULTS: Neoadjuvant transarterial chemoembolization or adjuvant regional transarterial chemotherapy embolization+systemic chemotherapy did not add benefit. Both adjuvant transarterial radioembolization with (131)I-lipiodol and adjuvant systemic interferon showed promising results. However, there were only a limited number of such studies. CONCLUSIONS: Further randomized controlled studies need to be carried out. Currently, there is no consensus on a standard neoadjuvant/adjuvant/chemoprevention therapy in partial hepatectomy for HCC.

The development of defect-free composite membrane(CM) is often challenging due to poor dispersion and distribution of filler particles in the polymer matrix. Despite the attractive physicochemical properties and gas separation performance of carbon nanotube(CNT) based CM, CNT displayed poor dispersion characteristics in most polymer matrix domain. Instead of incorporating CNT, a viable alternative, carbon nanofiber(CNF) which exhibits similar properties as CNT, but improved dispersion quality in the polymer matrix is found. In this work,CNF particles were incorporated in poly(2,6-dimethyl-1,4-phenylene oxide)(PPOdm) polymer continuous phase for CM development. The optimum gas separation performance of the PPOdm-CNF CM(11.25 at 197.02 barrer of CO_2 permeability) was obtained at 3 wt% of CNF loading. Compared to pristine PPOdmmembrane,CO_2 permeability and CO_2/CH_4 selectivity of PPOdm-3 wt% CNF CM were enhanced by 180% and 55%, *** 3 wt% CNF loading, the filler particles were dispersed and distributed more homogenously, in which no obvious CNF agglomeration was observed. In addition, the incorporation of CNF particles also enhanced the mechanical and thermal properties of the resultant CM.

The detection and reconstruction of transparent objects have remained challenging due to the absence of their features and variations in the local features with variations in *** this paper,both compressive sensing(CS)and super-resolution convolutional neural network(SRCNN)techniques are combined to capture transparent *** the proposed method,the transparent object’s details are extracted accurately using a single pixel detector during the surface *** resultant images obtained from the experimental setup are low in quality due to speckles and deformations on the ***,the implemented SRCNN algorithm has obviated the mentioned drawbacks and reconstructed images visually *** developed algorithm locates the deformities in the resultant images and improves the image ***,the inclusion of compressive sensing minimizes the measurements required for reconstruction,thereby reducing image post-processing and hardware requirements during network *** result obtained indicates that the visual quality of the reconstructed images has increased from a structural similarity index(SSIM)value of 0.2 to *** this work,we demonstrate the efficiency of the proposed method in imaging and reconstructing transparent objects with the application of a compressive single pixel imaging technique and improving the image quality to a satisfactory level using the SRCNN algorithm.

Hemoperitoneum is a rare and potentially life-threatening complication of GIST. We reported a 54- year-old man who developed disseminated intra-abdominal recurrence from a previously resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically 8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery in the management of GIST. In our patient, disease progression after second line targeted therapy and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy and aggressive surgical treatment produced reasonably good survival outcome.

In this paper, we designed and evaluated a duplex detection strategy for micro RNAs（mi RNAs） using universal probe-based target-triggered double hybridization and fluorescent microsphere-based assay system（x MAP array）. In the absence of target mi RNA, reporter DNA cannot hybridize stably with the immobilized capture DNA due to its low melting temperature. Only after adding target mi RNA, can reporter probe hybridize with capture probe to form a stable three-component complex. This targettriggered stable hybridization makes this method possible for highly selective and sensitive detection of multiple mi RNAs. We exemplified a quantitative detection of duplex mi RNAs with a limit of detection of40 p M. The x MAP array platform holds the potential of extending this approach to simultaneous detection of up to 100 mi RNA targets. Considering the simplicity, rapidity and multiplexing, this work promised a potential detection of multiple mi RNA biomarkers for early disease diagnosis and prognosis.

Evolving treatment landscape for early and advanced pancreatic cancer

Pancreatic ductal adenocarcinoma is an infrequent cancer with a high disease related mortality rate, even in the context of early stage disease. Until recently, the rate of death from pancreatic cancer has remained largely similar whereby gemcitabine monotherapy was the mainstay of systemic treatment for most stages of disease. With the discovery of active multiagent chemotherapy regimens, namely FOLFIRINOX and gemcitabine plus nab-paclitaxel, the treatment landscape of pancreatic cancer is slowly evolving. FOLFIRINOX and gemcitabine plus nab-paclitaxel are now considered standard first line treatment options in metastatic pancreatic cancer. Studies are ongoing to investigate the utility of these same regimens in the adjuvant setting. The potential of these treatments to downstage disease is also being actively examined in the locally advanced context since neoadjuvant approaches may improve resection rates and surgical outcomes. As more emerging data become available, the management of pancreatic cancer is anticipated to change significantly in the coming years.

One of the main goals of modern cosmic microwave background （CMB） missions is to measure the tensor-to-scalar ratio r accurately to constrain inflation models. Due to ignorance about the reionization history Xe （z）, this analysis is usu- ally done by assuming an instantaneous reionization Xe （z） which, however, can bias the best-fit value of r. Moreover, due to the strong mixing of B-mode and E-mode polarizations in cut-sky measurements, multiplying the sky coverage fraction fsky by the full-sky likelihood would not give satisfactory results. In this work, we forecast constraints on r for the Planck mission taking into account the general reionization scenario and cut-sky effects. Our results show that by applying an N-point interpo- lation analysis to the reionization history, the bias induced by the assumption of in- stantaneous reionization is removed and the value of r is constrained within 5% error level, if the true value of r is greater than about 0.1.

Deterministic approaches for noncoherent communicationswith chaotic carriers

Two problems are proposed. The first one is the noise decontamination of chaotic carriers using a deterministic approach to reconstruct pseudo trajectories, the second is the design of communications schemes with chaotic carriers. After presenting our deterministic noise decontamination algorithm, conventional chaos shift keying (CSK) communication system is applied. The difference of Euclidean distance between noisy trajectory and decontaminated trajectory in phase space could be utilized to non-coherently detect the sent symbol simply and effectively. It is shown that this detection method can achieve the bit error rate performance comparable to other non-coherent systems.

Primary immunodeficiency disorders(PIDs)are rare inborn errors of the immune *** with PIDs are unique models that exemplify the functional and phenotypic consequences of various immune defects underlying infections,autoimmunity,lymphoproliferation,allergy and *** 150 PID syndromes were characterized in the past 60 years,with an ever growing list of new entities being *** of their rarity,multi-center collaboration for pooled data analysis and molecular studies is important to gain meaningful insights into the phenotypic and genetic diversities of *** this article,we summarize our research findings on PIDs in Chinese population in the past 20 *** collaboration among various immunology centers,cross-referrals and systematic data analysis constitute the foundation for research on *** directions include establishment of a national PID registry,raising awareness of PIDs and securing sufficient resources for patient care and scientific research.
