Needle knife fstulotomy in flat and diverticular papillae:Is it time for redemption?

Background: European Society of Gastrointestinal Endoscopy( ESGE) recommends needle-knife fstulotomy(NKF) as the preferred precut technique in cases when standard cannulation techniques fail. despite scarce scientifc evidence, flat and diverticular papillae are thought not to be ideal for NKF, as they are associated with poor outcomes. The present study aimed to determine the outcomes of the use of NKF in relation to flat and intradiverticular papillae. Methods: This prospective multicenter study enrolled consecutive patients, evidencing na?ve flat(group A, n = 49) or diverticular papilla(group B, n = 28), who underwent NKF after failure of standard cannulation techniques. Diverticular morphology was subdivided into intradiverticular(group B1, n = 14) and diverticular border papillae(group B2, n = 14), using a previously validated endoscopic classifcation of the major papilla. The success of biliary cannulation at initial endoscopic retrograde cholangiopancreatography(ERCP), overall biliary cannulation, overall cannulation time, and the rate of adverse events were assessed in the study. Results: The initial cannulation rates were 93.9%, 64.3% and 71.4% for group A, B1, and B2, respectively( P = 0.005);overall cannulation rates after a second ERCP were 98.0%, 92.9% and 85.7%, respectively( P = 0.134). Adverse events occurred in 11.7% of patients, with post-ERCP pancreatitis(PEP) being the most common adverse event(10.4%). Although there was a trend towards a higher incidence of PEP in flat papillae, univariate and multivariate analyses did not show any signifcant relationship between pancreatitis and trainee involvement, papillary morphology, nor overall cannulation time. Conclusions: Although flat papillae are associated with high success rates of biliary cannulation using NKF, the rate of PEP is not negligible. NKF is feasible in diverticular papillae, but it is associated with a modest success rate in the initial ERCP.

Structural characteristics of Mulato Grass I under Different Cutting Heights

This work aimed to evaluate the effects of different individual heights of cut on structural characteristics of Brachiaria hibrida cv. Mulato I in four periods of the year. The study was carried out at the Fazenda Experimental Vale do Acaraú, in Sobral, Ceará, Brazil, from January to december, 2008. The experimental design was a split plot, with the plots representing four pasture conditions through manual cuts of 10 cm, 20 cm, 30 cm and 40 cm heights, which were done every 30 days. Each treatment had three replications. The subplots were represented by four periods of evaluation: January-February-March, April-May-June, July-August-September and October-November-december. Fertilization was performed with 350 kg of N/ha, divided into monthly applications. Irrigation was conducted in alternate days during the dry season. The variables studied were: mean height before cutting (cm);dry matter content (%);total dry matter production (t/ha);percentage of leaves;percentage of stems;percentage of dead material;leaf blade/stem ratios;number of total leaves per tiller;number of live leaves per tiller;average size of leaves;and tiller density. Cutting at 10 cm caused a decrease in total dry matter production, underestimating the potential production. Cutting heights between 20 and 30 cm contributed positively to increase the total number of leaves, number of green leaves, average size of the leafs and total dry matter production. Under the present experimental conditions, cutting around 40 cm was not recommended because of a decreasing in the leaf/stem ratio and a increasing in stem fraction and dry matter content, compromising the structure of the canopy.

Soil Water Availability on Growth and development of Safflower Plants

Safflower (Carthamus tinctorius L.) is a promising culture to be widespread in Brazil. However, the lack of basic knowledge about cultivation techniques, such as water demand by the culture, is still obstacle to the expansion of safflower in that country. The objective was, then, to evaluate the effect of the soil water availability on growth and development of safflower in the Cerrado soil of Mato Grosso, Brazil. The experiment was conducted in a greenhouse, in a completely randomized design with five water availabilities (25%, 50%, 75%, 100% and 125% of the maximum water holding capacity in the soil) and four replications. Maintenance soil moisture was performed by gravimetric method with daily weighing of experimental units. The variables analyzed were: plant height, stem diameter, number of leaves, number of heads, heads diameter, dry mass of shoots, heads, and roots. The results were submitted to analysis of variance and regression test at 5% probability by SISVAR program. All variables set to the quadratic regression model, showing the best results in the water availability between 64% and 76%. Safflower is shown to be more sensitive to water stress with increased tolerance to water deficit in the soil than to flooding.

Soil Water Availabilities in the Content and Accumulation of Nitrogen and chlorophyll Index in the Safflower

The safflower (Carthamus tinctorius L.) is adapted to grow in adverse conditions, is tolerant to salinity and water deficit, however, its productivity increases when grown in areas with greater availability of water and in fertilized conditions, showing that the knowledge of crop management can bring income gains for producers. It is known that extreme conditions of soil moisture damage the crop development. Given this context, the objective was to evaluate the influence of water availability on the nutritional characteristics of the safflower. The experiment was conducted in a greenhouse in a completely randomized design with five water availabilities (25%, 50%, 75%, 100% and 125% of the maximum water holding capacity of the soil) in four replications. Maintaining soil moisture was performed by gravimetric method with daily weighing of experimental units. The variables analyzed were SPAD reading (chlorophyll content), concentration and accumulation of nitrogen in the shoot and in the sections. The results were submitted to analysis of variance and regression test at 5% probability by Sisvar program. There was adjusting of variables to linear and quadratic regression models. Water availabilities between 65% and 75% have greater potential to promote better nutrition for safflower plants. Both deficit and excess of water in the soil are detrimental to the absorption of nitrogen and reduce the chlorophyll content. Safflower is less tolerant to excess than to deficit of water.

