Intraductal Prostatic Carcinoma: Epidemiological and Anatomopathological Aspects in Dakar

Introduction: Intraductal carcinoma is often associated with high-grade, high-stage adenocarcinoma. Its frequency is variable and it is considered a poor prognostic factor. In our context, when prostatic carcinoma is diagnosed, pathologists do not always report the presence of this anatomopathological entity. We therefore conducted a study to determine the epidemiological and anatomopathological profile of patients with this lesion in Dakar. Materials and Methods: This is a retrospective descriptive study covering a 1-year period from January to December 2022. It focused on cases of intraductal carcinoma diagnosed among prostatic carcinomas collected in the anatomopathology laboratories of Hôpital Général Idrissa Pouye (HOGIP) and Hôpital Militaire de Ouakam (HMO). It was based on archives of anatomopathological reports, blocks and slides. A total of 200 cases of prostatic carcinoma were collated and reviewed to identify those presenting with intraductal carcinoma according to the diagnostic criteria of Guo and Epstein. Results: 87 cases of intraductal carcinoma were found, representing 43.5% of prostatic carcinomas. The mean age was 71 years. Patients in their seventh decade were the most represented, i.e. 42.5%. The majority of samples examined were biopsies (72.4%). The mean PSA level was 965.91 ng/ml, with extremes ranging from 0.03 to 10,000 ng/ml. Histologically, 96.5% of cases (N = 84) were invasive prostatic carcinoma. Gleason score 8 (4 + 4) was the most common, accounting for 42.53% (N = 37). On average, the study found four (04) foci of intraductal carcinoma per specimen, with extremes ranging from 1 to 30. Dense cribriform architecture accounted for 78.16%, loose cribriform for 11.5%, solid for 8.04% and micropapillary for 2.3%. Six cases (6.9%) showed foci of comedonecrosis. Histopathologic Profile of Miscarriages during First Trimester of Pregnancy in Teaching Hospital of Grand Yoff in Dakar (Senegal)

Background: Mainly for economic reasons, histopathologic analysis is not a systematic practice in medical structures in Senegal, and the utility of this exam is being questioned by many authors. The purpose of this paper is to report the results of this exam, in our medical practice and try to determine if this exam should be systematic of kept only for individualized cases. Material and Methods: It was a retrospective study from January 2010 to December 2018, carried out in the Laboratory of Pathology of the Teaching Hospital of Grand Yoff in Dakar. The data were collected from the register of pathologic examinations of the laboratory. Results: We registered 543 results of histopathologic examination of products of conception from the first trimester abortion. The women mean age was 22.4 years ± 6.2 with extremes of 17 and 46 years. The routine histopathologic assessment of products of first- trimester miscarriages highlighted in our study three pathologies: hydatidiform mole (24.7%), infection (4.6%) and ectopic pregnancy (0.09%). Only 11.7% of cases of Hydatidiform mole (HM) were suspected before the histopathologic assessment. In our sample, molar pregnancy seems to be higher in anembryonic conception with a prevalence of 45%. The prevalence of Complete Hydatidiform Mole (chM) was higher than the one of Partial Hydatidiform Mole (PHM) (14.8% vs. 9.9% of miscarriages). Indeed, both require follow up to prevent or manage at time the occurrence of choriocarcinoma. Conclusion: The results of the histologic analysis after abortion in the first trimester of pregnancy show that this exam should be practiced systematically in routine in our context because of high prevalence of hydatidiform mole.

Ballistic design and testing of a composite armour reinforced by CNTs suitable for armoured vehicles

This paper is investigating the use of composite armour reinforced by nanomaterials, for the protection of light armoured(LAV) and medium armoured military vehicles(MAV), and the interaction between the composite materials and high-performance ballistic projectiles. Four armour materials, consisted of front hybrid fibre reinforced polymer cover layer, ceramic strike-face, fibre reinforced polymer intermediate layer and the metal matrix composite reinforced backplate, were manufactured and assembled by adhesive technology. The proposed laminated protection system is suitable for armoured ground vehicles;however, it could be used as armour on ground, air and naval platforms. The design of the protection system, including material selection and thickness, was elaborated depending on the performance requirements of Level 4 + STANAG 4569 military standard(projectile 14.5 mm × 114 mm API B32) and especially on a design philosophy which is analysed with the specifications. The backplate of this new composite is a hybrid material of Metal Matrix Composite(MMC) reinforced with carbon nanotubes(CNTs), manufactured with the use of powder metallurgy technique. The composite backplate material was morphologically, mechanically and chemically analysed. Results show that all plates are presenting high mechanical properties and ballistic characteristics, compared to commonly used armour plates. Real military ballistic tests according to AEP-STANAG 4569 were carried out for the total composite armour systems. After the ballistic tests, AA2024-CNT3 showed the best protection results, compared with the other plates(AA2024-CNT1 and AA2024-CNT2), with the projectile being unable to fully penetrate the composite plate.

