Recent trends in liver transplantation for alcoholic liver disease in the United States

AIM To examine temporal changes in the indications for liver transplantation(LT) and characteristics of patients transplanted for alcoholic liver disease(ALD).MeTHODS We performed a retrospective cohort analysis of trends in the indication for LT using the United Network for Organ Sharing(UNOS) database between 2002 and 2015. Patients were grouped by etiology of the liver disease and characteristics were compared using χ~2 and t-tests. Time series analysis was used identifying any year with a significant change in the number of transplants per year for ALD, and before and after eras were modeled using a general linear model. Subgroup analysis of recipients with ALD was performed by age group, gender, UNOS region and etiology(alcoholic cirrhosis, alcoholic hepatitis and hepatitis C-alcoholic cirrhosis dual listing).ReSULTS Of 74216 liver transplant recipients, ALD(n = 9400, 12.7%) was the third leading indication for transplant after hepatitis C and hepatocellular carcinoma. Transplants for ALD, increased from 12.8%(553) in 2002 to 16.5%(1020) in 2015. Time series analysis indicated a significant increase in the number of transplants per year for ALD in 2013(P = 0.03). There were a stable number of transplants per year between 2002 and 2012(linear coefficient 3, 95%CI:-4.6, 11.2) an increase of 177 per year between 2013 and 2015(95%CI: 119, 234). This increase was significant for all age groups except those 71-83 years old, was observed for both genders, and was incompletely explained by a decrease in transplants for hepatitis C and ALD dual listing. All UNOS regions except region 9 saw an increase in the mean number of transplants per year when comparing eras, and this increase was significant in regions 2, 3, 4, 5, 6, 8, 10 and 11.CONCLUSION There has been a dramatic increase in the number of transplants for ALD starting in 2013.

Changing landscape of hepatitis C virus-positive donors

With the introduction of the new highly effective antiviral therapies, there has been a dramatic increase in the use of the hepatitis C virus(HCV)-positive livers in HCV-positive recipients. In the majority of studies, HCV positivity was defined as a donor testing HCV Ab positive. In 2015, all Organ Procurement Organizations were mandated to perform and report HCV Nucleic Acid Amplification Testing(NAT) results on all deceased and living donors. Studies are not yet available on how organs are being utilized based on NAT status and whether NAT status affects recipient outcomes. Further studies are needed to maximize the use of these organs.

Muscle energy technique for chronic obstructive pulmonary disease:A feasibility study

Objective:This study aimed to investigate the feasibility of implementing a manual therapy technique(muscle energy technique,MeT)protocol in a hospital pulmonary rehabilitation(PR)program for patients with moderate to severe chronic obstructive pulmonary disease(COPD).Methods:Participants aged 40 years and over,with moderate to severe COPD,were recruited into this 12-week study.The primary outcome measures were feasibility(acceptability of the intervention and attendance/adherence to the trial)and safety(adverse events,Aes).All participants received the MeT and PR therapies.Participants and assessors were unblinded.Semi-standardized MeT was delivered on 6 occasions(a maximum of once per week)at the hospital directly before a PR session.Participants undertook PR sessions as per the hospital program at a frequency of two days per week for 8 weeks.Participants were contacted 4 weeks after their final MeT treatment via a telephone call to assess acceptability of the intervention.Results:Thirty-three participants were enrolled,with a median age of 74 years(range 45-89 years).The median number of MeT sessions that participants attended was 5(range O-6)out of a possible 6 sessions(83%attendance).At follow-up,participants overwhelmingly enjoyed the MeT treatment with some subjectively reporting improved breathing.There were no major Aes related to the intervention,with the majority of Aes classified as expected events related to COPD exacerbations.Conclusion:It is feasible to implement a manual therapy protocol using MeT as an adjunct to PR in a hospital setting.Recruitment rates were satisfactory and there were no Aes related to the MeT component of the intervention.

