Experience with the surgical treatment of hepatic hydatidosis: Case series with follow-up

AIM: To describe patient characteristics and the results of surgical treatment for hepatic hydatidosis (HH) in the Hospital Hernán Henríquez Aravena from December 2001 to March 2005. METHODS: Subjects older than 16 years with suspected HH were included based on the clinical picture and radiological studies. Variables examined included clinical and laboratory aspects, outcome, features of the parasite and surgical aspects. Descriptive statistics were used, calculating central tendency, dispersion and extreme tendency. RESULTS: The series was comprised of 122 patients, with an average age of 44 ± 16.9 years. The most frequently used surgical technique was subtotal cys-tectomy in 90% of the patients, followed by hepatic resection (hepatectomy, segmentectomy and subsegmentectomy) in 5%. In 2%, a combination of subtotal cystectomy and segmentectomy was performed. In addition, 28% of the series presented complications in the postoperative period and mortality was 2%. CONCLUSION: The most frequently used surgical technique for HH was subtotal cystectomy and the morbidity and mortality rates in this Chilean series are comparable to other national and international series.

Background: Coupling biomass models with nutrient concentrations can provide sound estimations of carbon and nutrient contents, enabling the improvement of carbon and nutrient balance in forest ecosystems. Although nutrient concentrations are often assumed to be constant for some species and specific tree components, at least in mature stands,the concentrations usually vary with age, site index and even with tree density. The main objective of this study was to evaluate the sources of variation in nutrient concentrations in biomass compartments usually removed during harvesting operations, covering a range of species and management conditions: semi-natural forest, conventional forest plantations and short rotation forestry(SRF). Five species(Betula pubescens, Quercus robur, Eucalyptus globulus, Eucalyptus nitens and Populus spp.) and 14 genotypes were considered. A total of 430 trees were sampled in 61 plots to obtain 6 biomass components:leaves, twigs, thin branches, thick branches, bark and wood. Aboveground leafless biomass was pooled together *** concentrations of C, N, K, P, Ca, Mg, S, Fe, Mn, Cu, Zn and B were measured and the total biomass of each sampled tree and plot were determined. The data were analysed using boosted regression trees and conventional ***: The main sources of variation in nutrient concentrations were biomass component > > genotype(species) ≈ age >tree diameter. The concentrations of Ca, Mg and K were most strongly affected by genotype and age. The concentrations of P, K, Ca, Mg, S and Cu in the wood component decreased with age, whereas C concentrations increased, with a trend to reach 50% in the older trees. In the SRF, interamerican poplar and P. trichocarpa genotypes were comparatively more efficient in terms of Ca and K nutrient assimilation index(NAI)(+65-85%) than eucalypts, mainly because leafless biomass can be removed. Landscape genetics reveals inbreeding and genetic bottlenecks in the extremely rare short-globose cacti Mammillaria pectinifera (Cactaceae) as a result of habitat fragmentation

Mammillaria pectinifera is an endemic, short-globose cactus species, included in the IUCN list as a threatened species with only 18 remaining populations in the Tehuacfin-Cuicatlan Valley in central Mexico. We evaluated the population genetic diversity and structure, connectivity, recent bottlenecks and population size, using nuclear microsatellites. M. pectinifera showed high genetic diversity but some evidence of heterozygote deficiency （F15）, recent bottlenecks in some populations and reductions in population size. Also, we found low population genetic differentiation and high values of connectivity for M. pectinifera, as the result of historical events of gene flow through pollen and seed dispersal. M. pectinifera occurs in sites with some degree of disturbance leading to the isolation of its populations and decreasing the levels of gene flow among them. Excessive deforestation also changes the original vegetation damaging the natural habitats. This species will become extinct if it is not properly preserved. Furthermore, this species has some ecological features that make them more vulnerable to disturbance such as a very low growth rates and long life cycles. We suggest in situ conservation to prevent the decrease of population sizes and loss of genetic diversity in the natural protected areas such as the Tehuacan-Cuicatlan Biosphere Reserve. In addition, a long-term ex situ conservation program is need to construct seed banks, and optimize seed germination and plant establishment protocols that restore disturbed habitats. Furthermore, creating a supply of living plants for trade is critical to avoid further extraction of plants from nature.

Structural health monitoring (SHM) is a relevant topic for civil systems and involves the monitoring, data processing and interpretation to evaluate the condition of a structure, in order to detect damage. In real structures, two or more sites or types of damage can be present at the same time. It has been shown that one kind of damaged condition can interfere with the detection of another kind of damage, leading to an incorrect assessment about the structure condition. Identifying combined damage on structures still represents a challenge for condition monitoring, because the reliable identification of a combined damaged condition is a difficult task. Thus, this work presents a fusion of methodologies, where a single wavelet-packet and the empirical mode decomposition (EMD) method are combined with artificial neural networks (ANNs) for the automated and online identification-location of single or multiple-combined damage in a scaled model of a five-bay truss-type structure. Results showed that the proposed methodology is very efficient and reliable for identifying and locating the three kinds of damage, as well as their combinations. Therefore, this methodology could be applied to detection-location of damage in real truss-type structures, which would help to improve the characteristics and life span of real structures.

OBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).METHODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CONCLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.