Contribution of Anti-p63 Antibodies in the Interpretation of Benign Label Prostatic Biopsies

Introduction: Prostate cancer is the second most common cancer in men. The diagnosis is most often based on the prostate biopsies’ analysis and on histological criteria recognizable on standard coloring. In some cases, the use of immunohistochemistry is important. Objectives: This paper aims to specify the p63 phenotypic profile of lesions diagnosed benign, with minimal suspect foci, difficult to interpret, HGPIN (high grade intraepithelial neoplasia) and LGPIN (low-grade prostatic intraepithelial neoplasia) and evaluate the manual technique of p63 immunohistochemistry. Patients and Method: This was a retrospective, descriptive study of prostate biopsies recorded in the PAC service of the HALD from January 1st, 2018 to December 31st, 2018. It was completed by a manual immunohistochemical study of the blocks enrolled from November 19th to December 4th, 2020 in the PAC department of the HPD. The studied parameters were: registry number, age, clinical stage, prostate volume, PSA level, microscopic appearance and p63 immunohistochemical profile. Results: Our study included 60 prostate biopsies. The ages of our patients varied from 45 to 77 years, with an average of 64.2 years and a standard deviation of 6.2. The majority of patients were at clinical stage cT2b (33%) with a prostate volume varying between 33.15 and 169.4 cc. The minimum value of PSA in our series is 5 ng/ml, the maximum being 100 ng/ml with an average level of 24.1 ng/ml and a standard deviation of 21.2. Our series included 50 adenomyomatous hyperplasias, 7 adenomyomatous hyperplasias associated with chronic prostatitis, 2 HGPIN and 1 LGPIN. After re-reading we found 5 discordant cases, which corresponded to minimal suspect foci (kappa = 0.5098). The p63 marking was informative in 53 cases, i.e. 88%, and non-informative in 7 cases, i.e. 12%. Among the uninformative markings, 2 were due to lack of tissue adhesion to the slides. With the prevalence of the Internet of Things(IoT)systems,smart cities comprise complex networks,including sensors,actuators,appliances,and cyber *** complexity and heterogeneity of smart cities have become vulnerable to sophisticated cyber-attacks,especially privacy-related attacks such as inference and data poisoning *** Learning(FL)has been regarded as a hopeful method to enable distributed learning with privacypreserved intelligence in IoT *** though the significance of developing privacy-preserving FL has drawn as a great research interest,the current research only concentrates on FL with independent identically distributed(i.i.d)data and few studies have addressed the non-i.i.d *** is known to be vulnerable to Generative Adversarial Network(GAN)attacks,where an adversary can presume to act as a contributor participating in the training process to acquire the private data of other *** paper proposes an innovative Privacy Protection-based Federated Deep Learning(PP-FDL)framework,which accomplishes data protection against privacy-related GAN attacks,along with high classification rates from non-i.i.d ***-FDL is designed to enable fog nodes to cooperate to train the FDL model in a way that ensures contributors have no access to the data of each other,where class probabilities are protected utilizing a private identifier generated for each *** PP-FDL framework is evaluated for image classification using simple convolutional networks which are trained using MNIST and CIFAR-10 *** empirical results have revealed that PF-DFL can achieve data protection and the framework outperforms the other three state-of-the-art models with 3%–8%as accuracy improvements.

Comparison between seismic analysis of twisting and regular 52-story towers considering soil-structure interaction

A dynamic analysis of both twisting and regular towers is carried out to determine the results of considering soil-structure interaction(SSI)on high-rise *** addition,the difference between the seismic performance of using twisting towers over regular ones is *** twisting tower is a simulation of the Evolution Tower(Moscow).The towers’skeletons consist of RC elements and rest on a reinforced concrete piled-raft *** soil model is considered as multi-layered with the same soil properties as the zone chosen for the analysis(New Mansoura City,Egypt).The only difference between both towers is their shape in *** whole system is modelled and analyzed in a single step as one full 3D model,which is known as the direct approach in *** analyses are carried out using finite-element software(Midas GTS NX).Dynamic output responses due to three records of seismic loads are proposed and presented in some *** on the results,it is concluded that SSI has a considerable effect on the dynamic response of tall buildings mainly because of the foundation flexibility,as it leads to lengthening the vibration period,increasing the story drift and the base shear for both cases.

The conversion of the cartesian coordinates of a point to its geodetic equivalent coordinates in reference to the geodetic ellipsoid is one of the main challenges in *** ellipse equation in the meridian plane significantly influences the value of the geodetic *** research analyzes this influence and how it can contribute to their *** study investigates the mathematical relation between them and presents an exact formula relating to the geodetic height and the ellipse *** addition,a heuristic formula for the relation between the geodetic height and the ellipse equation is proposed,which is independent of the geodetic latitude and has a relative accuracy better than 99.9 %.The calculation is stable,and the cost is low.

Impact of lifestyle interventions on pathogenesis of nonalcoholic fatty liver disease