CRISPR/Cas-mediated genome editing in human pluripotent stem cells(hPSCs)offers unprecedented opportunities for developing in vitro disease modeling,drug screening and cell-based therapies.To efficiently deliver the CRISPR components,here we developed two all-in-one vectors containing Cas9/gRNA and inducible Cas13d/gRNA cassettes for robust genome editing and RNA interference respectively.These vectors utilized the PiggyBac transposon system,which allows stable expression of CRISPR components in hPSCs.The Cas9 vector PB-CRISPR exhibited high efficiency(up to 99%)of inducing gene knockout in both protein-coding genes and long non-coding RNAs.The other inducible Cas13d vector achieved extremely high efficiency in RNA knockdown(98%knockdown for CD90)with optimized gRNA designs.Taken together,our PiggyBac CRISPR vectors can serve as powerful toolkits for studying gene functions in hPSCs.

The majority of patients with congenitally corrected transposition of the great arteries,also known as transposition of the great arteries{S,L,L}have ventricular septal defects(VSD),most commonly perimembranous VSD(pmVSD).Transcatheter device closure of pmVSD in these patients has not been widely described.We present a case of device closure of pmVSD in L-TGA with an Amplatzer Duct Occluder II(ADOII)device using a deployment starting in the subpulmonary left ventricle.The case demonstrates some of the technical advantages of the ADOII device for VSD closure,specifically its low profile,symmetric shape,and soft material.These characteristics are advantageous in closing conventional pmVSD but are especially useful in patients with challenging anatomic substrates.

BACKGROUND Diabetes rates among pregnant women in the United States have been increasing and are associated with adverse pregnancy outcomes.AIM To investigate differences in birth outcomes(preterm birth,macrosomia,and neonatal death)by diabetes status.MeTHODS Cross-sectional design,using linked Missouri birth and death certificates(singleton births only),2010 to 2012(n=204057).exposure was diabetes non-diabetic,pre-pregnancy diabetes-insulin dependent(PD-I),pre-pregnancy diabetes-non-insulin dependent(PD-NI),gestational diabetes-insulin dependent(GD-I),and gestational diabetes-non-insulin dependent(GD-NI).Outcomes included preterm birth,macrosomia,and infant mortality.Confounders included demographic characteristics,adequacy of prenatal care,body mass index,smoking,hypertension,and previous preterm birth.Bivariate and multivariate logistic regression assessed differences in outcomes by diabetes status.ReSULTS Women with PD-I,PD-NI,and GD-I remained at a significantly increased odds for preterm birth(aOR 2.87,aOR 1.77,and aOR 1.73,respectively)and having a very large baby[macrosomia](aOR 3.01,aOR 2.12,and aOR 1.96,respectively);in reference to non-diabetic women.Women with GDNI were at a significantly increased risk for macrosomia(aOR1.53),decreased risk for their baby to die before their first birthday(aOR 0.41)and no difference in risk for preterm birth in reference to non-diabetic women.CONCLUSION Diabetes is associated with the poor birth outcomes.Clinical management of diabetes during pregnancy and healthy lifestyle behaviors before pregnancy can reduce the risk for diabetes and poor birth outcomes.

This study aims to investigate the ability of an imidazolium biobased Zwitterionic Ionic Liquids(ZILs)in enhancing the phytoavailability of copper from garden(G)and vineyard(V)soils using the model plant ryegrass.Uncontaminated and artificially contaminated CuSO_(4) soils,unamended and ZIL-amended soil modalities were designed.The copper/ZIL molar ratio(1/4)introduced was rationally established based on molecular modeling and on the maximal copper concentration in artificially contaminated soil.Higher accumulation of copper in the shoots was detected for the uncontaminated and copper contaminated ZIL amended V soils(18.9 and 23.3 mg/kg,respectively)contrary to G soils together with a ZIL concentration of around 3%(W/W)detected by LC-MS analyses.These data evidenced a Cu-accumulation improvement of 38%and 66%compared to non-amended V soils(13.6 and 13.9 mg/kg respectively).ZIL would be mainly present under Cu(II)-ZIL_(4) complexes in the shoots.The impact on the chemical composition of shoot was also studied.The results show that depending on the soils modalitity,the presence of free copper and/or ZIL led to different chemical compositions in lignin and monomeric sugar contents.In the biorefinery context,performances of enzymatic hydrolysis of shoots were also related to the presence of both ZIL and copper under free or complex forms.ecotoxicity assessment of the vineyard soil samples indicated that the quantity of copper and ZIL remaining in the soils had no significant toxicity.ZIL amendment in a copper-contaminated soil was demonstrated as being a promising way to promote the valorization of phytoremediation plants.