Self-reported participation restrictions in normal hearing individuals in thyroid ontogeny: Evidence of subclinical changes

Purpose: To verify evidence of subclinical alterations through self-reported participation restrictions in normal hearing individuals and congenital hypothyroidism ***: An analytical, quantitative, cross-sectional exploratory study with a 1:1 ratio, consisting of a convenience sample of 86 normal hearing individuals with(n = 42) and without(n = 44) congenital hypothyroidism diagnosis. All participants underwent tonal and speech audiometry, immitance and distortion product otoacoustic emissions. The researchers excluded people with hearing loss, genetic syndromes and metabolic diseases. The instrument used for evaluate of self-reported participation restrictions was the HHIE-adapted questionnaire, composed of 25 questions, 12 of which were social domain and 13 emotional domain. Student's ttest and chi-square test were used for statistical analysis at a significance level of 5%.Results: There was a significant(p ch(61.9%), with a greater relevance for the social domain(p = 0.002). There was a greater frequency of mild/moderate(40.5%)and higher prevalence of association with clinical factors and adherence to ***: The findings indicate that self-reported participation restrictions in normal hearing individuals with congenital hypothyroidism was more significant than in the non-exposed group, suggesting evidence of subclinical auditory abnormalities in this population.

Multiscalar Geomorphometric Generalization for Soil-Landscape Modeling by Random Forest: A Case Study in the Eastern Amazon

Multiscalar topography influence on soil distribution has a complex pattern that is related to overlay of pedological processes which occurred at different times, and these driving forces are correlated with many geomorphologic scales. In this sense, the present study tested the hypothesis whether multiscale geomorphometric generalized covariables can improve pedometric modeling. To achieve this goal, this case study applied the Random Forest algorithm to a multiscale geomorphometric database to predict soil surface attributes. The study area is in phanerozoic sedimentary basins, in the Alter do chão geological formation, Eastern Amazon, Brazil. The multiscale geomorphometric generalization was applied at general and specific geomorphometric covariables, producing groups for each scale combination. The modeling was run using Random Forest for A-horizon thickness, pH, silt and sand content. For model evaluation, visual analysis of digital maps, metrics of forest structures and effect of variables on prediction were used. For evaluation of soil textural classifications, the confusion matrix with a Kappa index, and the user’s and producer’s accuracies were employed. The geomorphometry generalization tends to smooth curvatures and produces identifiable geomorphic representations at sub-watershed and watershed levels. The forest structures and effect of variables on prediction are in agreement with pedological knowledge. The multiscale geomorphometric generalized covariables improved accuracy metrics of soil surface texture classification, with the Kappa Index going from 43% to 62%. Therefore, it can be argued that topography influences soil distribution at combined coarser spatial scales and is able to predict soil particle size contents in the studied watershed. Hydrometeorological Modeling of Limpopo River Basin in Mozambique with TOPMOdeL and Remote Sensing

The Limpopo River basin (LRB) is known for its vulnerability to floods, high rates of evapotranspiration, and droughts that cause significant losses to the local community. The present study aimed to perform simulations of flood events occurring in two Mozambican sub-basins of LRB, namely chókwè and Xai-Xai from 2000 to 2015 with TOPography-based hydrological MOdeL (TOPMOdeL) and satellite remote sensing data. As input in TOPMOdeL, data from two high-resolution global satellite-based precipitation products: Climate Prediction Center MORPHing technique (CMORPH) and Integrated Multi-Satellite Retrievals for the Global Precipitation Mission (GPM) algorithm (IMERG), 8-day MOD16 evapotranspiration product and surface runoff data estimated by Global Land Data Assimilation System (GLDAS) were used. The sensitivity tests of TOPMOdeL parameters were applied using the Monte Carlo simulation. Calibration and validation of the model were performed by the Shuffled Complex Evolution (SCE-UA) method and were evaluated with the Kling-Gupta Efficiency (KGE) index. The results indicated that simulations with the GPM-IMERG (KGE: 0.59 and 0.65) tended to underestimate the stream flows, while with the CMORPH product the performance was much better (KGE: 0.66 and 0.77) in both sub-basins. Thus, TOPMOdeL can help to develop flood monitoring systems from satellite remotely sensed data in similar regions of Mozambique.

Phase Transformations in PZT Thin Films Prepared by Polymeric chemical Method

Lead zirconate titanate Pb(Zr0.50Ti0.50)O3 (PZT) thin films were deposited by a polymeric chemical method on Pt(111)/ Ti/sio2/Si substrates to understand the mechanisms of phase transformations in these films. PZT films pyrolyzed at temperatures higher than 350℃ present a coexistence of pyrochlore and perovskite phases, while only perovskite phase is present in films pyrolyzed at temperatures lower than 300℃. For films where the pyrochlore and perovskite phase coexists the amount of pyrochlore phase decreases from top surface to the bottom film-electrode interface and the PZT structure near top surface are Ti-rich compositions while near the bottom film-electrode interface the compositions are Zr-rich. For pyrochlore-free PZT thin film, a small (100) orientation tendency near the film-electrode interface was observed.