The aminothiol cysteamine,derived from coenzyme A degradation in mammalian cells,presents several biological applications.However,the bitter taste and sickening odor,chemical instability,hygroscopicity,and poor pharmacokinetic profile of cysteamine limit its efficacy.The use of encapsulation systems is a good methodology to overcome these undesirable properties and improve the pharmacokinetic behavior of cysteamine.Besides,the conjugation of cysteamine to the surface of nanoparticles is generally proposed to improve the intra-oral delivery of cyclodextrin-drug inclusion complexes,as well as to enhance the colorimetric detection of compounds by a gold nanoparticle aggregation method.On the other hand,the detection and quantification of cysteamine is a challenging mission due to the lack of a chromophore in its structure and its susceptibility to oxidation before or during the analysis.Derivatization agents are therefore applied for the quantification of this molecule.To our knowledge,the derivatization techniques and the encapsulation systems used for cysteamine delivery were not reviewed previously.Thus,this review aims to compile all the data on these methods as well as to provide an overview of the various biological applications of cysteamine focusing on its skin application.

Long-term efficacy of a computer-tailored physical activity intervention for prostate and colorectal cancer patients and survivors:A randomized controlled trial

Background Physical activity(PA)can improve the physical and psychological health of prostate and colorectal cancer survivors,but PA behavior change maintenance is necessary for long-term health benefits.OncoActive is a print-and web-based intervention in which prostate and colorectal cancer patients and survivors receive automatically generated,personalized feedback aimed at integrating PA into daily life to increase and maintain PA.We evaluated the long-term outcomes of OncoActive by examining the 12-month follow-up differences between OncoActive and a control group,and we explored whether PA was maintained during a 6-month non-intervention follow-up period.Methods Prostate or colorectal cancer patients were randomly assigned to an OncoActive(n=249)or a usual care waitlist control group(n=229).OncoActive participants received PA advice and a pedometer.PA outcomes(i.e.,ActiGraph and self-report moderate-to-vigorous intensity PA(MVPA)min/week and days with≥30 min PA)and health-related outcomes(i.e.,fatigue,depression,physical functioning)were assessed at baseline,6 months,and 12 months.Differences between groups and changes over time were assessed with multilevel linear regressions for the primary outcome(ActiGraph MVPA min/week)and all additional outcomes.Results At 12 months,OncoActive participants did not perform better than control group participants at ActiGraph MVPA min/week,self-report MVPA min/week,or ActiGraph days with PA.Only self-report days with PA were significantly higher in OncoActive compared to the control group.For health-related outcomes only long-term fatigue was significantly lower in OncoActive.When exploratively examining PA within OncoActive,the previously found PA effects at the end of the intervention(6 months follow-up)were maintained at 12 months.Furthermore,all PA outcomes improved significantly from baseline to 12 months.The control group showed small but non-significant improvements from 6 months to 12 months(and from baseline to 12 months),resulting in a decline of differences bet

The application of digital holography for accurate three-dimensional localisation of mosquito-bednet interaction

Understanding mosquito interaction with long-lasting insecticidal bednets is crucial in the development of more effective intervention methods to protect humans from malaria transmission.As such,a 240×240×1000 mm laboratory setup for the in-line recording of digital holograms and subsequent in-focus reconstruction and 3D localisation of mosquitoes is presented.Simple bednet background removal methods are used to accurately localise a mosquito obscured by a bednet in 3D coordinates.Simulations and physical data demonstrate that this method is suitable for mosquitoes positioned 3−1000 mm behind a bednet.A novel post-processing technique,involving a cascade-correlation of a Tamura of Intensity focus metric extracted from digitally reconstructed scenes,accurately localises mosquitoes positioned 35−100 mm behind a bednet from a single digital hologram.The result of this study is a scalable digital holographic methodology to examine mosquito-bednet interaction in 3D at a level of accuracy previously only seen in 2D imaging of mosquitoes in a much smaller volume.
